It addresses housing needs, goals and strategies. This plan is required by HUD and must be submitted annually. Submission deadline is October 15, PROPOSAL The PHA Annual Plan for Fiscal Year 2016; Capital Fund program open grant 2015; Capital Fund 5 Year Plan 2016 through 2020; and Public Housing Admission and Continue Occupancy Policy (ACOP) for the Red Wing HRA will be presented at the Public Hearing. Included in the plan along with all other financial information related to the agency are the following: PHA Annual Plan - HUD form Annual Statement/Performance & Evaluation Report for CFP 2015 and CFP 2016; HUD form Capital Fund Five-year Action Plan for Fiscal Years , HUD form Public Housing ACOP, see Attachment 1. Red Wing HRA reviewed the PHA Annual Plan, Capital Funds 2015, Capital Funds 2016 through 2020 and ACOP with the Resident Council at their August 19, 2015 meeting. See attached: 1) PHA Plan Overview; and 2) PHA Plan Comments. RECOMMENDATION Staff and Finance Committee recommends to the HRA Board of Commissioners to adopt Resolution # approving; 1) the PHA Annual Plan; 2) Annual Statement/ Performance & Evaluation Report for CFP 2015 and CFP 2016; 3) Capital Fund 5-Year Plan for Fiscal Years ; 4) Public Housing Admission and Continued Occupancy Policy; and 5) Authorize the Executive Director to execute all necessary documents. 3 PHA PLAN COMMENTS AUGUST 19, Reviewed PHA Annual Plan. No comments were received. 2. Reviewed open grants CFP 2015 and 5-Year plan 2016 through Comments received: Walkway wall leaks when there is a heavy rain. Tenant would like to see this repaired. Tenant would like to postpone repairs to Jordan Tower I parking lot in favor of completing repairs that are of greater priority. 3. Reviewed summary of Admission and Continued Occupancy Policy. 6 g j Capital Fund Financing Program (CFFF). D Check if the PHA proposes to use any portion of its Capital Fund Program (CFPVReplacement Housing Factor (RHF) to repay debt irkurred to finance capital improvements. Housing Needs. Based on information provided by the applicable Consolidated Plan, information provided by HUD, and other generally available data, make a reasonable effort to identify the housing needs of the low-income, very low-income, and extremely low-income families who reside in the jurisdiction served by the PHA, including elderly families, families with disabilities, and households of various races and ethnic groups, and other families who are on the public housing and Section 8 tenant-based assistance waiting lists. The identiiication of housing needs must address issues of affordability, supply, quality, accessibility, size of units, and location. Currently, the local market rate rental units are in short supply. Affordable imits are not available and are in great demand. Housing for disabled households is also in great demand. Housing Choice Vouchers are slow to turn over and the waiting list is closed. Shelter + Care program is full. Public Housing units turn over in 6 months to a year. The Domestic Violence Shelter is full. Strategy for Addressing Housing Needs. Provide a brief description of the PH A's strategy for addressing the housing needs of families in the jurisdiction and on the waiting list in the upcoming year. Note: Small, Section 8 only, and High Performing PHAs complete only for Annual Plan submission with the 5-Year Plan. 9.1 The HRA just completed a Housing Needs Assessment which shows a need for 1,055 housing units in the next ten years. The HRA is working with non-profit and private developers to bring additional housing units to Red Wing. Presently, there is a proposed development of 36 LIHTC units in the application stage, of which the HRA has dedicated 4 Shelter + Care units, if awarded. In addition, the HRA is working with the City of Red Wing to create an Assisted Housing Trust Fund. Another top priority is to maintain the existing HRA owned property including a $1.2 million window replacement project, in which $923,000 is being funded through Publicly Owned Housing Program (POHP) from the State of Minnesota. In addition, the HRA just completed a total modernization of the 2 elevators; 2 public restrooms and tub room making them handicap accessible. In Public Housing family housing we are working with the County to assess head of households for economic, health and qualifications to apply for additional assistance. Page 2 of 2 form HUD (4/2008)

