2 H o m e C a r e O n t a r i o S y m p o s i u m About Home Care Ontario Home Care Ontario is a provincial association with a mandate to promote growth and development of the home care sector through advocacy, knowledge transfer and member service. In Ontario, members provide nursing care, home support services, personal care, physiotherapy, occupational therapy, social work, dietetics, speech language therapy and medical equipment and supplies in the home to individuals of all ages. An estimated 58 million hours of publicly and privately purchased home care services are provided annually across the province. 3 Welcome by the Chair On behalf of the Board of Home Care Ontario, it is a great pleasure to welcome you to our 2017 Symposium. This year s theme, Better Home Care in Ontario, provides the important context for our discussions over the course of the day. Home Care Providers are unwavering in their commitment to an improved health care system for Ontarians that puts Patients First and today s agenda will challenge us all to think about constantly getting better. Our speakers and panellists have been chosen to bring their experience and advice on patient and family caregiver engagement, new government directions, technology advancement and health human resource planning. We have also allowed time for you to ask questions and share perspectives during the day. I am also particularly proud to announce the Association s first Frontline Worker of the Year Awards. The purpose is to recognize excellence in Home Care service provision in Ontario by honouring an outstanding person in each of two categories: Care Provider and Staffing Coordinator. I am told that there were dozens of worthy submissions. You can read more about our Frontline Workers of the Year in our program. Thanks so much for being here to share this day with us! 4 Frontline Workers of the Year 2017 Sylvia Bradbury, Companion, Right at Home Canada, Georgian Triangle Sylvia always goes above and beyond in every aspect of her caregiving. She became a professional caregiver after being the primary caregiver for her late husband in the last few years of his life. She treats all the people she cares for with the same compassion as she did for him. Everywhere she goes she brightens the room with her smile and lovely demeanor. No task is too big or small for Sylvia, she is always able to find the positives even in some difficult situations. Sylvia is punctual, impeccable and reliable. Her documentation is detailed and accurate. She is dedicated to improving the quality of life for seniors and people with disabilities. Sylvia is the family when the family is not able to be there. Janis Collins, Staffing Coordinator, CBI Home Health, Guelph Janis is a steady, reliable member of the CBI team. Janis coordinates therapy service for CBI s adult occupational therapy program. Demands on her are high as many referrals are urgent or time specific and are spread out across a vast geography. She is consistently calm and composed and never loses focus on the client experience. She is patient even when required to repeat information for a client. She is respected so much by her team of therapists and callers appreciate her patient and calm demeanour and helpfulness. Janis has coordinated the Big Bike Ride Heart and Stroke fundraiser for the office team over the last two summers. Her motivation was to bring the team together with a common goal of helping others. She is also an active volunteer outside of work with many organizations including Habitat for Humanity. 4

5 H o m e C a r e O n t a r i o S y m p o s i u m Opening Keynote Presentation Transforming the System to Achieve Better Home Care in Ontario Dr. Bob Bell, MDCM, MSc, FRCSC, FACS, Deputy Minister, Ministry of Health and Long-Term Care Dr. Robert Bell was appointed Deputy Minister of Health and Long-Term Care, effective June 2, Prior to this role, he served as President and Chief Executive Officer of University Health Network for nine years. He was previously the Chief Operating Officer at Princess Margaret Hospital and Chair of both Cancer Care Ontario s Clinical Council and the Cancer Quality Council of Ontario. Dr. Bell received his Doctor of Medicine from McGill University and a Master of Science from the University of Toronto. He also completed a Fellowship in Orthopaedic Oncology at Massachusetts General Hospital and Harvard University. Dr. Bell is a Fellow of the Royal College of Physicians and Surgeons of Canada, the American College of Surgeons and an Honorary Fellow of the Royal College of Surgeons of Edinburgh. An internationally recognized orthopedic surgeon, health care executive, clinician-scientist, and educator, Dr. Bell brings more than 40 years of health care experience to his current role. #symp2017 5

