3 Introduction is a purpose built unit providing care and support for patients and their families. The building consists of an eight bed inpatient unit, a central general sitting and reception area known as "The Green Conservatory Area" and a Day Care Unit. This booklet has been designed to provide information about the services available for you and your family. Please remember that everyone in the hospital is concerned with your comfort and well being and will be happy to answer any questions or deal with any problems you may have. We would also welcome any comments or suggestions which you think may help to improve the service we provide. This booklet is also available in the Welsh language. 2
4 Philosophy Through our respect for an individual's needs we aim to maximise quality of life, preserve dignity and provide choice enabling patients family and carers to remain in control of their lives. Care should create an environment for patients and families/carers which is conducive to independence, self esteem and participation in daily life. Care should ensure that patient and family/carers needs are assessed, planned, implemented and evaluated on an individual basis. Care should ensure that each individual is enabled to be as informed of his/her condition as he/she wishes to be. Care should provide access to information on local and national services for professionals, patients and carers. 3
5 The Specialist Palliative Care Team The Specialist Palliative Care Team works closely with your General Practitioner and District Nurse who will continue to be your main Health Care Professionals. The Specialist Palliative Care Team does not provide a twenty-four hour service for people in their own homes. If you experience problems please contact your General Practitioner. serves as a base for many different professionals who each have a specific role in patient care. Some of the professionals will only work within but others will see you not only in but also at home and other hospital settings. The professionals involved are: Medical Staff The medical staff are based at but you may see them on other wards, outpatients clinics and they may visit you at home at the request of your General Practitioner. Nursing Staff There are specially trained nursing staff, including Macmillan nurses who provide care within the unit, in the hospital, the community and day care settings Therapy Staff Occupational Therapists, Physiotherapists and a Dietician provide assessment and treatment for inpatients, day care patients and outpatients. 4
6 Social Worker Social Workers from Bridgend County Borough Council Social Services Department visit the unit on a regular basis. Contact is also made with the other Social Services Departments throughout the locality and the Tenovus social worker. Chaplain A Chaplaincy service is provided as part of the Trust service. A minister of your choice is welcome to visit. Pharmacist The Pharmacist visits the ward regularly. Ancillary Staff There are many ancillary staff who support the day to day running of the unit. 5
7 How to get to Where is is located in the grounds of the Princess of Wales Hospital. On entering the main hospital site turn right at the roundabout. Drive past the Accident and Emergency department, the main entrance to the hospital and through the car park. Y Bwthyn Newydd is the single storey building at the end of the car park. Travelling by Car Parking facilities are available outside. There is a charge which is displayed on the pay and display machines. Concessions are available for prolonged periods. Ask a member of staff for further details. Travelling by Bus The telephone Number of the local bus company - First Cymru There is a regular bus service to the hospital. Travelling by Train The nearest train station is situated in Bridgend town centre. The telephone No is

9 Reception and The Green Area The entrance leads directly to the reception area. All visitors should report to the reception desk on each visit. For security reasons all access doors are kept locked other than at times when the reception area is manned. There is an intercom system situated to the right of the main doors. Please ring for entry. Please be patient as the staff may be busy. The reception area is used by inpatients, day care patients and visiting relatives and friends. Visitors and outpatients attending clinics are not permitted to smoke in this area. A smoking area is provided by the patio area for inpatients and day care patients only. Although visitors may sit in this area please be aware that patients have priority. Patio doors lead on to the garden where seats are provided. A public telephone is provided in main reception. There is also a display of relevant information booklets. 8
10 Attending Day Care Once you have been referred to day care you can expect to meet a member of the team who will welcome you to the unit. The staff and volunteers create a welcoming environment and you will be made to feel at home. The volunteers will provide you with morning tea when you arrive and an alcoholic appetiser (if you so wish) before lunch. Afternoon tea is served at 3.00 pm before you depart at 3.30 pm. Any special dietary requirements should be communicated to the nurse as soon as you arrive. On your first visit it would be helpful if you could bring all your medication. You will be given the opportunity to discuss any issues you may have and to meet appropriate members of the team. Volunteer drivers will provide transport if necessary. There is no charge for this service and it will be arranged by the day care staff. However if you need an ambulance for day care the staff will arrange this. Day care consists of the following rooms: Lounge This is the main sitting area and craft activities are undertaken here. Quiet Room Daily relaxation classes are held here, together with hand and foot massage sessions. Dining Room Lunch is provided at no cost. During the afternoon other activities may be held. 9
