4 I. Preface A. Scope and Purpose of the Athletic Handbook This Athletic Handbook is the operating manual to educate each Keiser University (the "University") studentathlete about the policies and procedures of the University's Athletic Department (the "Athletic Department"). The University's expectation is that each student-athlete will read, understand and comply with the provisions of this Handbook as a condition to competing in intercollegiate athletics at the University. If you have any questions regarding this Athletic Handbook, please contact the Director of Athletics. B. Keiser University Athletic Priorities and Guiding Principles The Athletic Department recognizes that intercollegiate athletics is an important part of a complete University educational experience. The University will continue to emphasize responsibility, a strong work ethic, and a positive attitude to help our student-athletes succeed academically, athletically, and socially. All individuals involved, directly or indirectly, in the athletic program must maintain and foster the values and goals associated with the University's Athletic Mission Statement. D. Keiser University Athletic Objectives It is the objective of the Athletics Department to provide the most positive student-athlete experience as they compete for Keiser University. The department aims to lead the university in recruiting, enrolling, retaining, graduating, and developing leaders. As an NAIA institution we continually strive to be a 5-star Champion of Character university, and to meet all five of the Champion of Character objectives (Integrity, Respect, Responsibility, Sportsmanship and Servant Leadership). We will accomplish these objectives by: Recognizing that educating and graduating students exceeds all other responsibilities. Commitment and acknowledgement that health and safety play a major role in the success of our intercollegiate athletic programs. The assurance that student-athletes will be held to a high level of accountability and understand that it is a privilege to compete in intercollegiate athletics and not a right. Understanding and implementing equitable opportunities for both men and women. Recognize that excellence in intercollegiate athletics may bring local, regional and national exposure to the University. 4

5 Displaying a high level of ethics, integrity, fairness, and respect for diversity. Being leaders when involved in an extra-curricular activity and in all behaviors on and off campus. Being courteous and well-mannered when representing the student-athlete's activity and the University. The Athletic Code of Conduct Student Handbook: Academic Policy: Student-athletes have specific responsibilities in the pursuit of academic, as well as athletic, excellence. University student-athletes are responsible for reading and understanding the athletic handbook, including the Keiser University Student Handbook and the Judicial Process at the Keiser University Flagship Campus. Student-athletes may be sanctioned under either or both of these sets of rules. A student-athlete's privilege of engaging in athletics is the main focus of the Athletic Code of Conduct, and the student's overall relationship to the University is the main focus of the Keiser University Student Handbook and the Judicial Process at the Keiser University Flagship Campus. Personal Presentation: A student- athlete's physical presentation is a reflection of the University. While representing the University, athletes must be well groomed and presentable as determined by the coaching staff and/or Athletic Department. Logos, body art and tattoos deemed inappropriate by coaches or University officials must be covered when student -athletes are representing the University. Athletes may also be subject to additional presentation standards issued by their specific team and coaching staff. Behavior and Unlawful Actions: Each student-athlete is expected to comply with the regulations outlined in this Athletic Handbook, the Keiser University Student Handbook, as well as all federal, state and local laws. Your behavior on- campus, off-campus, during your season, and out of season, creates an image that is a direct reflection on yourself, your team and the University. It is expected that every student-athletes knows, understands and honors the universities expectations, displays positive behavior and treats everyone with respect and civility. In addition to sanctions under this Athletic Code and under the Judicial Process at the Keiser University Flagship Campus, any student-athlete who is charged with a violation of law, including, without limitation, any misdemeanor or felony, must notify their Head Coach, Athletic Director or Assistant Athletic Director verbally and in writing within 48 hours, and the student-athlete may be subject to punishment, including suspension from his/her athletic activity pending resolution of charges. Drug and Alcohol Policies: The University expects all student-athletes to adhere to all state, federal and local laws, as well as all applicable rules regarding the use of alcohol and controlled substances established by the NAIA or other organizations with which the University is affiliated. Student-athletes must also comply with the requirements of the Keiser University Student Handbook and Judicial Process at the Keiser University Flagship Campus applicable to alcohol and drugs. We expect our student-athletes to represent Keiser University in a manner that creates a positive image of the people, values, and traditions associated with the University. Disciplinary Action: Any student-athlete found to be in violation of any provision of this Athletic Handbook will be subject to disciplinary action. Such disciplinary action will be determined by the Athletic Department in accordance with the severity of the violation, the student -athlete's record of past conduct, and University policies. In addition to the specific sanctions discussed under the Alcohol, Drug, and Social Media Policies (please see each individual section in the handbook for specific sanctions for violations of Alcohol, Drug, and Social Media violations), disciplinary actions may include, but are not limited to, one or more of the following: 5

