2 Dear Student, Welcome to the Parkland College Medical Assisting Certificate Program/s. The program will prepare you as a generalist in the area of the medical office. This handbook has been prepared to help you become familiar with the educational program, roles, responsibilities and rules, and the occupational outlook for a Medical Assistant student. It is the responsibility of each student to review this handbook and refer to it as needed during the progression through the program. The hand book may have some minor changes through- out the chosen program. The additions/changes will be shared with you each semester. The CB Profile and health records procedure is for those in the G.CMA.CER 12 month program. It includes the documentation of health records, proof of immunizations, TB 2-step skin test/chest x-ray, background check and drug screening for the G.MAS.CER and G. accelerated programs. When you receive an acceptance letter a packet of materials will be included for the CBC-health records process. It is advisable that each semester you meet with the Program Director/advising Faculty to keep on track with your course work. Enjoy the journey of establishing a new career in health care. 5 INTRODUCTION Purpose of Handbook The Medical Assisting Student Handbook has been prepared to assist you in your progression through the Medical Assisting Certificate Programs at Parkland College. A Medical Assisting Student needs to have a thorough understanding of the program- learning outcomes, policies, and procedures essential to the successful completion of the program. A Hard Copy of the handbook will be obtained from the Program Director. Original-2008, Portions of the handbook has been adopted from other health professions programs handbook. 6 Medical Assisting Certificate G.MAS-CER: Program Information Definition: The Medical Assisting Programs prepare students to act as a liaison between the physician and the patient. Medical assistants are skilled in both clinical and administrative areas. In the administrative area typical tasks include medical records management and EMR, awareness of insurance, billing and coding, and patient scheduling. Clinical tasks may include taking all or some of the following: vital signs, height/weight measurements, specimen collection EKGs, venipuncture, administering medications and immunizations, sterile instrumentation, and point of care testing. Medical assistants primarily work in outpatient settings but may work in hospitals, medical billing companies, or any place office skills and medical knowledge merge. Approval/Registry/Certification: The Medical Assisting Program-G.CMA. CER is approved by the American Registry of Medical Assistants, 61 Union Street, Suite 5, Westfield, MA Phone: 413/ Upon successful completion of the Medical Assisting program at Parkland College the student may apply for registry with the American Registry of Medical Assistants. Certification: The Medical Assisting (G.MAS.CER, G. CMA.CER, & G.) graduates may sit for the National Certification exam through NHA-National Healthcareer Association. 7 MISSION AND PURPOSE STATEMENT The Mission of the Medical Assisting programs at Parkland College is dedicated to providing a high-quality, balanced curriculum of general education, medical assistant practice in theory, and the administrative and clinical skills essential for the position of medical assistant. The Purpose of the Medical Assisting programs is to: Maintain a state-of-the-art program of learning that meets the needs of the students, the community, and health care facilities. Provide academic advising that will assist students in achieving their career goals. Develop a climate where students feel free to discuss ideas and concerns regarding social and professional issues. Offer learning opportunities that allow for demonstration of caring behaviors. Provide a nurturing, quality work environment that allows for free expression of ideas, implementation of a vision for the future, and pedagogical methods that will enhance student learning. Prepare safe, qualified medical assistant graduates who meet the requirements for beginning employment. The Mission and Purpose statements of the Medical Assisting programs are in support of the Parkland College Mission and Purposes and follow all federal and state laws and College policies. 7 P age

8 PHILOSOPHY The medical assistant programs at Parkland College believes that learning is a lifelong process that results in changes in thought processes, attitudes, values and behaviors. The medical assistant programs recognizes that students, like their clients, come from diverse and varied ethnic, cultural, religious and educational backgrounds. Students bring to the classroom different learning styles, personal goals, age representation, life styles, experiences, and needs. A non-threatening, supportive environment in which frequent feedback is provided is essential for optional learning. Learning progresses from the simple to the complex and involves the active participation of both the student and the instructor. The instructor serves as a mentor, role model, resource, coach and/or facilitator of learning. The student is a mentee, role apprentice, and consumer of educational resources and a manager of her/her own learning. 8 P age