11 Hairdressing Salon The salon is staffed by volunteer hairdressers. There is no charge Therapy Room Physiotherapy and occupational therapy assessments may be undertaken here. Treatment Room This room is used for Nursing and Medical consultations. Kitchen This may be used for occupational therapy assessments. Bathroom Following assessment appropriate patients may be offered an assisted bath during day care. Toilets Toilets are situated off the main day care corridor, including facilities for people with disabilities. 10
12 The Inpatients unit The eight beds are used as a short stay unit for assessment and symptom control. Patients requiring longer term nursing care will be transferred to more appropriate facilities. On arrival to the ward you will be met by a member of staff. Your care will be managed by a team of doctors and nurses. There are six single rooms and one two bedded room. Each room has a nurse call bell system a television, video recorder and radio. A telephone trolley phone can be used in each room. Patients are encouraged to wear clothing of their own choice. A combined sitting and dining area is provided for the use of patients only. A relatives room is available which accommodates overnight stays. Relatives are able to use the kitchen facilities during specific times, which are displayed on the kitchen door. However this is at their own risk. Relatives are permitted to bring in food for patients, however, the food can only be kept in the refrigerator for 24 hours. All food must be covered. The date, time and patient s name must be displayed on the food item. In the interest of food hygiene this policy is strictly adhered to. The food items will be removed from the fridge after 24 hours or earlier if the food item is not labelled. The unit operates an open visiting policy, however, between 1.00 pm 2.30 pm visiting is only allowed by arrangement with the nursing staff. 11
13 Children are welcome, but should be supervised by an adult at all times. Please speak to a member of staff regarding very young children visiting. A box containing toys is located in the reception area. Relatives should use the telephone in the reception area. Pets are welcome, please discuss arrangements with the nursing staff. Discharge During your hospital admission you will be assessed by the palliative care team, your needs identified and the appropriate plan of care including discharge will be discussed with you. On discharge you will be given a transfer of care information sheet for your personal use and a seven day supply of medication. Your District Nurse and your General Practitioner will be informed of your discharge. Outpatients Service A palliative medicine outpatient clinic is held every Friday morning in the main out patient department at the Princess of Wales Hospital. Following consultation with the doctor you may need to have x-rays or blood tests. The doctor will contact your GP with the results of such investigations. A Macmillan Clinical Nurse Specialist attends many of the oncology clinics held at the outpatient department in the Princess of Wales Hospital. He/she will be able to provide you with further information and answer any questions you may have. 12
14 Chemotherapy Clinic A chemotherapy clinic is held at every Tuesday afternoon. This is an outreach clinic from Velindre hospital. Macmillan Nurse Specialists will be available at the clinic to provide information and support. Please do not ring reception desk with inquiries regarding the chemotherapy clinic outside of the clinic hours. Please ring the Macmillan Nurses who support the clinic or ring Velindre hospital directly. Wig Clinic A wig clinic is held at the hairdressing salon of on the first Wednesday of each month. Appointments for this clinic can be arranged by contacting the Macmillan Clinical Nurse Specialists /910 Prior to your appointment you will be asked to attend Y Bwthyn Newydd to discuss the style and colour of the wig you would like. At your appointment a selection of wigs will be shown to you by Salon Wills specialists. The cost of the wigs and exemption categories will be discussed with you prior to your appointment. You will be given written information on caring for your wig during the time you need to wear it. 13
15 Lymphoedema Clinic A clinic is held every Monday morning. Appointments for this clinic are made through your medical Practitioner via Velindre Hospital. 14
16 Volunteers The Palliative Care Service receives invaluable support from a dedicated group of carefully selected and trained volunteers. The volunteers fulfill many roles in the unit and these include reception duties, craft activities, hairdressing, making teas, serving lunches and spending time talking to patients. We also have a voluntary driver service. Patients are picked up from their own homes to attend day care. 15

Macmillan Cancer Relief Macmillan cancer relief support the service through the fixed term period of funding for nursing, medical and professionals allied to medicine. Macmillan Cancer Relief also provide support with staff education and patient grants. Donations and Gifts The whole unit has benefited from the generous support of the community. The service continues to develop due to the donations given to: A. Money is used for patient comforts and staff education. Cheques should be made payable to endowment fund and handed into reception. 16
