10 I felt surprised and wondered why did that statute pass in the first place? I felt frustrated and angry and resoluted in the sense that even though something has been put in my way, I was going to overcome it somehow. When I took the job, I vowed to visit every facility. In 10 years I had gotten to about 90%, and it was very frustrating, what I was seeing. So then I was told by a staff person about Chase and sought to visit it. That experience was just amazing. I knew I was seeing something I had never seen before. Amazing, unique, we ve got to bottle it. I told Dr. Thomas that if there was ever anything I could do, to let me know. It had the potential to revolutionize the system, and here I was presented with a law that no one even knew about, and I was supposed to stop them? I got so ticked off I decided this isn t going to stop me. I could lose my job, but so what? Not on this one, I m on the side of the angels with this one. I thought, what the heck, let s go! 15 Keep Deficiencies & CC Separate There are two problems that invoke blame of the regs: 1) that providers sometimes haven't read them or use them as an excuse for not doing anything new, and 2) just because you are trying to do culture change doesn't mean that you might not be doing something badly - good intentions don't substitute for good performance. 23 NFPA Proposals Residential Cooking - kitchen hood with grease removal capability and air flow, protected per UL 300A, a residential suppression system, 24 or less residents (Type I commercial hood not required). Open Kitchen - residential scale kitchens, 24 or fewer residents, to be open to the common areas and corridors, deactivation switch for cooking equipment, locked, only accessible to staff and a fire extinguisher in each kitchen area.
24 Corridors - Where the corridor is 8 feet, permits areas along the corridor to contain secured chair, bench or grouping of chairs that are specifically placed at strategic intervals to allow residents to sit and rest during Decorations - Increases percentage of wall and door areas for decorations (photographs, paintings) from 20% to 50% inside resident sleeping rooms, with sprinkler system, same requirement of day care centers and schools.
25 Fireplaces Currently fireplace is defined as an item having a fire box, a hearth and a chimney which includes gas log fireplaces that are actually heaters with gas fired logs but not real fireplaces. Clarifies that a gas heating device designed to look like a fireplace is not a solid fuel fireplace so it can be permitted inside the sleeping room areas.

47 Recommended Course of Practice Diet determined with the person not exclusively by diagnosis. Assess the condition of the person. Include quality of life i.e. satisfaction with food, meal time service, control, independence. Unless a medical condition warrants a restricted diet, consider beginning with a regular diet and monitoring Empower and honor the person first, whole interdisciplinary team second, to look at concerns and create effective solutions. Support self-direction and individualize the plan of care. Ensure that the physician and consultant pharmacist are aware of resident food and dining preferences so that medication issues can be addressed and coordinated i.e. medication timing and impact on appetite.
48 Monitor person and condition related to their goals regarding nutritional status, physical, mental and psychosocial well-being. Although a person may have not been able to make decisions about certain aspects of their life, that does not mean they cannot make choices in dining. When one makes risky decisions, plan of care will be adjusted to honor informed choice, provide support to mitigate risks. Most professional codes of ethics require professional to support the person in making their own decisions.
49 When caring for frail elders there is often no clear right answer. Possible interventions often have the potential to both help and harm the elder. This is why the physician must explain the risks and benefits to both the resident and interdisciplinary team. The information should be discussed amongst the team and resident/family. The resident then has the right to make his/her informed choice even if it is not to follow recommended medical advice and the team supports the person and his/her decision, mitigating risks by offering support, i.e. offering foods of natural pureed consistency when one refuses recommended tube feeding. It is when the team makes decisions for the person without acknowledgement by all that problems arise. The agreed upon plan of care should then be monitored to make sure the community is best meeting the resident's needs.
