Business arising from previous minutes 3.1 Contact list Suzanne is planning to update the WA Diabetes IG dietitian contact list to include more information including place of work and contact details. Members happy to have details circulated provided their details (name, site, ) 3.2 Meeting minutes Meeting minutes were distributed on the DAA DIG distribution list. This Continue to use this created more interest and increased number attending. 3.3 Speaker at NOV meeting Jill Sherriff had been invited to do a presentation and is available for our November meeting. Jill Usually provides an overview of recent topics presented at conferences. platform Jill will provide a list of current topics for the DIG to select from closer to the date. 2 3.4 PD at meetings PD is covered in other IG meetings. The members present discussed presenting ideas about format of meeting and PD as well as topics to be covered case studies/ articles/ conference papers/ media controversies/ frequency - each meeting or alternate 4. Correspondence 4.1 Correspondence No new business discussed. 5. ADEA Report 5.1 ADEA WA meeting Meeting next week. 6. DAA WA Report 6.1 PD Patricia Marshall 6.2 Diabetes and endocrinology network Sophie / Denise State conference 2016 to be hosted in New Norcia Fri/Sat 5/6 May in New Norcia Scientific meeting before national conference th August 2015 in Adelaide 4 th Sept- Agriculture and food event where the impact of big business on food supply can be explored. There will be representatives from Farmers organisation consumer groups- Simone P food regulation- Dt F&V producers were less keen to be represented 30 Oct- Childhood obesity DAA president visit and sundowner. Nov- Annual branch meeting- aiming for a social component This is a networking groups consisting of DE, Dt, WACHS, endocrinologist and other health professionals in diabetes care Diabetes standards still at policy level discussions have been able to provide feedback on draft standards Networks website contains list of diabetes service delivery across state Quarterly meetings may be the new meeting timetable as the network undergoes structural change Members to send ideas through for discussion when planning format for 2016 DIG meetings.

3 7. Diabetes WA Report 7.1 Position on agenda To be included in the worksite updates Removed from agenda and added as sub category of worksite reports 8. Health Service / Worksite Updates 8.1 Diabetes WA DAPHNE (T1D) - 2 reps went to Melbourne to conduct training. This may be presented as either 5 consecutive days or once a week for 5 weeks to increase reach of the program. DESMOND (T2D)- self-management group education style. Increase the number of health professionals undergoing training, Australia wide (NSW-18) Vic (October) Perth and NZ. Sheryl conducting the training, which can be undertaken by people who are not DE if they are interested. Lay trainers can be other health professional or people living with T2D. Individual appointments; being railed in Subi and Mt Lawley (pilot) with cost recovery costing as Diabetes Australia a not for profit organisation. Meal smart- new alternative to cook smart to promote behaviour change- (portions, energy density, meal planning ) in a group setting using DESMOND principles. Feedback so far has been good and the health literacy of the group impact on feedback. Midland having their first one with 14 participants 11 th Aug. 8.2 Bethanie Olwyn Flay 8.4 Private practice Olwyn (OF) and Jessica (JB) GP s PD on DESMOND- conducting presentation at practices, which are well received to enhance referrals Health coach delivery community services to HBF members for care direction. Allied health in the team include dietitian (2), physio (1), OT (3), nurse (1) home visits or telephone after an initial face to face. Direction to other programs in the community is provided if needed. General information is provided and referral to other professionals for complex cases (eg DESMOND). Soon to expand to Bunbury, Geraldton and Albany. OF- Perth Diabetes Care Multi discipline team EP, DT, Podiatrist with the potential for consultant endocrinologists- (Teresa involved). It is attempting to replicate the public diabetes clinic where team care is all in one spot for the patient. JB- Essence of Eating : Moved back to Perth form Melbourne and looking to set up private practise in Perth. Compile a list of DT who are DE or T2D specialists to be able to refer patients to when encountered them in group sessions or through telehealth (DIAL or COACH)

(top and intermediate hospital, 12 month waiting period, and 2 years between health support programs)- need to manage member expectations. Telephone based program to enhance self management in T2D, rolled out for 12 months now. Has been part of HBF s offering for CVD secondary prevention since Understand and achieve the modifiable risk factors (HbA1c, LDL chol, BP, kidney health) Encourage you to adopt the recommended lifestyle goals- PA, nutrition, Alcohol and smoking Gain skills and knowledge to effectively monitor and manage your health- using motivational interviewing techniques All clinical data is kept separate within the HBF health insurance business Amalgamation of PCEMML + PSCML + goldfields Midwest ML- these are still running some program independently in the transition phase. As of the 30 th Sept there will be no geographical boundaries for programs PCEMML- change for life still has 6-12months funding. Wt management program along with Metro healthy lifestyles. Requires GP referral as eligibility is T2D, those at risk of T2D or at risk of CVD. PSCML- living well transition program for those post PMH BAML= Arche PNML= Black Swan Mel team leader, continuing the Metro healthy lifestyle program Chronic disease mapping- reconfiguring for service delivery post hospital discharge. Looking at endocrinologist, clinics with teams including endocrinologist, DE and DT. (Dr Chandran) Allocation of post hospital services between RPH and FSH needs clarification as it is messy at the moment. Bentley diabetes services in the future are uncertain. GDM- 2 groups a week as numbers are increasing. Adopting the new diagnosis standards (Sept 1) DE online course (4 units over up to 4 years) + 2 weeks placement is underway with 49 applicants for 25 places. Allocation was by eligibility for credentialing (nurses, Dt, physio, pharmacy, already working in diabetes services) Reaccreditation course being planned Aboriginal DE advisors are available to assist aboriginal health workers DWA Study day- this is for enhancing diabetes education for health professionals who wish to up skill in diabetes care as an option if unable to enrol in the DE course.. Covers medications; insulin devices, health Janelle: Provide information to DWA for the DIAL info pack inclusion

5 8.9 North Metro Ambulatory Care Service Denise 9. New Business 9.2 Federal Government changes to NDSS professional updates. Pharmacist are generally really keen to be DE- specialist courses can be developed. Business as usual. No update Product perspective to the pharmacy guild but there is no change planned for service delivery. 9.3 Professional development Agreed to place this on agenda for 2016 meetings as a means of sharing expertise in the room, as well as guest speakers on occasion.
