2 Monday, February 22, 2016 Call to Order/Review agenda Crystal Stordahl Meeting was called to order. Introductions were made around the table. Crystal asked that everyone review the agenda. 3 echam roduct Updates User Management System: Speed has diminished again and we ve asked Dakota to evaluate to increase the speed again. However this does not affect the end user in any way. DF: Finishing fixing hyperlinking to ensure hyperlinks (page numbers in DF) go to the exact right location and then the DF will be released for mass use. o CV finished o Ref/roc and Medicine in progress Website: Have been experiencing issues with speed and backbutton on both access and QA website. Back button issues and speed reported at the last meeting are mostly resolved, back button fails infrequently. iad app: Tested v. 35 and returned issues requiring resolution. App is functioning well except bookmarking issue that Dakota is working to resolve. Anticipate releasing for external testing as soon as this issue is resolved. DMS: Working well at this time. ARC/RAC Report Group See attached ARC and RAC Report. ARC: Unit 9a/9b-no action needed. The CHA Education rogram Evaluation Checklist had a typo in review time. It has been corrected and will be reposted on the akchap.org website. Motion: Karen S. motioned to approve changes to ost-session ractice Checklist. assed. receptorship Critical Skills List Add language to other regional skills we have been taught not required for preceptorship, follow regional guidelines. Reference all the lab skills that come with basic training into preceptorship skills. atient types at least 15 patients and has to be accompanied by 15 encounters. Community Health Aide rogram Certification Board Doug Finney CHACB approved the request to change 3 words in the CHACB Standards on the first reading; it has to go back to the Board for a second reading. o Section b(1) rimary rovider from: the primary provider must initiate the history and examination to: the primary provider must perform the history and examination. o Section (3) Session II: change in ratio of patient type from: primary provider in at least 6 patient encounters at least 4 encounters as active participants to: primary provider in at least 4 patients at least 6 encounters as active participants. 3

4 There is a vacancy on CHACB. Bill S. was elected to that seat; it still has to be approved. CHACB has chosen to uphold a priority ranking for admission to training centers. Letters will be sent out to training centers. Health Aide Emergency Response Tonya James Tonya proposes to revise the CHAM chapter, What to do in an Emergency with an oversight committee. She will develop an emergency preparedness course to begin in April. Eventually, she ll be developing response tools for the regions that want them. Strategic lanning See attached Strategic lanning. Crystal Stordahl CHA Cerner Sarah Smith ANTHC Cerner has a strict change management policy. They only make changes on Wednesday s. They have an approval process. Sarah will resize text boxes and check for consistency with capitalizations, etc. Eye Exam - Sarah will ask eye doctor which position to place right or left eye on form. Change #1: CHAM Respiratory/CHAM Resp. HI Cont. - The question is Active and playful, or acting tired or fussy? The current response it only Other. NSHC has requested the change to have responses of Active and playful, Tired, Fussy and Other. Change #2: CHAM Well Child 2 wk-5 yr/wcc hysical Exam and WCC hysical Exam cont. Currently these 2 sections are out of order and the responses are very generic. The change is that most of the questions response boxes be updated with more relevant choices. The change would also arrange the body sections to be in a more appropriate order. Change #3: CHAM Eye Exam/Eye Exam-Add 2 options to the sections highlighted below. The 2 options to add are Good Reaction to Light and Equal Reaction to Light. Tuesday, February 24, 2016 Call to Order/Announcements Crystal Stordahl Meeting was called to order. The group will continue to work on Strategic lanning this afternoon. AFCHAN Sarah Freeman AFCHAN presented on the different carts available. There was a request for the list of the 5 carts with advantages/disadvantages. AFCHAN will come back with the list at the May meeting. 4

