4 A Message from CAPHC s Board Chair & CEO A unified national voice is essential in addressing the current and emerging challenges that threaten the health, well-being and future of Canada s children. All children and youth must have access to a healthcare system supported by national standards and best practices that will appropriately position them for optimal transition to adulthood. Health promotion and healthcare delivery are complex issues, especially for more vulnerable infant, child and youth populations. CAPHC is committed to improving and promoting health service delivery for all children and youth across the continuum of care and to enhancing the application of knowledge from research to practice and practice to health policy. We are proud to present our 2013 Annual Review to our member organizations, national partners and networks in recognition of the tremendous degree of collaboration that represents the cornerstone of all CAPHC s work. Bringing new knowledge to impact practice change at a system wide level is a key factor and commitment of all CAPHC programs. This annual review summarizes our activities throughout 2013, highlighting our key milestones and future direction. In the year ahead, CAPHC will remain committed to its primary goal of facilitating change to improve child and youth healthcare services across the continuum of care. In 2014, we will focus on the development of national paediatric guidelines in the following areas - transition from paediatric to adult care; management of medically complex children through the continuum of care; sepsis; paediatric pain and the delivery, utilization and weaning of nitric oxide in neonatal and paediatric acute care. We will also continue our guideline development work in collaboration with Accreditation Canada in the area of interfacility transport of critically ill children and youth and with the Canadian Institute for Health Information (CIHI), in the development of a National Paediatric Rehabilitation Reporting System. 6 ... through Programs and Activities that Address Child and Youth Healthcare Priorities All CAPHC s programs are implemented based on broad national consultation and engagement. We would like to dedicate this year s report to the many child and youth healthcare leaders who bring expertise, passion and leadership to all CAPHC programs. Advancing Paediatric Empirical Decision Support and National Benchmarking: CAPHC - Canadian Paediatric Decision Support Network (CPDSN) A national data sharing and benchmarking program within CAPHC, CPDSN utilizes a collaborative and networking model to broaden the scope of understanding and analysis of paediatric health services in Canada. The Network s membership is made up of Canada s Academic Paediatric Health Centres, as well as many Community Hospitals from across the country. Leveraging our long-standing partnership with the Canadian Institute of Health Information (CIHI), we consistently advance the agenda in pursuit of access to better data, relevant, timely and useful information in response to the changing needs of our paediatric community of practice was a productive year for CAPHC-CPDSN, achieving important milestones that underpin our commitment to advancing paediatric empirical decision support and benchmarking in Canada. The following chart highlights our key activities in For more information about CPDSN please go to: through Improving Paediatric Quality of Care, Safety and Efficiency Across the Continuum: 1) CAPHC Paediatric Practice Guidelines (PPG) Collaborative About The PPG Collaborative: In October 2011, CAPHC established a National Paediatric Practice Guidelines Collaborative. The goal of the PPG is to develop, implement and evaluate paediatric practice guidelines aimed at improving and promoting quality of care, safety and efficiency across the continuum of care. Our Paediatric Practice Guideline Collaborative is lead and facilitated through inter-professional communities of practice, represented by physicians, nursing leaders, quality & decision support experts, researchers, senior administrators, allied health professionals and families Highlights: The four Communities of Practice (CoPs) established in 2013 are working toward their respective goals of developing national standards and practice guidelines within each of the following priority areas of child health. Key 2013 milestones and achievements are described in the table below. Management of Medically Complex Children & Youth Through the Continuum of Care Survey of current practice, Development of a survey tool to determine family needs and priorities; Catalogue of national complex care programs; Glossary of terms to promote a common language; and Knowledge exchange through monthly web meetings. Paediatric Pain Systematic review of published guidelines for procedural pain in children; Survey of current practice: CAPHC, PERC, Alberta Health Services Emergency Departments; Environmental scan of Grey literature; and Dissemination & implementation planning. 5 Each step of developing, implementing, evaluating, reviewing and modifying paediatric practice guidelines is facilitated, enhanced and transformed through institution collaboration at a national level. Transition from Paediatric to Adult Health Care Knowledge exchange through monthly web meetings; Survey of current practice; Stakeholder engagement; and Analysis of ongoing best practices.