9 ETHICS Students are expected to observe the standards of conduct established by the college and the clinical practice agency. Students will work within the scope of practice of a MOA/CMA/RMA. Students demonstrating unethical conduct (i.e., cheating, deception) related to examinations, class, laboratory assignments, or clinical laboratory practice will be subject to course faculty review to determine disciplinary action (see Academic Honesty Policy in the Parkland College Student Policies and Procedures Manual). Ten points total are awarded for the ethics criteria listed. Each work ethic is worth 1 point per day/week. Ethics for Evaluation in all HCS/MAS courses: o Appearance..uniform neat, clean; hair & beard groomed, no perfume, one stud earring, dress in labs are non- revealing. Student will adhere to the clinical facility s dress code. ID badge, second- hand watch, black pen and stethoscope are part of the uniform. Uniform is preferred for labs on campus. o Attendance.. 100% attendance is expected and encouraged. Punctuality is expected. On time. Notifies instructor for class/lab if going to be late or absent. Notifies both the instructor and clinical site of being late or absent. Consequence. Absences require notification of faculty and/or clinic manager if appropriate. Absent for more than three days may result in student not able to complete the required skills. Does not leave early from scheduled classes/labs/clinical. No call /no show will result in loss of ethics points. Unforeseen closure of Parkland College. Closure of Parkland College for any reason will not affect the student s grade. Make up of hours. Students are required to notify the faculty and or clinic manager for possible make up hours for approval. Not all courses have allotted make up time. The student is expected to come prepared to the following scheduled class /lab/or clinic. 9 P age

Does not engage in negative discussions and behaviors. o Character..Displays a high level of commitment to performing and completing work. Demonstrate trustworthiness and honesty. Notifies instructor of tardy or absence in advance if possible. o Communication..Listens attentively to others. Demonstrate appropriate nonverbal communication skills. Communicate accurate information to others in a professional and courteous manner. Demonstrates ethical behavior in all social media related to Parkland College. Students will not discuss their own physical experiences with the clients. o Cooperation..Conveys willingness to assist others. Work to resolve conflicts and to identify solutions that will benefit all parties. Follows the chain of command in resolving conflict. o Organization.. Organize work efficiently and effectively. Observe established policies on safety. Stay on task and utilize time constructively. Maintain equipment and supplies. o Productivity..Prioritize and manage time effectively. Demonstrate ability to adapt to change. Be prepared for class/lab/clinical. Contribute to class discussion, complete assignments, and be involved in lab activities. Begin and end work as expected using class/lab/clinical time appropriately. o Respect..Treat instructors, staff, clients and fellow students with respect, courtesy, and tact. Do not engage in harassment of any kind. Respect the rights of others. Demonstrate an understanding of the value of diversity in the classroom/lab/clinical setting through appropriate behavior and use of language. Cell phones may be in vibrate mode during lecture and lab for Public Safety access. Cell phone use including but not limited to texting messaging during lecture, lab or clinical is not allowed unless requested by the faculty for educational purposes. o Teamwork..Encourage and facilitate cooperation, pride, trust, and group identity. Foster commitment and team spirit. Recognize and use skills of members when completing assigned teamwork. 10 P age

11 SCOPE of PRACTICE CMA/RMA Student The utilization of medical assistants continues to increase and diversify. The scope of practice may vary from one institution to another. Questions concerning the scope of practice for the clinic assignment will need to be directed to the institution s human resource department and/or the Physician or Healthcare Provider. The American Association of Medical Assistants website provides a resource for questions regarding permissible scope of practice across the United States. Scope of Practice General List provided from AAMA and most states. In most states a medical assistant can: Perform clinical and administrative tasks to keep the workflow going, if supervised by a physician or other health care practitioner. Determine the acuity of a visit and the visit length for appointment scheduling purposes using an office protocol provided by the supervising physician. Measure and record vital sign. Record patient demographics and basic information about the presenting and previous condition. Use medical terminology and accepted charting abbreviations. Escort patients to the exam room and prepare them for an exam Use scientific methods to solve problems and choose a mathematical method or formula to solve problems. Convey clinical information on behalf of the physician Arrange examining-room instruments and equipment. Change wound dressings and obtain wound cultures Remove sutures or staples from superficial incisions or lacerations Operate diagnostic equipment but cannot interpret tests. Provide patient information and instructions. Provide a single dose of oral medication as ordered by the physician to a patient for immediate self-administration under observation. Administer medications topically, sublingually, vaginally, rectally and by injection. Perform CPR and render First Aid in an emergency Prepare patients for examination, including draping, shaving, and disinfecting treatment sites. Perform aseptic procedures such as wound care. 11 P age