7 Additional informatiod. Describe the following, as well as any additional information HUD has requested. (a) Progress in Meeting Mission and Goals. Provide a brief statement of the PHA's progress in meeting the mission and goals described in the 5- Year Plan We help elderly and persons with disabilities maintain their independence by providing, improving, and expanding sustainable housing programs and social services in partnerships with the community: Continue to maintain a successful Independent Living Services Program offering home health care, homemaker services and Customized Living Services. Services are offered to residents under a sliding fee scale. Customized Living Services are provided through ACCRA Home Health Care providing on-site nursing and homemaker services 12 hours per day, 7 days per week; 24 hour emergency response access through a pendant worn; 2 safety checks per day; continental breakfast, escort to SEMCAC dining, prepared and delivered evening meal; homemaker services 1-2 times per month. Contracted a part-time Community Support Technician to assist the Social Service Coordinator during the evening hours to assist with maintaining and improving housing programs to better serve the needs of the elderly and disabled. Provide dining site for SEMCAC dining which provides meals 7 days per week Strong Resident Council which provides social activities and support of housing programs Continue successful administration of the capital fund program. Capital improvements include addition of security cameras, repair of structure and new flooring in family Public Housing units, and replacement of signage. Future capital improvements include replacement of sprinkler heads and replacement of boilers. Successfully obtained general obligation bond funding to replace 225 windows in Jordan Tower I with energy efficient H-Windows which are ADA compliant and replacement of panels under windows which contain asbestos. Continue to apply for general obligation bond funding for the replacement of boilers and air condition in common areas at Jordan Tower I. We help families maintain safe and affordable housing during their times of economic transition through viable subsidized housing programs. Maintain standard to high REAC score Continue to make improvements to family Public Housing properties: Replacement of roofs Renovation of kitchens Repair of structure and flooring Maintain Family Self Sufficient program through the Housing Choice Voucher program. Currently there are 7 on the program with 1 receiving an escrow and 7 successfully completing the program. (b) Significant Amendment and Substantial Deviation/Modification. Provide the PHA's definition of "significant amendment" and "substantial deviation/modification" Substantial Deviation: A substantial deviation is a decision made by the Board of Commissioners to change the HRA's mission statement, goals, or objectives identified in the 5-Year Plan. It is also when goals or objectives are changed that affect the residents or have a significant impact of the HRA's financial situation. Significant Amendment or Modification: A significant cunendment or modification is a change in HRA plans or policies that require formal approval by the Board of Commissioners. Items (a) through (g) may be submitted with signature by mail or electronically with scanned signatures, but electronic submission is encouraged. Items (h) through (i) must be attached electronically with the PHA Plan. Note: Faxed copies of these documents will not be accepted by the Field Office. 19 RESOLUTION # PHA ANNUAL PLAN; CFP 2015, CFP 2016; AlVD CFP 5-YEAR PLAN FOR FISCAL YEARS WHEREAS, The PHA Plan is a guidebook, which outlines the PHA mission statement and other housing related goals for the upcoming year and five years beyond. It addresses housing needs, goals and strategies. This plan is required by HUD and must be submitted annually. Submission deadline is October 15, 2015; and WHEREAS, Included in the plan along with all other financial information related to the agency are the following: 1) PHA Annual Plan for Fiscal Year 2016; 2) Capital Fund program open grant 2015; 3) Capital Fund 5 Year Plan 2016 through 2020; and 4) Public Housing Admission and Continue Occupancy Policy (ACOP); and WHEREAS, Red Wing HRA reviewed the PHA Annual Plan, Capital Funds 2015, Capital Funds 2016 through 2020 and ACOP with the Resident Council at their August 19, 2015 meeting; and Therefore, let it be resolved that the Red Wing Housing and Redevelopment Authority Board of Commissioners adopt Resolution # approving: 1) the PHA Annual Plan; 2) Annual Statement/ Performance & Evaluation Report for CFP 2015 and CFP 2016; 3) Capital Fund 5- Year Plan for Fiscal Years ; 4) Public Housing Admission and Continued Occupancy Policy; and 5) Authorize the Executive Director to execute all necessary documents.
