1 Call for Proposals 2017 Solutions East Toronto s Health Collaborative is delighted to launch a call for proposals to fund high-potential projects aligned with Solutions mandate and goals. Solution s Mandate: To develop innovative approaches that leverage resources across the system to ensure that individuals in East Toronto have equitable access to high quality health care experiences that help to build healthy communities. Solution s Goals: 1. To work together to ensure the availability of quality health services that respond to the social determinants of health and result in equitable health outcomes and healthier communities. 2. To leverage the services and expertise of the network s cross-sectoral membership for the common purpose of improving health care experiences, particularly for those who are marginalized. 3. To work collaboratively with public policy decision makers, informing and influencing, as appropriate. 2 Overview I. Objective: To provide funding for projects which will further the mandate and goals of Solutions East Toronto s Health Collaborative. II. Funding& Eligibility: Funding of up to $40,000 is available for a project of up to two years. A maximum of two projects will be selected. Applicant groups must include a minimum of two members of the Solutions East Toronto s Health Collaborative. Funding will be granted only for costs directly attributable to the project direct costs that are appropriate and justifiable for the work proposed. Each item and its cost must be clearly described in the budget. Any grant provided shall be directed to the Solutions member on the project team. Responsibility for planning, directing, reporting and execution of the proposed project will rest solely with the primary applicants (pay master). III. Review Criteria: As per the mandate and goals of Solutions, the proposal will be considered based on its ability to meet the following aims: To improve patient/client experience To involve patient/clients in the process. To be community-based and benefit the people (clients, patients, community) who live in or use services in East Toronto To collaboratively work in partnership with other community members and stakeholders To creatively utilize system resources in new and innovative ways To demonstrate a commitment to have a positive impact on the East Toronto community To demonstrate incorporation of sustainability measures and or lasting impact beyond terms of the project To help improve health equity in our community. Timelines: Expression of Interest Due: Invitation for submission: Closing Date for Submission: November 17, :30 p.m. December 3, :30 p.m. February 2, 2018, 4:30 p.m. Notification of successful project(s) will occur at the end of March. V. Application Process: The application will be divided into two sections. Any interested parties are welcomed to submit an expression of interest based on the template found below. After an evaluation of the expressions of interest, selected projects will received an invitation to complete the full application. The full application includes the following sections: 1) Project Lead & Partners (mandatory) 2) Project Description (30 points) 3) Relevance to Solutions Mandate and Goals (25 points) 4) Sustainability Plan (15 points) 5) Project schedule (10 points) 6) Budget (15 points) 7) Additional sources of funding (5 points) 8) Reporting (mandatory) VI. Submitting your application: Please submit your Expression of Interest application via to Erica Di Maio at by the closing date. A confirmation will be sent upon successful receipt. VII. 5 Expression of Interest Submission of Expressions of Interest (EOI) letters will be accepted until November 17, :30 p.m. Please submit EOI s via to Erica.dimaio@tehn.ca. Please include the following elements in your EOI: 1. Organizations & individuals involved, identifying the lead organization. 2. Provide a brief description of how the organizations involved have worked in this area. (100 words) (Project Lead and Partners section of the full application) 3. Project Description. Detailed description of the project. Include how the project will improve the lives of clients/patients in East Toronto and how clients/patients/community will be included in planning, design and/or delivery of the project. 6 Application Please note, only applications from projects invited to submit an application based on the EOI evaluation will be reviewed. The application includes the following sections: 1) Project Lead & Partners (same as EOI) Please list the names of all individuals and organization involved in the project. Please identify one individual and origination to act as the main liaison with Solutions for the duration of the project and be the pay master. 2) Project Description (an elaboration on EOI, 500 word limit) Please provide a detailed description of the project including the following elements: a. Background b. Objective c. Description of Project d. Evaluation, Impact and Metrics e. Elements of Co-design: How will you involve clients/patients in planning, design and/or delivery f. Health equity: How does this project attempt to improve a health inequity? 3) Relevance to Solutions Mandate and Goals (200 word limit) a. What is the anticipated impact to both community and patient/client experience b. Describe the partnership with community members and stakeholders c. 7 4) Sustainability Plan (250 word limit) Please describe the how the project will make a longer term impact beyond the 2 year limit to the project funding. Include elements of knowledge translations, communications plan, spread and experiential learnings. 5) Project schedule (1-page limit) Please describe key milestones and timelines for the project. 6) Budget (1page) Please outline major expenses for this project. Include in-kind and other funding source if applicable. 7) Additional sources of funding Describe any in kind services or resources provided by partner agencies or any additional grants or subsidies confirmed for this project. 8) Charter & Reporting (please check) Team members are committed to Submitting a Charter prior to launch of the project. Team members are committed to reporting updates on progress regularly to Solutions.