12 Collect blood specimens via capillary and venipuncture technique. Obtain specimens by noninvasive techniques, such as wound cultures. Perform simple laboratory and screening test customarily performed STAT in a medical office, such as urinalysis. Administer different types of cryotherapy to reduce pain or swelling. Filing and bookkeeping Process insurance claims Transcribe medical dictation for medical records. Call in prescription orders or refills to the pharmacy, but only as ordered and approved by physician, nurse practitioner, or physician assistant. The sites used for this information warns that medical assistants CANNOT: Independently perform telephone triage (medical assistants are not legally authorized to interpret data or diagnose symptoms) Independently diagnose or treat patients. Independently prescribe medications Independently give out medication samples Independently refill prescription requests. Independently do triage Inject medications into a vein (most states) unless permitted by law. Start, flush, or discontinue IV s (most states) unless permitted by state law. Provide medical treatment, analyze, or interpret test results. Advise patients about their condition or treatment regimen Make assessments or perform any kind of medical care decision making. Administer any anesthetic agent (except topical numbing agents such as EMLA cream) Perform test that involve the penetration of human tissues except for skin tests and drawing blood as provided by law. 13 SKILLS LIST and Required Equipment GMAS CER Program Students in the G. CMA. CER and the G. accelerated programs are required to have the CMA uniform from Star Uniform Store, clinical closed toed shoes, Stethoscope, watch with second hand and black pen. Blood Pressure cuff and sphygmomanometer are also required. Depending the program of study, the student will have all or some of the following skills lists. The student will be able to perform (obtain or prepare) the following clinical skills: Vital signs Venipuncture Capillary puncture Pulmonary function testing Electrocardiography Patient screening using established protocols Select sites for administering parenteral medication Oral medications Parenteral (excluding IV) medications Quality control measures CLIA waived hematology, chemistry, urinalysis, immunology testing Screen test results Prepare proper dosages of medication for administration Maintain laboratory test results using flow sheets Maintain growth carts Standard Precautions: select appropriate barrier/personal protective equipment (PPE) for potentially infectious situations Hand washing/hand hygiene Prepare items for autoclaving Perform sterilization procedures Specimens for microbiological testing CLIA waived microbiology testing 13 P age

14 The student will be able to perform (obtain or prepare) the following administrative skills: Obtain a patient history using reflection, restatement and clarification techniques Report relevant information Correctly use medical terminology Explain office policies Instruct patients according to their needs to promote health maintenance and disease prevention Prepare patient for procedures and /or treatments. Demonstrate telephone techniques Documentation of patient care and patient education Compose professional/business letters Develop and maintain a current list of community resources related to patient s healthcare needs Advocate on behalf of patients Manage appointments: scheduling, admissions and procedures Organize medical records Use EMR records data Maintain office hardware and software systems Utilize internet to access information Organize filing Perform routine maintenance of office equipment Perform office inventory Apply HIPAA rules in regard to privacy/release of information. Incorporate the Patient s Bill of Rights into personal practice and medical office policies and procedures Complete an incident report Apply local, state and federal health care legislation and regulation appropriate to the medical assisting practice setting Report illegal and/or unsafe activities and behaviors that affect health, safety and welfare of others to proper authorities Develop a plan for separation of personal and professional ethics Comply and evaluate safety within the work environment Develop a safety plan for the patient, employee and work environment. Demonstrate proper use of the eyewash, fire extinguishers and sharps disposal containers Participate in mock environmental exposure events Demonstrate methods of fire prevention in the healthcare setting. Retrieved April 5, 2013, from Health Insurance Portability and Accountability Act (HIPAA) Policies and Procedures Parkland College Health Professions Department will implement and adhere to the Health Insurance Portability and Accountability Act (HIPAA) of All students who will be attending to patients/clients/residents will receive education and training. It is the practice and philosophy of the Health Professions programs to protect the interest of patients and to fulfill the legal obligations mandated under HIPAA. Definition Protected Health Information (PHI) is any information that identifies an individual and relates to: 1) the individual s past, present or future physical or mental health; or 2) the provision of health care to the individual; or 3) the past, present or future payment for health care. Information is deemed to identify an individual if it includes either the patient s name or any other information that taken together could enable someone to determine an individual s identity such as: 15 P age

