He will share how these shifts impact older adults and help define ways we can become more aware, informed and involved. 1. SESSION 4: OUTCOME MEASURES IN LOWER LIMB PROSTHETICS: EMPOWERING THROUGH MOBILITY Justin Pfaff LCPO, Regional Clinical Manager Hanger Clinic SESSION ABSTRACT: By utilizing validated mobility outcomes for lower limb prosthetic patients, health care teams can empower patients to understand and participate in their own care leading to higher satisfaction. This introductory level course designed for nurses and case managers teaches on new methodology on collecting outcomes, utilizing the Prosthetics Limb Users Survey of Mobility (PLUS-M) self-reporting instrument. This course ensures the learner understands patient and output research trends, and more specifically, how health care teams can collaborate with physicians and patients alike to improve patient care. 1. Communicate the benefits of using outcome measures with lower limb prosthetic patients 2. Discuss the Prosthetics Limb User Survey of Mobility (PLUS-M) assessment, how it was developed, and how it is used to assess mobility for patients that use lower limb prosthetics to provide objective outcomes 3. Define correlation with outcomes and standard of care SESSION 5: NURSE NAVIGATION: ACROSS THE CONTINUUM OF CARE Amy Scott, BSN, RN Registered Nurse, Population Health and Clinical Quality Austin Regional Clinic Winonah Hoffman Director, Population Health and Clinical Quality Austin Regional Clinic SESSION ABSTRACT: This session will provide a discussion of the Nurse Navigator program in detail and how to use centralized and embedded RN s to help our most at risk patients. We will detail how the Nurse Navigator program impacts patient compliance, therefore decreasing ER visits and hospitalizations. This session will also elaborate on the working relationship between the Nurse Navigator program and other entities such as hospital case managers, insurance groups, home health, palliative care and hospice, helps stabilize patient health, close care gaps, and empower patients. Finally, our discussion with touch on how the program is assisting clinics to transition for fee for service care to value based care. 1. Define Population Health and Nurse Navigation 2. Identify the relationship between Hospital Case Manager and Clinic Nurse Navigator 3. Illustrate the effects of Nurse Navigation on hospitalizations, re-hospitalizations, ER visits and overall patient health