7 CAPHC ) Inhaled Nitric Oxide Community of Practice About this Community of Practice (CoP): This CoP was launched in 2012 with the goal to establish national standards and guidelines for the delivery, utilization and weaning of inhaled nitric oxide for neonatal and paediatric patients across Canada. This project has a strong emphasis on quality and patient safety for this highly specialized population Highlights: Facilitated a national stakeholder engagement webinar; Established a national, inter-professional steering committee; Developed and completed a national survey of current practice; Completed a Review of Canadian Hospital Guidelines and a Systematic Review of the Literature; Coordinated a national workshop held in conjunction with CAPHC s 2013 Annual Conference in Toronto to review recommendations from the systematic review; and Established two working groups to develop recommendations for guidelines in the areas of neonatal and paediatric use of in haled nitric oxide with a completion target of December For more information: 3) CAPHC Patient Safety Collaborative About CAPHC s Patient Safety Collaborative: Since 2003, our Patient Safety Collaborative has provided a national forum to unite individuals, groups and organizations to facilitate partnerships, improve communication and undertake collective action to improve patient safety for all children and youth. Our virtual collaborative brings key stakeholders together to share ideas, experiences and new innovative programs through monthly webinars Webinar Highlights Include: Implementing standards and guidelines in areas such as Sepsis and Opioid safety; Using insurance claims to support safety efforts and risk assessment checklists; Patient safety education from the Canadian Patient Safety Institute - Patient Safety Education Program (PSEP); Fire and Evacuation experiences of Children s Hospital at the Health Sciences Centre, Winnipeg; Improving Adolescent Medication Adherence; and Family Leadership Councils Vehicles of Excellence in Patient & Family Centered Care. 4) Interfacility Critical Care Transport of Maternal, Neonatal and Paediatric Patients The Development of National Standards About CAPHC s Interfacility Critical Care Transport Program: Extreme variability exists across Canada regarding transport system design, transport team models & training, transport equipment and vehicles, consideration of the principles of family centred care, and many other aspects of inter-facility transport. In 2011 CAPHC created a National Steering Committee of content experts to look into the state of paediatric air medical interfacility transport in Canada and to develop national standards. The goal of the CAPHC Transport Systems Program is to identify and introduce standards of care (national guidelines) in the area of inter-facility transport in order to improve care and outcomes through a focus on education, safety and quality. There is a growing appreciation for the value of a Canadian process of accreditation for specialized air and ground medical transport systems. The publication of the Accreditation Canada Qmentum Standards for EMS has given impetus to the idea that similar standards could be developed for critical care interfacility transport. The Transport Practitioner Competency Profile was finalized and published on July 4, , Canadian Association of Paediatric Health Centres Additional Reference - Accreditation Canada Qmentum Standards for EMS - Updated version released in January 2013 with Required Organization Practices (ROPs) and references to CAPHC Practitioner Profile Guidelines. 8 (continued from page 6) 2013 Highlights: The type of standards envisioned by CAPHC s Interfacility Transport Steering Committee members falls clearly in the domain of Accreditation Canada. Discussions between CAPHC and Accreditation Canada suggest that there is mutual interest and a potential opportunity to develop a Made-in-Canada system of accreditation for air medical specialty / interfacility transport. A new working group of content experts was struck by CAPHC in June 2013 to prepare a draft document that would help to inform potential future standards development processes by Accreditation Canada. The working group used the existing Qmentum EMS Standards document as a framework. Key Areas Include: Increasing community awareness and establishing strong partnerships; Preventing infections and minimizing exposure to hazards; Providing high quality medical transport services; Maintaining user-friendly and effective clinical information systems; and Partnering with families in the transport setting: familycentred care. 5) Interfacility Critical Care Transport of Maternal, Neonatal and Paediatric Patients: National Symposium On November 19, 2013 CAPHC and Accreditation Canada held a national symposium that brought together inter-professional stakeholders from across the country. i. ii. iii. The primary objectives of this symposium were: To identify strengths and opportunities for enhancing requirements related to interfacility transport in the Qmentum standards; To review CAPHC s recommendations for interfacility critical care transport of neonatal, paediatric, and high risk maternal patient groups; and To obtain feedback from stakeholders regarding the key safety and quality issues for interfacility transport and the potential uptake of these standards on a national scale. Plans for the Year Ahead: CAPHC will continue to work with Accreditation Canada to address these key maternal, neonatal and paediatric population gaps bringing focus to the following: Existing Qmentum EMS Standards that can be applied with only minor modification; Additional standards to address the specialized needs of all critically ill patients needing interfacility transport; Additional standards to address the sub-specialized needs of neonatal, paediatric and maternal patients; and Additional standards to address the safe and effective operation of fixed wing and helicopter air medical transport vehicles. 7

