Hawke s Bay District Health Board Position Profile / Terms & Conditions Position holder (title) Registered Health Practitioner Reports to (title) Department / Service Purpose of the position Clinical Team Leader Pregnancy and Parental Service (PPS) Community Mental Health Provision of high quality case consultation, co-ordination and case management services to clients who are pregnant or parents of children under three years of age, who experience Alcohol and Drug issues and are poorly connected to health and social services. PPS aims to reduce harm and improve the wellbeing of children by addressing the needs of parents using a strength based approach and working to strengthen the family environment. Key Accountabilities SERVICE PROVISION Provide a community based outreach service to Patients/consumers/Whaiora Facilitate engagement and retention of Whaiora in alcohol and other drug services, antenatal and post natal services. Develop and implement case co-ordination/goal plans in conjunction with Whaiora and relevant stakeholders. Utilise relevant expertise to contribute to multidisciplinary review. Co-ordination of Well-Child services. Screening for child protection issues are in accordance with the MHS Child Protection Standard Operating Procedure and HBDHB abuse reporting policy guidelines. Provide advocacy and support on custody, and care and protection issues. ASSESSMENT, INTERVENTION & EVALUATION Provide a range of clinical/social Work/ Occupational Therapy/Nursing/Psychology interventions which will include - Assessment - Case management - Crisis management - Referral - Liaison - Case consultation - Case co-ordination - Multidisciplinary input Provide assessment and appropriate referral and advice for domestic violence issues. Applies a range of clinical interventions and critical thinking to support the service user and family. Coordinates the involvement of other services to provide a range of choices for the service user and family. Professional standards of practice and ethics are utilised. Networks are developed and strenghtened. Ongoing evaluation methods used. Reduction of harm from the effects of alcohol and drug use to pregnancy, unborn child and children. Coordinated provision of services to pregnant and parenting Whaiora and their families/ significant others. Increased awareness and management of Whaiora s specific issues. Whaiora s child(ren) receive ongoing health and development assessments and appropriate interventions. Appropriate action plans are developed and implemented in conjunction with child protection services. Referred Whaiora are assessed to determine A&D intervention and bio psychosocial needs. Provide crisis intervention as required to ensure the immediate safety of persons concerned. Develop therapeutic alliance with the Whaiora acknowledging and respecting diversity of cultural norms. Treatment/referral processes are implemented using a multi-disciplinary approach and active Whaiora participation in decision making. Clinician attends and presents cases weekly at MDT Clinician attends for weekly risk management meetings To reduce harm to families from domestic violence Internal and external liaison focuses on appropriate and timely service delivery. Practice standards and ethical conduct conforms to the relevant statutory codes. Evidence of service user and family notification is held in the clinical file. Cases are actioned in a timely manner. File audits will identify: - Assessments are comprehensive and cover all relevant aspects of the PPS process. - Assessments meet service requirements

RELATIONSHIP WITH WHAIORA AND WHANAU Assist client engage with parenting interventions. Provide advocacy and support on parenting issues. Mental health issues are screened and appropriate referral is made. Education is provided on HIV, Hepatitis C and STD s. Screening is provided as necessary. Clients are referred to appropriate treatment interventions. Alcohol and drug screening and provision of brief interventions. Provide advice and assistance for Whaiora to access support services in direct relation to their social and welfare requirements. Assist in developing and facilitating groups with an education and support focus to pregnant and parenting clients. Include whanau and/or significant others in goal planning as directed by client. Families are supported to keep their children safe. Whaiora receives information and support regarding parenting support and skills. Whaiora is able to parent in a safe and nurturing way. Whaiora have access to appropriate mental health treatment. Whaiora has information about HIV, Hepatitis C and STD s and access to screening. Whaiora has strategies that reduce disease transmission, improve resistance to disease and promote good health. Whaiora acesses care related to HIV, Hepatitis C and STD s. Access Whaiora who are not engaged with health and social services with the aim of engaging them in these services. Development of an appropriate goal plan with Whaiora. Social and welfare needs will be met enabling clients to address alcohol and drug issues. To increase Whaiora s knowledge to empower them to make positive life changes and to facilitate whanau to support each other. PROFESSIONAL PRACTICE Develop and maintain working relationships with community health, social services and Lead Maternity carers. Provide consultation and education for MHS clinicians, other health and social agencies on issues relating to drug abuse and parenting/ social needs. Communication skills both verbal and written are expert. Cultural diversity is recognised within clinical practice and people are treated with respect. Undertakes regular clinical supervision within own profession. Linking wit Professional Advisor and colleagues, participates in specific Professional development activities. Maintains Competency Certificate through relevant Professional Registration Board. Registration is maintained. Professional education and training is attended. Improve whanau and/or significant others understanding of Whaiora needs so they are better able to support them. Ensure sound communication of relevant information takes place between professionals involved in the care of pregnant and parenting Whaiora. Raise awareness and increase knowledge of needs specific to pregnant and parenting Whaiora in order to improve service provision. Service user and family satisfaction surveys are completed annually and reflect the above. Case worker roles are established and evidenced in the clinical file. Knowledge of professional ethics and professional standards are evident. A training plan is developed agreed training undertaken. Clinical standards of practice are maintained and enhanced. Annual practicing certificate is current.

Applies knowledge of legislation / policies that impact on clinical practice. Minimum of 12 supervision sessions attended in a 12 month period. Supervision record evidences this. Supervision contract is on personnel file. Demonstrates knowledge of legislation / policies. PLANNING AND PROCESSES Proactive approach to service planning and review processes that guide actions and decision making so as to effect smooth day to day service delivery and continuous quality improvement. This includes policies, standards, audits, research, quality, training. Continuing professional development log is up to date. Participate fully in service implementation of MHS and HBDHB Policy/ Procedure, Practice Guidelines and Practice Standards and statistical reporting requirements. Participate in and contribute to organisational reviews and audits of Policy/Procedure, Practice Guidelines and Clinical Practice Standards. Actively participate in service and MHS wide activities that promote the development of best practice standards within HBDHB MHS. PROFESSIONAL STANDARDS & SUPERVISION To meet Hawke s Bay District Health Board s Standards (i.e. legislative, professional, contractual, ethical and organisation) by knowing what the applicable standards are and undertaking any steps necessary to remedy shortfalls in practice and knowledge. Develops supervision contract and receives supervision in accordance to the relevant professional guidelines, registration requirements and organisational policies. Participate in and promote all research and quality initiatives within the service. Continue to develop own knowledge base and skills as required to function effectively within the alcohol and drug field generally and within MHS and PPS specifically. Develop and maintain a high level of expertise in all issues relating to the clinical management of pregnant or parenting substance using clients, keeping abreast of new developments and research. Attend regular supervision for purposes of reflecting on practice, enhancing clinical skills and adhering to HBDHB best practice standards and outputs. Attends supervision as per standards for professional registration. Evidence of superiviviosn as per HBDHB Professional Supervision Policy.
