3 Company Background d Care Dynamix is a national provider of turnkey, on-site flu clinics as well as for other vaccinepreventable diseases and health screenings d Based in Atlanta, Care Dynamix was founded in 2002 when a practicing family care physician identified the need for high quality, affordable, preventative health care that was based on convenience, no matter the location or time d The company expanded nationally and in 2008 signed an arrangement to be the exclusive immunizer for all PTA schools d In 2009, HIG Capital, an $8 billion investment fund, made a minority investment in Care Dynamix to aid the company s expansion in the healthcare services field d In 2010, Care Dynamix partnered with Families Fighting Flu to expand the message of the importance of flu vaccinations

9 Clinic Basics d All Care Dynamix nurses are screened, background checked and trained by Care Dynamix prior to the clinic - Specific protocols and emergency procedures are the core of their training, and available for review by clients d Care Dynamix nurses arrive 30 minutes in advance of the clinic with all required supplies, including a VIS sheet and consent form for each participant d Nurses clean up all waste, including sharps and medical, before departing d Immunization information from the clinic can be uploaded directly to your state s registry, if desired
