7 CBO Planning Grant In order to achieve the DSRIP goal of a 25% reduction in avoidable emergency room visits and hospital admissions, it is vital to address to various social determinants of health that contribute to these avoidable visits/admissions. Housing Nutrition Transportation Legal-Aid Community Based Organizations (CBO) are designed to address many of these issues. Participation from CBOs, many who are unable to bill Medicaid, are essential to the success of DSRIP Bronx Health Access 6

8 CBO Planning Grant DOH developed a RFA to provide planning grant dollars to support CBOs who do not bill Medicaid to participate in DSRIP and PPS networks. Final grant award will be issued to three regional consortiums of CBOs in the State. The grant is to assist CBO Consortiums in planning activities to identify business requirements and formulate strategies for shortterm needs as well as longer term plans If CBO meets DOH eligibility requirement, BHA PPS will provide an initial consultation with our Grant Consultant, One River Grants, at no cost If a CBO decides to apply as Consortium Lead, BHA PPS will write a letter of support (application requirement) Due Date for Applications: 8/16/2016 by 4:00 p.m. ET Bronx Health Access 7

26 About Us Began over 25 years ago as a support group for women with AIDS Currently offers prevention and self-care education and services for HIV/AIDS, diabetes and other chronic diseases, as well as award-winning youth mentoring programs for children with ill or deceased parents and for young men on probation Utilize, in all programs, peer model of training people most affected by disease to educate others and advocate for the community Empower residents by teaching them how to manage and/or prevent chronic illness through effective health management and self-care Inspire hope, create jobs and improve the health of thousands of people in need 25

27 Purpose of this Presentation Health People is very excited to collaborate with Bronx Lebanon Hospital Center s PPS, Bronx Health Access to engage community partners to help patients with diabetes live healthier lives and reduce hospitalizations and ED use. This webinar will go over the steps to create a Diabetes Wellness Community: 1) Train Staff in Stanford Diabetes Self-Management Program (DSMP) 2) Recommend & Train individuals with type 2 Diabetes 3) Implementation of Stanford DSMP for participants 26

28 Diabetes Self-Management Program (DSMP) Developed at Stanford University Evaluated with randomized, controlled study One year after workshop completion, participants show significant improvements: - Depression - Communication with physicians - Healthy eating - Patient activation - Self-efficacy Additional evaluation shows: - Fewer hospital days - More PCP visits It is the process in which the program is taught that makes it effective. Classes are highly participative, where mutual support and success build the participants confidence in their ability to manage their health and maintain active and fulfilling lives. 27

29 Diabetes Self-Management Program (DSMP) Subjects covered include: - Techniques to deal with symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, emotional problems such as depression, anger, fear and frustration - Healthy eating & exercise - Appropriate use of medication - Working more effectively with health care providers Participants: - Make weekly action plans - Share experiences - Help each other solve problems they encounter with creating & carrying out their self-management program 28

30 Diabetes Self-Management Program (DSMP) 6-session Workshop: - One 2 ½ hour class per week - Facilitated by 2 Trained Leaders o 1 or both of whom have diabetes participants per Workshop - Offered at scheduled times & identified partner locations Participants receive: - Stanford-recommended workbook & CD - Nutritious Snack - Support from other group members & Coaches 29

31 Overview The Bronx Lebanon Bronx Health Access and Health People PPS Project is creating a Diabetes Wellness Community in 3 Steps: 1. Train Staff as Leaders to facilitate DSMP Care team staff from clinical & community partners will be trained by Health People to facilitate the Stanford DSMP 2. Staff recommends potential Peer Leaders to be trained to facilitate the DSMP Individuals with Type 2 Diabetes & Medicaid and other qualifications will be trained by Health People 3. Implement DSMP Workshops for participants with Type 2 Diabetes Participants are referred by Staff to attend 6-session DSMP, which is facilitated by Staff and/or Peer Leaders 30

32 Step 1: DSMP Staff Leader Training 4-Day Training: - 9am-5pm each day (split 2 days a week for 2 weeks) - Facilitated by 2 Stanford-approved Master Trainers - Up to 20 Potential Staff Leaders per Training Participants: - Learn to facilitate the Stanford DSMP 6-session workshop - Receive a Certificate of Completion & Recognition as Stanford Educator - Are approved to deliver life-changing workshops to patients with Type 2 Diabetes - Receive Stanford Instructor Manuals - Have support from group & Master Trainers - Get nutritious snacks or meal at each day of training 31

33 Step 2: Recommend Potential Peer Leaders for 5-Week Training Health People expects to interview 60 potential peer leaders to have 20 begin Training Good candidates are men and women who have Type 2 Diabetes and Medicaid and: 8 th grade literacy level Take good care of themselves Can likely commit to 5 Week Training (4 days/week) 6 Month Internship Part-Time work schedule Want to help the community and enjoy working with people Willing to facilitate DSMP 6-Session Workshops Our peer educators are directly representative of their community and they reach, teach and inspire others to fight for better health. 32

34 Selected Potential Peer Leaders who attend the 5-Week Training will receive: 5-Week Training - 4 days a week - Includes both Health People Peer Educator Training & Stanford DSMP Training Certificate of Completion & Recognition as Stanford Educator Stipends ($10/day) & MetroCards for each day of training Eligibility for Internship, then Part- Time Employment After Training, Peer Leaders have: 6-Month Internship ($50 stipend & MetroCards for 2 Workshops + 1 Peer Meeting per week), then Part-Time Employment (10 Hours per above minimum wage) 33

35 Step 3: Implementing DSMP Courses Stanford indicates that workshops are facilitated from a highly detailed manual by two trained Leaders, one or both of whom are peer leaders with diabetes themselves. To facilitate DSMP courses, options are: - 2 Staff Leaders (if one has diabetes) - 2 Health People Peer Leaders - 1 Staff + 1 Health People Peer Leader Coming soon! 49 Spanish for Dental Residents The 1199SEIU Foreign Language Program offers contextualized language training for dental staff to enable them to communicate with their non-english speaking patients and family. Basic training is 40 hours, consisting of 2-hour sessions, twice a week for 10 weeks. We can also accommodate half day or full day trainings (7hrs/day for 6 weeks). Class size consists of up to 12 participants.

66 Context The Bronx Health Access PPS (BHA) has created this document in response to the NYSDOH Cultural Competency & Health Literacy (CC/HL) requirement to develop a training strategy with a focus on addressing the drivers of health disparities beyond the availability of language appropriate material. We have reviewed the community needs assessment and the results of the workforce survey that was conducted in January We identified challenges to providing culturally competent care as well as assessments and tools that will help us achieve the vision of providing care the results in optimal health outcomes and a satisfying patient experience. We identified three overarching elements of the training strategy: Trainings Materials Resources to improve organizational practices in CC/HL This training strategy is meant to address the needs that were identified in the CNA and the workforce survey among the key targeted groups. In addition to these elements, BHA plans to hire a CC/HL Coordinator who will be responsible for overseeing the implementation of the CC/HL training strategy across the project workgroups and partner organizations.
