Program Descriptions Families and Children: Families and Children is a managed care program that covers health care for the following people who have Medical Assistance: Children under the age of 21 Parents and caretakers of a dependent child Pregnant women Low income adults without a dependent child MinnesotaCare: MinnesotaCare is a managed care program that covers health care for people who do not have access to affordable health care coverage. Some members may be required to pay a premium to the State. Minnesota Senior Care Plus (MSC+): MSC+ is a managed care program that covers health care and Elderly Waiver services for people who: are age 65 and over, and have Medical Assistance. If you are enrolled in MSC+, you may have another option. Check with your county financial worker to see if you can enroll in the Minnesota Senior Health Options (MSHO) program. MSHO is a voluntary program. Important Information Clinics The clinic information printed in this listing was correct as of October 2017. You need to choose a primary care clinic when you enroll. Your primary care clinic can provide most of the health care services you need and will help coordinate your care. Please confirm with Customer Services that your primary care clinic is still a provider with our plan before signing up. If you think you need to see a specialist, your primary care physician can help you make that decision. Check this listing or call Customer Services. How to choose a clinic: Select the primary care clinic from this listing that you plan to visit most often for preventive care and annual exams. Then write the six digit primary care clinic code on your enrollment form. To ensure that UCare is able to coordinate your care with your provider, be sure to contact UCare Customer Services at the numbers on page i if you change your primary care clinic. Health Care Homes Health Care Homes (also known as s) are health care providers that have been certified by the State of Minnesota. The program is designed to provide additional care coordination for members who need additional help with chronic or complex health care conditions. There is a process that members must follow to participate with a Health Care Home in our network. Contact any of the Health Care Home providers included in this listing for more information about their program. Health Care Homes are designated by an asterisk (*) in this listing. Cost Sharing You may be required to pay an amount toward some health care services. This is called cost sharing. Some examples of cost sharing are copays or deductibles for certain services. You are responsible to pay your cost sharing amount to your provider. Member Identification (ID) Cards and Member/Welcome Packet Members will get a health plan Member Identification (ID) Card. You must show this card whenever you get health care services, including when you get a prescription filled. You will get your ID Card and a new member packet containing more information about UCare and your benefits soon after you are enrolled. Service Accessibility If you need special access to get services from a provider, you can obtain information by calling Customer Services at 612-676-3200 or 1-800-203-7225. TTY/hearing impaired call 612-676-6810 or 1-800-688-2534. This could include, but is not limited to, access for services such as availability of flexible hours, wheelchair access, or parking lot access. Service Authorization Our approval is needed for some services to be covered. You must get the approval before you get certain services or before we pay for them. For those services, your provider will be required to obtain prior authorization from us. To see a doctor or specialist who is not in UCare s network, you or your primary care provider must obtain a service authorization from UCare in order for those services to be covered. Members will get a Member Handbook with more information about which services require authorization. Services Case Management for MSC+ members When you enroll in the MSC+ program, you choose a primary care clinic. If you do not select a primary care clinic, you will be assigned one. After you enroll, you will be assigned a UCare Case Manager, who will offer you an initial health risk assessment and will work with you to coordinate the care and services you need. Your UCare Case Manager s job is to help you connect to services and get the most out of your benefits with the least amount of hassle and paperwork. Whether you re at home, in the hospital, or in a nursing home, you ll have one person to contact who understands and makes it easier for you to receive the services you need. You, your doctors, and others providing your care can work together with your UCare Case Manager on your behalf. Whenever you have questions, concerns, or need services, you can simply call your Case Manager. If you are getting home and community based services through the Elderly Waiver at the time you enroll, your UCare Case Manager will help you coordinate these services. If you reside in the community and are not getting home and community based services through the Elderly Waiver at the time of enrollment, your first screening will include an assessment of your need for these services. If you are assessed as eligible for home and community based services under the Elderly Waiver, your Case Manager will work with you to get these services. Members in the MSC+ program residing in the community or in nursing homes will be assessed annually for their ongoing care needs. You can get more information on case management services by calling Customer Services at the numbers listed on page i. Chemical Dependency Chemical Dependency services provide care and treatment for chemical substance abuse. This can include abuse of alcohol, narcotics or other drugs. UCare uses a network of chemical dependency providers in your community. You can get current information about those providers by using the Find a Doctor feature on ucare.org or by calling Customer Services at the numbers listed on page i. After hours and on weekends, members can call the UCare 24/7 Families and Children/MinnesotaCare/Minnesota Senior Care Plus PCNL, October 2017 iii

