14 Craft the Right Rounds NURSE LEADER PATIENT ROUNDING THIS IS HOW WE DO IT 1. BUILDING AN EFFECTIVE ROUNDING TEMPLATE QUESTIONS ARE KEY HOW MANY QUESTIONS? NO MORE THAN 5 Limit your round to No More Than 5 questions (plus observations). WHY? 6+ QUESTIONS: Adds Time Adds Complexity Reduces Adoption Burdens Roundee 5 QUESTIONS: Increases Adoption Simplifies Results Leaves Space APPROPRIATE QUESTIONS? For Conversation WHICH QUESTIONS? REMEMBER! Explain Did you answer yes to any of these filter questions? If so, reevaluate your round question! ROUNDEE LEVEL QUESTIONS DEPARTMENT LEVEL QUESTIONS Acknowledge Knock, wait for response Demonstrate hand hygiene Greet patient and family by name Explain the purpose of your round Demonstrate confidence, empathy, and humility Check room, make sure everything is within reach Explain white board, date, staff, plan of care, discharge date Manage up! Your team is the best! ORGANIZATION LEVEL QUESTION Tip: Swap out a question once a goal is achieved MYROUNDING QUESTION FILTER GOOD ROUNDING STARTS WITH ASKING THE RIGHT QUESTIONS OBSERVABLE? Can we answer the question without asking the roundee? I heard you say that you re being kept up at night due to squeaky cart wheels in the hall. Did I get that right? Thank QUALIFIED? Is the question too technical? Is it unreasonable for the roundee to answer? TATTLE? Are we asking the roundee to manage the team? KNOW? Will the roundee feel foolish if they don t know the answer? GOAL? Is this question outside of our current goal scope?