9 CAPHC through Advancing the Quality of Life for Children and Youth with Disabilities About the CAPHC Network of Child and Youth Rehabilitation (CN-CYR): Established in 2004, CAPHC s Rehabilitation and Child Development program is focused on improving rehabilitation and child development services for children and youth in Canada. This program has two main areas of concentration: The development of a national data set for children s rehab services; and The knowledge translation and engagement of the paediatric rehabilitation community. Vancouver 2010 Olympic Gold medalist (Moguls) Alexandre Bilodeau & brother Frédéric Bilodeau, honorary CAPHC member 2013 Highlights: National Paediatric Rehab Database Completed the second pilot of the national data set project; Included 8 sites, collected approximately 50 data elements on children with Cerebral Palsy and Autism; Where the first pilot focused on the feasibility of a diverse set of rehab facilities to collect basic data, the second pilot extended that work to begin to incorporate wait times data, functional measures, and participation/activity measurements. KT and Research The KT and Research Committee s primary goal is to improve KT across the child and youth rehab community in Canada, bringing research to the bedside, and engaging the research community; In the past 12 months, CAPHC s rehab community has embraced the CAPHC Presents webinar program and has provided advice and leadership on 8 national and international webinars focused on rehab and child development; The KT and Research Committee also continues to engage the rehab and child development research community by nurturing partnerships with organizations such as NeuroDevNet, CanChild and CanFASD.... through Fetal Alcohol Spectrum Disorder: A National Screening Tool Kit for Children & Youth Identified and Potentially Affected by FASD About CAPHC s FASD Program: The National Screening Tool Kit for Children and Youth Identified and Potentially Affected by FASD was developed with support from the Public Health Agency of Canada (PHAC) and the leadership of the Canadian Association of Paediatric Health Centres (CAPHC) and its National Steering Committee of FASD experts. The Tool Kit includes five screening tools that are applicable to various populations, implementation settings, sectors and jurisdictions. During , PHAC funded an implementation strategy which included further field testing of the effectiveness and practical application of the screening tools. These field tests provided critical information on the tools predictive values, client characteristics, practice settings and training requirements. In addition, knowledge transfer activities provided a forum for tool development, discussion and improvements. Highlights in 2013: Pilot the multiple components of the National FASD Screening Tool Kit in different jurisdictions and population across Canada in order to further validate and evaluate the following 4 tools to identify improvements and gaps: Meconium Testing in the neonatal population Neurobehavioral Screening Tool (NST): 6-18 years of age Maternal Drinking Guide - Pre-natal exposure for at risk women Youth Probation Officer Tool Youth Justice System Implementation of a multi-faceted knowledge translation and mobilization strategy to further promote the uptake and on-going development of the Screening Tool Kit. 8

10 ... through Our Commitment to Knowledge Translation and Mobilization Knowledge translation and mobilization has been facilitated in numerous ways over the past year. CAPHC is very committed to serving our members as a knowledge broker, facilitating the transfer, exchange and mobilization of knowledge across the child and youth healthcare community, within and far beyond Canada s borders. A comprehensive catalogue/repository and video library of all CAPHC webinars has been created and made available on the CAPHC s Knowledge Exchange Network (KEN). CAPHC Presents! is a bi-weekly newsletter designed to help streamline communications related to CAPHC s various live webinar presentations, recorded presentations and other knowledge exchange opportunities. Our communications involve live webinar presentations, on-demand video, CAPHC s Knowledge Exchange Network, our YouTube Channel and our podcast channel on the itunes Music Store. The online views of the recorded webinars and other resources on Knowledge Exchange Network are: Page views: 93,681 Unique Visitors: 13,627 CAPHC s 2013 Annual Conference Innovations in Children s Healthcare: From Inspiration to Application Our commitment to knowledge translation and mobilization can be seen each year at CAPHC s Annual Conference. Our conference goal is to ensure that all who attend have the opportunity to reflect on best practices and the latest innovations as a base for creating new programs that enhance the quality of child and youth healthcare. CAPHC s 2013 Annual Conference was hosted by the Hospital for Sick Children and took place in the vibrant city of Toronto. This year s conference welcomed a record number of delegates, almost 500 multi-disciplinary health professionals from across the country, as well as from many other international, national, provincial and regional health related organizations Highlights: CAPHC promoted 45 live webinars, viewed by more than 2,500 individuals and organizations.