6 1. A verbal warning; 2. A written warning documented in the student-athletes file; 3. Mandatory counseling sessions, including required compliance with recommendations of the counselor; 4. Suspensions from participation with team functions; 5. Dismissal from the athletic team; or 6. Forfeiture and/or disqualification from receipt of athletic financial aid. Violations of this Athletic Handbook may also be violations of the Keiser University Student Handbook and/ or of federal, state or local laws. Any violation of the Keiser University Student Handbook may also involve a Misconduct proceeding under the Judicial Process at the Keiser University Flagship Campus. In such a case, any disciplinary action taken under this Athletic Handbook will be in addition to any sanction imposed under the Judicial Process at the Keiser University Flagship Campus or by legal authorities. A report detailing any disciplinary action taken under this Athletic Handbook will be filed with the Athletic Department. The Athletic Department recognizes the use of alcohol and illegal drugs as a significant problem on college campuses and for many student-athletes. Their use results in negative effects on behavior, relationships, academic performance, and the overall learning environment. The use and misuse of these substances also affects athletic performance and team morale. Although we realize that problems with alcohol and other drugs are not confined to student-athletes, they are of special concern to us because of the high visibility and additional social pressures athletes often face as representatives of our institution. A. Academic Policy The Athletic Department is committed to helping students achieve excellence in the classroom measured by eligibility, grade point average, retention, and graduation rates. Academic Integrity: Student-athletes experience additional academic pressures in their pursuit of maintaining a balance between academics and athletics. Academic honesty is fundamental to the activities and principles of a university. All members of the academic community must be confident that a person s work has been responsibly and honestly acquired, developed, and presented by the individual submitting the work. Students must be aware that academic integrity violations include, but are not limited to, giving or receiving unauthorized aid on an examination, assignment, paper, or lab report. In addition, violations may also include, without limitation, giving false reasons for taking a make-up examination, resubmitting work as original or falsifying data. Failure to take responsible action is also a violation. If at any time there are questions or concerns about what does or does not constitute a violation of academic honesty, please consult the Undergraduate Catalog or contact the appropriate instructor or academic advisor. 6

7 Attendance Policy: Student-athletes are expected to attend all classes and scheduled academic events. Punctuality and regular attendance is required and the student-athlete accepts the consequences of failure to attend. Student-athletes will be provided with game and practice schedules in advance and encouraged to schedule accordingly to avoid conflicts if at all possible. If there is a conflict with a class and a scheduled competition, it is up to student-athlete to: Inform the professor at the beginning of the semester (within the 1 st 2 weeks of the start of the semester) and provide an approved from the Athletic Department with details of absence. Update professors immediately if games are cancelled or rescheduled, and attend the classed that would have been previously missed. There will be a separate class release form for conference tournaments and national tournaments once the schedule is finalized and seeding s have been released. Request from professor any work or assignments in advance. Be accountable for all work and/or assignments. Make arrangements for taking any examination affected by team travel. Provide both coaches and professors with any last minute changes as soon as possible. If there are any questions or concerns regarding class attendance, examinations or assignments, it is the responsibility of the student-athlete to contact academic services and/or the Athletic Department. The Athletic Department will receive academic reports for mid-term grades and final grades to track GPA, class attendance, and overall academic performance. Keiser Athletics follows the university policies for attendance. Please see your syllabus for a specific explanation for attendance. B. Alcohol Policy Student-athletes must comply with all applicable Federal laws and rules related to alcohol. The legal drinking age in Florida is 21 years of age, any student -athlete under the age of 21 that violates this rule (either on or off-campus) will be subject to state/federal laws and can be charged as a 2nd offender by the Athletic Department (see below). In addition, student-athletes of any age may not consume alcohol 48 hours prior to any athletic game or contest. Violations of this policy include, but are not limited to, the following: Possession or consumption of alcohol by a student-athlete under the legal age, as mandated by Florida law; Providing alcohol to a person under the legal age and/or drinking in the presence of a minor, as mandated by Florida law; Charged misdemeanor or felony alcohol-related offense; Possession, consumption or under the influence of alcohol on campus, in residence hall or at any athletic contest or function. Violation of 48 rule Alcohol related disturbances or conflicts either on or off campus. 7

