1 Job Details Job Title: Location: Reports to: Project Worker Glasgow Housing Support Services - Barrowfield Service Project Manager Job Purpose As a Project Worker within a Housing Support Service you will provide support and care to people who have a sensory impairment and who may be deaf, blind or Deafblind. The people may also have additional needs such as mental ill health, physical disability or other health needs. You will support them to live the lives that they want and to live in their own home. You will work as an integral part of the team, providing excellent standards of support and care. Your work as a Project Worker may involve you working on a 1 to 1 basis with tenants or as part of a bigger team providing support to individuals at any given time 1. Day to day housing support You will help the people we support to participate fully in the dayto-day management of their home, including: - General counselling, encouragement and support that enables tenants to maintain their tenancy - Assisting with the security of the persons home - Assisting with the maintenance of the safety of the persons home - Advising and supervising tenants on the use of domestic equipment and appliances - Assisting with arranging minor repairs to and servicing of a tenants own domestic equipment and appliances - Helping individuals to develop life skills needed to maintain their dwelling in an appropriate and safe condition i.e. how to keep their house clean, disposal of refuse, managing entry of visitors to home and assisting individuals to deal with unwelcome or undesirable visitors etc - Arranging adaptations to enable the tenants to cope with disability

2 - Advising or assisting the tenant with personal budgeting and debt counselling - Advising/assisting or indeed assisting services users to access services that relate to benefit claims and other official correspondence relevant to sustaining occupancy of the dwelling - Assisting tenants to manage their finances, including helping them with the payment of household bills - Advising, assisting or making arrangements for tenants to deal with relationships and disputes with neighbours - Advising or assisting the tenant to move onto other accommodation if this is his/her preference or is reflective of an increase in independence etc - Assisting with shopping and errands - Assisting individuals with the cleaning of their own rooms and windows 2. Person Centred Care and Supports - You will help people to develop and maintain friendships and networks of support within the local community. - You will help people to achieve their defined outcomes, goals and dreams as detailed in the persons support and care plan as well as in individual outcome based support plans. - You will help people you are working with to maintain their personal safety, health and wellbeing, including supporting them with personal care. You will work to the care and support plan instructions agreed to achieve this. - You will assist the people we support to participate in social, recreational and leisure activities that have been defined and chosen by them, and you will work to the care and support plan instructions agreed to achieve this - You will work in a key worker role to identified individuals, taking the lead in ensuring their support is delivered in line with their Support and Care Plans and Outcome Based Support Plans. You will be responsible for ensuring that these plans, and any required risk assessments are reviewed and updated as required and in line with statutory and organisational expectations. - You will contribute to the formation of Support and Care plans and Outcome Based Support Plans for all tenants you support.

Communication - You will support people to communicate in the manner they want and are most comfortable with to this could be through using British Sign Language (BSL), Deafblind Manual Alphabet (Hands on Signing), Braille or using a person s own individual way of signing. - As English is not a deaf or Deafblind person s first language you may need to support a person with their mail etc - You will support people to access interpreters where they are required possibly for meetings, care reviews etc 4. Planning for the future At The Mungo Foundation we want everyone we work with to have a Care and support plan. This Plan sets out the kind of life the person wants to live and the kind of help they need to achieve this. In addition to this, individuals may have what is called an outcomes based support plan which further focuses on the outcomes that the ongoing support a person receives will help the person achieve. The first step in doing this is usually to bring together everyone who is important in the person s life to talk things over. As a Project Worker you may need to provide some practical help to the person to make sure that these meetings happen and to help the person to express their own opinion about how they want their life to be. You will also help the person to adapt and improve their Care and support plan by assisting them at regular meetings and reviews. 5. Training and Supervision - You will participate in a full Induction Training Programme. - You will have the opportunity to participate in training relating to the specific support needs of the individual(s) that you are working with. - You will participate in other training, as and when required.

4 - You will have the opportunity to be involved in Individual Supervision and Appraisal and you will make yourself available for such sessions and will provide items for the agenda - You will participate in Team Planning and Team Building - You will take personal responsibility for putting your training and supervision into practice and for advising your line manager of any training need. 6. Organisational Responsibilities - You will adhere at all times to The Mungo Foundation Policies and Procedures. - You will participate in regular team meetings and provide items for the agenda. - You will recognise that how you act/behave outside of work has an impact on you and the reputation of The Mungo Foundation. - You will maintain a safe working environment ensuring your own personal safety and the safety of others - You will ensure that all statistical and/or factual information required by the organisation is collated and forwarded to the relevant personnel. - You will communicate clearly with colleagues so that our services operate effectively. - You will ensure that work plans are adhered to and any deviation is discussed with your line manager at the earliest opportunity. - To bring to the attention of the manager any amendments required to work plans. 7. Personal Responsibilities - You will encourage positive attitudes toward the people we support. - You will adhere and promote the organisation s values of Life, Justice and Community at all times in your practice. - You will adhere at all times to the SSSC Codes of Practice. - You will ensure that no action or omission on your part is detrimental to the interests, condition or safety of the people we support. - You will work in a flexible way recognising the individual needs of the people we support. - You will provide sleepover and night time cover if required.

5 - You will represent the organisation in a positive manner at all times. This Job Description provides the basis of the role of the Project Worker in a Housing Support Service.
