3 NOTICE No. 1 Solicitation No Provide Pest and Termite Control Services THE FOLLOWING INFORMATION SHALL BE INCORPORATED AS PART OF THE ABOVE MENTIONED SOLICITATION; ALL OTHER TERMS AND CONDITIONS SHALL REMAIN THE SAME: Revisions to the Solicitation 1. Pages # lll. SCOPE OF WORK/ SPECIFICATIONS A. Pest Control Services 1. This scope of work covers a complete service to be provided by qualified firms, designed to control the following pests in the schools and other facilities located within Richland County School District One, but not limited to: Water Bugs Fleas Nats Moth Mites Head Lice Silverfish Lice Bees Fruit Flies Roaches Ants Ticks Spiders Flying/Stinging Insects (excluding Honey Bees) Indoor Swarming Termites Insects and Arthropods 2. Schedule / Times of Service All treatments performed while school is in session shall be scheduled after students/staff have been dismissed. Listed below are anticipated treatment times: b. The only exception to the above schedule will be treatment of Cafeterias (including dining areas, kitchens, pantries and offices). These areas have limited access and should be treated between the hours of 3:00 p.m. - 4:15 p.m. Monday-Friday under the direction of a Student Nutrition representative. Treatments should be done after each school s dinner time. Elementary Schools-2:30 p.m.-3:00p.m. Middle Schools- 3:45 p.m. - 4:15 p.m. High Schools- 3:15 p.m. - 3:35p.m. 3. Treatment Requirements Richland County School District One has adopted an Integrated Pest Management (IPM) methodology in addressing pest control management. Strict adherence to the following plan will be required to protect the safety and health of students and staff while controlling unwanted pests: d. Emergency Treatment - Contractor must be available to address emergency treatment requests within by close of business the same day. Service is expected at no additional charge for treatment of pests listed under this contract. Monday-Friday service is required. Saturday/Sunday and holiday service is non-routine, but may be required in extreme CBVB No No.1 Page 3 of 7

4 circumstances. Contractor should specify any additional charges (if any) related to weekend/holiday emergency response. Repeated emergency calls for the same issue will constitute the need to perform an additional site evaluation to ascertain the root cause of the reoccurring problem. A formal report from the Contractor will be required within 24 hours following any site evaluation. B. Termite Control Services and Other Services 5. Contractor should include bid prices for the following services: a. Annual termite/wood destroying organism inspections service (which includes annual inspections, treatments as needed, and all follow-up visits). 2. Page 14 # lv. INFORMATION FOR OFFERORS TO SUBMIT C. Special Contract clauses special to this solicitation 2. The successful bidder prior to being awarded a contract shall provide an opportunity to the MCO to view chemicals and equipment that will be used by their company. All chemicals shall be in the original containers and labels must display EPA regulations. MSDS information on all products must be submitted to the MCO prior to the bid being awarded.. 3. References - Contractor must provide three (3) written references FOR EACH LOT THAT A BID IS SUBMITTED including current or previous projects within the last three (3) years. With at least one (1) of those references being a client of a larger or similar size. Point of contact name and phone number must be provided for references. Contractors should have at least (5) years experience in the area for which bids are received (commercial pest and termite control, and/or animal control services). A SLED background check must be performed on all technicians prior to entering the Richland County School District One s property. A copy of the SLED background check should be submitted to the office of Procurement Services to be obtained placed in the contract file. 4. Permits and Licenses All permits and licenses required by local, state and federal authorities for pest and termite, and/ or animal control services shall be required by RSCD1 prior to contract award. All permits and licenses shall be obtained, paid for by the contractor, and kept current during the duration of this contract. The Contractor bears all financial responsibilities for obtaining and maintaining all permits and licenses. Contractor personnel performing pest control on any RCSD1 site shall be individually certified in category 7A Pest Control by the State of South Carolina. CBVB No No.1 Page 4 of 7

BID SCHEDULE/ PRICE PROPOSAL PEST CONTROL SERVICES PRICE PROPOSAL NOTE: Standard pest control service to be provided every month at every site AND includes a special interior treatment of every building twice a year (either at the end of Summer Break or during Christmas Break. Description: Richland County School District One shall require services for all its locations (some locations have several separate buildings. Separate annual bid sums shall be made on each location as indicated below, taking into consideration the frequency (two total site treatments and the remaining standard monthly treatments) and areas to be serviced in strict accordance with the requirements as set forth in the specifications.