6 Panel - Implementing LHIN Renewal Alison Blair Director, Local Health Integration Network (LHIN) Renewal Branch, Health System Accountability and Performance Division of the Ministry of Health and Long-Term Care Alison provides leadership and direction on the Patients First project and its implementation. She serves as the orchestra conductor for this ambitious project, to ensure alignment, coordination and engagement among key implementation partners across the healthcare system. Alison previously worked in the Health System Information Management Division where, since 2007, she provided leadership on policy and legislation to ensure the effective provision of information to support patient care and health system management, in a privacy-protective manner. Amy Olmstead Director, Health System Accountability & Performance Division, Home and Community Care Branch, Ministry of Health and Long-Term Care Among other work, the Home and Community Care Branch is leading the implementation of Patients First: A Roadmap to Strengthen Home and Community Care (2015) and supporting The Patients First Act, 2016, which expands the mandate of Local Health Integration Networks. Prior to her appointment, Amy was Manager of the Universities Unit in the Ministry of Training, Colleges and Universities. Amy also served as Manager in the Capital Programs Branch in the Ministry of Education, and as Policy Advisor and as Policy Director to the Minister of Education. Panel - Technology to Enable Better Home Care in Ontario Mary Gavel ehealth Privacy Specialist, Healthcare Information Technology Services (HITS) Mary Gavel is an ehealth Privacy Specialist in the Health Information Technology (HITs) ehealth Office, Hamilton Health Sciences. She supports health care providers through connecting South West Ontario Change Management and Delivery Partners to ensure privacy compliance in accordance with the Personal Health Information Protection Act (PHIPA) with the privacy assessment process for becoming a ClinicalConnect participant. She provides privacy guidance on appropriate uses for ClinicalConnect, develops and maintains provincial and program policies and procedures and addresses privacy related questions and concerns. Janet Daglish MBA, CMC, PMP National Director, Business Development and Government Relations, Bayshore HealthCare Janet has over 20 years business development and project management experience and has worked for Bayshore since Janet s role oversees business development, project implementations, government relations and establishes strong ties with industry stakeholders. Her work includes program and business development, including bundled funding models, integrated care solutions and delivery networks, digital health and information technology, quality and service excellence, innovation, wellness and coaching programs. Deb Mulholland Vice President, Global Services, CellTrak Technologies Inc. Deb Mulholland brings senior management experience from positions including government, non-profit and private organizations. In her role as Vice President of Care Delivery Performance at CellTrak, Deb focuses on creating partnerships and supporting organizations in the achievement of systems integration and leveraging technology to realize improved organizational and client outcomes. 6

8 8 Panel - Dementia Care - Better at Home Chris Dennis CEO, Alzheimer Society of Ontario Chris Dennis joined the Alzheimer Society of Ontario (ASO) in June, Prior to this, he had a long, multifaceted career in the health-care sector, focussed on strategy development, organizational design, team building, financial management, sales acceleration and process excellence. His priorities since joining ASO have included working with the Ontario Government to establish an Ontario Dementia Strategy and the establishment of a more streamlined and efficient organizational structure for ASO and its 29 Member Alzheimer Societies. Faryar Foroodi Owner, Home Instead Senior Care, Richmond Hill & Vaughan For over ten years, Faryar has worked in the healthcare industry as a breast cancer researcher and pharmaceutical marketing professional, as well as a volunteer in numerous long-term care facilities. He has dedicated himself and his business to providing compassionate home care services, educating the community, and enhancing the lives of senior citizens and their families. Judy Richards Family Caregiver Judy is a daughter who has the privilege to take the lead and facilitate care for her mom as she moves along her dementia journey. Denyse Lynch Family Caregiver Denyse began caregiving for her much loved Dad in 1999 in her home, leaving her own career behind resulting in financial instability. There were many health care professionals along the way that provided welcomed expertise and compassion in her journey navigating health system elements. Since her father s passing in 2014, Denyse has become a strong advocate for improvements in the health care system to benefit other caregiver s experiences. Panel - Workforce Planning to Enable Better Home Care in Ontario Denise Cole Assistant Deputy Minister, Health Workforce Planning and Regulatory Affairs Division, Ministry of Health and Long-Term Care Denise Allyson Cole has been involved in formulating public policy for more than three decades and has worked in senior positions within Canada s federal, provincial and municipal levels of government as well as in the private sector. She is currently the Assistant Deputy Minister responsible for health workforce planning and regulatory affairs in the Ontario Ministry of Health and Long-Term Care. Sarah Palubeski Director of Human Resources, CarePartners Sarah contributes over 15 years of experience in the HR profession and she is a proud alumni of the University of Waterloo, Conestoga College and York University. She is a designated Certified Human Resources Leader. Although Sarah is proud of her academic awards, her greatest life lessons and achievements have come from her family: Mike, Chloe, Brooklyn and Max. Erin Kuzz Partner, Sherrard Kuzz LLP As one of the founding members of Sherrard Kuzz in Toronto, Canada Erin represents employers in all areas of employment and labour law. Erin has been recognized by Lexpert, Chambers Global, Who s Who Legal and Best Lawyers as one of Canada s leading employment and labour lawyers. Having practised labour and employment law for more than 20 years, she has earned a reputation for practical, results-oriented advice.

9 H o m e C a r e O n t a r i o S y m p o s i u m Closing Keynote Reflections on the Audit Process of Home and Community Care Bonnie Lysyk MBA, CPA, CA, LPA Auditor General of Ontario Bonnie Lysyk became the 13th Auditor General of Ontario on September 3, Ms. Lysyk has held senior positions in both the private and public sectors. She served as the Deputy Auditor General and Chief Operating Officer for the Office of the Auditor General of Manitoba and, most recently, as Provincial Auditor of Saskatchewan. She also held a variety of senior roles at Manitoba Hydro and served as Chief Audit Executive of the Manitoba Liquor Control Commission. A native of Winnipeg, Ms. Lysyk graduated from the University of Manitoba with a Bachelor of Administrative Studies (Honours) degree and subsequently obtained her designation as a Chartered Accountant while working with Coopers & Lybrand (now PricewaterhouseCoopers). She also has a Master s in Business Administration and is a Certified Internal Auditor. Ms. Lysyk is currently a member of the Canadian Council of Legislative Auditors and sits on the Committee of Professional Conduct of the Canadian Institute of Actuaries and on the Board of the CCAF-FCVI Inc. #symp2017 9