8 Following are minimum sanctions that may be imposed by the Athletic Department for any violation of this alcohol policy. Additional sanctions may be imposed by the University, the Director of Athletics, or via specific team contract/rules. Disciplinary actions designated by Athletic Department may be separate from actions taken by other involved departments within the university. NOTE: Any of the following listed violations may be considered by the Athletic Department as a second offense violation, even if it is the student-athlete's first offense: Peace disturbance/misdemeanor alcohol related; or Alcohol related motor vehicle violations. 1st Offense = 8 hours community service and additional actions optional by coach 2nd offense = 16 hours community service and suspension of 20% of season and/or 25 % scholarship reduction 3rd offense = immediate removal from athletic program and immediate removal of all athletic aid *NOTE: THE ABOVE LISTED SANCTIONS ARE MINIMUM SANCTIONS. ANY CODE OF CONDUCT OR TEAM CONTRACT THE STUDENT ATHLETE HAS SIGNED CAN PLACE ADDITIONAL SANCTIONS ON THE STUDENT ATHLETE. C. Student-Athlete Drug Policy Student-athletes must comply with applicable laws and rules related to controlled substances. Substance abuse is not limited to "street" drugs (e.g., marijuana, cocaine, etc.). While the use of medical marijuana is legal in some states it is still considered illegal by Federal law and will be considered an illegal and banned substance at Keiser University. In the best interests of student-athletes, coaches, staff and the University, performance-enhancing drugs (e.g., steroids, etc.) are also prohibited. If there are questions or concerns regarding the use of any drug or medication, please contact the Athletic Department prior to using such substance. Password: naialive5 All student-athletes are required to sign a Drug Testing Consent Form before being allowed to participate in athletics at the University. This form allows the athletic director and athletic training staff to request that an athlete be tested for substance abuse at any time for illegal or banned drugs. An additional NAIA drug testing consent form will also be required for individual student-athletes and teams advancing to national championship competition. The form will be signed prior to any championship level competition, including but not limited to opening rounds and final sites. Keiser University reserves the right to randomly select student-athletes from any team for drug testing. The University may also drug test any team or student-athlete upon the discretion of the Athletic Department. Student-athletes chosen for random drug testing will be notified prior to testing and must report to the designated test site at the designated time of testing, typically the morning after notification. When called upon for drug testing, the student must provide identification and verify his/her presence by signature, to the test administrator. Random testing will be conducted monthly. 8

9 If a staff member is suspicious of a student-athlete s use or potential use of a banned or illegal substance and wants to initiate a test, the staff member must: (1) fill out the suspicion based testing form (on web-site) and submit to the Director of Athletics; and (2) meet with the Director of Athletics to discuss whether further investigation or other intervention is appropriate. The Athletic Director will determine whether to solicit a drug test from the student-athlete. Students selected for "suspicion based" drug testing will follow the same procedure of the random drug testing procedure. Random or suspicion testing will involve a urine sample, though the University reserves to itself the right to collect blood, oral fluid, or hair follicle samples to determine compliance with this policy. Anyone who feels they have a problem with substance abuse and needs help should call is a state-wide referral network for social services. Furthermore, any Keiser University staff member would be willing to help if possible and would be able to direct you where to go if needed. The campus CRT or Crisis Response Team is always available as well. Consequences of Failure to Participate in or Cooperate with Testing A student-athlete who is found to have not signed a Consent Form or who refuses to sign the Student Athlete Notification Form or Custody and Control Form, fails to arrive at the collection station at the designated time without justification, fails to provide a urine specimen according to protocol, leaves the collection station before providing a specimen according to protocol, or attempts to alter the integrity or validity of the urine specimen and/or collection process, will be in breach of protocol and treated as if there was a positive test for a banned substance. The student-athlete will be penalized in accordance to the Effects of positive results violations. The certified collector will inform the student-athlete of these implications (in the presence of a witness) and record such. If the student-athlete is not available, the certified collector will notify the University staff member whom is present. Drug Testing Procedures All student-athletes will be eligible for random testing. This includes any student-athlete who is on a roster, regardless of eligibility status. In addition, suspicion based testing will be performed as deemed necessary by Keiser University staff. Testing will be performed each month, the dates for testing will be determined by the Athletic Director and Head Athletic Trainer. The drug testing service provider, Drug Free Sport, will randomly select the student athletes each month. A Keiser University representative will inform those student- athletes selected for testing. Testing will take place at indicated time given during notification, typically the following morning. Student-athletes will remain at the collection site until a complete urine specimen is provided. FAILURE TO REPORT SHALL BE CONSIDERED A POSITIVE TEST. See also, Consequences of failure to participate in or cooperate with testing. A certified collector from Drug Free Sport will collect the specimen according to protocol (see end of policy for Drug Free Sport Urine Collection Guidelines). Such persons observing the collection of the specimen will be of the same gender as the athlete. Once an athlete has submitted a complete specimen, he/she will remain eligible to participate in athletics at the University until the head athletic trainer secures the final results of the drug test. Once a positive test in confirmed there will be a 12-hour notice before suspension begins. This allows for flexibility with time-toconfirmed-results being received. e.g. a confirmed positive result is received the day of a game and said student/coach had the game planned around this student-athlete. The ability to start suspension prior to confirmed positive result will lie at the discretion of the coach and athletic director. 9

