1 Marion County Board of Health Meeting Minutes September 18, :00 P.M. Board Members Present: Richard Haney, Mike Morton, Tom Turner, Keith Kessler, Elizabeth Franczyk, Stan Morrison, Paula Strother. Others Present: Lori Ryan, Administrator; Melissa Mallow, Environmental Health Director; Shelly Yoder, Director of Nursing; Tara DeWitt, Health Educator; Cathy Hays, Administrative Secretary; and Will Doolen, WJBD Radio. I. CALL TO ORDER The Marion County Board of Health met on September 18, 2012, at the Marion County Health Department, Centralia office. Mike Morton called the meeting to order at 7:00 p.m. II. QUORUM A quorum of seven board members were present. III. MINUTES Mike Morton made a motion to accept the June 26, 2012, quarterly Board of Health meeting minutes as presented; Dick Haney seconded the motion. All were in favor; none opposed. The minutes were approved. IV. SPECIAL ORDERS Board Appointment Letter was submitted to the County Board, however was missed on the agenda. Will be addressed in the October County Board Meeting. a. New Board Appointments Dr. Stan Morrison December 1, 2012 November 30, 2015 Pam Hawkins December 1, November 30, 2013 b. Selection of Board Positions A motion was made for Tom Turner to be new Board President. All was in favor, none opposed. Tom Turner, Board of Health President. Keith Kessler agreed to be the new Board Secretary. All was in favor; none opposed. Keith Kessler, Board of Health Secretary. c. 2 V. FINANCIAL REPORT Morton called for a review of the financial report. Lori Ryan reported the cash flow balance at the end of the 2nd Quarter is $735, The revenue for the end of the 3rd Quarter was $871,149.10, 77.38% of the budgeted amount. Expenditures for the end of the 3rd Quarter is $936, which is 83% of the budget. Expenditures over revenue is $64, Accounts Receivable is $155, FCM $45,000 WIC/ BFPC $32,000 PA $51,000 Mike Morton made a motion to accept the financial report; Dick Haney seconded the motion. All were in favor; none opposed. The financial report was approved. VI. OLD BUSINESS a. Parking Lot Expansion: Lori Ryan spoke with Steve Hotze, who is trustee over the adjoining land to the Health Department. Parking is getting worse on Tuesday, Wednesday and Thursday s. Ms. Ryan transferred $30,000 in the building fund for FY 13. Parking lot expansion is tabled until spring 2013 Board Meeting. VII. NEW BUSINESS a. Activities: 1. Health Education: Tara DeWitt, Health Educator Quitline Cessation Program - Year to Date: 33 clients have been referred to the health department by the IL Tobacco Quitline; 5 clients have completed the program this Fiscal Year. Tobacco Coalition / CADEA Resumed September 12, Coalition agreed to again this year include Kick Butts Day (KBD). Meetings - As part of the Tobacco grant the Health Department is to develop and implement outreach strategies with Health Care Provider Systems to promote the Illinois Tobacco Quitline. Page 2 of 9

3 Initial contacts: June 28, DeWitt met with Stephanie Hilton-Siebert, President of Salem Township Hospital. September 5, Tara met with Yvonne Wagner Head of Social Services and Aimee Nix Head of the Respiratory Department at St. Mary s Good Samaritan in Centralia. The purpose of these meetings were to provide information about the Illinois Tobacco Quitline (ITQ), the Fax Referral systems for the Quitline and the 5 A s and 5 R s of the Public Health Service cessation guidelines. Also, to obtain a Cessation Assessment Survey to learn each hospitals cessation practices and awareness of the ITQ. 2. Environmental Health: Melissa Mallow, Director of Environmental Health West Nile Virus Illinois has had 95 human cases (mostly northern Illinois; Bond and Crawford counties had 1 each). There have been 3 deaths (DuPage county 2; Kane county 1. Marion County has had no positive human cases, horses or birds. Sent 2 birds from the Salem area for WNV testing and each was negative. Currently, we have 10 mosquito batches that are waiting to be tested for WNV. Food On August 28, we had a local restaurant employee test positive for Salmonella. Marion County Health Department required that the employee be restricted from any job duties that could result in the transmission of the disease. The restaurant complied. August 22, IDPH requested that all local health departments forward information concerning the multistate salmonella outbreak linked to cantaloupes from southwest Indiana to all Farmer Markets in their jurisdiction. The information was given to the Farmer Market masters in Salem and Centralia; Dan s Produce in Centralia and the Amish Food Stand in Salem. Page 3 of 9

