What is RPM? Using wireless technology, RPM connects clients living with chronic illnesses, to a health care professional who monitors their vital signs remotely and encourages medication compliance and provides information about managing their chronic condition(s). ReActwas created in 2010 and has consistently had over 100 clients participating at any one time since then. Currently primarily focused on chronic disease patients presenting a medium-to-high risk of admission to a hospital, RPM remains an emerging enabler for health system transformation with the potential to improve outcomes and the patient experience through self-management and home-based care. RPM is an evolving technology and CBI continues to engage with healthcare and technology leaders in this space, as demonstrated by the content of this presentation to move this area of practice forward.