10 All results are confidential, except as otherwise required by law, and will remain in student athletes permanent file. The athlete/donor, head athletic trainer, coaching staff of the specific sport and athletic director will be notified of positive results. Certain circumstances require further notification of necessary individuals for the purposes of Cleary Act reporting. Failed drug tests will accumulate over a student-athlete s time at Keiser University. A second failed drug test at any time will be considered a second violation. For example, an athlete fails a drug test their freshman year, passes during their sophomore year, then fails again their junior year. This second fail will face second violation penalties. Negative Results The athlete will be contacted by the head athletic trainer about the results and remain eligible to participate in athletics at the University. The results of the test will be shared with the athlete/donor, the head athletic trainer, the head coach of the specific sport, and the athletic director. If an athlete tests negative for a drug test, he/she may be tested again during the school year. Effects of Positive Results (the presence of banned substance(s)) The Head Athletic Trainer will notify the student-athlete, Athletic Director, and coach regarding the results of the positive drug test. The results will be scanned into pdf format and attached in an to the athletic director and the coach. Following this notification, the Athletic Director and Head Coach will meet with the student athlete. Following are minimum sanctions that may be imposed by the Athletic Department for any violation of this drug policy. The University and the Director of Athletics may impose additional sanctions. 1. FIRST VIOLATION: a. The student athlete will be required to complete an individualized education and counseling program. Throughout the program, the student athlete will be subject to random drug testing using the protocol described above. b. The student athlete will be suspended from all participation in competition at Keiser University for a period of 20% of their eligible season, to include exhibition games, regular season, and post-season games. If the positive test takes place in season, the suspension will begin immediately following receipt of the positive drug test and can carry over from one season to the next if the 20% is not satisfied. If the positive drug test takes place in the student athlete s off-season, the suspension will be served at the beginning of the following season. 20% of the season will be based on regular season games and will be rounded down to the nearest whole number. This is to avoid team sanctions and long term individual sanctions resulting from further supervised drug testing. c. Any athletic scholarship monies can be revoked for the remainder of the student athlete s career at Keiser University. 2. SECOND VIOLATION: (Second confirmed positive drug test result or failure to comply with the sanctions imposed or failure to participate in the education and counseling program for the first violation). a. The student athlete will be subject to the sanctions described above and will be suspended immediately from all athletic teams at Keiser University for one full calendar year from the date of the second violation. b. Any athletic scholarship monies can be revoked for the remainder of the student athlete s career at Keiser University. *It is a requirement that the student-athlete must continue his/her education at Keiser University throughout suspension to be considered for reinstatement at the end of the calendar year. 10

11 3. THIRD VIOLATION: (Subsequent confirmed positive drug test results or failure to comply with the sanctions imposed or failure to participate in the education and counseling program for a second violation). a. The student athlete will be dismissed immediately and permanently from the Keiser University athletic team(s) and athletic scholarship will not be renewed. b. The student-athlete will be encouraged to remain in the drug education rehabilitation/treatment program. *NOTE: THE ABOVE LISTED SANCTIONS ARE MINIMUM SANCTIONS. ANY CODE OF CONDUCT OR TEAM CONTRACT THE STUDENT ATHLETE HAS SIGNED CAN PLACE ADDITIONAL SANCTIONS ON THE STUDENT ATHLETE. A POSITIVE TEST MAY RESULT IN ADDITIONAL DISCIPLINE THORUGH THE UNIVERSITY S JUDICIAL PROCESS. Appeal Process Student-athletes who test positive for a banned substance by the laboratory retained by the institution may, within 72 hours following receipt of notice of the laboratory finding, contest the finding. Upon the studentathlete s request for additional testing of the sample, the Director of Athletics/designee will formally request the laboratory retained by Keiser University to perform testing on specimen B. Specimen B findings will be final, subject to the results of any appeal. If specimen B results are negative, the drug test will be considered negative. Student-athletes who test positive under the terms of this policy will be entitled to a hearing with the Director of Athletics or his/her designee prior to the imposition of any sanction. Requests for such a hearing must be made within forty-eight (48) hours of notification of a positive test result. If the forty-eight hours would end on a weekend, the request must be made by noon on the next business day. Requests must be in writing and received by the Director of Athletics or his/her designee. The student-athlete must present his or her own case. The meeting should take place no more than seventy-two (72) hours after the written request is received. The student-athlete may request an extension of time to the Director of Athletics, who will consider whether to grant the extension upon a showing of good cause. These proceedings shall include an opportunity for the student-athlete to present evidence, as well as to review the results of the drug test. The proceedings shall be confidential. The decision by the Director of Athletics or his/her designee regarding the sanction to be imposed shall be final. 11

