Each year, we engage over 40,000 New Yorkers through our network of 31 sites in Manhattan and Brooklyn. Our mission is to empower the individuals we serve through comprehensive, quality services that strengthen families and meet the current needs of the community. Established older adults program provides comprehensive services to the diverse population of the Lower East Side. These services incorporate the Department for the Aging Health and Wellness model into the daily activities. Services include congregate and homebound meals, case assistance, social and recreational activities. Ongoing training and professional supervision provided. Responsibilities: Perform intake, on going assessment, and develop individualized, goal oriented service plans to ensure that participants remain healthy, in their homes, and receive the social, recreational, and health services they need. Provide outreach, information, case assistance, entitlement evaluation, referrals and advocacy Link services to homebound and at risk older adults to promote successful community living. Provide crisis intervention and counseling as needed. Coordinate and bridge services and case conferences and ensure seamless service delivery. Document services provided to clients in writing (hard files and web based case management system); keep client files up to date; provide regular statistical reporting on outcomes. Coordinate with LEARN staff for quality control and integration of services Assist in designing documents or forms for record keeping and outreach purposes; assist in program planning and evaluation Assist with daily tasks for Meals on Wheels program ordering, phone calls, changing route sheets, coordinating with meals on wheels case managers and contractor Assist with program activities Complete data entry into DFTA database system and Salesforce daily, weekly and monthly Participate in team meetings and coalition meetings Attend professional development trainings Keep up to date on all information and resources pertaining to older adults Collaborate with program staff and supervisors in a professional manner Other duties as assigned by program director Qualifications: 04/26/2018

University Settlement s Families Thriving Program is a new program that will serve families and children on the Lower East Side of Manhattan through community and home based services. The program will include a team of 5 staff members (a director, two social workers, a family support worker and an administrative support staff) who will be trained in an evidence based model centered on building on the strengths of families. The Social Worker will assist the Program Director and community partners with serving families and youth with a range of needs. The Social Worker will meet with this cohort of families and youth on a regular basis and work closely with partners such as schools, community based organizations, and after school programs in the planning and facilitation of successful outcomes. Responsibilities: Participates with interdisciplinary team members in developing and implementing program policies, procedures and individualized services Provides direct, individual and/or group social services in the home/community to each family referred by self and community partners Assesses the strengths and needs of clients and families Conducts intensive outreach to clients and families Tracks and follows up on all referrals made Coordinates social work and supportive services between the program and other agencies providing social and educational services in both the school and local community. Serves as an advocate for families rights within the community and as a mediator between community partners as needed Remains available for crisis intervention social work as the need arises or arranges for such coverage by emergency social services Records professional assessments, identifies problems, maintains treatment plans and progress notes as required by program regulations, policies, procedures and the Agency Develops and facilitates individualized service plans Participates in the planning and implementation of psychoeducational and therapeutic workshops, presentations and trainings in the community and with community partners as needed Participates in training and execution of evidence based models Performs other related duties, as required 07/16/2018

5 1574 Job Announcement Mental Health and Personal Development Position: Supervisor: Assistant Director for Clinical Services Director of Mental Health Services Overview: The Door is an unparalleled model for youth development, offering a comprehensive range of integrated services within a single site for nearly 10,000 New York City youth each year. Our mission is to empower young adults to reach their full potential by providing comprehensive youth development services in a diverse and caring environment. At The Door, youth can access health care and education, mental health counseling and crisis assistance, legal assistance, college preparation services, career development, housing supports, arts, sports and recreational activities, and nutritious meals all for free and under one roof. By providing participants with our suite of integrated services, we seek to provide any motivated young person with the tools, resources, and opportunities needed to successfully transition to adulthood. The Assistant Director for Clinical Services will provide clinical leadership to the agency s mental health program by giving ongoing training, clinical supervision, and task management to the mental health team, and by reporting to the Director of Mental Health Services on quality assurance and clinical compliance with regulatory and quality assurance standards. The Assistant Director for Clinical Services will also provide clinical consultation to other programs and units within The Door, and will work with the Director of Mental Health and other management staff across the agency to ensure integration and accessibility of mental health services among all Door programming. Responsibilities: Maintain an agency wide youth development focused framework (including methods, modalities and strategies) for the provision of mental health services Oversee and continue to improve implementation of a clinical framework for screening, engaging and supporting youth and young adults (up to age 25), including those with severe and persistent mental illnesses and/or who have histories or risk of homelessness Provide daily oversight of Article 28 mental health services, including clinical supervision of an interdisciplinary team of psychotherapists and interns; oversight of crisis, short term and long term services that address needs of young people; assurance of documentation compliance; implementation and ongoing development of policies and protocols; participation (with Director of Mental Health Services and other stakeholders) in program planning and evaluation; and adherence to contractual requirements as managed by Director of Mental Health Services Provide field placement supervision for Master's Level students Work collaboratively with University Settlement's Article 31 staff to promote satellite services Provide support to onsite Article 31 therapists Work with other supervisors to ensure appropriate integration of departmental programming Participate on agency wide task forces or teams including Behavior Management Team Ensure compliance with funding sources and contracts, including timely submission of reports and data collection. Ensure tracking of behavioral health outcomes and goals for the department and integrate a system of data collection and outcome measurement (tools, tracking systems, etc.) In collaboration with leadership, develop and direct mental health services consistent with NYS Healthcare and Medicaid Reform Work in an ongoing way to ensure deeper integration of behavioral health and primary care at The Door Ensure ongoing quality assurance and clinical compliance with regulatory and quality assurance standards (DOH, HRSA, PCMH, etc.)

Programs within the Career and Education Services Department (CareerED) are designed to help both in school and out of school youth to set and attain their educational and career goals. CareerED services allow a young person to craft a personalized life plan defining the steps they will take to achieve those goals while developing the necessary skills for future life success and independence. The Door s EPOCH Program aims to prepare out of school and out of work young people for meaningful placements in college and/or employment through the provision of comprehensive education and career services, which develop potential for economic self sufficiency. The Supervisor of Housing Based Career and Education Initiatives will be responsible for management and outcomes achievement of career and education programs that The Door offers at our supportive housing program on the Lower East Side and through a new partnership with Women In Need in East New York. Specific duties include development and implementation of work readiness training, establishing and maintaining comprehensive referral systems, supervision of case management services, development of employment, internship, and training placements, and ensuring that young people are completing internships, and obtaining and retaining employment. Responsibilities: Provide training, support, supervision, and task supervision to staff across multiple sites including a Career Advancement Coach, Recruitment and Outreach Coordinator, and Placement Specialist Work in partnership with the managers and staff at The Door s supportive housing program and at Women In Need to develop on site career and education programming as well as connections to career and education programming offered at The Door that will meet the life skill, educational and employment needs of young people living in these residences Create, implement, and monitor work readiness curriculum and coordinate satellite and agency implementation of work readiness and job search workshops and coordinated care activities to attach youth to Career and Education program services Develop program activities and workshops to keep participants engaged during the placement search process.
