2 AHI PPS Innovation Funds DY3 DY5 Summary Innovation Funds Summary Purpose: Our efforts to significantly improve the quality of health care delivery in the region rests largely on our collective ability to identify and implement new and distinctive service methods. We re pleased to announce the availability of a new round of funding to help AHI PPS partners undertake innovative initiatives that promise to make a positive impact on Pay-for- Performance (P4P) measures during DSRIP Years 3-5 (DY3-5). If you have a ground-breaking project in mind that would have a significant, tangible influence on improving population health in our region, we strongly encourage you to apply. Eligibility: To participate in this unique funding opportunity, projects will involve at least two partner organizations, one of whom will take the role of lead applicant, and focus on measurable positive outcomes as we prepare to transition away from a fee-for-service system to one that rewards quality performance and results. Project Term: Project terms may be up to two years in length. Submitted projects will be moved through the review process swiftly as they are received. Exclusions: You have a myriad of options on how funds may be used, however they may not be used for major capital investments, to support lobbying or political activities, to establish or continue Regional Health Information Organization (RHIO) participation and/or connection, or for services that are eligible for reimbursement. 3 1. Executive Summary: Describe the project plan and goals. Identify the participating partners, timeline, impacted pay-for-performance metrics and summary of requested funds. Include the timeline for impact on P4P measures identified. 2. Project Progress Evaluation Plan: Describe the methodologies that will be used to track the progress of the project, including how the monitoring plan will include identification of barriers and strategies to resolve issues. 3. Sustainability Plan: Describe how the project will continue after the funding period is complete. 4. Ability of Replication: Describe how PPS partners in other regions may be able to replicate your project. 5. Work Plan Grid: Detailed grid outlining tasks, responsible person(s) and timeline. 6. Budget: Complete budget template. Be sure to include any other funding sources in addition to this innovation grant. Prefer funding request not to exceed $250,000 per year. Submission and Approval Process 1. Each application will be submitted to the AHI DSRIP mailbox at and then routed directly to the appropriate AHI Executive Director. 2. Each application will be reviewed by AHI to assure minimum requirements of the project application and financial stability of the applicant organization. 3. Application will be forwarded to appropriate Population Health Network (PHN) triad lead team for review and recommendation on disposition to the Steering Subcommittee. 4. AHI will review the Steering Subcommittee s recommendation and assure availability of funds. 5. Final disposition will be determined by AHI and applicants notified. 6. Steering Committee and subsequently the AHI Board of Directors will be made aware of all awards. 7. AHI Finance will complete contracts with partners to begin the funding process. 2

4 Innovation Fund Application Components When sharing with us the components of your project, please use the following recommended guidelines for application completion. Cover Page - Project Title, Applicant Organization and Contact Information Executive Summary (3,000 words maximum) Purpose - Goals and Objectives (3,000 words maximum) Statement of Need (3,000 words maximum) Organizational Capacity (3,000 words maximum) Project Narrative (6,000 words maximum) Plan for Evaluation (3,000 words maximum) Sustainability Plan (3,000 words maximum) Work Plan (template to be provided) Budget (template to be provided) Letters of Support (optional) (Notes: character limit no spaces; 3,000 characters is equivalent to one single-spaced page of text. Your application will be approximately eight (8) pages in total (this does not include the cover page, work plan, budget, or letters of support). 7 Innovation and Transformation Funds Comparison Innovation Funds New, innovative projects. Applications include two or more partners. Comprehensive application and scoring process. Awards monitored quarterly through work plan updates submitted during the invoicing process. Goal for a successful project to be expanded and replicated in other Population Health Networks (PHNs) Transformation Funds Projects that fill in the gaps. Single or multiple partners may apply. Simplified application and scoring process. Payments will be made in a lump sum or installments based on individual contracts. Project progress to be reported at the Population Health Network (PHN) level. Projects/funding are one time in nature.
