JOB PURPOSE To provide general information and support about gynaecology cancer and palliative care services, to enable women and their families to navigate the health and social care system and make choices that are best for them personally. Responsible for proactively identifying patient and carer needs using knowledge approved tools and procedures to ensure women get the right support to meet their needs, providing appropriate advice and escalating to the registered practitioner where and when appropriate. In conjunction with the Cancer Nurse Specialist (CNS), coordinate access to the right information and education resources to support women in making decisions about aspects of their own care, enable independence and support self-management as appropriate. Develop a partnership approach to working in order to empower the patient and carers.

7 3. DIMENSIONS The post holder will be employed within NHS Lothian and there is a requirement to work flexibly across Lothian to meet the service demands. This may include working in St John s Hospital, and the Western General Hospital, however the Royal Infirmary of Edinburgh will be the main base. The post holder will work within the Gynaecology Oncology Team which is based in the Royal Infirmary of Edinburgh. The team consists of one Gynaecology Oncology Nurse Specialist and four Consultant Gynaecology Oncologists. They provide specialist care for women across Lothian, the Borders, Fife and Dumfries & Galloway with gynaecological cancer. The post holder is not responsible for managing the budget but needs to be aware of the resources available, participate in ordering of supplies and the need to work within the financial envelope. 4. ROLE OF DEPARTMENT McMillan Cancer Support Worker (This Post) The department of gynaecology, including Gynaecology Oncology, sits within Women s Services, which includes obstetrics, neonatology and gynaecology. Gynaecology covers a number of inpatient and outpatient services for women. The department has two inpatient ward settings one in the Royal Infirmary of Edinburgh (RIE) and the other in St John s Hospital (SJH) Livingston. There are outpatient treatment facilities in RIE, SJH, the Western General Hospital, Chalmers Hospital and Roodlands Hospital. Women are referred to the team for diagnosis and surgical treatment from Lothian, Borders, Dumfries and Galloway and Fife.

8 6. KEY RESULT AREAS 1. To support NHS Lothian s values of quality, teamwork, care and compassion, dignity and respect, and openness, honesty and responsibility through the application of appropriate behaviours and attitudes. 2. Triage incoming calls, using a risk assessment framework, and initiate an appropriate response, according to protocols and individual care pathways. 3. Make regular contact with the woman affected by cancer to assess their wellbeing, provide basic telephone advice, refer on or signpost and arrange a face to face visit where appropriate or requested by the woman affected by cancer. Frequency of contact will be depend on the woman s individual needs as assessed by the oncology team 4. Coordination and navigation of the complex health and social care system so that the woman affected by cancer can access the right help at the right time, both during and following completion of cancer treatment, so experiencing seamless care. 5. Assess the holistic needs of the woman affected by cancer, provide relevant information and advice and signpost to other services that can meet their needs. 6. Provide practical and emotional support to the woman affected by cancer and their families, this may be within the clinical areas or by phone 7. Coordinate the necessary assessments, appointments or investigations to fast track people back into the system if required, including recognising and responding appropriately when faced with a sudden deterioration or an emergency situation, alerting the team or enabling rapid response as appropriate 8. Signpost as to where to get advice about possible signs of recurrence or long term effects of treatment 9. Act as an advocate and facilitator to resolve issues that may be perceived as barriers to care or support 10. Provide patient feedback to the oncology gynaecology team regarding patient treatment, progress, presentation and response to intervention 11. Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit. 12. Chaperone and assist as directed by the Oncology Gynaecologist or CNS with collection of specimens and vital signs observations. 13. Responsible for sharing practice skills with other staff, students and contribute to clinical education within area of competency, as required

