3 SERVICE PROFILE Brief Overview Wellington House is a Day Service within the Northumberland Recovery and Rehabilitation sector. It caters for clients with ongoing Mental Health Problems. These clients will usually have experienced problems for many years. The service aims to offer a responsive, needs led approach to people who are vulnerable; this can encompass physical, psychological or social issues. We balance the goals of recovery, social inclusion, maintenance, crisis stabilisation and enhancement of function to enable the client to achieve and maintain a quality of life that is acceptable to them in their own community. The unit can also offer an extended hours and outreach service (in the evenings or at weekends), allowing the service to offer activity and interventions outside of the normal working day. We operate as an integrated team consisting of nurses, occupational therapists, support staff and medics. Care Delivery We offer a Primary Worker approach to the delivery of an individualised recovery package. A Primary Worker could be either a Registered Mental Nurse or an Occupational Therapist. Referrals (see clinical criteria) are discussed within the clinical team, then allocated to an initial contact person who will arrange an interview. This is then discussed with the team and if it is felt appropriate, allocated a Primary worker and admitted to the unit for a six-week assessment period. During this time we aim to establish whether or not we are able to meet the clients needs (see attached sheet for examples). If the answer is yes, the client is admitted on a more permanent basis, if not they would be referred back to the referrer (see attached sheet for examples). Who Can Use The Service? People who are 18 and above People who live in the Blyth Valley area People who have been referred via a Care Coordinator Clinical Criteria Clients must be suffering with mental health problems Clients must not have a primary diagnosis of substance misuse; (there are services specifically dealing with these areas). Clients must not have a degenerative cognitive illness; (we do not have facilities to ensure the safety of this group of clients).
4 Client Involvement The team aims to deliver a service, which is responsive to the needs of the individual clients. Clients (and their families when appropriate) are encouraged to take an active role in the assessment, recovery planning and discharge process. Wellington House works towards providing a service, which is based upon the development of therapeutic relationships, encouraging mutual respect, empowerment and is based around client hopes, goals and aspirations. Clients are also encouraged to contribute to the planning and development of services via staff/client meetings, recruitment of staff, and utilising volunteers as advocates. Clients are supported in making choices about their lives and are involved in their own recovery journey e.g., there is a section within our assessment for client signature, contribution and opinions. Hours of Operation We currently offer a Monday to Friday service, which operates from 9.00am until 5.00pm. However there are some occasional optional evening and weekend working. Student Placements Each student will be allocated a named mentor prior to his or her first day of placement. Students are generally expected to work the hours of 9am to 5pm Monday to Friday. However, they are encouraged to be flexible, and there is an expectation that they should be involved in some of the out of hours activities. This will enable the student to optimise the therapeutic relationship. At Wellington House an Induction Package is also available, this gives more detail on practical issues and includes more descriptive information. Students are encouraged to be actively involved in their own learning package. General Expectations Students are not required to wear a uniform. However, appropriate dress is expected. Casual and smart. Service Expectations of The Student The learning experience is the students personal responsibility, whilst we are happy to contribute we will not take the lead. The student will be expected to actively participate and will be expected to be motivated to learn. They must also be proactive in seeking knowledge and experience. They should show initiative and should be polite and professional at all times. In return we will ensure the student is welcomed into the clinical team and is fully supported in the achievement of their learning outcomes. Team Consists of

15 Glossary of Terms KGV - Krawiecka, Goldberg & Vaughan (1977) An assessment tool used with Psycho Social Interventions. This tool is a brief measure of mental state. It has a variety of uses and will help to establish a concise summary of symptoms experienced by an individual. There are fourteen sections to this tool including depression, mania, and includes some more subjective measures. BDI-II - Becks Depression Inventory An assessment tool, which has specific questions, which should give a baseline to the range of depressive symptoms, someone suffers. A multi choice type questionnaire. BAI - Becks Anxiety Inventory An assessment tool, which has specific questions, which should give a baseline to the range of anxiety symptoms, someone suffers. A questionnaire, which helps to rate the severity of symptoms. BSS- Becks Suicide Scale An assessment tool, which has specific questions, which should give a baseline to the range of suicidal ideation, someone suffers. A multi choice type questionnaire. There should also be an indication given as to the severity of risk and the likelihood of self-harm.
16 BHS - Becks Hopelessness Scale An assessment tool, which has specific questions, which should give a baseline to the range of hopelessness, someone suffers. A questionnaire, which offers true/false answers and will give an idea as to the extent of negative attitudes/pessimism to the future. HAD - Hospital Anxiety & Depression Scale A questionnaire that rates anxiety and depression symptoms. Lancashire Quality of Life Scale A scale, which helps to identify areas of mastery and pleasure for a client. ACTIVE- Actively Collaborating Towards Independent Ventures through Enablement A unit based assessment tool done in conjunction with the client. It will help identify risk areas; areas where a client will need extra support to enable them to function in a community setting. BVDS Assessment - Blyth Valley Day Service Assessment Tool A unit based holistic assessment tool done in conjunction with the client. It will help a client to identify areas of their life where they need extra support. It looks at the person as a bio psychosocial individual.
17 Recovery Star A tool for supporting and measuring change with service users. An outcome tool supporting recovery. It provides a map of the journey to recovery. Covering 10 core areas of assessment, which have been found to be critical to recovery. It is an organisational priority.
