2 FINANCIAL MATTERS We realize the need for a clear understanding between patient and doctor concerning financial arrangements for the provision of your medical needs. Our commitment is to provide the very best health care services for our patients while recognizing the need to exercise judgment and discretion in limiting services to only those necessary for each patient. We ask that you come prepared to pay our charges at the time services are rendered as the expense involved with monthly billing has a significant impact on our overhead cost and takes our staff away from their primary responsibility of providing patient care. We will file claims with those plans with which we participate. These include Anthem Blue Cross, Aetna and Medicare. We will give you an itemized bill for each visit so that it will be a simple matter for you to file your insurance claims for other plans. We reserve the right to submit a charge for additional services provided at your request, such as filling our various insurance and medical forms, writing medical letters, etc. We will gladly copy records at your request but will charge a fee for this service to defray our costs. As an appointment time is blocked uniquely for you, we will need to charge a fee for missed appointments unless you notify us within 24 hours. We do have an after-hours cancellation line on our phone system that you may use. Please realize that our services are provided directly to you, not to your insurance company. You are ultimately responsible for paying charges incurred through our office.

3 We appreciate the confidence that you have shown by selecting us to participate in your medical care. Dr. McClanahan is board-certified in the specialties of Diabetes, Endocrinology, Metabolism and Internal Medicine. He is a member of the Professional section of the American Diabetes Association, the Endocrine Society and several other professional organizations and is a Fellow of the American College of Endocrinologists. He has collaborated with numerous nationally and internationally known physicians and researchers and has had works published in several national professional medical journals and textbooks The purpose of this pamphlet is to acquaint you with the general office policies and specific scheduling practices of D.A.T.A. So that you can help us provide the best possible medical care for you and our other patients. PRACTICE ORIENTATION Because of Dr. McClanahan s additional training and special interest in the subspecialties of Diabetes, Endocrinology and Metabolism, our practice is concentrated in the area of hormonal and metabolic disorders. These would include thyroid, adrenal and pituitary disease, diabetes, osteoporosis, hirsuitism and polycystic ovary syndrome. We may initiate evaluation and treatment of other problems identified in the course of your evaluation but the ultimate responsibility for these non- endocrine conditions will generally be deferred to your primary care physician or to a specialist in the area concerned. APPOINTMENTS Our office is generally open from 8am-4pm Monday-Thursday and 8am-Noon on Friday. We make every effort to stay on schedule, but unanticipated emergencies or patients with unusually

4 Complex problems may cause unavoidable delays. When these occur, we appreciate your patience and understanding. We do realize that your time is valuable and if you are unable to wait, we will be happy to reschedule your appointment to a more convenient time. If your schedule is particularly tight on the day of your appointment, please call before you leave to verify the time that you may reasonably expect to be seen. INITIAL EVALUATION Please arrive at least 20 minutes before your scheduled appointment time since a permanent record will need to be created and you will need to fill out various forms prior to your evaluation. It is requested that your bring all of your medications or a complete list of them so that we may minimize the risk of medication errors or interactions. Also please bring any medical records that pertain to the problem for which you are being seen. We reserve the right not to reschedule a missed initial appointment. LABORATORY & OTHER DIAGNOSTIC TEST RESULTS We believe strongly that your deserve to hear and understand the results of any testing that is performed or ordered by our office. Abnormal test results are called to you when they become available. Normal results will either be called to you or be made available to you by our automated system unless we discuss otherwise. Because of the specialized nature of lab tests ordered by our office, some of them may not be completed for at least one week.

5 EMERGENCIES If you have an acute medical problem that you believe is directly related to a condition for which we are treating, please call and a nurse will be able to advise you when you may be seen in our office. At times if the nature of your complaint is likely to require facilities not available in our office a referral straight to the emergency room may be recommended. If your problems occurs at a time when our office is called and cannot wait until the next working day for evaluation, it is recommended that you call 911 to be evaluated in the emergency room. TELEPHONE CALLS Questions regarding treatment, billing, insurance, etc. may be called to our office. We have an automated system to speed your access to us. We take pride in the quality of our staff and they can answer many of your questions immediately. Others may need to be discussed with the physician before a final recommendation can be made. Telephone requests for medication refills can be overwhelming to a medical practice. Because of this we request that you review your need for refills prior to your appointments to minimize the drain on our phone resources. However, if you find yourself running out of medications, please call us or have your pharmacy contact us and we will send an electronic prescription.

6 GETTING TO KNOW US... Dr. Mark A McClanahan was born and raised in Abingdon, VA. He is a graduate of the University of Richmond and the Medical College of Virginia (MCV). After his internship and residency at MCV, he completed an advanced fellowship in endocrinology, diabetes and metabolism. He is a Diplomat of the American Board of Internal Medicine and a Charter Fellow of the American College of Endocrinology. He is an Adjunct Clinical Professor for the James Madison University PA program. He resides in Spotsylvania with his wife, Irene and their two sons, Alex and Jonathan. Scott Urquhart, Certified Physician Assistant was born and raised in Woodbridge, VA. He joined our practice in He is a graduate of George Mason University and serves as a Clinical Instructor for the University PA program. Scott is an Adjunct Clinical Professor for the James Madison University PA program and is the current President of the American Society of Endocrine PA s. He resides in Spotsylvania with his wife, Jodi and their twins, Noah and Carley. CLOSING We feel that the patient-physician relationship is unique, sacred and confidential.
