1 2014 SJWCFC Internship Job Descriptions About St. John s Well Child & Family Center Our mission is to eliminate health disparities and foster community well-being by providing and promoting the highest quality care in South Los Angeles. St. John s Well Child & Family Center (SJWCFC) is one of the largest nonprofit healthcare providers in Los Angeles County. We are a network of 10 federally qualified nonprofit community health centers and school-based clinics providing medical, dental and mental health care through more than 160,000 patient visits. With a passion for increasing access to quality, community-based healthcare, SJWCFC has grown from a small single clinic in 1964 to a $50 million organization serving uninsured, underserved and economically disadvantaged children and adults in South Los Angeles. Our work goes far beyond the traditional health care model to embrace the broader concept of health and well-being and to tackle the social determinants of health outside our health center walls. We place a high priority on developing supportive services to address this broader spectrum, such as families educational, socio-economic, environmental, and mental health needs. Our work includes health education, child development and literacy education, prevention programming, case management and insurance enrollment. We have an impressive history of successful partnerships with health, educational, social service, and development agencies to deliver a wide range of services that are responsive to local needs. You can learn more at About the Internships Interns work on a variety of projects conducting field research and providing related support to advance health and wellness in South Los Angeles and Compton. Ten program-specific internships are available based on the specific skills and interests of applicants. Interns will be given responsibility to create full project proposals and spearhead the work to complete them during the course of the internship. 1. Prevention Program Intern South LA is a region with an epidemic of obesity, diabetes, and other preventable chronic diseases and with shamefully low access to healthy food, nutrition education, or safe exercise space. Help SJWFC further develop prevention programming to reduce chronic diseases. SJWCFC has an edible urban gardening program, physical activity classes and healthy cooking and nutrition classes in our on-site demonstration kitchen. Additionally, SJWCFC has partnerships with Community Services Unlimited, the LA Neighborhood Land Trust, Enrich LA and other partners who work to improve access in South LA to

2 healthy, affordable food, parks and gardens, and physical activity space and programming for health and well-being. An intern for this program could have the following roles and responsibilities: 1) assist in mapping access points in communities served by St. John s health centers for healthy food, parks and gardens, and exercise space/routes, 2) design surveys to assess patients needs regarding healthy food, parks and gardens, 3) assist in outreach to patients about prevention programming and classes, 4) produce a report to disseminate patients needs and resources available, 5) assist in expanding prevention classes and implementing a fully operational demonstration kitchen, 6) assist in program development and recommendations to connect patients with healthy food, parks and gardens, and physical activity, and 7) support patients in biking for health and other needs by leading rides and identifying resources to support patients who want to bike. 2. Transgender Health Program Intern An intern for this program could have the following roles and responsibilities: 1) assist in identifying resources for the health and well-being of transgender people in South Los Angeles, 2) assist in conducting outreach to transgender communities regarding the SJWCFC Transgender Health Program, 3) assist in overall program development and recommendations to connect transgender communities with comprehensive primary healthcare as well as trans-specific health services such as hormone replacement therapy, 4) provide case management services, 5) assist in conducting patient assessments for potential housing, mental health, and other needed referrals 7. HIV/AIDS Testing and Care Services Intern St. John s houses an HIV Specialty Care Clinic that provides primary medical care and support services to people living with HIV/AIDS. Our interdisciplinary care team provides comprehensive services to patients, including individualized care plans, case management, psychoeducation, referrals to outside specialists, and access to medication coverage plans. St. John s also offers routine HIV testing to all patients ages during primary care visits and is currently implementing a program to routinize HIV and Hepatitis C screening at all of our clinics. An intern for this program could have the following roles and responsibilities: 1) Assist Program Coordinator in the overall development, implementation, and monitoring of all HIV/AIDS Testing and Care services, 2) Data entry and data transfer, 3) Develop monthly reports for HIV-related programs, 4) Attend HIV-related trainings and certification courses, 5) Participate in community HIV prevention planning groups, 6) Assist in developing and implementing outreach activities targeting people living with HIV in South Los Angeles, and 7) Develop resource list of support services for people living with HIV in Los Angeles County. 3. Healthy Homes Program Intern Healthy Homes provides in-home case management, health education, comprehensive housing assessments and tenant advocacy for children and adults suffering from housing-related health conditions such as asthma, lead poisoning and skin rashes. Healthy Homes is a collaborative project

3 between SJWCFC, Strategic Actions for a Just Economy, and Esperanza Community Housing Corporation. In addition to medical services, in-home hazard remediation, and tenants rights assistance, the project is focused on systematic public policy changes to housing code enforcement and to establish this as a reimbursable public health prevention model. An intern for this program could have the following roles and responsibilities: 1) Assist in running and implementing a pilot program with the County Department of Public Health and partner organizations, 2) Assist in producing grant reports on the program, 3) Assist in streamlining the referral process internally and between organizations, 4) Assist in policy research and meetings, 5) Participate in statewide and local policy calls around integrated pest management, Medi-Cal reimbursement for community health workers, asthma initiatives, and other housing policy issues, 6) Assist in overall program development and recommendations to improve and increase care and services to tenants and patients. 4. Healthy Work Program Intern SJWCFC partners with worker s centers and occupational health organizations to provide support services and partner on campaigns to improve occupational health. Additionally, SJWCFC has a fasttrack appointment system for certain workers, like carwash workers, who we know are exposed to multiple occupational health hazards. An intern for this program could have the following roles and responsibilities: 1) work with car wash, restaurant and other low-income workers to survey their health concerns, 2) develop a system to connect them to the healthcare they need at SJWCFC, 3) develop a report or other ongoing documentation of the occupational health-related problems car wash and restaurant workers face, 4) assist in the filing of Cal OSHA complaints, 5) develop an employer manual on Cal OSHA regulations including best practices and recommendations to help improve the health of workers in the industry, 6) support organizations in the development of reports or ongoing documentation of occupational or other health concerns workers face. 4. Integrated Behavioral Health Program Intern St. John s mental health services include a range of counseling, case management and education to children, youth and adults who have been marginalized due to poverty, race, mental illness, substance abuse, HIV/AIDS and homelessness. Our mental health providers address severe mental illness as well as depression, anxiety, substance abuse, parenting concerns, stress, among other issues. In the integrated behavioral health program, mental health clinicians work with primary care providers as a team to treat the mental and physical health needs of their patients. Mental health case management involves assessing the needs of a patient, implementing an individually tailored care plan and coordinating community services to ensure the best possible outcomes. A full list of our mental health services includes crisis intervention counseling, group sessions for youth and adults, short-term therapy, individual therapy/counseling, couples/family counseling, group counseling, psychiatric evaluation and care, and prescription medications.

