2 Individual and Family Consultations Given the challenges of caregiving for people with dementia, individuals and families often elect to work with a dementia specialist to develop a care management plan. Having a proactive care plan in place that changes as the disease progresses is essential for maintaining the highest quality of life for all involved. A skilled dementia specialist can anticipate the changing needs of the caregiver and the care recipient, provide guidance and support on how to best manage the disease progression for all involved, and connect families to the vast number of community resources available to provide support throughout the journey ahead. This brings optimal support to caregivers allowing them to better care for their loved ones and keep them at home for a longer period of time. The caregiver support group has allowed me to share my inner feelings with others who feel the same way. Caregiving can go on for years and requires special understanding, patience and skills. Hope Dementia & Alzheimer s Services is here to help. Our goal is to work with you to create a comprehensive care plan starting with an initial assessment to determine your unique individual needs. We offer a wide range of services for those caring for a loved one with dementia, as well as other family members, healthcare professionals and community agencies.