4 Septic Terry Johnston, Marion County Board Chairman, forwarded a complaint to Marion County Health Department. The nature of the complaint was a man living in an old camper in Hubbard Woods, Centralia. The man has no electricity, water or septic system, thus is using the bathroom outside. Ms. Mallow performed an inspection to verify that the allegations were true and they were. Mallow notified the property owner via certified mail that an Illinois approved private sewage system permitted by the health department had to be installed. The property owner did not install a private sewage system in the allotted time frame, so the complaint has been forwarded to IDPH for legal enforcement. General Environmental Health Missy Mallow wrote a letter of support to a new storm water system for the village of Iuka. 3. Nursing: Shelley Yoder, Director of Nursing WIC Caseload: 1511 increased to 1549 in July 2012 June: 1407 P-181, BF-60, PP-89, I-344, C % of caseload July: 1410 P-184, BF-54 PP-91, I-339, C % of caseload with increased caseload August : 1413 P-177, BF-55, PP-96, I-328, C % of caseload Our goal is to maintain a caseload of greater than 90% of our assigned caseload Breastfeeding Progress: Breastfeeding initiation with a goal of 85% The calculations are based on clients that were enrolled prenatally with information on breastfeeding obtained during their infant s certification visit. Centralia: 75% in June, 56% July, 33% in August Salem: 90% in June, 63% July, 69% in August 6 month duration with a goal of 50% Centralia: 18% in June, 14% July, 6% in August Salem: 9% in June, 8% July, 13% in August Page 4 of 9

5 12 month duration with a goal of 15%. Centralia: 15% in June, 10% July, 8% in August Salem: 0% in June, 14% July, 8% in August STD Testing: 11 HIV tests were done this quarter with 1 positive result. 28 GC and Chlamydia tests were done with 2 positive results for Chlamydia and 2 positive results for GC. 25 syphilis tests were done with no positive results. Positive HIV Case: In July MCHD provided STD testing services for an out of county adolescent. The client tested + for HIV. We arranged for case management services and counseling to be provided to the family at our health department at the time of notification of test results as well as completed all the required case reports and interview records d/t the circumstances of the case. Outreach for STD Treatment: In August 2012 the CDC revised its treatment guidelines for Gonorrhea due to the bacteria s ability to develop resistance. Cefixime is no longer recommended as a first line treatment for this infection. In an effort to notify local healthcare facilities of the new guidelines, I sent out letters along with a copy of the weekly morbidity and mortality weekly report which outlines the rationale and recommendations of the new CDC tx guidelines. The packets were mailed out to the SMH ER, STH ER, Salem Medical Center, Salem Hospital Rural Health Clinic, St. Mary s Family Health Center in Salem and Heartlands Women s Healthcare Center in Centralia. Lead Poisoning Prevention Services: This quarter 230 lead tests were completed. 6 children are currently being followed for lead poisoning case management services. 4 lead nursing home visits were conducted this quarter. 1 child with a high capillary lead identified in June still hasn t had a confirmatory venous lead test result. The parent was non-compliant so a report was made to DCFS for an investigation on medical neglect, which is currently ongoing. Page 5 of 9

