3 Introduction Welcome to the Sheffield Cystic Fibrosis Unit. The aim of this booklet is to provide you with helpful information about the service. Please note we will use the abbreviation CF throughout the booklet rather than using the term Cystic Fibrosis in full. What is the CF service? The CF service provides both in and outpatient care for people over the age of 16 who have CF. The service provides four types of care: Regular outpatient appointments at our clinics A yearly annual review for all our patients Inpatient admissions Home visits if appropriate Where is the service based? The Sheffield Adult CF Centre is located in the Brearley Outpatients department on A floor. The centre is highlighted on the hospital plan on the following page. It is best to enter the hospital at the Barnsley Road entrance. Parking is available in the visitor's car parks where there is a pay and display system. There are some disabled parking spaces outside the outpatient building. If you are admitted to hospital, your inpatient stay will be in the Cystic Fibrosis Ward or Brearley 2 (for those who have specific infections that prevent them from being nursed on the CF ward). There is a visitor's car park just outside the CF ward which you can use if spaces are available. A permit for this can be issued by the staff on the CF ward. You are best to enter the hospital from Barnsley Road entrance and turn right, the car page 3 of 12

8 How and when can I get to see the CF team? You can call for advice or to arrange an appointment from 8.00am to 4.00pm ( or ). Please leave your details on our answer machine. If you need to be seen the same day, try to contact us before 10.00am. The phone will be answered by a CF nurse between 9.00am am and 2.00pm pm Monday to Friday if available. Outside these hours staff on the CF ward may be able to give you advice. However, if you need to be seen urgently go to Accident & Emergency. The CF nurses can also be contacted by call or text on the mobile phone. The number is When you call the Nurses' Office you will be given 4 options. Please press options 1-4 as follows to be directed to the correct person: 1. To change your appointment 2. To order prescriptions (medicines issued by the hospital only) 3. If you are unwell or need advice 4. If you need a social worker or psychologist The CF team will review you in clinics which are on Monday mornings, Wednesday afternoons and Friday afternoons. If you need to start any IV treatments we will arrange a time for you. What happens when I go to the outpatient clinic? Prior to your clinic appointment you will receive an automated telephone reminder. The CF outpatient nurses may also text or call you. On arrival at the clinic please report to the nurse reception desk and staff will show you to a room. page 8 of 12

9 During your appointment, you will see each member of the team individually. To reduce the risk of cross infection you will be given a room for your appointment and the CF team will all come to see you. When are outpatient clinics? We have clinics on Monday mornings, Wednesday afternoons and Friday afternoons. We also have a combined CF and Diabetes clinic most months, for those patients with diabetes. It helps us if you arrive on time for your appointment, as the clinic can get very busy. This also reduces your risk of cross infection with other patients. Please remember to bring to clinic any devices that you have such as ineb or blood glucose monitors. If you are unable to attend, please call us as soon as possible so that we can allocate your appointment to someone else. Are home visits available? Visits by the CF team to your home can be arranged where clinically appropriate. This can be discussed further with any of the CF team. Annual reviews At your annual review we will look at many different aspects of your health that relate to CF - it is a bit like an MOT. The review happens every year and we will send you a separate appointment for this. Always remember to bring your nebulisers and physio equipment to the annual review for servicing where appropriate. It is also very useful if you can bring your glucose monitor if you have one and a list of your current medications. page 9 of 12

10 Annual reviews are carried out on the CF Ward. During your review you will see all the members of the team. The review may include additional investigations such as: Glucose tolerance test Chest x-rays Bone scans Ultrasound scans of the abdomen Blood tests Exercise tests Your contact details Please tell us if any of your contact details or GP details change so that we can update our records.