5 Forum & CHA/ Awards Rebecca azdernik Registration is open; the link is on the akchap.org website. The registration fee is $300 before 4/8, $400 after. We are using the event app again this year. We need the attendees address so they can log in to event app. Room block is Springhill Suites for $109/night plus tax. Shining Star nominations are due 3/21; right now we ve received only one. Immunization Update Tania Smallenburg & Rebekah Clark Temperatures in all permanent storage units storing state-supplied vaccine must be monitored using the calibrated LogTag TRED 30-7 and interface cradle provided by the Alaska Immunization rogram Required to have a back-up calibrated continuous monitoring device on hand in case of emergency or vaccine transport All vaccine order transports from the tribal hub to a village will need to contain a LogTag temperature monitoring device with a probe in a plastic vial of glass beads The Alaska Immunization rogram will supply LogTags for all permanent storage units, as well as additional LogTags for transport from tribal hub to villages ($150/LogTag) The new requirements will be in place no later than August 1, 2016 The Alaska Immunization rogram will provide tribal hub facilities with: o One-on-one trainings on how to follow the new guidelines when transporting o Customizable LogTag instructions for training village staff o Instructions on how to pack vaccine transport according to State and Federal guidelines o Develop individualized tribal hub Implementation lans in collaboration with ANTHC s Immunization Coordinators Billable Codes Motion: Sue S. motioned to forgo the round robin reports, have the billable codes discussion then adjourn. 6 Report to CHA Directors & ARC EMS Training Committee 1/27/16-1/29/16 Submitted by Jean Rounds-Riley, A-C 2/22/16 State EMS Office: NOTE: lease see the white paper prepared by the State Section of Emergency rograms. Available on The State EMS Office (via Mark Miller Unit Manager) made a proposal to the Alaska Council on EMS (ACEMS) in Juneau in Feb. to use National Registry EMT (NREMT) exams as the sole source of testing. This was recommended in the 2014 National Highway Traffic Safety Administration (NHTSA) Re-assessment of the Alaska EMS system (Available at ). This is being recommended to improve inter-state consistency, and based on the possibility that future federal funding and recognition will depend on adapting uniform national guidelines and certifications. The recently revised State of Alaska EMT-1 cognitive exam may have been compromised and has not been validated. Given the current Alaska State budget situation, funding may not be available for the Training Committee to develop and revise Alaska-specific testing. Currently the NREMT is administered and offered only via earson Vue computer sites. Concerns expressed by the Training Committee are costs to volunteers, availability of test opportunities (location, hours, and limits on simultaneous testing), as well a test language/terminology and format. To all EMS ersonnel in Alaska; Feb 12, 2016 EMS roviders, EMS Instructors; EMS Medical Directors; The EMS Unit at the request of the Alaska Council of EMS has prepared a survey regarding Alaska EMS Testing and Certification. The survey questions should take no more than 5 minutes to complete with opportunties to add comments. Emergency Medical Services is a rapidly evolving field and the many people who support it want to provide the best services possible. Contact Mark Miller at mark.miller@alaska.gov with any survey issues. EMS providers of all levels, Medical Directors, Instructors and EMS Administrators are encouraged to fill this survey out just once using your primary role in the EMS system. 7 NREMT does not have an exam which corresponds to Alaska EMT-2 or EMT-3. The Advanced EMT (AEMT) national training course is much longer than the EMT-2 or EMT-3 courses, which may make it difficult to implement in Alaska. There was discussion about the needs for advanced EMS providers in Alaska. NOTE: lease contact me, your regional EMS Training Committee representative or Mark Miller at with comments about needs in your regions villages for advanced EMS skills and how these might be met. Regulations changes: Another set of Regulation changes is in review within the department DHSS, but not yet ready for public comment. EMS Training and CME: The State EMS Unit and Training Committee made recommendations for clarifications of the sychomotor Examination, so that all test sites would be more uniform. State Training Committee is working on writing objectives for the EMT-1 course, which will follow the National EMS Guidelines, and provide more guidance for teaching. Anticipating a change in regulations, Training Committee is working on revising the process to become an EMS teacher. This would include multiple levels of teacher/instructors, such as an instructor level who would be able to check off skills for primary and recertification and assist with classes, but not be the primary instructor. SREMSC started free on-line webinar synchronous CME beginning in Jan. 13, The 2nd Wednesday of the month 6pm-9pm. It will be a go-to-meeting format. 