July, 2013 INTRODUCTION: In an effrt t prvide a safe envirnment fr ur individuals, visitrs and staff, this Emergency Preparedness plan has been develped fr ur facility at 24141 JEB Stuart Hwy. in Stuart, VA. All staff members are required t review and be familiar with this plan and act in accrdance with the guidance included in this plan. While this plan is intended t utline steps fr staff t take in emergency situatins, staff shuld always fllw the directin f the prfessinal emergency services persnnel. Any health and safety issues as well as fire hazards r ther risks shuld be reprted t the PC Hrizns Safety Officer (Prgram Manager) immediately. In additin, a wrk rder shuld be cmpleted indicating that a safety hazard exists. EMERGENCY RESPONSE PLAN: NATURAL DISASTER OR SEVERE WEATHER: In the event f a natural disaster r severe weather such as severe strm, earthquake, trnad, fld etc., all staff is instructed t fllw emergency bradcast instructins. If cnditins warrant and an evacuatin t a SAFE AREA is indicated, the basement r hallway and surrunding area is designated as the safe area fr this facility. All staff members, clients and visitrs shuld remain in the designated area until ntified that the threat has passed. FIRE: In the event f a fire, staff shall take apprpriate actin with a fire extinguisher (lcated in the stairwells at either end f the building) r ther means immediately available t them. Staff SHOULD NOT put themselves r thers at risk. If the fire is nt immediately extinguished: Ntify all building ccupants verbally, r pressing buttns n fire alarm. Staff shuld call 9 1 1 (when and where it is safe t d s) t ntify emergency persnnel. When a fire alarm has sunded the safety fficer, r designee in their absence, shall: Cnfirm that 9 1 1 has been ntified Make a circle thrugh the building (if safe t d s) t be sure all staff and visitrs are aware f the emergency and are mving ut f the building withut difficulty. Make certain that peple in the waiting rms and rest rms are aware f the emergency and instruct them t exit the building. When a fire alarm has sunded each staff shall: Direct any persn in their ffice/area t immediately exit the building. Prvide assistance if necessary. Ntify the Safety Officer f any persn needing assistance r refusing t exit the building. Clse drs in their ffice/area. Lend assistance t ther staff as necessary and apprpriate. Exit the building After reaching the designated assembly pint, ntify the Safety Officer f any persn pssibly still in the building. EVACUATING THE BUILDING: All persns shuld prceed t the nearest exit. Escape rutes are psted in the hallways thrughut the building. If any exit is blcked due t fire, excessive smke, debris, etc., prceed t an alternate exit. Designated assembly pints are as fllws: Tree in middle f frnt yard, ff the drive way and ut f the way f any emergency persnnel. The Safety Officer and Staff shall nte the presence f all expected staff, clients and visitrs within the assembled areas. A cunt f persns present shall be taken by the Safety Officer r designee. If anyne is absent and pssibly still within the building, the Safety Officer r emergency services persnnel shuld be ntified immediately. This ntificatin shuld include any infrmatin available n the missing persn(s) pssible lcatin in the building. Page 2
OFFICE EMERGENCY OR THREATENING SITUATION If yu experience an emergency r threatening situatin in yur ffice: 1. Alert ther staff t the threat by asking fr them t bring yu a Dr. Pepper. a. Staff shuld then call extensin f the area in questin and ask: i. D yu need help? ii. Shuld I call 9 1 1? iii. Answers shuld nly be yes r n. b. If indicated, 9 1 1 shuld als be called. 2. When help is needed, the first persn n the scene takes the lead in emergency situatins, except in situatins where a security r law enfrcement fficer cmes n the scene. 3. Staff shuld always assume that a weapn is invlved until it is determined therwise and get the ther clients t a safe lcatin. 4. The first persn respnding shuld knck n the dr and enter if necessary. Staff members are encuraged t exercise discretin in decisins befre entering an ffice, but t always act in the interest f maintaining rder and preservatin f the lives and health f all invlved. 5. 