nurse line for help with finding provider information. You can find the phone number for the nurse line on the back of your Member Identification card. Chiropractic Care Chiropractic benefits are managed through Fulcrum Health (formerly known as Chiropractic Care of Minnesota, Inc.). A list of providers will be in the member packet you ll receive when you enroll with UCare. You can get more information on how to access chiropractic services by calling UCare Customer Services at the numbers listed on page i or by using the Find a Chiropractor feature on ucare.org. Dental UCare contracts with Delta Dental of Minnesota (Delta Dental) to manage your dental benefits. For more information on UCare s dental services, you can go online to www.dentalcareforu.org, or call the UCare Dental Connection at 651-768-1415, or 1-855-648-1415 toll free from 7 a.m. to 7 p.m., Monday through Friday. TTY users can call 711. The UCare Dental Connection can help you: Find a Delta Dental CivicSmiles provider or dental home. Schedule dental appointments, including appointments for follow up and specialty care. Coordinate transportation to dental appointments. Coordinate interpreter services for dental appointments. Answer dental benefit and claims questions Elderly Waiver Home and Community Based Services for MSC+ members If you are age 65 or over and need help to stay in your home, a program called Elderly Waiver (EW) can help. EW offers more services than regular Medical Assistance. Services may include visits by a nurse, home care services, homemaker services, chore services, and more. Your Case Manager can arrange these services for you. If you want more information, please call the Customer Services numbers listed on page i. Elderly Waiver Obligation for MSC+ members If you get EW services and your income is over the income limit, you may have to pay a monthly amount. This is called the EW waiver obligation. Your county will tell you about any EW waiver obligation when your eligibility is determined. Health Education As a UCare member, you can protect yourself and your family from serious illness by completing your annual checkup with a primary care physician at no charge. Ask your doctor which tests, screenings and shots are right for you. Get answers to questions about your health and the choices you are making. See the back page of this booklet for fitness and wellness options available to you. Home Care Home Care services include the following: Skilled nursing Rehabilitation therapies (for example: speech, physical, respiratory and occupational) Home health aide Medical equipment and supplies Personal Care Assistant (PCA) Home Care Nursing (HCN) services For more information, including how you can access home care services, please call UCare Customer Services at the numbers listed on page i. s The hospitals in the listing are contracted with UCare. If you do not know which hospital your doctor uses, ask. Unless it is an emergency, you should use a hospital that is in UCare s provider network, and is associated with your doctor. Remember to call your primary care clinic before going to the hospital, except in an emergency situation. Interpreter Services American Sign Language (ASL) and language interpreter services are available to help you get services. Oral interpretation can be given to you in your language. You may be able to get written information in your language. Face to face oral language interpreter services are only covered by a UCare contracted agency, and the interpreter must be listed on the Minnesota Department of Health s Spoken Language Health Care Interpreter Roster. For more information, call UCare Customer Services at the numbers listed on page i. Medical Equipment and Supplies Medical Equipment and Supplies is any medical equipment you can use in the home to improve quality of living. It can also provide therapeutic benefits to people with certain medical conditions or illnesses. Examples are nebulizers, wheelchairs, crutches and many others. You can get information on Medical Equipment and Supplies by calling UCare Customer Services at the numbers listed on page i. Mental Health Mental health services include care and treatment for depression, anxiety or other mental health issues that interfere with your daily functioning. You can get current information about mental health providers by using the Find a Doctor feature on ucare.org or calling Customer Services at the numbers listed on page i. After hours and on weekends, members can call the UCare 24/7 nurse line for help with finding provider information. You can find