EQUIPMENT AND MACHINERY The following are examples of equipment which will be used when undertaking the role: This list is not exhaustive: Clinical Equipment for example digital thermometers, sats monitors, blood glucose monitors, portable ECG machines therapeutic mattresses and beds. IT Equipment for example Personal Computer, phones, mobile phone, telehealth units, teleconference, videoconference. The post holder will be expected to be responsible and knowledgeable in the safe use of all clinical and non clinical equipment used within the area ensuring this is checked and maintained and where problem are identified these resolved so that all equipment is fit for purpose. Note: New equipment may be introduced as the organisation and technology develops, however training will be provided. 7b. SYSTEMS The following are examples of systems which may be used when undertaking the role: Maintaining patient records documenting all patient interventions and non-direct contacts e.g. TRAK, MIM system, Picture Archiving Communication Systems (PACS), PIMs. Internet based clinical information sources e.g. E-Library Update department shared drive/intranet site Use of intranet to access information within NHS Lothian Daily use of for communication Microsoft Office - Formatting and populating spreadsheets and databases to produce statistics and reports as required Health & Safety, Datix and COSHH Systems Note: New systems may be introduced as the organisation and technology develops, however training will be provided 8. ASSIGNMENT AND REVIEW OF WORK The clinical caseload is generated by the specific service needs of the clinical area. Further tasks may be delegated on a daily/weekly basis. Access to advice and support from the Gynaecology Oncology nurse specialist / senior staff will be available as required, and may be indirect. The workload will be delegated by the nurse specialist and the post holder may work independently on a day-to-day basis, being accountable for own actions, with input/supervision as required. Formal and/or informal meetings/support with senior staff for purposes of clinical discussion will be regularly available and form part of routine practice. Participation in the appraisal process through Personal Development Planning and review in line with the Knowledge and Skills Framework and continuing professional development will be facilitated via the nurse specialist.

DECISIONS AND JUDGEMENTS The post holder is responsible for carrying out delegated patient holistic assessments in line with the level of competency and assessment tools, in a safe and effective manner. These may be direct face to face consultations or indirect via telephone, or post. Triage incoming calls using a risk assessment framework and escalate clinical concerns to the appropriate clinician. Assess the holistic needs of the woman affected by cancer and provide relevant information, advice and signpost to other services as required. Prioritise own delegated caseload. Make judgments regarding the current condition and status of women receiving treatment identifying when to escalate to a more senior member of the clinical team. Through analysis of information presented determine when to refer to other healthcare professionals and statutory and voluntary services E.g. smoking cessation or social work Determine when delegation of tasks is appropriate to other members of the team e.g. the inpatient nursing team for care whilst in hospital. Participate in service/team discussion to inform and advise on decision-making processes e.g. use of resources, service developments, and patient pathways. 10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB Utilisation of time management skills to provide a service incorporating management of personal caseload, support to other staff, administrative tasks including producing service reports. Ongoing patient monitoring and evaluation of outcomes to ensure appropriate caseload management, maintain patient safety and working within scope of practice Dealing with emotional and/or distressing situations including patients with co-morbidities that require the utilisation of motivational, negotiating and persuasion skills to support behaviour and lifestyle change. Supporting self-management strategies to embed and sustain behaviour change and effectively manage their condition.

11 11. COMMUNICATIONS AND RELATIONSHIPS Provides advice, education and encouragement to individuals and their families. Apply knowledge of clinical conditions to facilitate effective communications with women and others involved in their care. Liaise with the senior clinicians regarding the management of complex cases, communicating and discussing these as appropriate to level of competency, including contributing to multidisciplinary case conferences. Receive information directly relevant to patient care and encourage/advise on a range of patient management strategies appropriate to level of competency. Deal with verbal complaints and challenging behaviour, including aggression either in person or via telephone and refer up to line manager Participate in meetings, and telephone conversations ensuring a two-way flow of information. Disseminate service information as appropriate Advise and negotiate with senior staff/nurse specialist regarding service review and development for their areas of responsibility Promote and share ideas Liaise with other members of the multi disciplinary team, for purposes of receiving guidance and advice for the exchange of information to ensure clinically effective care. Feedback patient information regarding treatment, progress, presentation and response to intervention to the CNS or directly to the oncology gynaecologist in her absence Share practice skills with other staff, students and contribute to clinical education as possible within area of expertise and as required. Utilise team support for emotional and challenging patients/situations. Liaise with third party agencies for example McMillan for ongoing support and advice. Engage with the general public and service users in consultation as required. 12. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB Physical skills Accurate IT skills daily use of computer e.g. communicate with other colleagues, complete data input. Concentration required dealing with practical matters e.g. assistance with directing patients and or carers in obtaining information on a range of topics.
