In any of these situations, people debate whether or not to seek medical attention, and what level of care they should seek. When Two Twelve Medical Center in Chaska opens on Feb. 1, that decision making will be left to the experts. A new model of health care in the Midwest, Two Twelve Medical Center offers 24/7 Emergency & Urgent Care along with comprehensive family and specialty care, and advanced diagnostics in one destination. A multi-tenant facility, Two Twelve was developed by award-winning. is the master lease-holder of the 163,000-square-foot, four-story building and will provide core services including emergency and urgent care, imaging and a number of specialty practices. Additional providers will round out the services with primary and specialty care, as well as a pharmacy, offering consumers highquality care in an outpatient setting that is easily accessible to people in the west-metro region. Right care. Upon arriving at the Emergency & Urgent Care department at Two Twelve, patients will be assessed and then treated at the appropriate level of care either as an urgent care patient or an emergent patient. And, they ll be billed accordingly. If treated for an urgent care type injury or illness, patients will be billed at an urgent care rate; alternatively, if the patient s treatment is through the emergency department, he/she will be billed at an emergency room level. If hospitalization is required, patients will be discharged to a hospital although only 12 percent of s annual Emergency Department visits result in hospitalization. For emergent cases, where ambulance service is necessary, Ambulance will provide transportation to the patient s hospital of choice, and the service will be free if the patient chooses Medical Center in Waconia, or local hospitals: St. Francis Regional Medical Center in Shakopee, Methodist Hospital in St. Louis Park, Fairview Southdale Hospital in Edina, or Children s Hospitals and Clinics of Minnesota in Minneapolis. 3 Ask a Physician Board-certified endocrinologist Sidney Jones, MD, recently joined Clinics growing team of medical specialists. As an endocrinologist, Dr. Jones specializes in treating disorders of the endocrine system the organs that secrete hormones, such as the pituitary, thyroid, adrenals and pancreas including diabetes. Dr. Jones addresses some common questions associated with the disorder. Sidney Jones, MD, Endocrinologist Q A Q A What causes diabetes? The cause of type 1 and type 2 diabetes is considered to be a mix of genetics and the environment. In type 1 diabetes, there is a genetic predisposition that is set off by an environmental trigger. This trigger, most likely of viral origin, starts a complicated immunologic response leading to progressive destruction of insulin-making cells. Type 2 diabetes is also caused by underlying genetics which may play a larger role than in type 1. Environmental causes, including sedentary lifestyle and poor dietary choices, also play a role in the development of type 2 diabetes. Is diabetes preventable? Type 1 diabetes is not presently thought to be preventable. However, the National Institutes of Health is funding a large trial called TRIALNET, which is searching for ways to prevent onset of type 1 diabetes in those at risk. There are several sites for this trial around the Twin Cities area, and any firstdegree relative of a person with type 1 diabetes can be considered for screening. Q A Type 2 diabetes can be prevented, or at least delayed, by lifestyle choices. In those who are at high risk for diabetes and/or those who have already been diagnosed with pre-diabetes, a regimen of diet, exercise and weight loss can prevent progression to diabetes. In addition, medications have been used successfully in this group to reduce progression to diabetes. What general suggestions do you have for managing diabetes? Diabetes is a chronic disease and is generally considered modifiable but not curable. In a person with diabetes, it is important to monitor glucose levels regularly and follow the treatment regimen outlined by his/her provider. There are many exciting changes in therapy that have occurred over the past decade. These new therapeutic options have allowed better overall glucose control in people with type 1 and type 2 diabetes. A person with diabetes should educate themselves as much as possible about their disease to empower themselves in terms of treatment options and lifestyle choices. 4 Partnerships provide best-possible pediatric emergency care minutes from home collaborates with Children s Hospitals and Clinics of Minnesota In the middle of a wintry night, an infant in Chaska spikes a fever. A toddler in Eden Prairie wakes up crying, tugging at his ear. Their parents contemplate waiting until the morning to consult their children s primary care providers. But they worry that waiting isn t in the best interest of their children. Through a partnership with a national leader, will provide the bestpossible care to emergent pediatric patients at its new Two Twelve Medical Center in Chaska. Around-the-clock emergency and urgent care at Two Twelve, when it opens Feb. 1, will be provided by s emergency department, in partnership with Children s Hospitals and Clinics of Minnesota. Children s is excited to bring emergency care that is tailored to the specific needs of children to the southwest metro area, said Robert Sicoli, MD, Emergency Department co-medical director, Children s. We have earned a reputation for providing high-quality, familycentered health care meaning we look out for the needs of the patient and the entire family. We anticipate pediatric patients and families will recognize the unique benefits of this partnership with and will ultimately make Two Twelve Medical Center their preferred location for emergency and after-hours urgent care, Sicoli said. Right care, right partners The 24/7 Emergency & Urgent Care at Two Twelve will be staffed by Emergency Department physicians and physician assistants, with a physician on staff at all times. When a pediatric patient arrives at Two Twelve s Emergency & Urgent Care just like patients of any age he/she will be assessed and then triaged for either urgent or emergency care, whichever is appropriate for his/her condition. Whether an urgent care or emergent patient, the child may be treated by one of s physicians or physician assistants, or one of Children s pediatric emergency physicians. Patient volumes