8 Attendance COMMUNITY HEALTH AIDE ROGRAM Academic Review Committee Report to the CHA Directors (CDs) resented to CD Chair by Dan Thomas, ARC Chair, 12/12/15 for ARC meeting December 9-11, 2015 Topic Action by ARC Requested Action by CDs See ARC minutes. Election of Officers New Chair: Al Shear New Vice Chair: Stephannie Christian Secretary: Marlene Katcheak 2016 meeting dates and times The next CHA meetings will be the week of 2/22-2/26. Tentative RAC and ARC meetings for February 2016: CDs: 2/22-2/23 CHA leadership: 2/24 8:30-10am RAC: 2/24 10am-12pm Training Center Coordinators: 2/ pm ARC: 2/ pm, 2/25-2/26 CHAM/Curriculum alignment for body system units 1. Work accomplished this meeting: a. The final drafts of Units 9a/9b (Mouth and Teeth) were reviewed and approved. After the meeting, it was noted that CHACB has not yet approved the removal of flossing from skills practice in Session II, so this was added back in as a Level 2 skill. b. The final drafts of Unit 5c11 (test for GC/chlamydia),14a (Male), 14b Female were reviewed and approved. c. Work on Units 25d (STI) and 20h (Sexual Abuse) was completed and approved. d. Work on Unit 21a Family lanning was continued. 1. Linda will do some final formatting to the approved units and forward them to Torie. 2. Torie will draft a cover letter to accompany the new units for distribution to the training centers, to be signed by CD and ARC Chairs. The training centers would like them ASA. 3. ARC Chair will ask CD Chair what review of the units CDs would like to do before they go out to the TCs. 4. Linda will make the changes to Unit 25d and send to Dan for final proofing. 5. Linda will reformat Unit 21a and bring it and Unit 19 to the next ARC Curriculum teleconference for more work. 6. Stephannie will work with Jenny to add student-friendly language, then to Linda for lease advise ARC if you would like to review of these units before they are implemented. 1.

9 e. Work on Unit 19 (Skin) was started. f. Jenny Brown of TCC presented by teleconference her work on the emergency CHAM chapter and Curriculum units. ARC liked the degree of detail Jenny added to the Curriculum. formatting, then back to ARC for unit revision. Jean Rounds Riley will bring some of the changes to the ANTHC trauma surgeons for their input. With one exception, the opinion of ARC members is that emergency training should be left in Basic Training, or if done by field programs instead, it should be a requirement. resession Handbook Updating of other old CHA documents Math tool Stephannie described her ongoing work on revising the resession Handbook. 1. Training Center Coordinators meeting 1. We reviewed and recommended approval of a request for CE hours for re-entry clinical evaluations. 2. We plan to develop a sample schedule for Re-entry as CE. 3. 10 RAC Report to CHA Directors December 9, 2015 No Action is requested from CD, from this RAC meeting. Membership: Approval for RAC reappointment of Doug Finney and Cyndy Langmade was received from CD. Faith Walsh is the new TC representative from TCC and attended. 1. New Business 6. Documents a. NSHC Site Review is scheduled for 4/12- to 4/14/16, with travel days 4/11-4/15/16. Site review Team Members: Carolyn Craig: Leader, Al Shear: Co-Leader, Elaine hillips: member, Faith Walsh: 4 th member paying her own way, with backups Elsie Dexter and Cyndy Langmade. The Nome RAC Notebook will be finished and distributed by February 1, The extra team member will receive orientation. b. New TCC Training Center Site Review is scheduled for 2/9/16 to 2/11/16, with travel days 2/8 to 2/12/2016. The RAC notebook will be finished and distributed January 1, Tasks for reviewing notebook contents were divided and assigned for be review by Al Shear: Leader, Carolyn Craig: Co-Leader, and Dan Thomas, member, after 1/1/16. Elsie Dexter hopes to attend as the 4 th member, for orientation. Review Committee members will have access to the resumes and licenses of faculty on a secure website, rather than credentials copied into the RAC notebook, to be reviewed online then deleted when complete. Torie will get a password for members who need access for review. The new TCC Training Center is getting ready for SI and SII 2/16. c. RAC chair will send letters to CHA CB, team members, TCC and NSHC, and CHA Directors re: site review plans for the next year, team members, where, and when, and also scan Site Review rep lan documents to members. d. RAC chair is ing the Annual Self Evaluation Checklist to ANTHC and YKHC for 2015, and will send out thank-you letters to supervisors of RAC members, cc to each RAC member. a. Reviewed documents since last meeting: CD accepted and approved the action of changes to the CB Standards, to be passed along to CHACB. b. Review/Approve minutes Group The group reviewed the AACHAD February 22-23, 2016 minutes. Motion: Bill S. motioned to approve the minutes. Herman G. 2 nd. assed. AFHCAN Sarah Freeman Design of cart: they normally hide all the cables so users can t unplug or change something. They have an issue when they change the cameras. They will put a USB hub in one of the drawers of the cart. They are talking to Clinical Directors about what version the organizations are using. 