1 Physical Activity Training in Wandsworth License Application License application form for the period 01 April 2017 to 31 March 2018 This document provides licensing permit options which must be obtained by commercial physical activity providers operating in Wandsworth parks, Commons and Open spaces. 1. Aims and Purpose Help to regulate physical training in the parks, commons and open spaces Ensuring trainer(s) are adequately qualified to operate within Wandsworth Ensuring the safety of trainer(s) and their client(s) Licensing Permit works to enforce the Wandsworth Byelaws which state: No person shall in any open space drill or practice military evolutions or exercises without the consent of the council in writing under the hand of its clerk. 2. Minimum Standards Physical activity providers must meet the following requirements to be eligible to obtain a license: a. REPs Level 3 (or equivalent) b. Public Liability insurance (Minimum 5 Million) c. Risk Assessment d. DBS (if working with Under 18 or disabled/vulnerable people) e. Proof of identification (Both a photo and address is required) f. Electronic Photo (please for ID card) g. First Aid Certificate 3. Enforcement Delegated council staff, parks and events police staff, Quadron services Ltd trading as idverde staff and nominated representatives have the delegated authority to enforce the physical activity licensing permits. Breach of the byelaws may result in prosecution and licenses being revoked. 4. Licensing permits Permits available are set in 2 categories: Personal Trainer Fitness Trainer a. Personal Trainer Permit b. Fitness Trainer Permit 1:1 1:2 Clients (no limitations on days, times locations** per annum) 1:3-1:25 Clients (limited to a day of the week, time slot on a location** per annum) ** Wandsworth Common is limited to only the approved areas which have been set out by Wandsworth Council (see map: WC approved areas for fitness training for more information) 5. Billing Permit fee is to be invoiced on license agreement. Invoices are to be paid in full in advance of the license permit being collected from the sport offices, Tooting Common. Invoices are valid for 30 days from the date of the invoice, after which license agreements will be cancelled.

5 7. Hiring Agreement Hiring Wandsworth parks, commons or open spaces for personal or fitness training will incur fees. Fees are charged based on your activities you identified in 2. Session(s) Information 1. This application does not mean you have the permission to hold the session and that you may be required to provide more information before the application is processed and finally granted permission with a signed agreement and issued a permit license 2. I warrant and represent that I the undersigned have the full authority of my organisation/company to make this application. I certify that the information I supplied is accurate. 3. I agree to pay all fees in relation to the physical activity license permit as quoted by Quadron Services Ltd trading as idverde as per Wandsworth Council sports contract. 4. I acknowledge that once the permit license and payment is accepted no refunds or credits are offered. Any amendments will charged an admin fee of and must be put into writing 7 days prior to any changes being agreed and approved. 5. I acknowledge in some circumstances Quadron Services Ltd trading as idverde in conjunction with Enable leisure and culture may need to cancel or issue a grass ban on a temporary basis (this is usually due to poor weather conditions which may result in your planned activity causing damage to the grounds) in this case no refund is offered and permits are not transferable. Notification of this will be via I certify that the organisation/company will conform to all legal requirements and abide by the terms of any license agreement required for physical activity sessions. 8. Terms and Conditions of Wandsworth Council & Enable Leisure and Culture (who want to remind you) You must abide by all byelaws including those relating to noise and including No person shall in any open space: Sell, offer for sale, exhibit for sale, or distribute any book, pamphlet, leaflet, card, bill, advertisement or literature of any kind whatsoever; Vehicles are not permitted on the commons, parks or open spaces including on any roadway or path under and circumstances with no exceptions (not even to drop equipment or bags). No Erection of structures including pop ups and gazebos You do not have exclusive access to, or use of, the location you have been permitted to use as specified in your license. You must be mindful at all times of other users within the space and that you may not obstruct them, nor prevent them going about their business, nor verbally admonish them. You may only use equipment identified in your permit license agreement. You may not use any other equipment, plant or machinery. You may not affix equipment to any tree, bench, fence, barrier or other structure, including fixed items of fitness equipment provided/installed by the council. When using fitness equipment provided/installed by the council. It must be used only for its proper purpose. Such equipment must not be used for extended periods of time to the detriment/disadvantage of others waiting to use equipment (this is particularly important in the early morning and late afternoons/evenings and at weekends). You are responsible for removing all litter and rubbish generated by you or client(s) and taking it away from site. Misuse of park(s) and greenspace(s) and/or failure to abide the terms and conditions will not be tolerated and may result in your permission/license agreement being relinquished.
