Hawke s Bay District Health Board Position Profile / Terms & Conditions Position holder (title) Reports to (title) Department / Service Social Worker Associate Clinical Manager Emergency Mental Health Team Older Persons, Mental Health & Allied Health Services The Social Work profession promotes social change, problem solving in human relationships and the empowerment and liberation of people to enhance well being. Utilising theories of human behaviour and social systems, social work intervenes at the points where people interact with their environments. Principles of human rights and social justice are fundamental to social work. Purpose of the position The registered social worker will be an active member of the integrated multi-disciplinary team, that provides a responsive 24/7 comprehensive assessment, diagnostic, planning and treatment service for persons experiencing a mental health emergency. Care and Treatment will be provided in accordance with both organisational and professional competencies and standards and will involve whanau/families/support persons. This position will be require work from the Emergency Department (ED)/Acute Assessment Unit (AAU) and will contribute to the Model of Care for patients in ED and formulate part of the wider multi-disciplinary team (MDT). Providing quality mental health care is paramount. The position will contribute to the provision of 24/7 Consultation Liaison Psychiatry ensuring general medicine and surgical services receive any necessary support and advice. Performing Duly Authorised Officer (DAO) responsibilities is expected. Support the delivery of organisational wide KPI s e.g., MOH targets, financial targets and the provision of patient safety and quality of care. Key Accountabilities PROFESSIONAL RESPONSIBILITY Has knowledge and judgement in professional, legal, ethical responsibilities and cultural safety. Applies knowledge of legislation / policies that impact on social work practice. Service user and family satisfaction surveys are completed annually and reflect the above. Case worker roles are established and evidenced in the clinical file. Knowledge of social work ethics and professional standards are evident. A training plan is developed agreed training undertaken. Clinical standards of practice are maintained and enhanced. Annual practicing certificate is current. Social Worker accesses regular and uninterupted supervision at least monthly and the record of supervision evidences this. Supervision contract is on personnel file. Demonstrates knowledge of legislation / policies. Continuing professional development log is maintained as per requirements of Social Work Registrtaion Board. ASSESSMENT A comprehensive mental health assessment, is completed with all service users. Assessments will involve the client and family where possible and other sources of information including previous documentation as appropriate. Assessments are timely, inclusive and meet, documentation standards. Social Work clinical standards of practice are maintained. Evidence that integration with other services has been implemented and they are involved in the review of goals. Informed consent, or its absence is evidenced and explained. Service user feedback reveals inclusion and partnership. Attends relevent network meetings. File audits will identify:- Interventions are clearly documented in case notes. INTERVENTION AND EVALUATION Applies a range of clinical interventions and critical thinking to support the service user and family. Coordinates the involvement of other services to provide a range of choices for the service user and family. Social work standards of practice and ethics are utilised. RELATIONSHIP WITH CONSUMER Consumers will be seen for an assessment as soon as possible All contacts are documented and meet professional standards. Core documentation is completed on all service users. Families are provided with education opportunities around early warning signs, diagnosis, symptoms and treatment options. Rights (as defined by the Health and Disability Commssioner) are actively promoted and respected. Empowering relationships are established with service users and families. The clinical file is up to date, entries are legible, dated and signed with designation clearly visible. Evidence in progress notes that the service user and family were involved (or were given the opportunity and declined). Audits and feedback evidence shows evidence of service user s social system being enhanced. Consumer / tangata whaiora and their families are aware of their rights and their choices and are empowered through their relationship with the service PROFESSIONAL STANDARDS To meet Hawke s Bay District Health Board s Standards (i.e. legislative, professional, contractual, ethical and organisation) by knowing what the applicable standards are and undertaking any steps necessary to remedy shortfalls in practice and knowledge. Professional standards are met. The risk of harm to service users, staff and others is minimised. Service users have confidence in the employee s standard of delivery of care. Annual practicing certificate with Social Work Registration Board is current. SUPERVISION Develops supervision contact and receives social work supervision in accordance to ANZASW guidelines, Social Work Registration requirements and organisational policies. Provides social work student supervision in accordance with professional objectives and criteria and as agreed with Professional Advisor and Service Manager. Attends supervision as per standards. Evidence of superiviviosn as per HBDHB Professional Supervisiosn Policy. Student supervision is undertaken regularly.