17 There was a State Medicaid meeting last week. Currently when a tribal member receives care at a tribally operated facility, it is 100% FMA. If they go to a non-tribal facility, it is a 50% match and the State pays for 50%. The State would like more to be 100% FMA, such as referrals. One of the tribes priorities is to get the encounter rate for CHAs and BHAs. The next meeting is June th. A State lan Amendment needs to be submitted to CMS to request an encounter rate. Currently we get 85% of fee for service. Coping Skills Victorie Heart The Coping Skills course is scheduled for September 27-29, ANTHC will take lead in planning. The next planning meeting will be on May 24 th. Committee members are Victorie Heart, Herman Geffe, Al Shear, Jo Miller, Crystal Stordahl, Bill Schreiner, aula Ciniero, Sue Steward, Laura Baez, Tina Woods, Dr. Michelle Hensel, Andrea Thomas, and Lucy Tennyson. Well Child form Sarah will send the updated form to the approval group. Tuesday, May 10, 2016 Call to Order/Announcements Meeting was called to order. UAF Crystal Stordahl Linda Curda Registration procedure list process for billing/grading. They are looking at how they can improve this. The cost is $75, and right now the THO pays $30 and balance is $45. When a payment is not received, the student often gets included in the billing cycle. If an organization chooses not to pay, let the CHA know and UAF. Linda will be invited to the CHA EXO meeting, either in June or July. State of Alaska Renee Gayhart Last week, there was a week-long meeting between the State and the Tribes. The Tribes are requesting that the State submit a State lan Amendment to fully compensate CHAs I through ractitioner, and BHAs I through ractitioner. And to come up with a single set encounter rate for those services. There is further discussion with organizations to come up with number of 4

18 encounters and utilization to set the encounter rate. The tribes also brought up CHAs to potentially take on additional roles to take over palliative care and elder services. Some tribes want to take over transportation for their beneficiaries. RAC Recommendation Letter Group CHACB is writing a letter to training centers regarding statewide priority for training. RAC review team came to TCC and NSHC. RAC constructed an issue paper regarding the standard. CHA Directors need to take action. RAC RECOMMENDATION #1: The standard either be revised or deleted. Suggested language for a revision would be: CHA Training Centers supported through regional tribal organizations and that organization s funding will announce through a statewide venue, any training slots available to out-of-region students and acceptance for those slots will based on statewide training priorities. RAC RECOMMENDATION #2: Revision to the CHA Education rogram Evaluation Checklist to reflect edits (or deletion) of standard Until such time the standard is clarified/revised/deleted the RAC site review teams should not score this item as it is currently written on the evaluation checklist. AACHAD worked on recommendations on how to change the riority List. The intention is to bring it to THOs before bringing it to CHACB. Trying to keep EMS in the public (and legislative) eye, with press releases etc. State office had several webinars in March and issued surveys to keep instructors informed of changes in EMS. EMS 2021: The State Office will be working with Regional Directors and ACEMS on a 5 year plan. State EMS Training Committee will be included in training/education and testing issues at the Sept meeting in Anchorage. Village Health Aides are specifically listed among the health care providers (Sec. 5.AS ). The Section of Emergency rograms is working on clarifying training and authorizations to allow these health care providers to dispense and administer opioid overdose drugs, and to train and distribute these to family/associates of those using opioids. The State is applying for a federal grant, and plans to utilize the only FDA-approved nasal delivery device (Naloxone Nasal Spray 0.4mg) 2 bottle kits at +/- $70/kit. Considering moving recertification dates to spread out the work load throughout the year. Ron Quinsey is interested in having EMS included in the newly renovated State Museum. 