14 D. Gambling Keiser Athletics opposes all forms of legal and illegal sports wagering on college sports. The NAIA and Keiser University follow NCAA rules and guidelines in regards to gambling. The NCAA has developed a Web site to educate student-athletes, coaches, athletic administrators and the general public about NCAA rules about sports wagering. The Don t Bet on It Web site ( Keiser University will adhere to the rules and consequences of the Don t Bet on It guidelines. A student-athlete involved in sports wagering on the student- athlete's institution permanently loses all remaining regular-season and postseason eligibility in all sports. A student-athlete who is involved in any sports wagering activity that involves college sports or professional athletics, through an internet gambling, bookmaker, a parlay card or any other method employed by organized gambling, will be ineligible for all regular-season and postseason competition for at least one year. Sports wagering is defined as putting something at risk such as an entry fee - with the opportunity to win something in return. Because of this, student-athletes, coaches and administrators may not participate in contests (examples: fantasy leagues and bracket challenges) where there is both a required entry fee and an opportunity to win a prize. Student-athletes and administrators may participate in contests where there is no entry fee but a possibility of winning a prize. E. Hazing Hazing is defined in the Keiser University Student Handbook as any action taken or situation created, intentionally, whether on or off University premises, to produce mental or physical discomfort, embarrassment, ridicule, or that demeans the character, or inflicts bodily harm on any person. Hazing is strictly prohibited and violations will be dealt with severely. Consequences under this Athletic Handbook may include suspensions, community service and loss of participation, as determined by University officials and the Athletic Department. As with other violations of the Keiser University Student Handbook, hazing incidents may also result in sanctions under the Judicial Process at the Keiser University Flagship Campus and criminal prosecution. Sportsmanship 1) Student-athletes bear the responsibility of displaying good sportsmanship during games and competition. Behavior at all times must reflect the high standards of honor and dignity that should characterize participation in competitive intercollegiate sports. Student-athletes shall conduct themselves in a manner reflecting positively on the reputation of the University both on and off the "field of play," in pre-game and post-game comments to media, and when traveling and participating at other institutions. 2) Student-athletes are expected to maintain an attitude of dignity and respect toward opponents. The following are examples of unsportsmanlike behavior: fighting opponents, fans or officials, taunting opponents, inappropriate "celebrations" with the intent to demean opponents, disrespectful attitude toward opponents, inciting crowd hostility toward opponents in an unsportsmanlike manner, profane or vulgar language, or any other action that will lead to ejection from any/all athletic contests or events. 3) As a member of the NAIA (National Association of Intercollegiate Athletics), we are committed to establishing sportsmanship and developing healthy environments for competition through the "Champions of Character" program. The five core values include integrity, respect, responsibility, sportsmanship and leadership. These character values help young people and those associated with their development make good choices in all aspects of their life and reflect the true spirit of competition. 4) Student-athletes displaying unsportsmanlike behavior will be subject to disciplinary action, as determined by the coaching staff and/or Director of Athletics. 14

15 Social/ New Media Policies for Student-Athletes Overlying Principles Definitions Social and New Media are defined as media designed to communicate online through social interaction using highly accessible public publishing forums such as, but not limited to, Facebook, Twitter, YouTube, LinkedIn, SnapChat, blogs, message boards, Flickr, and other social platforms. Student-athletes may not be aware that third parties, including the media, faculty, future employers and NAIA officials or other athletic affiliations we are members, can easily access their profiles and view all personal information. This includes all pictures, videos, comments and posters. Inappropriate material found by third parties affects the perception of the student, the athletic department and the University. This may also be detrimental to a student-athletes future employment options. General Guiding Principles Rules/Guidelines Disclosure Do not post information about specific student-athletes, unless it is related to an award or honor. Please follow the guidelines of FERPA (see FERPA website) and good taste. Sportsmanship Don't talk about internal matters in the department, the KU campus, the conference or the NAIA and/or any other affiliation. Practice generosity. Find something good to say, or don't say it. Don't bash the competition. Politics and religion are off limits. Don't gender bash. Common sense and courtesy should always prevail. Content Do not post content that mentions the use of alcohol, drugs, or the use of either. Also, do not post images that contain alcohol, drugs, or drug paraphernalia (either in your possession or in the background of the image). Please remember that anything that goes online does not, and will not disappear. Once images and posts are uploaded they can be accessed at any time, including after you graduate and are applying for jobs. Proprietary Information and Confidentiality All photos, podcasts, videos, news stories or other information not authored by an account manager should be used only in compliance with copyright laws. Account managers will not comment on confidential information related to department matters that may include litigation, personnel matters, including the hiring of firing of staff members, or other confidential 15