4 An intern for this program could have the following roles and responsibilities: 1) develop a report on what school-based mental health and well-being programs should consist of, 2) research and develop curriculum for mental health care needs of our Medicare population, 3) conduct a South LA community needs assessment of youth mental and sexual health issues, 4) support overall integrated behavioral health program evaluation, 5) do an analysis of integrated behavioral health no-show rates and develop potential solutions to improving show rates. 5. Communications, Marketing, & Social Media Intern SJWCFC has multiple social media outlets, a website, and ongoing press events and coverage. An intern for this program could have the following roles and responsibilities: 1) Develop and post content for all SJWCFC social media outlets on a daily and/or weekly basis, including writing posts and blogs and taking and uploading pictures 2) Track the growth and impact of the social media and reply to comments and messages, 3) Create external marketing and internal branding materials, 4) Conduct media outreach, press calls, press packets, and event coverage for a variety of media events. 6. Women s Health Program Internship SJWCFC is in the process of expanding and improving the services it offers for its Women s Health Program. While much thinking has been done about that, the program is in need of a business/design plan or description. That plan would include a description of current operations and the target population, key components of the program, a marketing strategy (internal and external marketing and outreach), a quality plan, and a financial plan. This intern would work with staff from the Operations, Medical and Financial departments to assist in the development, recommendations for and initial implementation of that program plan. 7. Diabetes Health Program Intern SJWCFC serves a large and growing population of pre-diabetic and diabetic patients. For several years we have provided services to this population. We are looking to improve and expand the services offered to this population. This program is in need of a business/design plan or description. That plan would include a description of current operations and the target population, key components of the program, a marketing strategy (internal and external marketing and outreach), and a financial plan. The intern for this program would with staff from Operations, Medical, Community Health Initiatives, and Financial departments to assist in the development, recommendations for, and initial implementation of that program plan 8. Infrastructure Improvements Project Internship Health programs such as SJWCFC are required to meet a variety of contractual, regulatory and other requirements, as well as professional and evidence-based standards of care in order to effectively delivery services to populations. Understanding, harvesting, communicating and implementing those requirements and standards is the bread and butter of health care delivery. SJWCFC is in the process of reviewing and refining many of those foundational resources. The Intern for this project would assist staff in reviewing, prioritizing, revising documents, policies and procedures for the organization, as well as developing implementation and communication plans for key policies and procedures.

5 9. Quality Improvements Project Internship At any given time there are a number of quality improvement (QI) projects that are or could be developed to improve the delivery of services in a health care delivery organization. These QI Projects involve the development of teams to work on specific projects for a period of time. The intern for this project would learn about models for quality improvement and work with staff in various departments to design and develop, and begin implementation on at least one QI project. This would include: the identification of QI opportunities; development of an Aims Statement; designing options for interventions to try (i.e., change packages); metrics for ongoing monitoring (i.e., run charts to include baseline and ongoing data); presentation of QI project design and preliminary results. 10. Chief Financial Officer Internship SJWCFC is looking for interns to work directly with our CFO. An intern for this program could have the following roles and responsibilities: 1) Prioritizing and standardizing monthly budget vs. actual variance feedback and follow up tools, 2) Designing and implementing metrics and tools to standardize, automate and streamline procurement to save time, meet federal procurement guidelines and manage costs, 3) Implement payroll cost monitoring through regular analysis of hours worked vs. hours paid, 4) Conducting ad hoc business analyses for financing, new clinic sites, renovation/construction, 5) Designing, monitoring, and modifying team and individual work plans to increase accountability, reliability, timeliness of performance and coach for professional development for FQHC finance staff, 6) Participating in the mid-year re-budgeting process as applicable, and 7) Supporting the CFO, as needed, on day-to-day work and projects as assigned. Some of the general roles and responsibilities interns carry out include: Conduct Internet, library, field-based and other types of research on topics related to the chosen project topic Conduct outreach to patients, community members, and partner organizations Check facts, compile information, and synthesize information and data to prepare solid draft material for literature reviews, reports, and/or grant proposals, news releases and social media Design and administer surveys; interview patients; conduct needs assessments; conduct data collection and analysis Conduct pre-proposal research to flesh out ideas and best practices Assist in the development of written and presentation materials designed to disseminate results of projects Assist in internal coordination needed to implement projects All internships will be based out of our administrative offices at 808 W. 58 th St. LA, CA For more information, contact Regina Clemente at rclemente@wellchild.org or x.4090.