9 1 1 is t be called anytime a weapn is threatened r bserved in the situatin. LOSS OF UTILITIES: If utilities are lst, all staff is encuraged t take apprpriate precautins t prtect everyne in building. If utilities are ff fr mre than 45 minutes, health and/r safety risks exist since there is n water r ability t flush tilets, ntify the Safety Officer immediately. The Safety Office may clse the prgram with crdinatin frm PSR Crdinatr r Management n call. Apprpriate steps will be taken t ntify staff/caretakers and any ther ffices. SEVERE INJURY: In the event f a severe injury, the staff persn first nticing the injury shall: Immediately ntify 9 1 1. Request assistance frm thers in the building until assistance arrives. Page 3
Initiate CPR and First Aid as needed. Staff shall als ntify the Safety Officer when a severe injury ccurs and cmplete any necessary dcumentatin. COORDINATION WITH LOCAL AUTHORITIES: The Human Resurces Directr and Facilities Manager will participate n the lcal emergency planning cmmittee fr Stuart& Henry Cunty t determine lcal disaster risks and cmmunity wide plans t address different disasters and emergency situatins. Infrmatin frm participatin n this cmmittee will be disseminated t all PCS Safety Managers. In additin, the Safety Officer fr Clay Street will register with the Stuart& Henry Cunty Citizens Alert System, which autmatically ntifies anyne, registered f ptential emergency situatins in ur service area. CRITICAL INCIDENT REPORTING: In the event f a disaster, fire, emergency r any ther cnditin that may jepardize the health, safety r welfare f individuals, staff shuld first respnd and stabilize the situatin. After the disaster/emergency is stabilized, staff shuld cmplete a Critical Incident Reprt. Cpies f the cmpleted reprt shall be given within ne wrking day t the immediate supervisr, Divisin Directr, Human Resurces Directr, Executive Directr and Safety Officer. The PCS Human Resurces Directr shuld reprt the disaster/emergency t DBHDS n later than 24 hurs fllwing the incident. FIRE DRILLS AND INSPECTIONS Within 24141 JEB Stuart Hwy., the Safety Officer r designee shall be respnsible fr: Quarterly inspectins f each fire extinguisher t assure that each unit is readily available and in gd perating cnditin. Als, each unit will be prfessinally inspected at least annually. Quarterly testing f emergency lighting and smke detectrs, as well as dcumentatin f the testing and subsequent findings. Annual testing f the autmatic sprinkler system, including dcumentatin f findings. Semi annual fire drills will be cnducted fr staff and shall include: Sunding the alarms. Practice evacuating the building. Practice alerting 9 1 1. False alarms cannt be cunted as fire drills. The Safety Officer r designee is respnsible fr ntifying the 9 1 1 and the Alarm Cmpany when false alarms ccur. Page 4

TRAINING IN FIRE PROCEDURES Each new staff member shall be trained in fire prcedures and fire drill prcedures upn their first day f emplyment. The immediate supervisr shall be respnsible fr the training and rientatin f each new emplyee. ACCESS TO TELEPHONES In the event f an emergency situatin, access t telephnes shall be made available. Instructins fr cntacting emergency services and the telephne numbers shall be prminently psted near the telephne including hw t cntact PCS medical staff as apprpriate. MAINTAINING A 24 HOUR TELEPHONE ANSWERING SYSTEM T maintain capability t respnd t emergencies fr individuals receiving services Piedmnt Cmmunity Services in partnership with lcal cmmunity agencies accesses a lcal htline knwn as CONTACT. CONTACT can be reached 24 hurs a day seven days a week by dialing 1 877 934 3576. Page 5