Disciplinary Actions If a student-athlete s profile and its contents are found to be inappropriate in accordance with the above behaviors, he/she will be subject to the following penalties: Written warning A meeting with Director of Athletics and Head Coach Penalties as determined by the athletics department, for example, suspension or termination from athletic team. Other offensive behaviors may also be subject to discipline so please use proper judgment when posting content. For your own safety, please keep the following recommendations in mind as you participate in social networking websites: Set your security settings so that only your friends can view your profile. You should not post your , home address, local address, telephone number(s), or other personal information as it could lead to unwanted attention, stalking, identity theft, etc. Be aware of who you add as a friend to your site many people are looking to take advantage of student-athletes or to seek connection with student-athletes. Consider how the above behaviors can be reflected in all Facebook/Twitter applications. If you are ever in doubt of the appropriateness of your online public material, consider whether it upholds and positively reflects your own values and ethics as well as the athletics department and the University s. Remember to always present a positive image and don t do anything to embarrass yourself, the team, your family or the University. H. Athletic Formal Complaint Policy The following steps are needed if you, the student athlete, or your parent/guardian wish to make a formal complaint to the athletic department. A formal complaint constitutes any issues or events that would qualify as a safety concern for the studentathlete. This includes; unnecessary treatment, (language or physical), bullying from teammates, or any other matter that affects the well-being of the student-athlete in regards to athletics. This does not include playing time. The head coach makes the decision based on their best judgment as it pertains to winning a contest. The steps to make a formal complaint are as follows: 1) The student-athlete must discuss any issues with the coach. 2) If the issue is in direct involvement with the coach (physical abuse) or a resolution cannot be worked out please write out the complaint and see the Athletic Director. 3) Once you have seen the Athletic Director and if the issue cannot be resolved, please see the Campus President. 4) The Campus President will make the final decision. 16

17 The steps to make a formal complaint involving the parents: 1) The student-athlete must make sure that they have signed a release form with the registrar waving their rights as an adult and granting the parent/guardian the ability to communicate any and all issues with Keiser University faculty and staff. 2) Once this has happened the student-athlete must write an to the coach that gives the parents of the student-athlete permission to discuss all issues regarding their son or daughter s behavior, academics, team involvement, and living experience. (This means the coach can bring up all issues that have happened so that parents can get a very clear picture regarding the issues that have occurred). 3) After the parents and student-athlete have discussed the issues with the coach and a decision cannot be agreed upon the student-athlete must write an granting permission for the parents to discuss, everything mentioned above, with the AD. 4) If a decision cannot be agreed upon with the AD, the student-athlete must write an to the Campus President granting the President permission to talk with the student-athletes parent/guardian. 5) The Campus President will make the final decision. III. Eligibility Rules and Regulations Academic eligibility is essential in order for a student to be a member of a Keiser University extra-curricular activity. Student-athletes must be full-time students and pass the minimum required credit hours and maintain the required GPA outlined by the NAIA in order to be eligible to participate in any/all athletic contests. The University enforces the rules of the NAIA, TSC, NCA, NDA, and other various associations with which the University is affiliated. Academics and earning a degree are the main reason you are at the University. Students are expected to attend all classes and complete all assignments. All student- athletes must meet all applicable minimum academic standards to compete inter-collegiately on behalf of the University. The University is affiliated with the NAIA (National Association of Intercollegiate Athletics), TSC (The Sun Conference), NCA (National Cheer Association), and the NDA (National Dance Association). Listed below are certain guidelines that outline eligibility rules to compete. The following is a basic checklist; please contact the Athletic Department for additional information. Further, these rules may change over time. A. National Association of Intercollegiate Athletics (NAIA) General Rules A student-athlete must accumulate a minimum of 24 institutional credit hours in his/her last two full time semesters immediately previous the current term of attendance. These are always taken at face value, even if a transfer is involved and the current institution does not accept all hours. Students must be enrolled in a minimum of 12 credit hours per term (15 is recommended). Incoming freshman An entering freshman student must be a graduate of an accredited high school or be accepted as a regular student in good standing as defined by the enrolling institution. 17

18 An entering freshman must also meet at least two of the listed three minimum requirements to be eligible for intercollegiate competition at the University. 1) A minimum score of 18 on the Enhanced ACT or 860 on the SAT. 2) An overall high school grade point average of 2.0 or higher on a 4.0 scale. 3) Graduate in the upper half of the student's high school graduating class. Each student-athlete must also be identified and enrolled in a minimum of 12 institutional credit hours at the time of participation, or, if the participation takes place between terms, the student-athlete must have been identified with the institution the term immediately before the date of competition. Transfer Students Rules and regulations regarding eligibility for transfer students are very detailed. All transfer students are required to meet with Faculty Athletic Representative to determine eligibility. Play NAIA All first time athletic participants at the NAIA level must register and receive an ELIGIBLE determination with play NAIA ( prior to participating in order to determine initial eligibility. For more information contact the head coach or Assistant Athletic Director for Compliance. Please contact the Athletic Department for more information about NAIA eligibility. B. National Dance Association (NDA) Eligibility Rules and Regulations The University requires all NDA student-athletes must maintain a cumulative GPA of at least 2.0 by the beginning of a student -athlete's second year of eligibility. A student-athlete must accumulate a minimum of 24 institutional credit hours in his/her last two full time semesters immediately previous the current term of attendance. Each participating team member must be a current full-time student of the institution. Graduating seniors and graduate students may be enrolled in fewer hours. NDA student athletes shall be limited to ten (10) semesters of eligibility, and four (4) years of eligibility. 18