6 Communicable Diseases: This quarter 77 communicable diseases were investigated. Out of those investigations the following were counted as cases. 36 Chlamydia, 12 GC, 10 HCV, 4 RMSF, 6 salmonella, 1 chickenpox, 1-erlichia, 1-tularemia, 1-HBV, 2 rabies exposures with 2 completed rabies PEP. Outreach for Rabies Testing: Since the animal disease lab closed on August 31 and it was our primary local resource for testing animals that were suspected of having rabies, I sent out letters to Marion County Veterinarians updating them on the procedure to utilize the IDPH Springfield lab for testing of suspected rabid animals. The packets included an explanation letter, rabies submission form and guidelines for submitting rabies specimens. TB Monitoring: 7 clients required screening for TB this quarter d/t either a positive TB skin test or from being a new nursing home resident with a hx of being an old + TB reactor. School Physicals: On July 27 th we held a school physical clinic at our Salem office. We completed 23 physicals: 11 sport and 12 school physicals. On August 3 rd we held a school physical clinic at our Centralia office. We completed 27 physicals: 6 sport and 21 school physicals. Shanda Swagler, ANP performed the physical assessments. The fee for the school and sports physicals was $ Immunizations were offered at an additional fee. Lab Testing: 163 lab draws were done this quarter for provided lab services. Outreach: Ms. Yoder attended the John Cavaletto Senior Fair on August 14 th at the Salem Community Center. Approx seniors attended. Yoder handed out health information and took blood pressures. Off-site TB clinics: On September 11 Shelley Yoder went to CHS and administered TB skin tests to 39 students. September 18 went to SHS and administered TB skin tests to 10 students. The students were either CNA or Child care students requiring a TB skin test for school clinicals. Page 6 of 9

7 Client Surveys for July 2012: General Surveys: 100 General surveys were conducted this quarter 87 females and 13 males responded to the surveys The primary age group that completed the surveys was the year old group The primary services provided were WIC followed by immunizations The following gave very satisfied responses: on a 1-4 scale with 4 being very satisfied. 86% to convenience of location 66% for length of time spent waiting (30% gave a #3 response) 83% to amount of privacy during the visit 88% to courtesy and respect shown by the staff 89% to skills and knowledge of staff When asked if client s would like other services offered. No comments were received. Comments made for ideas to improve services are as follows: - More advertising for current services. - The MCHD has a wide variety of services already offered. 4. Administration: Lori Ryan 1. Grants A total of 20 grants have been submitted. To date only 1 rejection; March of Dimes. Two grants are pending; Office of Women s Health and MRC. 2. We Choose Health Grant 13 health departments teamed together and wrote We Choose Health Transformation Grant. 3 topics chosen: 1. Coordinated School Health 2. Baby Friendly Hospital 3. Workplace Wellness In the process of hiring 5 contracted people to work throughout the 13 county area. 3. Affordable Care Act If the Affordable Care Act is passed, the health department will most likely loose immunization clients due to not being able to bill Insurance. Ms. Ryan along with other HD Administrators, are looking into a company that will assist us to set up contract with insurance companies. Page 7 of 9

8 Staff Appreciation Day July 31, 2012 the health department held our Staff Appreciation Day. Artina Fogerson and Pam Snyder have been employed with the health department for 10 years. Bright Smiles In a few months HD WIC nurses will provide fluoride varnish to the WIC children up to 36 months of age. Staff Education Day September 27 both health department offices will be closed for our Staff Education Day. 2 speakers will be present along with presentations from our staff. Changes in Staff Rose Straeter resigned as Breast Feeding Counselor. Linda Hawkins; Breast Feeding Counselor, is now full-time. Cathy Nattier resigned as the Centralia Front Desk Clerk and Erica Butts has been hired to replace Cathy Nattier. b. FY 13 Budget Ms. Ryan reviewed FY 13 budget with board members. Mike Morton made a motion to accept the FY 13 budget; Dick Haney seconded the motion; all in favor; none opposed. The FY 13 Budget was approved. c. Policy and Procedure Updates Ms. Ryan reviewed handouts with 2 new policy and procedures. Vehicle driver s license, insurance and driving distractions. Secondly, Marion County Health Department a non- smoking campus. Stan Morrison made a motion to accept the Policy and Procedure Updates; Tom Turner seconded the motion; all in favor; none opposed. The Policy and Procedure Updates were approved. d. Tobacco Free Campus / Board of Health Resolution Lori Ryan presented a resolution for Marion County to be a smoke free campus and grounds. Keith Kessler made a motion to accept the resolution, Paula Strother seconded the motion. All were in favor; none opposed. The resolution was approved. Page 8 of 9

9 VII. CLOSED SESSION Closed Session was not needed at this time. VIII. ADJOURNMENT Mike Morton made a motion for adjournment at 7:35 p.m.; Keith Kessler seconded the motion.