16 Names ALL geographic subdivisions smaller than the state All elements of dates smaller than a year (i.e. birth date, admission, discharge, death, etc.) Phone numbers Fax numbers addresses SS numbers Medical record number Health plan beneficiary Any other account numbers Certificate/license numbers Vehicle identifiers Device identification numbers WEB URL s Internet IP address numbers Biometric identifiers (fingerprint, voice prints, retina scan, etc.) Full face photographs or comparable images Any other unique number, characteristic or code. 16 P age

17 Implications for Students HIPAA has important implications for Health Profession students and their education. Protecting the privacy of your patients health information must be foremost in your mind as you are considering how you will communicate what you are learning with faculty, clinical staff and fellow students. When preparing case specific presentations, papers, discussions and reports, you must avoid disclosing patient information that could identify the patient. The anonymity of the staff, caretakers, physicians etc., should be protected by referring to them only by their title such nurse. Each Health Profession student is responsible for understanding and following Parkland s HIPAA policies and procedures. Health Career students at Parkland College have an affirmative obligation to safeguard information and minimize disclosures. HIPAA Compliance Policies Any violation of confidentiality may result in dismissal from the program. Students are expected to observe the standards of conduct established by the college and clinical practice agency. For purpose of reference, the term patient will be used in the following statements and is meant to include: patients, clients, residents, facility and all health care providers. 1. All patient records are confidential. Patient Records include the chart and any other information, verbal or written, and those notes taken from the record. Students must be protective of patients information (i.e. notes not left on desk, in classrooms, or out for any public viewing). All identifying patient information (surgical schedules, printed orders) should be disposed of properly at clinical and should not leave the healthcare setting. 2. Patient records may not be duplicated or removed from the healthcare setting (i.e., notes from records must not contain client-identifiable information or be left on desks, in classrooms, in cafeterias, or out for any public viewing). 17 P age

18 3. Personal electronic devices cannot be carried or used in patient care areas. 4. Students must not discuss patient, staff, or care issues with other patients, friends, family or others in public places such as cafeterias, elevators, bars, restaurants, etc. 5. Students must report observations of violations of the policies to the program directors or the course/clinical instructor immediately. A Parkland College Incident Report will be filed. 6. All students will review the HIPAA policy at Parkland College, pass an assessment and sign a HIPAA agreement before attending clinical. Additional training may be required at Parkland or at clinical sites. 7. Students are responsible for all activities that occur under their login. Do not leave: Electronic charts left open Sharing logins Inappropriate use of internet 8. Release of medical information to other agencies requires a signed release from the patient. If there is a signed release, only send the information they requested and nothing extra. Refer to the specific clinical site policy, procedure and process for scope of practice within the site. 9. If patient chooses to be anonymous in the hospital then even room number is not given out. Consequences Students that violate HIPAA to any form of media will be dismissed immediately. The student will receive a written warning when the HIPAA violation appears accidental (like leaving a chart open). The student will be expelled if a willful violation occurs such as looking up information on a patient without good reason. 18 P age

19 DISMISSAL FROM ANY OF THE MEDICAL ASSISTING PROGRAMs or Courses A student may be immediately dismissed from the medical assisting program if his/her behavior in a classroom, laboratory, or clinical setting jeopardizes the safety of others or does not meet with safe standards of care/practice for a medical assistant. The student will be dismissed if he or she fails to follow the ethical and legal limitations of a MOA/CMA/RMA as a student. Only the Dean of Health Professions may take this action after recommendation from the Program Director. If a student is asked to leave a clinical site by a health care professional and not return for any reason, the student may receive a failing grade for the course of MAS 135 or MAS 170 and possible removal from the program. A review of the cause will be discussed with the Program Director and Dean of Health Professions. A faculty review committee may be requested to determine the outcome for the student. If the student fails the course, he student may not be accepted back into the program of Medical Assistant. Student might have the opportunity to repeat any of the Program Courses for only one time after demonstrating remediation and academic advising. 19 P age