19 IV. Athletic Training A. Purpose Statement The Athletic Training team at Keiser University has a primary goal to provide the best medical care possible to its studentathletes while doing so in a professional, efficient, and service-orientated manner. The staff and its medical team assume the primary responsibility of injury care, injury prevention, and rehabilitation for student-athlete injuries. Our commitment is to provide this quality health care while educating the student-athlete on good health practices and injury maintenance. All athletic injuries are to be reported immediately to a coach and athletic trainer for evaluation and care. After an evaluation of the reported injury, the coach, athletic trainer, and student-athlete will work together in treating the injury through appropriate means. It is the student-athlete's responsibility to report back to the coach and athletic trainer if the injury becomes worse, persists, or if an outside physician is consulted. B. ATHLETIC TRAINING ROOM RULES: Failure to comply with any of these rules may result in your removal from the athletic training room or may revoke athletic training privileges All athletes must sign-in prior to getting treatment. Before getting taped, rehab must be performed. All personal items must be placed on the shelves located by the door. No tobacco products, No food, and drinks must have lids. No cell phones, headphones, and/or tablets during evaluations or rehab. Appropriate clothing must be worn at all times, no cleats. No profanity or other derogatory/abusive language. No loud or obnoxious behavior. Athletes must shower before post-practice/post-game whirlpool or modality use, unless authorized by athletic trainer. Return all equipment, supplies, etc. to the appropriate place when done using. Clean up after yourselves (i.e. Put used towels in hamper, throw away trash). Nothing leaves the athletic training room unless approved by an athletic trainer. Athlete is responsible for all medical equipment issued to them. Equipment should be returned as soon as it no longer needs to be used. Loss or damage to equipment will be the financial responsibility of the athlete. Athletes may obtain ice and ice only, when athletic training room is closed. Equipment may not be used without prior approval, especially modalities. All athletes are required to have active primary medical insurance coverage that is approved by Keiser University. All athletes must have a current copy (front and back) of their medical insurance card on file with the athletic training department. All staff is to be treated in a professional, courteous, and respectful manner. Failure to do so will revoke athletic training room privileges. Only KU student athletes and staff are permitted in the athletic training room. It is the athlete s responsibility to get to practice/game on time. Plan accordingly. Limited staffing & facilities=wait time. If an injury/illness is not reported until practice time, athlete is expected to participate. If an athlete cannot make or will be late for an appointment, they must notify the respective athletic trainer immediately. Failure to, may result in refusal of treatment. Athletes are not allowed in the staff office without authorization from an athletic trainer. Athletes are not permitted to dictate their own treatment. All treatments are at the discretion of a staff athletic trainer. 19

20 C. Sport Coverage/ Athlete Treatment Sports that are in-season, especially post-season play, will get priority in regards to treatment times and athletic training room space. We will see and treat everyone but sometimes you may have to be patient as there are over 550 athletes. It is the athlete s responsibility to get to practice/game on time. Plan accordingly. Limited staffing & facilities=wait time. There will be a sign-up sheet for rehab sessions. D. Return to Play Policy Detailed information regarding our concussion policy may be found under student-athletes tab on KU Seahawks website. This policy states that a Keiser University approved team physicians has the final authority regarding full clearance for student- athletes returning to play. All doctors will be chosen by the Athletic Trainers. The current team Orthopedic Doctor is Dr. Anand Panchal, D.O. The team Concussion Doctor is Dr. Evan Peck, M.D. Colleagues of these two physicians will be consulted for specific cases based on need. The NCAA Sports Medicine Handbook is used by Keiser University and the NAIA as a guideline for medical recommendations to ensure a safe and competitive environment for athletic participation. E. Emergency Situations In the event of an emergency you may be called upon to help retrieve equipment and/or help with the direction of EMS services (ambulance or fire truck). Spine board and straps are located behind the taping tables, against the wall, to the right when you walk into the athletic training room (ATR). Splint bag (red with blue handles) and red emergency kit (with AED) will be on-site for home games. During practices, they will be located in the ATR under the first two treatment tables. o Soccer- under the clock table o Basketball, Volleyball- behind the home bench o Baseball, Softball- on golf cart or in dugout if raining Crutches are located behind file cabinet/ next to storage cabinet in the back left corner Physicals, Medications All athletes must have a physical done annually by an MD, DO, ARNP or PA. They are provided by Keiser University on specified dates but you are able to get one on your own. However, our paperwork must be used. A copy of your primary insurance card must be on file before you will be cleared to participate. If you choose to use the primary policy offered by Keiser University please bring a print out from the website confirming you are enrolled in the policy. If you have asthma and use an inhaler we will need an inhaler with your name on it in the ATR or in one of our sideline kits. If you have allergies and have an epi-pen, we will need one with your name on it in the ATR or in one of our sideline kits. Athletic Accident Insurance The Keiser University Athletic Department requires all student-athletes to be covered by valid primary insurance, from an American based company. International students will be enrolled in the Student Health Insurance Plan (SHIP policy) offered by Keiser University. No international or other insurance plans will be accepted for international students as their primary insurance coverage. The cost of this policy will be automatically included on every student's bill. Domestic students have the ability to waive this policy with qualified health plans at the beginning of the academic year. If at any time during the academic year, the studentathlete s coverage changes or lapses, it is the responsibility of the student-athlete to notify the athletic training staff and to renew immediately. 20

