Kuopio University Hospital Regional collaboration in falls prevention - making a continuum Tarja Tervo-Heikkinen, Clinical Nurse Consultant, RN, PhD Arja Holopainen, RN, PhD, Research Director, Nursing Research Foundation Kaisa Haatainen, RN, Associate Professor, Patient Safety Manager Merja Miettinen, RN, CENO, Associate Professor

Above - Kuopio University Hospital (KUH) KUH regional responsibility area Below - Kuopio city seen from the Puijo tower Regional Responsibilities of the five University Hospitals in Finland

Speech in the background, slide 2 Kuopio University Hospital is responsible for specialized medical care for a total of more than 840,000 patients in Central and Eastern Finland. Patient questionnaires indicate that patients are very satisfied with the care they receive. Nursing science is stimulated by the existence of Finland's largest nursing teaching unit at the University of Eastern Finland. The Hospital is located in the Kuopio Science Park and is thus engaged in important collaboration with the Science Park's research centers and the University of Eastern Finland

What Happens in Finland? In 2012 the amount of hip fractures was 7806 In 2012 falls, slips and trips caused 112,5 treatment periods per citizen Over 65 years old the frequency was 177,1 treatment periods per citizen of the same age SOTKAnet Statistics and Indicator Bank ( National Institute for Health and Welfare ( Pajala

Speech in the background, slide 4 More and longer treatment periods and even deaths in Finland are consequences of accidental falls. Over treatment periods in hospitals are caused by the falls, slips and trips. The risk of falling accidents rises up linearly with age

What are the Costs of Falls? In 2012 in Finland occurred 7806 hip fractures to the citizens of over the age of 65 in 2-3 % of the hip fractures the reason is a fall The costs of hip fractures after fall were about 250 m in 2010 SOTKAnet Statistics and Indicator Bank ( Piirtola et al Gerontology 2012;58:

Speech in the background, slide 6 The estimated costs of accidental falls among 64 year old citizens were almost 40 million euros in Most of these costs were caused by treatment of hip fractures. The costs of hip fractures after fall have raised in ten years so that they were about 250 m in 2010 In addition, there is a lot of human suffering which we cannot evaluate in monetary terms. Piirtola et al

The Regional Fall Prevention Network (RFPN) the Goal To promote the awareness of fall prevention among patients and their families as well as social and health care personnel by organizing education and training by following falls in different organizations by promoting evidence based uniform practices by writing recommendations and guidelines 8

Speech in the background, slide 8 The Regional Fall Prevention network was established in The goal is to promote the awareness of fall prevention among patients and their families as well as social and health care personnel. The aim is to decrease human suffering and reduce costs by preventing the increase of falls

Multiprofessional RFPN Members are from Kuopio University Hospital other hospitals in the area public health care (Health Care Centers) academies (Savo Consortium for Education, Savonia University of Applied Sciences and University of Eastern Finland) 10

Speech in the background, slide 10 Those 23 members of the network are by profession for excmple nurse, nursing leader, doctor, pharmacist, nutrition therapist, physiotherapist, teacher, patient safety manager etc. We have members in many different organizations in regional responsibility area of Kuopio university hospital such as other hospitals, public health care health centers and educational organizations

Outcomes Well functioning Network Follow-up of the falls in the area Uniform instructions for fall prevention Regional education of health care personnel and students The use of Fall Risk Assessment Tool Checklist tasks after fall Keep Standing guide Process description 12

Speech in the background, slide 12 Well functioning network need highly dedicated members and it is my pleasure to say we have had them The outcomes of network are for example: - uniform instructions - regional education and training - the use of risk screening tool - an so on - Keep Standing guide is on process. This guide is aimed at not only the elderly but also at their loved ones and health care professionals in counseling support. - In this whole work we have good cooperation with The Nursing Research Foundation (Hotus) which is a non-profit research and development organization in Finland. - All of our material will be transferred to the nursing research foundation internet pages from where it is freely available to anyone

In the Near Future Implementing uniform practices in the area health care social care academies The evaluation of the actions 14

Speech in the background, slide 14 The near future the next year will be reserved for implementing uniform practices in the area and also planning the systematic evaluation of the actions

Making a continuum Our new hospital building will be introduced at the beginning of 2015

Speech in the background, slide 16 In the Future we can imagine that our patient comes to the hospital with information of her risk to fall. Also she already has knowledge of her risk and how she can reduce it by herself. She has the drug card with her and her medication has been checked in view of the risk of falls. Hospital staff is also aware of the risks and the use of the assessment tool is routine and they know how to react to the results of the tool. Also when patient leaves the hospital she will be given a Keep Standing guide if she does not have it already. She will also be given nursing summary from which she can find, among other things, the fall risk information. All social and health care staff knows what this information means and they also know what to do and they do it together with the patient or client whatever name we call her
