3 Event List 1. Event List October 6, 1:15 A.M. A large transformer near Taylor, Michigan fails. The transformer while not new, is not old enough that failure was expected. Power is out in much of Livingston, Oakland, Washtenaw and Wayne counties. Detroit Edison has not yet given an expected repair time. October 6, 3:12 A.M. Telephone service has failed in and around Grayling, Michigan. The cause of the failure is not yet clear, nor is the breadth of the disruption. It is believed it may extend beyond Crawford County, but that is not known at this time. October 6, 5:13 A.M. The reactor at the Enrico Fermi Nuclear Generating Station has SCRAMed unexpectedly. The Fermi plant is located in the Frenchtown Charter Township on the shore of Lake Erie in Monroe County. The single reactor known as E. Fermi 2 was commissioned in The reactor is a GE boiling water reactor, similar to the Fukushima reactors, but significantly modernized. (E. Fermi 1 was decommissioned in 1975.) At this time, the cause of the shutdown is unknown. October 6, 5:19 A.M. DTE Energy notifies the State Police Hot Line that a Site Area Emergency (HS3 Appendix R, HS3 Other Conditions) has been declared at E. Fermi 2. October 6, 5:32 A.M. Governor Rick Snyder orders the activation of the State Emergency Operations Center as a result of the incident at E. Fermi 2. October 6, 6:00 A.M. The Section Emergency Coordinator notifies the DEC for District 2 and the Net Manager for SEMTN of the SEOC activation. October 6, 6:45 A.M. A large water main breaks near Kentwood, Michigan, leaving residents in much of Ionia, Kent and Montcalm counties without water. (Appendix I, District 6 Water Outage) October 6, 7:26 A.M. Incident Commander Inspector Capp declares the State Emergency Operations Center fully operational. 3

4 October 6, 8:12 A.M. KD8MTL advises the Section that E. Fermi SCRAM still unexplained, (Appendix A, E. Fermi 2 SCRAM notification), requests status of R2 counties (Appendix B, Request for R2 status update). October 6, 8:37 A.M. Power has been interrupted in a large section of District 3, as well as parts of adjacent Districts. At this time the cause is unknown, and there is no estimate of when power will be restored. (Appendix J, District 3 Power Outage) October 6, 9:11 A.M. A major natural gas pipeline near Thurston has burst. Much of the Detroit metropolitan area is without natural gas. The cause of the break is under investigation. (Appendix K, Metro Area Natural Gas Outage) October 6, 9:30 A.M. KD8MTL advises Section that SEOC is concerned that there may be more than a problem at Fermi. Relays Inspector's request for hourly 214. (Appendix C, Request for 214). October 6, 9:45 A.M. The telephone disruption around Grayling appears to have expanded to include much of the lower peninsula north of M-46 Appendix L, Telephone Outage North of M-46 October 6, 9:48 A.M. Tuscola county notifies D3 of telephone outage. October 6, 9:51 A.M. Benzie county notifies D7 of telephone outage. October 6, 9:52 A.M. Montmorency county notifies D7 of telephone outage. October 6, 9:55 A.M. Alcona county notifies D3 of telephone outage. October 6, 9:59 A.M. Saginaw county notifies D3 of telephone outage in the northern parts of the county. October 6, 10:17 A.M. 5 Event List October 6, 10:18 A.M. D.C. Cook reactor 2 SCRAMs October 6, 10:19 A.M. Saginaw county notifies D3 of Internet outage. October 6, 10:21 A.M. Bay county notifies D3 of Internet outage. October 6, 10:21 A.M. Entergy calls the MSP hotline to notify the state of the unexpected SCRAM of reactor 2 at the D.C. Cook Nuclear Generating Station. The plant has declared an Unusual Event (HU5.1 - Appendix T, HU5.1 Other Conditions Exist Which in the Judgement of the Emergency Director Warrant the Declaration of a UE). The Donald C. Cook Nuclear Generating Station is located north of the city of Bridgeman in Berrien county. October 6, 10:32 A.M. As a precaution, D. C. Cook shuts down reactor 1. October 6, 10:35 A.M. Entergy notifies the duty officer to notify MSP that reactor 1 has been shut down as a precaution. The plant had declared an HU4.1 Unusual Event (HU4.1 - Appendix S, HU4.1 Confirmed Security Event which Indicates a Potential Degradation in the Level of Safety of the Plant). October 6, 10:41 A.M. KD8MTL notifies the section of the SCRAM at D.C.Cook, updates on other incidents (Appendix D, Notification of D.C. Cook Status). October 6, 11:00 A.M. The incident commander at the SEOC Requests Palisades shut down due to suspicious events at the other nuclear plants. October 6, 11:13 A.M. KD8MTL requests District 5 counties send their 205A (Appendix E, Request for R5 status update). October 6, 11:27 A.M. CMS Energy advises the SEOC they will not shut down the Palisades reactor. October 6, 11:31 A.M. 5

6 Internet outage across D8 east of Iron Mountain and and D7 north of Onaway (Appendix O, District 8 Internet Outage) October 6, 11:45 a.m. Palisades declares SS6.1 Site Area Emergency due to irregularities in their control systems (Appendix U, SS6.1 Loss of annunciators). October 6, 11:48 A.M. MDCH orders KI distribution in Van Buren, Allegan and Muskegon. October 6, 11:51 A.M. KD8MTL notifies District 5 of the order to distribute KI (Appendix F, KI Distribution notification). October 6, 12:01 P.M. Palisades declares GENERAL EMERGENCY HG1.1 due to apparent hostile action at the plant (Appendix Q, HG1.1 Hostile Force). October 6, 12:04 P.M. KD8MTL notifies the section of the general emergency at Palisades (Appendix G, Palisades General Emergency). October 6, 12:07 P.M. Complete loss of power S of I94 and W of Hillsdale Appendix M, Power Outage South of I-94 October 6, 12:12 P.M. Branch county notifies D5 of widespread power outages across the county. October 6, 12:19 P.M. Governor Rick Snyder orders evacuation of areas 1,2,3,4,5 around the Palisades Nuclear Plant (Appendix P, Palisades Protective Action Areas). This affects parts of Van Buren, Allegan and Berrien counties. Marine traffic is to be cleared within a 10 mile radius of the plant. October 6, 12:21 P.M. KD8MTL advises the section on the evacuation order for Palisades, and reminds jurisdictions to keep the SEOC updated (Appendix H, Notification of Evacuation). October 6, 12:59 P.M. Exercise ends. 6