and the complexity of each patient s situation, at any time, will play a role in determining which providers care for which patients, said Ben Nielsen, executive director, Medical Cener. Children s emergency physicians will provide coverage at Two Twelve during the busiest times within an emergency department, explained Nielsen, with coverage weekdays from 5 to 10 p.m. and weekends and holidays from 4 to 10 p.m. has also developed partnerships with other national leaders, including Hennepin County Medical Center, for neurology services, and Minneapolis Heart Institute, for cardiology services. The right care for all ages Q: Retail clinics have restrictions regarding the minimum age at which they can see patients for one clinic, patients must be 18 months and older. Are there age restrictions for pediatric patients at Two Twelve Emergency & Urgent Care? A. No, there will not be age restrictions. If the patient is a newborn, it s likely he/she will be considered an emergent patient, but parents can bring children of any age, with any illness or injury, to Two Twelve.... at the right price. Q. How are costs different between emergency care and urgent care at Two Twelve Medical Center? A. By providing both emergency and urgent care at Two Twelve, will provide patients with the appropriate care, priced accordingly. If treated for an urgent care type injury or illness, patients will be billed at an urgent care rate; alternatively, if the patient s treatment is through the emergency department, he/she will be billed at an emergency room level. 4

5 introduces ImPACT: A new tool for managing concussions After being hit at the end of a football playoff game this fall, Waconia High School senior and right guard Crosby Smith mentioned a headache and dizziness to his mom that evening. They agreed that he would visit his team s athletic trainer provided by Sports Medicine the next day and in doing so, was asked to rest for a few days before taking an ImPACT assessment. Introduced by this fall, ImPACT (Immediate Post-Concussion Assessment and Cognitive Testing) is a new tool in helping to identify and manage concussions. This 20-minute computer test measures several cognitive functions, and an abnormality in one area is enough to trigger concern. Research has found that although some head injuries appear to be mild, an even minor hit can have serious, long-term effects, especially with repeat concussions or when an athlete is returned to play too soon. Smith s results came back abnormal compared to the baseline he had taken at the beginning of the season. His verbal score had dropped severely. The test gave us solid data to confirm his concussion and manage his care, said Jessica Hess, MA, ATC,. After his test, Smith was monitored by Sports Medicine staff over the following week and then retested. At that point, his results were within the normal range. For many athletes, recovery from a concussion can take much longer. Rest, not risk Because of Smith s injury and symptoms, he was pulled from play and missed the final playoff game of the season. Sometimes athletes try to mask or minimize their injury because they want play so badly, said Hess, and that can be very dangerous. Smith was smart to share his symptoms and probably more accepting of his situation because he knows a student from another state who has suffered permanent brain damage due to a sports-related head injury. It s important to rest both physically and cognitively after a concussion. That means no physical activity, no TV and no video games, Hess added. The brain simply needs time to heal. When modified its concussion guidelines, it modeled them after NCAA (National Collegiate Athletic Association) guidelines, the Centers for Diesease Control and Prevention (CDC) and a consensus statement from the International Sports Medicine Conference in Zurich in Athletic Trainer Jessica Hess and Waconia High School athlete Crosby Smith review Smith s ImPACT scores. Not every concussion is as obvious as a direct blow to the head and loss of consciousness. Some concussions are hard to identify because symptoms can be subtle or short lived, or do not even present themselves until days following an injury. The guidelines state that an athlete must be symptom-free for one week before beginning a gradual return to play. Once the athlete is symptom-free for a week, they are typically retested and sent with their test results to their physician, who will give clearance to play. It is typically a two-week process from being symptom-free to full return to play. To date, Sports Medicine has performed ImPACT baseline tests on more than 700 students from its contracted schools and others. This includes Waconia High School, Central High School (Norwood Young America), Mayer Lutheran, Crown College and more. Although it is ideal to have a baseline for comparison, it is not necessary as post-injury results can be compared to standard norms, Hess said. Baseline standards are only available for ages 10 and up, so that s generally the youngest we ll test. ImPACT is now a standard at more than 1,000 high schools, colleges, sports medicine centers and professional teams throughout the world, including the NFL and NHL. According to Hess, This number will continue to grow as the public is educated about concussions and realizes the value in having a quick, easy and objective tool to assist in keeping our athletes safe. 9 Childbirth-Related Classes Physical Assessment for Golf Performance This class is designed to improve your golf game. Participants will go through the Titleist Performance Institute personalized movement screen to identify physical limitations that can contribute to common golf swing faults. Exercises will then be taught in a group setting by our team of physical therapists and certified athletic trainers to help correct specific limitations. This assessment is valued at $150 if done individually. Come in loose-fitting clothing, prepared to move and practice the exercises. 10 Childbirth-Related Classes Breastfeeding Breastfeeding is a unique experience for each mother. This class is beneficial whether you choose to breast feed for many months or just a few weeks. It can also be helpful if you are unsure about whether to breast feed or bottle feed. Subjects include advantages of breastfeeding, preparation for breastfeeding, nutrition, problem solving and returning to work. Bring your spouse or special support person.
