There are a broad range of medical cases seen through the doors. KCH ECC is also unique in that it is the main centre for accepting urology and vascular surgical patients. It is led by 6 FY2 doctors, 1 on-call HCOOP SHO and 1 medical SHO. Juniors are well supported by senior staff, with 3 consultants on the floor on a typical day. There is also a surgical on-call SHO, registrar and when needed consultant. ECC adopts a shift system of 8 hours daily, the start and end times of which will vary. The HCOOP and medical SHO s on calls work a 13 hour day. Patients are triaged. Obs are performed by the nursing staff, with the doctors then clerking patients in and following up their care for the remainder of the day. The latter is what sets our department apart from most other A&Es. As we don t have an MAU, patients care on the day of admission is the responsibility of their clerking clinician. This helps establish continuity of care and is also more efficient as patients are not clerked in twice. The placement provides an excellent learning opportunity in acute medicine, and also enables you to develop your history taking, examination and communication skills. As the on-call consultant will review the patient the same day you are also able to develop your presenting skills, and learn directly from your vastly experience senior clinicians. You may also be able to perform various procedures such as cannulas, ABGs. lumbar punctures, pleural & ascitic taps etc. There is always a medical registrar to assist you if needed. It is a very busy, demanding placement, and can

In addition to the doctors, the department has its own renal pharmacist, dietician, and a number of specialist nurses (e.g. peritoneal dialysis nurses, transplant nurses, vascular access nurses, haemodialysis nurses etc ) in addition to the standard ward staff. The department is a friendly and close-knit team. Many of the patients are known to the staff having been patients for many years and because they attend regularly for dialysis. A typical working day starts with a Ward round (9.00am). The junior doctor is expected to produce an updated list before the round and to have everything ready before the round. There are 2 Consultant ward rounds each week and 2 Registrar led ward rounds. On Wednesday there is a Radiology meeting at 8.30am, followed by a junior ward round for the chronic team and registrar led ward wound for acute team. Ward rounds are often a great learning opportunity since juniors are expected to present patients and to know the recent test results as well as trends. Discussion and ideas are encouraged. There is often a huge diversity of clinical presentations on the ward. You can encounter anything from a marathon runner with acute renal failure secondary to rhabdomyolysis, or someone with myeloma, or a transplant patient with sepsis, or a dialysis patient with an acutely ischaemic limb. This variety presents fantastic learning opportunities. The ward also has procedure room where lines for dialysis are inserted. As a junior you will have the

11 chance to place central lines (for temporary dialysis/plasma exchange) and to remove tunnelled lines. Where the placement is based Clinical supervisor(s) for the placement Main duties of the placement Typical working pattern in this placement (e.g. ward rounds, clinics, theatre sessions) The department effectively consists of 3 clinical areas; Marlowe ward (which has an acute end and a chronic end), the Thomas Beckett Dialysis Unit, and outpatients. As a junior doctor you will work primarily on the ward, patients will often be admitted from either outpatients or the dialysis unit. You may also be asked to review patients whilst they are on dialysis in the Thomas Beckett unit. Varies between the Renal Consultants In addition to standard ward duties (organising the ward round, taking bloods, writing discharge summaries etc ) you will also be expected to clerk in patients who are admitted to the ward. This is a common place event, and often you will be the first doctor they see on admission as many of the patients come straight onto the ward (bypassing Emergency Care Centre) having either been referred in by a specialist nurse, from dialysis, clinic or by a consultant at another hospital. There are a broad range of medical cases seen through the doors. KCH ECC is also unique in that it is the main centre for accepting urology and vascular surgical patients. It is led by 6 FY2 doctors, 1 on-call HCOOP SHO and 1 medical SHO. Juniors are well supported by senior staff, with 3 consultants on the floor on a typical day. There is also a surgical on-call SHO, registrar and when needed consultant. ECC adopts a shift system of 8 hours daily, the start and end times of which will vary. The HCOOP and medical SHO s on calls work a 13 hour day. Patients are triaged. Obs are performed by the nursing staff, with the doctors then clerking patients in and following up their care for the remainder of the day. The latter is what sets our department apart from most other A&Es. As we don t have an MAU, patients care on the day of admission is the responsibility of their clerking clinician. This helps establish continuity of care and is also more efficient as patients are not clerked in twice. The placement provides an excellent learning opportunity in acute medicine, and also enables you to develop your history taking, examination and communication skills. As the on-call consultant will review the patient the same day you are also able to develop your presenting skills, and learn directly from your vastly experience senior clinicians. You may also be able to perform various procedures such as cannulas, ABGs. lumbar punctures, pleural & ascitic taps etc. There is always a medical registrar to assist you if needed. It is a very busy, demanding placement, and can

As KCH is the centre in East Kent for stroke thrombolysis, we do get several of these patients presenting to us daily. It provides a unique opportunity to assess acute stroke patients, with the stroke consultant and specialist nurse. Several other medical conditions will also come into resus, where you can again expect to be supported by a senior member of staff. As you are the clinician responsible for the patients you clerk in, your duties will include their acute assessment (history, examination), organising appropriate investigations such as bloods, imaging and following these results up. You may have the opportunity to do procedures such as pleural taps and lumbar punctures. There is also paper work involved, which includes writing referral letters and discharge summaries. Patients are spread across all the hospital wards. Also do oncall days in ECC. Type of work to expect and learning opportunities Where the placement is based Clinical supervisor(s) for the placement Main duties of the placement Typical working pattern in this placement (e.g. ward rounds, clinics, theatre sessions) Ward rounds usually from 9am (meet in the mess). Rounds, however, start at 8am on day after consultant is on-call. Dr Batty usually on-call on Tuesdays but occasionally we are also on-call on Thursday, at the weekend, or from 5pm on a Sunday. After ward round we normally go through the job list and split up the jobs. On whole these include taking/checking bloods, ordering/chasing scans, doing EDNs, referring to other specialties, speaking to families, chasing social placements. Advised to try and attend one clinic, one social meeting, one physio session during attachment. Can volunteer to learn procedures if they present themselves but there aren t many opportunities. Lots of good opportunities for teaching within the department. Consultants are great mentors and supportive. Kent and Canterbury Hospital Wards Dr Batty Letters, EDN, bloods, cannulas, ward work. Will be expected to prepare for + present at journal club once during rotation. You are on-call as HCOOP SHO 8.30am-9.30pm roughly once a week. Also do approx two sets of day weekends and one set of weekend nights during 4months. Also do approx a further two sets of mid week nights. While on nights you clerk in ECC admissions as well as covering any ward jobs. You will be expected to present at journal clubs during this rotation.