Request for 214 ICS 213 GENERAL MESSAGE TO: All District RACES Officers POSITION: FROM: State RACES Officer POSITION: SUBJ: Potential Hostile Action DATE: OCT 6 TIME: 0930 MESSAGE: *** THIS IS AN EXERCISE *** There have been a rather startling number of apparently unrelated incidents occurring across the state. This has led Inspector Capp, Incident Commander at the State Emergency Operations Center, to be concerned that the incidents are somehow related. Please advise the SEOC of any unusual events in your jurisdiction. To date, the following incidents are known: - Taylor transformer failure - Grayling telephone disruption - SCRAM at E. Fermi 2 - Kentwood water main break - Widespread power outage in District 3 - Gas pipeline break near Chelsea The commander is concerned that with the widespread infrastructure issues it is important that the SEOC stay on top of all available communications assets. He has therefore requested that each jurisdiction deliver ICS-214 to the SEOC on an hourly basis until the situation is better understood. *** THIS IS AN EXERCISE *** SIGNATURE: John J. Cook Shutdown DATE: OCT 6 TIME: 1041 MESSAGE: *** THIS IS AN EXERCISE *** Reactor 2 at the D.C. Cook nuclear plant in Berrien County has shut down unexpectedly. The plant has shut down reactor 1 as a precaution. The telephone disruption around Grayling appears to have spread to much of the lower peninsula north of M-46. There also appear to be some Internet disruptions on the east side of the state. It is critical that you notify the SEOC of any unusual events in your jurisdiction, and update the SEOC with any changes in your ICS 214 or 205A promptly. *** THIS IS AN EXERCISE *** SIGNATURE: John J. Notification of Evacuation ICS 213 GENERAL MESSAGE TO: All District RACES Officers POSITION: FROM: State RACES Officer POSITION: SUBJ: Evacuation of parts of District 5 DATE: OCT 6 TIME: 1221 MESSAGE: *** THIS IS AN EXERCISE *** Governor Rick Snyder has ordered the evacuation of protective action areas 1, 2, 3, 4, and 5 around the Palisades Nuclear Plant in Van Buren County's Covert Township. In addition, marine and air traffic is to be cleared within a 10 mile radius of the plant. This area includes portions of Allegan, Berrien and Van Buren counties. At 12:01 P.M. the plant declared a General Emergency of the category HG1.1. This indicates that the plant safety officer believes that control of the plant is in the hands of a hostile force. Possibly related to the loss of both nuclear generating stations in the area, power has been lost in the area south of I-94 and west of Albion. All jurisdictions are reminded to report any unusual incidents to the State Emergency Operations Center IMMEDIATELY, and promptly update the SEOC with any changes in your 214 or 205A. *** THIS IS AN EXERCISE *** SIGNATURE: John J. HG1.1 Hostile Force HG1.1 A HOSTILE FORCE has taken control of plant equipment such that plant personnel are unable to operate equipment required to maintain safety functions as indicated by loss of physical control of EITHER - A VITAL AREA such that operation of equipment required for safe shutdown is lost OR - Spent fuel pool cooling systems if imminent fuel damage is likely (e.g. Freshly off-loaded reactor core in the pool) 23

HS3 Other Conditions HS3 Other Conditions Existing Which in the Judgement of the Emergency Director Warrant Declaration of Site Area Emergency HS3.1 Other conditions exist Which in the judgement of the Emergency Director indicate that events are in process or have occurred which involve an actual or likely major failures of plant functions needed for protection of the public or HOSTILE ACTION that results in intentional damage or malicious acts; (1) toward site personnel or equipment that could lead to the likely faiure of or, (2) that prevent effective access to equipment needed for the protection of the public. Any releases are not expected to result in exposure levels which exceed EPA Protective Action Guideline exposure levels beyond the site boundary 24

HU4.1 Confirmed Security Event which Indicates a Potential Degradation in the Level of Safety of the Plant Security Shift Leader reports ANY of the following - Suspected SABATOGE device discovered within plant PROTECTED AREA - Suspected SABATOGE device discovered outside PROTECTED AREA or within the plant switchyard - Confirmed tampering with safety-related equipment - A HOSTAGE/EXTORTION situation that disrupts NORMAL PLANT OPERATIONS - CIVIL DISTURBANCE or STRIKE ACTION which disrupts NORMAL PLANT OPERATION - Internal disturbance that is not short lived or that is a harmless outburst involving ANY individuals within the PROTECTED AREA - Malevolent use of a vehicle outside the PROTECTED AREA which disrupts NORMAL PLANT OPERATIONS 25

HU5.1 Other Conditions Exist Which in the Judgement of the Emergency Director Warrant the Declaration of a UE HU5.1 Other Conditions Exist Which in the Judgement of the Emergency Director indicate that events are in process or have occurred which indicate a potential degradation of the level of safety of the plant or indicate a security threat to facility protection has been initiated. No releases of radioactive material requiring offsite response or monitoring are expected unless further degradation of safety systems occurs. 26
