1 Request for Quotation Evaluation of Physiotherapy Primary Health Care Model in TCLHIN Community Health Centres February 26, 2016 INVITATION TO PROPONENTS: The Four Villages Community Health Centre ( Four Villages CHC ) is seeking applications from consultants with the ability to support the Four Villages CHC and a collaborative of 11 participating Community Health Centres across the TCLHIN in evaluating the implementation of physiotherapy services. 2 BACKGROUND: Eleven participating 1 Community Health Centres (CHCs) across the Greater Toronto Area received funding from the Ministry of Health and Long Term Care via the Toronto Central Local Health Integration Network (TCLHIN) to provide and expand access to physiotherapy services in their communities and priority populations in January The Clinical Directors of the participating CHCs were in agreement that a collaborative and streamlined approach to providing physiotherapy services throughout the various communities would lead to greater efficiencies, and result in better client and population outcomes. The CHC Primary Health Care model which states that comprehensive and community driven primary health care, illness prevention, community capacity building and health promotion are all necessary components of healthy communities 2, created an underlying framework for integration of physiotherapy services into CHCs through a population health approach. As a result, a CHC Physical Therapy Primary Health Care Model was developed containing 6 core elements 3 : Clinic Care (individualized, goal oriented assessment and intervention service delivery, including both individual services and group programs), Outreach (targeted outreach services for marginalized populations who would not otherwise access services, but who may be at risk), Case Management (first contact access for rehabilitative health care needs), Shared Care (provider partnerships for goal oriented assessment and intervention collaborative service delivery for coordinated care within the CHC), Integrated Care (external partnerships for collaborative service delivery for coordinated care across the continuum of care), and Programming Consultation(internal and external partnerships for programming development support to wellness services for preventative, maintenance care and health promotion). SCOPE OF WORK: 1) Supporting the TCLHIN CHC Physical Therapy Clinical Directors Network ( Committee, consisting of Senior Management of the participating CHCs) that will provide oversight of the Evaluation of Physiotherapy Primary Health Care Model in TCLHIN Community Health Centres project using the Evaluation Framework (see Appendix) and working collaboratively with the Physiotherapy Project Lead and Data Management Coordinator Lead, the Consultant(s) will: a) use the data collected in the CHC EHR (Nightingale on Demand) to measure the indicators outlined b) identify any additional relevant measures/indicators to evaluate the features of the model of care described above, c) conduct necessary research to develop client satisfaction and employee engagement tools and identify and execute processes for data collection using these tools, d) provide active project management to the evaluation process, 1 Flemingdon Health Centre, Davenport-Perth Neighbourhood CHC, LAMP CHC, Stonegate CHC, The The Four Villages CHC, Parkdale CHC, Central Toronto CHC, Anne Johnston Health Station, Regent Park CHC, South Riverdale CHC, Anishnawbe Health Toronto, East End CHC 2 Association of Ontario Health Centres 3 Report; Physical Therapy in Primary Health Care : A TC LHIN CHC collaborative initiative,

3 e) facilitate engagement of each CHC to ensure comprehensive and complete evaluation across the collaborative, f) coordinate the collection of data across CHCs, and complete comprehensive data analysis, g) draft all reports/documents/presentations, including a draft and final report, both in hard and electronic copy, h) offer advice and guidance to the Committee to enable a complete evaluation by May 30, 2016 of the elements of the Collaborative Physiotherapy Programming in order that the participating CHCs and funders can understand the successes and gaps in the provision of the first year of physiotherapy service implementation across the TCLHIN. 2) Tasks and Deliverables include: a) Conducting an evaluation that will assess the impact of all 6 elements of the Physiotherapy Primary Health Care Model including both process and outcome measures/indicators (see Appendix). The evaluation will incorporate the following elements, and will, where possible use existing best practice and evidence based measures to measure: i. Service Parameters ii. Client Demographics iii. Service Integration iv. Patient/Population Outcomes v. Model Adoption vi. Client Satisfaction vii. Client Engagement viii. Employee Engagement b) Generating a report for approval by the Oversight Committee, through the Clinical Director Network, for submission to funders. c) Providing recommendations for ongoing and sustainable evaluation of the physiotherapy collaborative programming. INFORMATION REQUIRED FROM PROPONENTS: A brief proposal including the following: 1) Profile and resumes of the consultant(s): qualifications, description of relevant experience of those who will be doing the actual work, research, facilitation, writing, etc.; 2) Description of approach and general plan with key milestones and timelines; 3) A firm cost indicating per diem for each member of the team and the number of days of each for each member necessary to complete the work as per expectations; 4) Out of the pocket expenses, if any expected; 5) References [minimum of two (2)] for similar work; 6) Samples of evaluation reports from previous engagements, if possible 3

4 TIME AND COST DETAILS REQUIRED IN THE PROPOSAL: Proposals should provide a breakdown of the approach, resource allocation, and time required to complete the evaluation. EVALUATION CRITERIA AND SELECTION PROCESS: Proposals will be evaluated based on criteria listed below. Maximum Value of Proposals: The Four Villages CHC will not consider proposals exceeding $25,000. 5 Communications Proponents may direct questions in writing or by to the Four Villages CHC contact. The Four Villages CHC is under no obligation to provide additional information but may do so at its sole discretion. The Four Villages CHC reserves the right to distribute the questions and responses to all Proponents. Intellectual Property If selected, the proponent agrees that the Four Villages CHC will have exclusive right to the unique intellectual property of this project and the proponent will waive in favour of the Four Villages CHC their Copyright Act rights to all concepts, techniques, ideas, information and materials, however recorded, (including images and data) of such intellectual property. Confidentiality If selected, the proponent will keep all information received to complete this project in strict confidence and use solely to perform the obligations for the purpose of the project. Upon completion of the project all information collected from the Four Villages CHC and partner CHCs will be returned to the Four Villages CHC or destroyed in a manner designated by the Four Villages CHC. Privacy If selected, the proponent will not have any contact with patients or patient health information for any purpose whatsoever unless expressly authorized by the Four Villages CHC. No Incorporation by Reference by Proponent The entire content of the proponent s proposal should be submitted in a fixed form and not be dependent on any external links or documents. The content of web sites or other external documents referred to in the proponent s proposal will not be considered to form part of its proposal. Proposal to be Retained by the Four Villages CHC The Four Villages CHC will not return the Proposal or any accompanying documentation submitted by the Proponent. Reserved Rights of the Four Villages CHC The Four Villages CHC may, at its sole and absolute discretion, carry out the RFQ process as it determines to be in the best interest of the Four Villages CHC and to be the most beneficial to the Four Villages CHC. Without prejudice to any other right or remedy available to the Four Villages CHC, the Four Villages CHC may make public the names of any or all proponents, do nothing, cancel the RFQ, reject any or all proposals, accept any proposal in whole or in part and under no circumstances shall the Four Villages CHC be liable for any indirect or consequential damages. 5
