3 JCI - Our Mission Transforming patient care across the globe The mission of JCI is to continuously improve the safety and quality of care in the international community through the provision of education and advisory services and international accreditation and certification. 2

4 The Safety Culture JCI has identified the safety objective as: The degree to which the risk of an intervention and the risk in the care environment are reduced for the patient and others, including the health care provider. 3

5 The World s Leading Practices Improving the quality, safety, and efficiency of health care is a goal shared by health care organizations and countries around the world. 4

6 Safety Quality The Joint Commission 5

7 International Essentials of Safety The Joint Commission has identified areas of risk on which to focus the improvement efforts. These areas were developed from an extensive international literature search on health care quality and safety. Specific criteria for each risk area were developed to provide clear and achievable risk-reduction strategies. (universal protocol-2003) 6

8 International Alliances for Safety Focus of attention: Development and implementation of problemspecific standardized operating protocols (SOPs) such as the Universal Protocol Creation of a comprehensive impact evaluation strategy (RCA) Collection, reporting, and analysis of data Establishment of an electronic collaborative learning community (sentinel events statistics and alerts) 7

9 Alliance with the WHO This focus on the highest patient care standards and results-oriented process improvement has earned the respect of health care leaders. That's why The World Health Organization (WHO) partnered with the JC in 2005 to establish the first WHO Collaborating Centre for Patient Safety Solutions 8

10 Other Alliances for Safety The SafeCare Foundation, a cooperation between JCI and the Council for Health Service Accreditation of Southern Africa, and the PharmAccess Foundation of the Netherlands, are designed to assist health care providers in step-wise quality improvement and the delivery of safer care to their patients. 9

11 Regional Advisory Councils Joint Commission International has three regional advisory councils: Asia-Pacific, Europe, and the Middle East and North Africa. These councils include key stakeholders whose charge is to facilitate JCI s awareness and understanding of important regional health care quality, patient safety, health financing, health policy issues, and cultural needs and requirements. 10

12 Other Alliances for Safety The JCI has corporate agreements with many governments in the Middle East to: Setup national accreditation programs Provide staff training on quality and safety issues Conduct initial assessments, gap analysis, and mock surveys to prepare individual organizations for accreditation. 11

14 Staff Safety in the JCI Standards Quality & Patient Safety (QPS.3) Quality improvement and patient safety are data driven. Effective use of the data is best accomplished in the broader context of evidence-based clinical and management practices: i. prevention and control of events that jeopardize the safety of patients, families, and staff. 13

15 Staff Safety in the JCI Standards Quality & Patient Safety (QPS.11) An ongoing program of risk management is used to identify and to reduce unanticipated adverse events and other safety risks to patients and staff. Organizations need to adopt a proactive approach to risk management (FMEA) 14

16 Staff Safety in the JCI Standards Prevention and Control of Infections (PCI.2) There is a designated coordination mechanism for all infection prevention and control activities that involves physicians, nurses, and other staff in central sterilization, microbiology, pharmacy, housekeeping, laundry, operating room, etc.) 15

17 Staff Safety in the JCI Standards Prevention and Control of Infections (PCI.4) The organization s leaders provide adequate resources to support the infection prevention and control program. An important resource is the information management system to support the tracking of risks, rates, and trends by using data analysis, interpretation, and presentation of findings. 16

18 Staff Safety in the JCI Standards Prevention and Control of Infections (PCI.5) The organization designs and implements a comprehensive program to reduce the risks of health care-associated infections in patients and health care workers. This is achieved through policies and procedures, staff education, risk assessment and action plans, employee health program, etc. 17

19 Staff Safety in the JCI Standards Prevention and Control of Infections (PCI.7.3) The organization has a policy and procedure on the disposal of sharps and needles. The organization ensures that a policy is implemented that adequately addresses all steps in the process including the selection and use of containers, the disposal of the containers, and the surveillance of the process of disposal. 18

20 Staff Safety in the JCI Standards Measurable Elements of PCI Sharps and needles are collected in dedicated, puncture-proof containers 2. The hospital disposes of sharps and needles safely 3. The disposal of sharps and needles is consistent with infection prevention and control policies 19

21 Staff Safety in the JCI Standards Prevention and Control of Infections (PCI.11) The organization provides education on infection prevention and control practices to staff, physicians, patients, families, and other caregivers when indicated by their involvement in care (specific, targeted, periodic, etc.) 20

22 Staff Safety in the JCI Standards Facility Management & Safety (FMS.4.1) The organization inspects all patient care buildings and has a plan to reduce evident risks and to provide a safe physical facility for patients, families, staff, and visitors. The goal is to prevent accidents and injuries; to maintain safe and secure conditions for patients, families, staff, and visitors; and to reduce / control hazards and risks. 21

23 Staff Safety in the JCI Standards Staff Qualifications & Education (SQE.8.4) Staff understand how to report, to be treated for, and to receive counseling and follow-up for such injuries as needle-sticks. The organization provides a staff health and safety program including an immunization schedule. 22

24 Staff Safety in the JCI Standards Staff Qualifications & Education (SQE.8.4) The program may also provide for initial employment health screening, periodic preventive immunizations and examinations, treatment for common work-related conditions, such as back injuries, or more urgent injuries. (to be documented in a hospital policy) 23

25 Sentinel Event Alerts Joint Commission sentinel event alerts related to staff safety: Sentinel Event Alert, Issue 22: Preventing needle-stick and sharps injuries The risk of injury from needles and sharps, including disposable needles, over-the-needle catheters, suture needles, lancets and scalpels, continues to expose health care workers to serious and potentially fatal infections from blood borne pathogens such as hepatitis B, hepatitis C, or human immunodeficiency virus (HIV). 24

26 Sentinel Event Alerts Joint Commission sentinel event alerts related to staff safety: Sentinel Event Alert, Issue 50: Behaviors that undermine a culture of safety Intimidating and disruptive behaviors can foster medical errors, contribute to poor patient satisfaction and to preventable adverse outcomes, increase the cost of care, and cause qualified clinicians, administrators and managers to seek new positions in more professional environments. 25

27 Speak-up Initiatives Prevent the spread of infections (including those contracted by healthcare professionals) Other speak-up initiatives include pain management, patient rights, fall assessment, etc. 26

28 Sustainable Results JCI strategies deliver sustainable results that work. Safe Health Design: Creating safer environments for patients, visitors, and staff JCI s expert clinicians and engineers apply evidence-based health design practices and JCI standards to help create physical environments that foster safety, quality, and operational effectiveness. 27

29 Improvement in Quality & Safety Health care organizations, and their patients, remain at risk from poor quality and unsafe practices if organizations do not learn from their good and bad experiences and take actions to continually improve. The Joint Commission 28
