2 2 Focus items 2.1 Healthwatch Hampshire (HH) update Madeline Close, Healthwatch Hampshire VCS and Diverse Communities Officer Page 2 Madeline informed the Group that she came into post three years ago and that HH has a close relationship with the Care Quality Commission. HH collects patient experiences of any NHS and Social Care Services. Madeline handed out information leaflets to the group. Madeline would appreciate being invited to GP surgeries and talking to patients in the waiting area. The comments received will be verbally fed back to the Practice Manager on the same day. Every GP in Hampshire has been telephoned and asked how new patients are able to register with that practice and what was required of the patient. Full terms and conditions of patient registration are covered in the following: Part 13 of the NHS England GMS Contract 2015/16 HH will carry out a mystery shopper programme and the results will be published. This will show the different ways patients can make appointments. Madeline took part in listening to Out of Hours GP telephone messages which can vary in clarity. There is a domiciliary care survey being sent out this week by Hampshire County Council and is available for anyone in receipt of domiciliary care. The results will be sent back to HH to be analysed. Madeline described an art project named My Face, My Story, which gathered experiences of people with learning disabilities about accessing health and social care services. This has just been completed for people with learning disabilities across Hampshire. Annual health checks are not always being carried out for people with learning disabilities and this is a recommendation from the Department of Health/NHS England. Practices are paid over 100 per health check. Elizabeth stated that the CCG are investigating this issue and that some practices are carrying out the health checks and do not want to receive the funding, but they are not recording the checks which they need to do. The process involves listening to the carers as well to make sure the health check takes place. Research was carried out about when people are first certified as being visually impaired. Madeline is working on this with West Hampshire CCG who is the lead for the wheelchair service. Nick asked about the involvement of the MCP with providers who specialise in supporting people with learning disabilities and asked if this could be fed back
3 to the CCG leads. Action: Elizabeth will follow-up on this. Madeline said there had been research carried out for people who are first registered with visual impairment to see if they receive advice, information and signposting to adult services. There are Eye Clinic Liaison Officers who liaise between health, eye clinics and adult services. It was found that most people had not heard of these officers so many people had not received mobility training. A report will be sent out in the next two weeks. Low vision aid clinics are commissioned by Fareham & Gosport and South Eastern Hampshire CCGs. HH are working with Hampshire County Council and have a number of recommendations. They also operate advice and information services together with formal complaints, using an advocacy service. The Royal National Institute of Blind People (RNIB) has reported that 180,000 people who are visually impaired do not leave their homes alone because they have not received advice on living independently. Nalin said that the PALS service is hospital based and is the first point of call for complaints. If the complaint is across different services then the initial complaint will go to the service(s) concerned and then HH can assist if the complainant wants to take the matter further. The ombudsman can be involved for example in the case of gender identity referrals. People can also direct complaints to the CCG. There are 148 Healthwatch offices in the UK and Healthwatch England is the National Board which the offices feed up to. The same branding is used by all. HH is run by a Community Interest Company and others are run by charities. All are seeking volunteers. HH has various paid managers. Linda asked how many people are aware of HH. Madeline replied that it is difficult to say but they produce numerous posters and packs and have circulated these to GP surgeries as well as other organisations. The Citizens Advice Bureau (CAB) Hampshire is one of the partners of HH. Much of the feedback is received through the CAB. Action Hampshire is another partner and the other partner is Health and Care, based in Dorset. They are all charities. Ann suggested that the surgery visual message boards could be used to advertise HH together with displays at the walk-in service. Norman informed the group that the walk-in service at the Emergency Eye Department at Queen Alexandra Hospital has been closed for three years. All referrals are by appointment only. 2.2 Multi-specialty Community Provider (MCP) Contract update Elizabeth Kerwood Elizabeth gave a brief overview of how the MCP contract is being formed which is attached below: Page 3
4 MCP contract briefing slide Norman and Steve recently attended the Vanguard meeting for Havant, Waterlooville and Hayling Island. Five services were identified to be tested, for example, the paramedics scheme and the leg ulcer clinic. Norman and Steve both felt that they had not been able to add any value to the Vanguard as they have not been involved in the process and are concerned that patients views are not being included. Elizabeth felt that board level may not be the best place for patient involvement and asked the group what would be the best place to give input and influence. Norman asked who is responsible for the projects in the Vanguard and could the learning taken from each project be shared across the patch. Madeline said that there have been learning events across the whole of the Vanguard area. Elizabeth offered to contact Rob Guile, Vanguard Locality and ask him to meet with Norman and Steve Crisis Care Project Homewell Practice Steve Southwell A GP in Bosmere was concerned about the difficulty in knowing where to refer patients who have mental health and substance misuse issues. One of the Vanguard projects is trying to pilot a crisis care project for emotional stress and mental health needs. This project will have a MIND practitioner based in the practice one day per week to manage the initial crisis, before directing the patient to the Improving Access to Psychological Therapies (IAPT) service. Steve asked the Group to find out if this problem exists in other surgeries and could each PPG member send feedback to Steve. Action: all Making Every Contact Count Training Norman Proudfoot Norman gave a brief summary of what was involved in the training and how useful it had been. 3 Standing Items Regular Updates 3.1 Feedback from PPG representatives Elms Practice, Hayling Island Ann Tate The last planned meeting of the PPG had to be cancelled owing to a bereavement and will now take place on 28 th November. Page 4 Ann reported that the impact of the Practice s in house Quick GP Message service continues to improve as more patients become familiar with it. The reduction in the number of telephone consultations has been considerable. Modifications have been made in the light of experience, and the patent application is going forward. It should be available for other practices to use in the future. Some of the concerns surrounding the grouping of practices have been eased following an area meeting. Therefore, the planned CCG meeting with the Elms
