Support the Physiotherapy Team Leader and ensure continuity of service delivery. 3. DIMENSIONS Range of Clinical Areas - One of the following areas will be the area of specialism and the postholder will be permanently based in one of these Domiciliary, General Rehabilitation, Learning Disability, Medical, Mental Health, Musculoskeletal Out-patients, Neurology, Paediatrics, Respiratory, Surgical, Women s Health, working with the Advanced Practitioner attached to Orthopaedic clinics Staff Responsibility Supervise clinical workload of Physiotherapists, Support Workers and/ or Students. 6 4. ORGANISATIONAL POSITION Team Leader Specialist Physiotherapist Support Worker/A/C 5. ROLE OF DEPARTMENT The Physiotherapy Service sits within WLHSCP Key responsibilities of the service are:- Provision of physiotherapy care to a diverse range of clinical conditions and patient types in a variety of environments across acute and community settings. Service management, planning and development within a defined budget and a multidisciplinary framework. Development, implementation and management of a Clinical Governance Strategy. 6. KEY RESULT AREAS Clinical 1. Act independently as a specialist to assess, analyse and provide a clinical diagnosis using highly developed clinical reasoning skills for individual patients who may have complex needs to determine their need for physiotherapy intervention within specialist area. 2. Act independently to plan, implement, evaluate, treat and progress patient care through patient centred goal setting to maximise rehabilitation potential within specialist area. 3. Manage an individual caseload of patients effectively and efficiently from initial assessment to discharge. 4. Liaise with Senior Physiotherapists to gain support and direction in management of their caseload when required. 5. Work as part of a team to ensure effective communication and delivery of care, including discharge planning. 6. Maintain patient documentation, records and accurate statistical information to reflect care provided and meet professional standards.

Communicate and make recommendations e.g. requesting appropriate investigations and screening for serious pathology to all relevant disciplines of staff to maximise patient care and promote multidisciplinary team working. 8. Implement appropriate clinical guidelines to optimise patient care. 9. Depending on the clinical area, provide physiotherapy on-call care to support delivery of the 24-hour service to acutely ill patients. 10. Educate patients, families and carers on their diagnosis and encourage self management Managerial 1. Deputise for Team Leader to maintain continuity of service delivery in their absence. 2. Supervise clinical workload of Physiotherapists, Support Workers and students to maximise efficiency and achieve optimal quality of care. 3. Participate in the NHS Lothian KSF Framework to promote personal and service developments. 4. Participate in clinical audit and service review to support the Physiotherapy Service Clinical Governance Strategy. 5. Attend and participate in relevant team meetings. Educational 1. Participate in physiotherapy in-service and lead team in-service training to promote development of self and others 2. To assist in maintaining and developing current knowledge of evidence based practice (e.g. critical appraisal, reflective practice) in the clinical areas to ensure the provision of up to date effective and efficient patient care. 3. Produce and present projects to promote service development and / or personal development. 4. Assist in the teaching of nursing, medical, other multidisciplinary team members, educational staff and others to promote knowledge of physiotherapy management to enhance patient care. 5. Supervise the clinical training of undergraduate and postgraduate physiotherapy students. 7a. EQUIPMENT AND MACHINERY The following equipment may be used depending on clinical area: Walking aids - Arjo Walker, Gutter frame, Mobilators, Zimmers, Elbow crutches, Quadrapods, Sticks, Crutches, Specialist walking frames to assist/progress the functional independence of patients. Manual Handling equipment - Mechanical Hoists, Samhall turner, Sliding boards, Rope Ladders to facilitate patient transfers. Electrical Equipment - Tens units, Muscle stimulators, EMG biofeedback units, Ultra-sound, Interferential, Curapulse /Short wave diathermy, Ultraviolet, Continuous Passive Movement Machine to assist rehabilitation of patients. Respiratory Equipment - IPPV (Bird), IPPB, Ultrasonic nebuliser, Ventilators, Suction units to assist rehabilitation of patients. Diagnostic Equipment Stethoscopes, respiradyne, peak flow meter, blood pressure cuffs to assist diagnosis and management of patients. Supports Full range of appliances and orthoses - to assist functional independence.

8 Rehabilitation Equipment Treadmill, Exercise bicycles, Steppers, Isokinetics, Standing frame (some specialist), Multigym, Weights, Traction beds. PPAM aid, Vari-table, Plinths, Parallel bars, Balance boards, Gymnastic balls, Wobble cushions, Blocks, Stairs, Full range of small exercise equipment, Specialist seating, Bodyweight supported treadmill to assist rehabilitation of patients. Thermal Equipment - Hot packs, Ice to assist pain relief of patients. Other Equipment Acupuncture needles to assist pain relief of patients. IT Equipment Personal Computer, Printers to assist inputting of patient data. Office Equipment Dictaphone- to assist provision of patient reports. Must ensure that all physiotherapy equipment is safe to use and be responsible for the safe and competent use of all physiotherapy equipment. 7b. SYSTEMS Patient Information Systems (CIS) to ensure accurate data collection. TRAK Picture Archiving Communication System (PACS) to examine patient data. MIM System to examine patient photographic records. Microsoft Office to compile patient information, advice sheets and to produce presentations and reports. to ensure efficient timeous communication with colleagues. Internet to access national and international databases thus enhancing knowledge base of clinically effective care. Intranet to access information within NHS Lothian Patient Records / Medical Notes to input/ departmental patient records and patients case notes. Guardian 24 when off site if available. 8. ASSIGNMENT AND REVIEW OF WORK Clinical caseload will be generated by the specific service needs of each clinical area. Physiotherapy Team Leader will also delegate other non-clinical tasks. Work independently on a day-to-day basis with available Senior Physiotherapist support as required. The Physiotherapy Team Leader on a six monthly basis will undertake performance reviews.

DECISIONS AND JUDGEMENTS Make decisions on diagnosis, treatment and ongoing management/discharge analysing information using highly developed analytical skills and available options to ensure an appropriate outcome for patients. Decide when to refer to senior physiotherapists to ensure patients receive optimal clinical care Prioritise caseload taking into account competing clinical priorities to ensure effective use of time. May decide on allocation of tasks / patients to Physiotherapists and Support Workers to ensure effective utilisation of skill mix. Deal with staff performance issues and refer onto senior staff as appropriate. In the absence of Physiotherapy Team Leader manage the clinical workload of the specialist clinical area. 10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB Develop knowledge, skills and evaluation processes in a wide variety of conditions and patient groups acting as a specialist within particular area of work with involvement in teaching. Exercise management and specialist clinical skills in the absence of a Physiotherapy Team Leader. Take responsibility for management and development of more junior staff and service. Undertake a mentally and physically demanding job, whilst at the same time taking care to safeguard own health and safety as well as that of colleagues and patients. If on-call, work independently with acutely ill patients with no direct professional support. 11. COMMUNICATIONS AND RELATIONSHIPS Patients 1. Provide and receive information regarding assessment, diagnosis, prognosis and treatment to encourage acceptance and compliance. 2. Patients will have a range of medical and psychosocial problems that require the physiotherapist to utilise motivational and persuasive skills to facilitate rehabilitation both in an individual and group setting.
