1 Prisons and Social Care Equipment in the Royal Borough of Greenwich The Royal Borough of Greenwich (RBG) is responsible for assessing and delivering social care in Greenwich prisons from 1 April 2015 in line with the Care Act 2014.

3 Three Greenwich Prisons HMP Belmarsh 910 prisoners including an average of 60 Cat A s HMP YOI Isis 628 prisoners ageing from 18 upwards HMP Thameside 1232 prisoners. Total 2770 prisoners of which less than 20% are Greenwich residents.

4 Definition of Social Care Social care in accordance with the Care Act 2014, aims to promote an individuals wellbeing and independence, in addition to preventative strategies and context of Greenwich prisons is defined as the provision of social support, personal care, protection or social support services to adults in need or at risk, or adults with needs arising from illness, disability and old age. This includes if the adult has a condition as a result of physical, mental, sensory, learning or cognitive disabilities or illnesses, substance misuse or brain injury. Often an overlap with health and social care services, partnership working is key

5 Eligibility for Social Care Eligibility for services is determined following an assessment by the RBG Social Care Team Difficulties with managing and maintaining nutrition managing personal care and hygiene managing toileting needs being appropriately dressed maintaining a habitable home environment maintaining family / personal relationships accessing and engaging in work, training or education Making use of facilities or services in the community

6 Referrals for Social Care Referrals to the council are via a secure referral that can be accessed by healthcare and prison colleagues Prisoners are screened in the first night centre by healthcare as part of their primary screen and referred from there. Prisoners can self refer requesting a formal referral to be sent to RBG by a staff member

7 Assessments for social care The Royal Borough of Greenwich (RBG) has an assessment team who will carry out a comprehensive Social Care assessment. RBG Social Care Assessment Team Counter Terrorism Clearance [CTC] Change Grow Live (CGL) are commissioned to provide the social care service for all eligible prisoners. Following assessment prisoners are notified of the outcome and a support plan will be provided to all eligible prisoners. Copies of assessments and support plans are held healthcare, CGL

8 Delivering Support Assistance with personal care tasks is delivered by CGL Domestic care is delivered by Care and Support Orderlies in line with Prison Service Instruction 17/2015 Prisoners Assisting Other Prisoners.

9 Transfer and release RBG are responsible for the continuity of care when prisoners are transferred or released. We liaise with other local authorities to arrange for care to be delivered in the community where prisoners are eligible. On release all prisoners ordinary residence rights return to what they were prior to coming into prison.
