UPC members/ Research subgroup UPC Summit poster sizes: 36 x 48 tri-fold; limited ability to display roll-up posters on the walls. Posters follow IOWA model. Research Subgroup to evaluate posters at the UPC Summit May 9 th, Magnet Rounds Tool (15 min) Access to Hardcopy Nursing Journals (5 min) Sam/Neil Review finalized answers based on feedback from peers and current structures and processes in place. Each unit/dpt should have access to one peerreviewed journal as a tool to guide evidencedbased practices and transfer of knowledge to improve patient care and nursing practices. Really need to make connection to raw data: narratives are great, but it is important to find ways to connect the responses to data on your unit (low fall rates, low rates of infection, etc.). List of approved journals as of 03/18/2014 Have CNS send request for journal to Minda Miyamoto: mmiyamoto@mednet.ucla.edu Council members Minda has sent confirmation Council members Transferor of Knowledge MyUCLAHealth changes: Went live in September. After March 24 th, adult patients will continue to see their access code on the AVS; also proxy access instructions will be printed on AVS. Care Connect Sub-committee (included in the Break-out Technology subgroup) Patty Sheehan / Diedre Keeves This section will now be part of the Technology Subgroup CareConnect updates: Beacon Go-Live; MyUCLAHealth in-patient portal Idea of feedback box on each unit, integrate CC into UPC meetings? Adolescent patients (12-17) do not have access to myuclahealth. Pediatric patients (<12 yrs old) will have proxy access instructions printed on the AVS so their parents are able to access myuclahealth. Access code on MyUCLAHealth now will be larger font and bolded. New letter print option will be available for inpatient nurses and ACP/CCP users. These letters will give directions to the patients and family members for how to go about getting proxy access and activating their myuclahealth account while in the hospital. Proxy access: family has to have their own myuclahealth account before they can set up proxy access to a family members account.
4 Letter print workflow: Letters (under more activities section in care-connect; will be on RN/CCP/ACP template), clock MyChart status on upper right corner to launch letter activity. Search and select letter: Use IP Proxy Letter UCLA for pediatric patients and MyUCLAhealth activation letter for adult patients. Can download myuclahealth app on mobile device and get their results while an inpatient. Care Connect Website: Search functionality is now updated. Lower left hand side. Brainstorm on how all these updates impact patient safety and patient satisfaction. My story = landing page; is something that separates UCLA. Find ways to pull patient into technology. Find a way to show how Magnet has made an improvement in a process in nursing care. Scientist Breakout Session Sub-Groups: -Research -Technology -Innovation (40 working 10 reporting out) Groups will continue working on: Defining purpose, identifying measureable goals/outcomes (initiate EBP Focus PDCA to guide the group). Needs assessment i. We need to develop a strategic way of disseminating information to the staff ii. Need to determine what information is the priority 1. CPC alerts, journal articles, core measures, hospital policies, weekly updates, etc. iii. When problems are identified on the unit, we need to ensure that our staff know how to go about looking for evidence to support their innovation evidence to support their innovation b. Barriers i. Lack of downtime during the shift ii. NK dissemination often seen as a task, we need to find a way to change the culture around this and make it a part of nurses daily workflow Discussion: ED clinical summary and handover report too generic, requiring optimization and ticket to be submitted.
6 and implementation of work flow that resulted in operational improvement, waste reduction, or clinical efficiency. Safety issues in communication because reports are not specific enough.
