2 Alberta Health Facility Listing October 1, 2017 Facility numbers and functional centre codes - claim submission requirements Claims to the Alberta Health Care Insurance Plan for insured services provided in formally recognized or accredited facilities in Alberta require a facility number and functional centre code. The attached Facility Listing contains the facility numbers and functional centre codes that Alberta Health has assigned to these facilities. The facility number identifies the physical location where the service was provided. The functional centre code identifies the specific area within the facility where the service was provided. Claim submissions must correctly describe the specific area within the facility where the services took place. Please refer to the Facility Listing when preparing claims for services provided in registered general (active treatment) hospitals, auxiliary hospitals, nursing homes, community mental health clinics, correctional centres, community ambulatory care centres and designated mental health facilities. The information it contains is current to the date displayed at the top of the listing. Updates to facility information will be communicated to practitioners via Alberta Health Care Insurance Plan Bulletins. Definitions of the facility types and functional centre codes contained in the Facility Listing are provided on the following pages. Information about facilities and functional centres in relation to claim submissions is located in Sections 1.8 and 3.4 in the Alberta Health Physician s Resource Guide and the Allied Health Practitioner s Resource Guide, as applicable to your practice. Please note: Claims for services provided in a publicly funded clinic located within a hospital to a registered in-patient or outpatient require the facility number and applicable functional centre code for the hospital. Claims for services provided in a practitioner s private, leased office/clinic located within a hospital to a patient who is not a registered in-patient or outpatient require the facility number for that office. If your private, leased office/clinic within a hospital has not been registered and assigned a facility number for claim submission purposes, please contact Professional and Facility Management for assistance Government of Alberta i

General hospital Facility and functional centre definitions In the Hospitals Act, a general hospital is defined as "a hospital providing diagnostic services and facilities for medical or surgical treatment in the acute phase for adults and children and obstetrical care." General hospitals have several functional centres. Each functional centre is associated with in-patient, outpatient, or diagnostic and therapeutic services. An in-patient is a person admitted to and assigned a bed in a hospital by the order of a physician, for the provision of diagnostic or treatment services or both. An outpatient is a patient registered to receive outpatient diagnostic, treatment or therapeutic services. In each case, the attending physician keeps a record of the services provided, for retention by the hospital. When submitting a claim for services provided in a general hospital, the functional centre code must be consistent with the information provided below. For example, a surgical service provided to an in-patient would indicate functional centre code, while a surgical service provided to an outpatient would reflect functional centre code D/N. Functional centre codes associated with in-patient services The functional centre codes defined below should be used only for registered in-patients. a) ICU1, ICU2, ICU3 (intensive care unit level 1, 2, 3) an area where medical services are provided to seriously ill patients who require constant supervision and monitoring. b) ICN1, ICN2, ICN3 (neonatal intensive care unit level 1, 2, 3) an area equipped for providing a defined level of intensive care services to newborns. c) ICO1, ICO2, ICO3 (obstetrical intensive care unit level 1, 2, 3 ) an area equipped for providing a defined level of intensive care services to obstetrical patients. d) (long-term care) an area designated for the provision of medical services to long-term patients awaiting placement in a long-term care facility. e) (medical) an area where medical and non-surgical services are provided to in-patients (for example, paediatrics, obstetrics). f) (surgical) an area designated for the provision of surgical services to in-patients. Functional centre codes associated with outpatient services The functional centre codes defined below should be used only for registered outpatients. a) (clinic) an area designated for the provision of medical and non-surgical services to registered outpatients. b) D/N (day/night care) an area designated for the provision of planned/scheduled medical and surgical services to patients attending for a number of hours of the day or night. c) (emergency) the area where medical services are provided to patients with conditions requiring immediate attention. d) PEMG (paediatric emergency) an area where medical services are provided to paediatric patients with conditions requiring immediate attention Government of Alberta ii

