3 Hello. You are very welcome to the adult renal department at St. James s Hospital in Leeds. This booklet will tell you a little bit about the young adult service and provide you with the contact details you may need. Transition from paediatric to adult services The transition clinic helps you and your family to plan, prepare and move from the children s renal department to the adult renal department. We understand that this may be a scary time with lots of change. You will have a new team of doctors and nurses caring for you as well as coming to a new hospital. We will help you get familiar to the new surroundings when we see you in clinic. This is also an exciting time. A time when you begin to become more independent and responsible for your own healthcare. The young person s team at St. James s will help support you to make the transition as easy as possible for you and your family. What are the main differences between children s and adult services? We know there are differences between children s and adult departments and it is likely you will notice these when you come to St. James s. The patient population is older here and the clinics and wards are run a little differently. These changes may sound quite daunting but we are here to support you and there are also lots of positive changes. You will grow in confidence, practice more independence and make more 3
4 decisions for yourself. You will also have the chance to meet lots of new people, staff member and patients alike and have the opportunity to get involved with social activities if you would like. Where do adult clinics take place? Renal clinics are held in Outpatients 2 on the ground floor of Lincoln Wing at St James s Hospital. On arrival you can check in with the reception staff at the desk. The Young Person s Renal Team The young person s team is made up of many different member including doctors, transition & young adult nurse, dieticians, psychologists and social workers. We can also refer to a number of different agencies to best support your care. In the young person s clinic you will regularly see a consultant and the transition nurse. 4
5 Dr. Madeleine Vernon Consultant Nephrologist I m Madeleine Vernon. I am one of the consultant adult nephrologists who works with and looks after young people both in the adult service and as they transition from paediatrics. I work closely with all the other members of the team and look forward to getting to know you. Dr. Andrew Mooney Consultant Nephrologist My name is Dr Andrew Mooney. I am a doctor who looks after adults with kidney problems. For quite a few years now, I have been looking after young adults as they transfer from the paediatric service. I look forward to meeting you and helping to look after you. Rebecca Shelton Renal Young Adult Clinical Nurse Specialist (CNS) Hi! I m Rebecca. My job is to help you settle into the adult renal service and answer any questions you may have. I can act as a key contact for you and help co-ordinate your care. I will come see you regularly in clinic, on dialysis or on the renal inpatient wards. 5
6 The Adult Renal Wards There are 2 adult renal wards at St. James s, located on the 1st Floor of Lincoln Wing. J49 is the Male ward and J50 is the Female ward. The staff here are very experienced in caring for a range of patients with renal illness. Parking is available nearby in the Pay and Display Beckett street car-parks or on site in the multi-story car park near Bexley Wing. There is an array of places to eat and have a hot drink in the nearby Gledhow wing, including a Marks and Spencer and Costa Coffee. J49 1st Floor Lincoln Wing J49 is the male ward with 22 beds. There are 3 bays and 7 side rooms. Each bed space has a television which is free to use and there is access to free WIFI. Visiting times are 12-8pm. The senior charge nurse of the ward is called Ian Henderson. 6
7 J50 1st Floor Lincoln Wing J50 is the female ward with 20 beds. There are 3 bays and 6 side rooms. Each bed space has a television which is free to use and there is access to free WIFI. Visiting times are 12-8pm. The senior sister of the ward is called Caroline Hills. 7
8 Who do I contact in adult services? If you are feeling poorly the first point of contact should be your GP, it may be that you need to come into hospital via A&E at St. James s.