20 State Training Committee is working on writing objectives for the EMT-1 course, which will follow the National EMS Guidelines, and provide more guidance for teaching and testing. Anticipate having these ready before the end of August IREMS: Symposium went well, included section especially for CHAs including suturing, splinting, spinal protection. NSB Will start synchronous blended ETT course Barrow/villages using Bridge-it software and smart boards. They recently had an MOI course, and now have 6 new ETT instructors in the villages who will help with the skills portion of this course. Also increasing BLS instructors and classes in the villages. SREMSC started free on-line webinar synchronous CME beginning in Jan. 2016, the 2nd Wed. of the month 6pm-9pm. It is in a go-to-meeting format; going well. Staff presented at the CHA Forum, especially reviewing changes in EMS such as spinal protection. University of Alaska: aramedic program (Kenai?) will be doing more by distance this fall. Also looking at distance ETT and AEMT next year. They will include didactic materials with a weekly contact with instructor. Anticipate using Jones & Bartlett s Navigate-2 software (YKHC has looked at this a well); customizable materials, some with an audio component to enhance learning modalities. Election of Officers: Elaine hillips Chair, Cyndy Langmade Vice-Chair, Faith Walsh Secretary CHACB Business Doug Finney reported on the status of the request to change the CHACB Standards with regards to the rimary rovider definition and change to the ratio of rimary rovider to Active articipant pts in Session II. The first reading was accepted with the provision of being reformatted. A second reading is required, therefore the rewritten version will be submitted for the next CHACB meeting. New Business QA process and criteria for Session Training field staff, and policies for orientation of Session Training field staff: NSHC has policies for eer Review, Clinical teaching Evals, and Classroom teaching Reviews. Dan will forward current NSHC documents to Doug. These are implemented 4 times yearly. Doug will ask ANTHC for their current QA documents. Wording for Field Trainer Training and QA policies will be explored at the next RAC meeting. This QA is already happening in NSHC, Manillaq and BBAHC in the field. The Nome Site Review is planned for 4/12- to 4/14/16. Site review Team Members are Al Shear, Carolyn Craig, Elaine hillips, with backups Elsie Dexter and Cyndy Langmade. Faith Walsh is also attending. RAC Site Review The new TCC Training Center RAC Site Review February 8, 2016 was described as Fascinating with all new staff starting a TC for the first time. Dan, Cyndy, Carolyn and Elsie attended. The report was reviewed. TCC s first part of Session I review successfully passed the RAC review. The second part of TCC Session I RAC review will take place by distance after the completion of the session, by reviewing the paperwork. There is also a recommendation to observe Session SIV clinical separated into 2 sessions, July 11 to 22, then July 25 to August 5 th with 3 students in each group. Concerns arose about the following during the stie visit review: CHACB Standards and rocedures Sec Trainees Selection rocess. (b) Statewide riorities. The CHA/ Training Center must adhere to statewide placement priorities for training applicants for a limited number of training slots. Two points allotted as nonessential on the RAC checklist are being discussed, concerning the Statewide Selection rocess priorities for limited training slots. This Standard and the Statewide

22 riorities document were written long ago, do not reflect current statewide realities, and will be discussed at the next RAC meeting concerning wording and criteria for filling slots in TCs. Dan will write a statement about how slots are now being filled, using the statewide priorities criteria whenever slots are empty. Dan will write a summary of this first portion of the RAC review, including the ongoing offer for support to TCC, and a return is planned in July for observing Session IV. The final write up will be submitted to RAC, forwarded by Leah for review by members before the next RAC meeting. Unfinished Business to be covered at the next meeting: RAC Training Center Annual Self Evaluation Checklist for each Training Center not receiving a site review- paperwork was distributed. ANTHC and YKHC will exchange completed selfevaluation checklists with each other after completion. They will tally the most recent schedule for each Session, comparing subjects and hours to those listed in the Basic Training Curriculum. RAC will send out letters to Training Centers listing any items on the Self Evaluation Checklist requiring improvement. NSHC team will start looking at the Nome notebook which Dan sent out feb 1 st. Documents posted or sent since last meeting: Letters to CHA Directors re: site review plan for Annual Self Evaluation Checklist to ANTHC and YKHC. Thank-you letter to supervisors of RAC members, cc to RAC member sent. Dan added dates to RAC Orientation for New TC Coordinators and RAC members, posted. Next agenda: May 1.