21 Student-athletes without current primary coverage will not be allowed to participate until updated coverage can be proven. Keiser University will not be responsible for any expenses that result from lack of primary insurance. All bills must first be submitted to the student-athletes primary insurance for payment. The Keiser University athletic department carries an Athletic Insurance policy to assist in the payments of medical services for its student- athletes injured during athletic participation. Keiser University is not responsible for the payment of medical bills. Failure to follow the proper procedures for obtaining treatment and submitting bills may result in denial of the claim. Coverage Keiser University provides secondary athletic accident coverage for its student-athletes participating in collegiate athletics. This policy provides coverage for outstanding medical bills incurred as a direct result of an athletic accident after your primary insurance coverage has been exceeded. An athletic accident is defined as an unexpected sudden and definable event, which is the direct cause of bodily injury, independent of any illness, prior injury, or congenital predisposition. This event must occur during a Keiser University Inter-Collegiate Sports event. All bills must first be submitted to the student-athletes primary insurance for payment. Remaining balances and deductibles will then be submitted to the secondary insurance. Coverage is only extended for events and activities that are authorized, organized or directly supervised by an official representative of Keiser University. This includes practices and games during both the traditional and non-traditional seasons. The secondary insurance will only be applied after the student-athletes primary insurance has been paid. Coverage is NOT provided for medical expenses resulting from illness, disease, or conditions unrelated to accidental injury while participating in intercollegiate activities at Keiser University. The policy DOES NOT cover general medical illnesses, or non-athletic related injuries or conditions. Additionally preexisting conditions and routine medical care (eye care, dental care or illness) are NOT covered. The benefit period for all eligible expenses is 1 year from original date of injury. Failure to provide necessary information in a timely manner may result in non-payment of the claim. This provision seizes when the injury has been deemed healed, to the extent reasonably possible. A claim submitted to the University for processing with our secondary insurance carrier does not guarantee payment of the claim. Student-athlete is responsible to make sure proper procedures required by their primary insurance plan are followed. Failure to do so can result in a denial of payment from the Universities secondary insurance carrier. The University is not responsible for out of network claims. Submitting Claims The Athletic Training Staff must submit the claim for your injury to the secondary insurance company. The insurance company will require additional information to process the payment. It is the responsibility of the athlete to submit all invoices, EOBs and other necessary paperwork for reimbursement. Please consult the Athletic Training staff for directions on how to submit invoices. 21

22 Primary Insurance Keiser University requires its student-athlete to have primary health insurance from an American based company. As mentioned above, international students must take the Keiser University SHIP policy as their primary insurance. Domestic students may waive out of institution insurance if they are already covered. Below is the process for opting out. For more detailed information on athletic insurance, medical procedures, athletic training room polices, and directions on submitting bills, please contact a member of the Athletic Training Staff. V. Equipment/Athletic Apparel If the University provides athletic equipment and apparel for its athletes, and if the equipment/apparel is lost, misplaced or stolen the student- athlete maybe financially responsible for replacing the items. All equipment is the property of the University and must be returned at the end of each athletic season/discontinuance of athletic career as determined by the University. Equipment/Apparel not returned within one week of season end, could result in the student-athlete's transcripts being withheld until replacement cost are paid to the University. Specific teams may impose additional consequences. Equipment/apparel provided by the University should only be used in conjunction with the University's athletic program. Failure to follow this policy could result in suspension, loss of eligibility or loss of other privileges determined by the coaching staff and/or the Athletic Department. VI. Facilities The University provides facilities on and off campus to aid in the pursuit of athletic excellence. Facilities, including weight rooms, locker rooms, and other athletic facilities, are only to be used under the supervision of a coach or another authorized member of the Athletic Department. Under no circumstances is a studentathlete to use a facility for recreational purposes or to aid persons not approved by the Athletic Department in accessing facilities, except as authorized by the University. The use of tobacco of any form by our student athletes is strictly prohibited at any university owned or leased facilities or sponsored athletic events. Failure to follow these rules could result in loss of eligibility, suspension or other consequences. ALCOHOL AND TOBACCO PRODUCTS (NAIA rule at all NAIA facilities and events) The use of alcohol, tobacco or tobacco products is not permitted by participants, coaches, cheerleaders, athletic trainers, game administrators, or officials in the playing areas during all NAIA regular season and national tournament competition and practices for such competition. The institution s coaching staff, event games committee, administrator or host site coordinator (as applicable) shall enforce this policy through the following steps. a) On the first offense, the individual(s) shall receive an official warning. b) A second offense will cause expulsion from the game, tournament or event. 22