Functional centre codes associated with diagnostic and therapeutic services The functional centre codes defined below should be used only when the service takes place in one of these areas within the hospital. a) CLAB (clinical laboratory) ** an area equipped for the provision of laboratory investigative procedures. b) DIMG (diagnostic imaging) ** the diagnostic imaging unit within the facility. c) ELEC (electrodiagnosis) ** an area designated for the measurement and recording of electrical impulses to evaluate the brain and associated physiologic functions. d) HBOC (hyperbaric oxygen chamber) an area containing a pressurized compartment in which 100% oxygen is administered to a patient for treating specified medical conditions. e) (other diagnostic laboratories) ** an area where highly specialized non-invasive procedures are provided. f) (radiation oncology) ** an area housing X-ray equipment and radioactive elements for therapeutic purposes. II. Auxiliary hospital Facility and functional centre definitions An auxiliary hospital is a facility designated for the provision of medical services to in-patients who have long-term or chronic illnesses, diseases or infirmities. Services may include acute palliative programs, geriatric day programs or day/night programs. Claims for services provided in an auxiliary hospital must include the applicable functional centre code as defined below. a) (long-term care) an area where medical services are provided for long-term patients. b) (medical) an area where care is provided to an in-patient in a sub-acute care program approved by and registered with Alberta Health. Sub-acute care is defined as care provided for patients requiring short-term treatment or convalescence of an active but stable condition, or following a medical procedure with the intent that the patient be discharged home after care is completed. c) (clinic) an area designated for the provision of medical and non-surgical services for registered patients. ** Fee-for-service claims cannot be submitted for laboratory, radiology and non-invasive diagnostic services provided in a general hospital. Payment for these services is the responsibility of the hospital Government of Alberta iii

5 Alberta Health Facility Listing October 1, 2017 III. Nursing home Facility and functional centre definitions A nursing home is a facility designated for the provision of nursing home care. Nursing home care means basic care and care provided under an approved program. Nursing homes have one associated functional centre, as follows: (long-term care) an area where medical services are provided to long-term patients. Claims for services provided in a nursing home can be submitted with a blank functional centre code to allow the claim processing system to default to. A facility number is still required in this case. IV. Community mental health clinic Facility and functional centre definitions A community mental health clinic is a facility operated by Alberta Health Services for the provision of community-based mental health services. Community mental health clinics have one associated functional centre, as follows: EXRM (examination room) an area designated as an examination room. Claims for services provided in a community mental health clinic can be submitted with a blank functional centre code to allow the claim processing system to default to EXRM. A facility number is still required in this case. V. Correctional centre Facility and functional centre definitions A correctional centre is a facility designated for the provision of insured health services within the confines of a provincial correctional centre, that are not being paid for on a contract or sessional basis. Provincial correctional centres have one associated functional centre, as follows: (clinic) a medical and non-surgical office or clinic located within a provincial correctional centre, where insured services are provided that are not being paid for on a contract or sessional basis. Claims for services provided in a correctional centre can be submitted with a blank functional centre code to allow the claim processing system to default to. Community ambulatory care centre Facility and functional centre definitions A community ambulatory care centre is a community-based service delivery site (non-hospital setting) primarily engaged in the provision of ambulatory care diagnostic and treatment services. This includes unscheduled primary care for clients who do not require hospital outpatient emergency care or inpatient treatment. Claims for services provided in a community ambulatory care centre that has been certified as an Urgent Care Centre (UCC) or Advanced Ambulatory Care Centre (AACC) must include the applicable functional centre code on the claim as defined below. a) AACC (advanced ambulatory care centre) An area where assessment, diagnostic and treatment services are provided for unscheduled clients with general injuries and conditions that require prompt attention. This includes the capability to treat up to urgent injuries or conditions, but is not intended to treat or stabilize clients with life-threatening conditions. Ground ambulance drop off to the AACC must have physician pre-clearance and is also supported by on-site laboratory and diagnostic imaging resources. b) UCC (urgent care centre) An area where assessment, diagnostic and treatment services are provided for unscheduled clients with general injuries and conditions that require prompt attention. This includes the capability to treat urgent conditions that could deteriorate and stabilize clients with life-threatening conditions. Ambulances may drop off clients to these locations if they have the resources to support them. They are supported by on-site laboratory and diagnostic imaging resources. c) (clinic) An area located within a community ambulatory care facility where medical (specialty clinics operated by Alberta Health Services) and mental health services are provided. VII. Designated mental health facility Facility and functional centre definitions A designated mental health facility is a facility operated by Alberta Health Services providing diagnosis and treatment for mental illness and addiction in the acute phase for adults and children. In-patient services refer to a person admitted to and assigned a bed in a facility by order of a physician for provision of diagnostic and/or treatment services. Designated mental health facilities have one associated functional centre, as follows: PRGR (patient room/group room) in-patient services provided within the patient s room or within a common group room within the designated mental health facility. Claims for services provided in a designated mental health facility can be submitted with a blank functional centre code to allow the claim processing system to default to PRGR.