FIRST KITS A well stcked first aid kit shall be readily maintained and available fr minr injuries and medical emergencies. First Aid kits are lcated in the prgram manager s ffice and n each van. EMERGENCY LIGHTING / FLASHLIGHTS Emergency lighting has been installed thrughut the Facility. In the event f a pwer failure, emergency lighting will illuminate the hallways and exits f the facility. Testing f the emergency lighting will take place at each quarterly safety inspectin. Wrking flashlights are als placed thrughut the facility. Flashlights shuld be tested during mnthly fire drills. POISON CONTROL The telephne number(s) f the reginal pisn cntrl center shall be psted n r next t the telephnes as needed. USE OF VEHICLES Blue Ridge Pisn Cntrl Center 1 (800) 451 1428 Any transprtatin prvided by the prgram, directly r thrugh cntract, fr individuals receiving services shall be in cmpliance with state and federal laws relating t vehicle safety and maintenance. In the event f a vehicle emergency/ accident staff shall initiate the vehicle peratin plicy and prcedure. 1. Ensure safety f individuals in vehicle. 2. Call 911- If applicable, administer First Aid and/ r CPR- yu must get a plice reprt. 3. Ntify supervisr 4. Cntact n-call management 5. Ntify legal guardian, authrize representative, r family member as sn as pssible. 6. Gather accident infrmatin frm emergency management persnnel. 7. Cmplete a critical incident reprt and Lgisticare Vehicle Accident reprt (if individuals served are in the vehicle) accrding t plicy and prcedure. Page 6
CONTROL OF DEVIANT OR CRIMINAL BEHAVIOR The apprpriate Divisin Directr and/r Safety Officer shall be ntified when staff bserves a situatin that may lead t this type f behavir. The Directr and/r Safety Officer shall assure that all reasnable precautins are taken t assure that n persn is expsed t, r instigates such behavir as might be physically r emtinally injurius t himself r anther persn. HEALTH SAFETY AND WELFARE In the event f a disaster, fire, emergency r any ther cnditin that may jepardize the health, safety r welfare f any individuals at 24141 JEB Stuart Hwy., the Safety Officer r designee shall take apprpriate actin t prtect staff, visitrs and individuals receiving services and take apprpriate actins t remedy the cnditins as sn as pssible. TRAINING IN EMERGENCY PREPAREDNESS Emplyees, cntractrs, students and vlunteers shall be knwledgeable f and prepared t implement this Emergency Preparedness Plan in the event f an emergency. Staff are required t cmplete the training at hire and annually. Once the training is cmpleted, staff shuld cmplete the Emergency Preparedness Training Frm (lcated n the intranet) and frward it t Human Resurces. INVESTIGATIONS Any incident relating t the peratin f this facility which results in serius injury r death shall be investigated by the Safety Officer and the Human Resurces Directr, apprpriately reprted t lcal authrities and immediately reprted t DBHDS licensure and human rights ffices. A written reprt f the incident shall be made and kept n file and made available fr review by authrized persnnel. PEER TO PEER INCIDENTS Prviders are asked t reprt and investigate peer-t-peer cnflicts/incidents t determine if their staff members have vilated, under the prvisins f the Human Rights Regulatin, Part III, 12 VAC 35-115-50, Dignity, Sectin B2 and Dl, 2 and 3. 1. Incidents when an individual attacks anther individual using physical frce, such as punches, which actually cnnect t anther individual s bdy. 2. Allegatins f sexual assault, and r ther nn-cnsensual sexual acting ut (tuching f anther individual s private areas). Page 7
3. Verbal threats where the aggressr(s) has the means t carry ut this act. Fr example, a peer states, I am ging t kill yu, and the individual has a knife, r anther bject in his r her pssessin that will aid them in carrying ut the threat. Generally, prviders d nt reprt threats where there is n visible means fr the individual t carry ut their actins. There culd, hwever, be a situatin that culd lead t a very serius act f aggressin frm a mere threat. We ask the prvider lk int each f these activities and take the apprpriate actins internally t assure the safety f each individual invlved. Shuld the prvider determine, thrugh the internal investigatin, that there has been neglect n the part f the staff, then they are asked t cntact the OHR and prvide the fllw-up infrmatin deemed apprpriate. 4. Pushing and shving is nt generally reprted unless it results in an injury, r when the alleged victim feels threatened r intimidated by the aggressr. Obviusly, this determinatin can be very subjective. 5. Incidents f spitting are nt rutinely reprted t the OHR. If hwever, an individual has been diagnsed (knw by prvider) with a cmmunicable disease such as AIDS and /r Hepatitis and uses this type f act t harm anther peer, then we ask the prvider t reprt. 6.
