2 MU Menu 04 Family Histry This guide will prvide yu the step by step instructins n hw t cnfigure the necessary setup and preferences t enable Enterprise t wrk fr reprting MU Menu 04: Recrd patient family health histry as structured data. Final Rule Requirements Mre than 20% f all unique patients seen by the Eligib le Prfessinal (EP) during the EHR reprting perid have a structured data entry fr ne r mre first -degree relatives. Cre: Menu: Numeratr: The number f patients in the denminatr with a structured data entry fr ne r mre first-degree relatives. Denminatr: Number f unique patients seen by the Eligible Prfessinal (EP) during the EHR reprting perid. Exclusin: Any EP wh has n ffice visits during the EHR reprting perid. TWAdmin Preferences and chse Alerts in drp dwn bx. Select preference, review value and change if apprpriate. Review Family Member Pick list fr Family Histry Prblems Preference: The Family Member Pick List fr Family Histry Prblems is used t set the ptins that are displayed by default in the Family Members list and the Family Member drp-dwn list. 4 The ptins in the Family Members list and the Family Member drp-dwn list are displayed in the same sequence as the dictinary entries are displayed n Clinical Relatinship Pick list fr a pick list. It is best practice t set t Allscripts delivered pick list r set t the pick list created fr yur enterprise. Ensure the pick list includes first -degree relatives (see fllwing Review clinical relatinship sectin). (Available v11.4.1) 1. TWAdmin Preferences and chse Prblem in drp dwn bx, select preference, review value and change if apprpriate. Review the Clinical Relatinship Pick List : This pick list is used t specify which clinical relatinships users ca n select when dcumenting family histry n a patient (available 11.4). 1. 5 2. Ensure the family member pick list created in Clinical Relatinship dictinary includes first-degree relatives. The First degree relative field is checked, by default, fr the family members that are cnsidered as first-degree relatives: Brther Child Daughter Father Mther Sibling Sister Sn Review Quick list Family Histry Setup : Ensure users have 'N pertinent family histry' set as a Quick List favrite in their Family Histry favrites. This can be dne thrugh Physician Admin (Favrites) r thrugh the SSMT categry Favrites: Patient Hx-Family Histry. 6 2. Select Specialty Open Patient Histry Favrites Open Family Histry Flder Master Favrites 3. Search fr 'N pertinent family histry' and duble click n it t add 4. Select prblem n right and select Add t Quick List 5. Select Save OR 1. Lgin t SSMT w/apprpriate credentials 2. Pick Favrites: Patient Hx Family Hx Cntent Categry 3. Select Headers in Extracted Data 4. Click Extract Data 1. Click in the extracted text 3. Click ctrl and a buttns t cpy all text 4. Click ctrl and s buttns t select all text 5. Open excel 6. 7 8. Make mdificatins as necessary in excel fr reference ranges 9. Cpy apprpriate rws and paste back int SSMT (after deleting text in windw) 10. Click Lad Data Wrkflw Cnsideratins In , it is mandatry t pick a family member t add a new family histry prblem. The reprt nly includes first degree family members in the numeratr (mther, father, brther, sister, sn, and daughter). The use f the family member family histry will nt be included in the numeratr. When yu are in a situatin where there is n knwn family histry dcument ed fr a family member, the N pertinent family histry term can be used. The term has t be assciated t ne r mre first degree family member t cunt in the numeratr and credit will be received. A cmmnly asked questin is hw t dcument alive and well. That term n lnger exists with yur upgrade t We recmmend t use N Pe rtinent Family Histry and select specific family members t link it t. When yu are in a situatin where there is family histry is unknwn fr a patient, in the example f adptin, yu can fllw the belw ptins. 8 OR create a synnym in the prblem dictinary fr family histry, unknwn called Family histry unknwn, patient adpted TWAdmin Dictinaries Prblem Search fr family histry unknwn Detail 3 Add New Put in the Prblem Name Ok Save It is recmmended t ensure bth terms (is using bth) are added t the prvider s favrites and quicklists. 9 Hw EEHR Calculates the MU2 Reprt Numeratr Fr each patient in the denminatr, the system will cunt nce: At least ne verified family histry prblem fr First Degree Family Members AND: The prblem must include at least ne f the fllwing Family Members types: Brther Child Daughter Father Mther Sibling Sister Sn AND: Is in a status f Active r Denied OR: N pertinent family histry dcumented and linked t a first degree family member Denminatr: The system will cunt nce each unique patient that was seen during the reprting perid. Exclusin: Any Eligible Prfessinal (EP) wh has n ffice visits during the EHR reprting perid. Indicatrs: When the numeratr divided by denminatr is greater than 20%.