5 Practice did not take place. Further information will be given to the PPG at the next meeting. Rowlands Castle Norman Proudfoot A meeting was held on 20 th October. It was the first attendance by one of the Partners. It was agreed that if possible, a Partner would attend alternate meetings. The annual flu jab session was held on a Saturday morning in October, with a member of the PPG in attendance to help as required. There was some discussion on the Surgery s participation in the MCP, which seems to be a grey area to most of us. The website is up and running, and is proving very popular with patients. Bosmere Medical Practice Nalin Deshpande The PPG meeting was held on Thursday 10 th November 2016 Consultations: Bosmere have contributed to the following consultations 1) Reducing noise at night in Queen Alexandra Hospital 2) End of Life Care at Queen Alexandra Hospital The lack of patient representatives and PPG members at these events is a concern raised by Linda Bumford. Working together: two representatives from the PPG went to the first Local Care and Learning event at Horton Heath. Practice developments: work has started on re-designing the practice newsletter. A small working party is sharing ideas to make sure the information provided works for both the practice and patients. A newsletter from another practice in Milton Keynes (produced by the PPG) has been shared with the view of generating new ideas. Ideas on fundraising are to be considered alongside this work. A new practice website is now up and running, members of the PPG were involved in the final stages of work before it went live. Regular reviews will be required and it is hoped members of the PPG will become involved in this. E-consult is now up and running at Bosmere Medical Practice. Surgery Signposters are now based in the Bosmere Practice. Sally Bigham, Project Manager from Gosport Voluntary Action (who is managing the pilot), will report to the PPG in November. A Mental Health Practitioner from MIND is now providing a service in the Practice. Practice concerns: Dr Ben Bracegirdle (Partner and PPG link) has raised serious concerns about the lack of support for patients who are trapped in a cycle of long-term mental health issues and drug and alcohol abuse. This is an area where care provision needs to be addressed and is being bought to today s Locality Patient Group meeting by Steve Southwell from Curlew.Homewell practice. Page 5
6 Page 6 Emsworth and Westbourne Surgeries Jim Strudwick and Joan Marsh During October the members of the PPG committee helped to direct the patients into four rooms for their annual influenza vaccinations. The nurses and doctor found the PPG members very useful in maintaining the steady flow of patients in and out of the surgery several hundred arrived on each Saturday morning. Unfortunately, the numbers were down this year, due to either local pharmacies also offering free vaccinations or the belief that last year s vaccine mixture of anticipated flu types were somewhat inaccurate so that some at risk patients still caught influenza. On Wednesday 26 th October the PPG held the latest of its health education programme talks by representatives of italk and The Samaritans. The speakers were interesting and spoke about the ways that they could help lonely or depressed patients come to terms with their problems. The PPG was pleased that one of the new breed of Surgery Signposters, Sheila Smith, also came to listen and contribute. This was held in the new Emsworth Baptist Church, which has an excellent auditorium with 330 seats. We are pleased that Emsworth Surgery has been awarded the status of Dementia Friendly Surgery as has the town of Emsworth itself. Horndean Surgery Caroline Footman The new surgery is progressing slowly, in spite of problems, still hopeful to open late spring next year. Latest initiative is a joint programme of sessions with two Petersfield surgeries to encourage awareness and self-management of conditions. Male Health is a topic for a Saturday morning in February the last session, on babies health, was so over-subscribed that they had to move venues. Many grandparents attended. The surgery has re-joined the National Association for Patient Participation (NAPP), more details at next meeting in January. The surgery is hoping to offer carousel clinics next year with other local surgeries, following MISSION model for asthma, breathlessness and COPD, and diabetes, maybe using a community venue. Homewell.Curlew Practice The last PPG meeting was held on 12 th October. Long term conditions: patients are called in for an annual review in their month of birth. A flag is set on EMIS so that receptionists can book longer appointments. Individual care plans are kept and updated. A health check is offered to patients who are over seventy years old. Prescription process: some concerns were raised about repeat dispensing. Some pharmacy assistants were insisting that patients take all items on the prescription even if some were not needed. This is a training issue that will be
7 fed back to the pharmacy. Dementia Friendly Practice: four groups of patients and carers have been identified and will be hosted at a walkthrough and question and answer session by Kate and a PPG member. Official go live was scheduled for 14 th December but has been put back due to work pressures Patient survey: this was conducted through Survey Monkey and by hard copy forms handed out at reception. There were 519 replies received online and 99 hard copies. Overall 71% of online respondents were very or fairly satisfied with the practice and 74% of respondents used the hard copy forms. This compares to 69% in the national survey which was based on 112 responses. Website update: web designers have been invited to attend to make proposals. New NHS England contract for GPs: Homewell.Curlew are participating in a pilot.