the phone number for the nurse line on the back of your Member Identification card. Nursing Home and Skilled Nursing Home Care for MSC+ members For MSC+ members, we will cover a total of 180 days of nursing home care. If you need nursing home care for more than 180 days, the Minnesota Department of Human Services (DHS) will pay for your care. This applies unless you were already in the nursing home when you joined our plan. You can get more information on how to access nursing home services by calling UCare Customer Services at the numbers listed on page i. Open Access Services For some services, you may choose to get care from any health care provider, clinic, hospital, pharmacy, or family planning agency even if it is not in our network. These are called open access services and they include the following: Family planning Diagnosis of infertility Testing and treatment of sexually transmitted diseases Testing for AIDS or HIV related conditions You can get more information on open access services by calling UCare Customer Services at the numbers listed on page i. Pharmacy Members must use our network of pharmacies for drugs covered by our health plan. You will get a list of pharmacies in the member packet you receive when you enroll with UCare. The drug must be on our covered drug list (formulary). Visit ucare.org for the most current formulary and pharmacy network information. Remember to have the following available when you call to refill your prescription: UCare Member Identification (ID) Card Minnesota Health Care Programs card Cards for any other health coverage you have, such as Medicare Part D or private insurance It is important to remember to show both your Member Identification (ID) card and your Minnesota Health Care Programs card when picking up your prescriptions. People with Medicare must enroll in a Medicare Prescription Drug Program (Medicare Part D) to get most of their drugs. You can get more information on pharmacy services by calling UCare Customer Services at the numbers listed on page i. Specialty Care Specialty care describes services you can get from doctors or surgeons whose training focuses primarily in one specific field of medicine. Examples include neurology, cardiology, and oncology, among others. UCare members can see any specialist in the UCare network without a referral. You can get more information on how to access specialty care by calling UCare Customer Services at the numbers listed on page i, from the Provider Directory you will receive when you enroll with UCare, or by using the Find a Doctor feature at ucare.org. Transition of Care When you join UCare we will help you continue to get necessary health care services that were authorized before you joined our plan. Some examples are the following: Orthodontia Mental health services Chemical dependency services Services for at risk pregnancy Prescriptions If you are currently getting care from a provider who is not in our provider network, we will help you transition to a participating provider. Transportation for MSC+ and Families and Children members Medical transportation is available for MinnesotaCare Children under age 21 through December 31, 2017 and under age 19 effective January 1, 2018. We are not required to provide transportation to your primary care clinic if it is over 30 miles from your home if another primary care clinic is available within 30 miles. Exceptions to this rule can be made in certain circumstances. If you need a ride and there isn't a closer clinic in our network please vi call our Health Ride service at 612-676-6830 or 1-800-864-2157 toll free. If you are hearing impaired, please call the TTY number at 612-676-6810 or 1-800-688-2534 toll free or 711 for Minnesota Relay Service. UCare covers rides to a specialty clinic that are 60 miles or less. Rides to a specialty clinic farther than 60 miles from your home are generally not covered, but may be approved by UCare under certain circumstances. If you have questions, please call Health Ride. You may also be eligible for network Special Transportation Services (STS) if needed. To be eligible for STS transportation, you must have a valid Certificate of Need filled out by your physician. If you have questions, or need help arranging a ride with a participating transportation provider, please call UCare Health Ride. Health Ride is not available to MinnesotaCare members, with the exception of transportation to and from mammograms and colon cancer screenings. Medical transportation is available for MinnesotaCare children under age 21 through December 31, 2017 and under age 19 effective January 1, 2018. Members will get a Member Handbook with more information about transportation. Urgent/After Hours/Emergency Care We know that you might need medical attention before your next doctor appointment. Plan members have several options available to them. Call your doctor s office for instructions on how to get urgent care or after hours care. You can also call the UCare 24/7 nurse line for advice on getting the best treatment for your health issue. The nurse line is answered by nurses 24 hours a day, seven days a week. You can find the number for the UCare 24/7 nurse line on the back of your Member Identification Card. You must call UCare s Customer Services Department within 48 hours, or as soon as you are able, if you are admitted to an out of network hospital.