20 METHOD OF PRESENTATION & EVALUATION The method of presentation may include but not limited to the following: Lecture Discussion Student Presentation Audiovisual including videos, DBDs websites Laboratory demonstrations/practice/simulations Return demonstrations Computer Software Assigned Activities Method of evaluation may include but not limited to the following: Exams (written, oral, computer assisted) Skills demonstrations Classroom presentations Clinical performance Work ethic behaviors Assigned activities GRADING SCALE A = % B = 89-80% C = 79-75% D = 74-70% F = 69 or below I = Incomplete with a grade of I (see college policy in the catalog page 41) 20

21 W = Withdrawal with a grade of W (see college policy in the (catalog page 45) Lecture (lead) faculty reserve the right to develop their own grading scale for the exams, quizzes, presentations, clinical and other activities associated with the course and program of study. Refer to each course syllabus and agenda. Final grading scale for the courses and program will be universal. See page 20 previous page. Faculty will provide the grading scale to the student at the beginning of the semester in written form. Students are required to maintain a 2.5 minimum PGPA and a C or higher in all required courses. See Program of Study catalog page 172, Parkland College Syllabus Addendum from the Vice President of Academics is posted on the front page of each course in COBRA. Students are encouraged to be familiar with the contents. 21

22 EXPOSURE FOLLOW-UP PROCEDURE FOR HEALTH CAREER STUDENTS Due to the nature of their work, health career students may be exposed to blood and/or other potentially infectious body fluids during their clinical rotations. In case a needle stick or other exposure incident occurs, the following procedures are to be followed. Each health-care institution should have its own protocol, but the following is a general procedure that is applicable at all facilities in case no protocol exists and for use when exposure incidents occur on campus. Please note that dental hygiene students who are exposed to blood/body fluids through their work in the Dental Hygiene Clinic must follow the protocol designed specifically for this group. It is available on the bulletin board in the Dental Hygiene clinic. Procedure: 1) Immediately after an exposure incident (e.g., needle or instrument stick, human bite, or blood splashed in eyes, nose, mouth, or open wound), the student should flood the contaminated area with water and clean any wound with soap and water or a skin disinfectant if available. 2) After cleansing the contaminated area, the student should report the incident to his or her clinical instructor, who in turn will report the incident to the appropriate staff member at the clinical site or school where the incident occurred. In addition to filling out an incident report at the facility, a Parkland incident report should be submitted to the Wellness Coordinator.. 3) If warranted, the student will be referred to the appropriate department in the health care facility where the incident occurred for medical evaluation. If the facility does not have a department that handles exposure incidents or if the incident occurred at a community agency or on-campus, the student should follow this procedure: If between 7 a.m. and 5 p.m., Monday-Friday, call the Carle Occupational Medicine Department ( ). The student should identify himself/herself as a Parkland health career student and explain there s been an occupational exposure to 22

23 blood/body fluids during clinical and s/he wishes to be evaluated. Department personnel will direct the student as to how to proceed. At other times, the student should go to the Carle Emergency Department (E.D.), 611 W. Park Street, Urbana, and tell the admitting clerk the same; there s no need to notify the E.D. in advance of arrival. 4) The facility where the incident occurred will determine according to their protocol whether or not the source (the person whose blood the student was exposed to) should be tested for HBV, HCV, HIV, and/or any other diseases. In situations where the incident occurs on-campus or at a facility which does not have a post-exposure protocol, the source should follow the same procedures as stated in #3 above. When speaking with Carle staff, s/he should explain that a Parkland student had an occupational exposure to his/her blood/body fluids and s/he needs to be evaluated. 5) If the cost of immediate post-exposure care for the student and source is not covered by the facility where the incident occurred, Parkland College will cover these costs. A bill should be sent to: Wellness Coordinator, Parkland College, 2400 W. Bradley Ave., Champaign, IL Any medical follow-up beyond that given at the time of the exposure is the responsibility of the student. The student should consult with his or her primary care physician or Parkland s Wellness Coordinator as soon as possible after the exposure incident to discuss recommended follow-up care. The student is expected to report the incident to the Wellness Coordinator, Parkland College, 2400 W. Bradley Ave., Champaign, IL in order to ensure proper payment of the initial evaluation. 6) The clinical instructor should report the incident to Wellness Coordinator ( ) at his/her earliest convenience. Any questions about the information provided or about exposure to bloodborne pathogens should be referred to Parkland s Wellness Coordinator at 217/
