4 INTRODUCTION Welcome to Terra State Community College Nursing Program. This student resource manual has been developed for your information and guidance. It contains policies and procedures specific to the Nursing Program as well as selected College policies. For additional Terra State Community College policies and procedures, please refer to the current Terra State Community College catalog. The College catalog may also be found on the web at It is the students responsibility to read and abide by the policies herein. Please keep this manual as a resource throughout your nursing education at Terra State Community College. Students will be notified of any changes made in policy or procedure prior to implementation of such changes. The mission of the Nursing Program, in keeping with the College mission, is accomplished through the provision of the generic Associate Degree in Nursing Program, the LPN to RN completion program and the availability of RN to BSN programs in collaboration with several area universities. Philosophy Of The Associate Degree In Nursing Program The Terra State Community College Associate Degree in Nursing Program is committed to providing the education and skills needed to enable the individual to successfully complete the requirements of the program, to successfully pass the licensure test, and to enter the profession as a Registered Nurse. Organizing Framework The organizing framework for the Associate Degree in Nursing Program is the Self-Care Deficit Theory of Nursing (SCDTN) as described by Dorothea Orem. SCDTN is based on the premise that: In caring for themselves, persons act deliberately in response to input from their internal and external environments. Actions are taken in response to these inputs to assure life and well-being, and to sustain functional and developmental well-being. This ability to respond is labeled agency, and by acting in response to stimuli, a person is demonstrating self-care agency. Abilities to act are learned and developed. 5 P a g e

6 Persons experience times when they lack the ability to identify needs or to respond to those needs. When a person is unable to provide self-care, society provides for assistance to those who lack the ability to care for themselves. Nursing care assists individuals in self-care when deficits exist in their knowledge or ability to provide care for self or dependent. SCDTN is the general theory describing the articulation of three specific theories: Theory of self-care/dependent care, theory of self-care deficit and theory of nursing. 6 P a g e Theory of Self-Care The theory of self/dependent care explains that persons perform actions or sequences of actions for self or dependents to meet self-care/dependent care requirements or requisites. Self-care requires the ability to engage in behaviors to promote well-being, the goal of which is to contribute to and promote health, well-being and life-maintenance. There are three identified types of self-care requisites: universal requisites are those common to all human beings; developmental requisites are those specific to conditions or events related to developmental states; and health deviation requisites are those arising from conditions of illness or injury to the health state. A person s ability to respond to self-care requisites is affected by basic conditioning factors (including age, gender, developmental stage, health state, socioeconomic orientation, and family systems), as well as knowledge, environment and available resources. Theory of Self-Care Deficit The theory of self-care deficit recognizes that humans may be affected by environmental change, or by changes in structural, functional or developmental integrity. These changes may result in new or anticipated needs that are beyond the abilities of the person to engage in self-care. This results in self-care deficits. Self-care deficits occur when therapeutic self-care demand (the summation of all self-care requisites for a particular person) exceeds self-care agency (the ability to meet selfcare requisites). Self-care deficits indicate a need for nursing. Persons with health derived or health related self-care or dependent care deficits are persons who have a need for and can be helped by nursing. Theory of Nursing System The theory of nursing system describes nursing as an action system performed by nurses to assist others and directed toward health related or health derived self-care deficits. Nurses utilize social, interpersonal and professional-technical skills to perform complex and deliberate actions from specialized learned abilities to provide nursing. This set of skills and behaviors defines nursing agency. The end goal of the actions and interactions produced by the nursing system is to protect, develop or regulate self/dependent care agency and to assist patients in achieving therapeutic selfcare. Nursing systems are designed to compensate for self/dependent care deficits for individuals or groups. A patient becomes a legitimate concern of nursing when there are existing or potential self-care deficits. Nursing agency can be wholly compensatory (the nurse provides total support of self-care for a patient who is unable to perform deliberate action, who should not perform deliberate actions, or who cannot make reasoned judgment or decisions about self-care); partially compensatory (the nurse assists the patient in those self-care measures the patient is unable to or should not perform); or educative-supportive (the nurse supplies education and support to the patient). Nurses use the framework of nursing theory to:

7 Determine the patients demand for self-care. Determine the patients ability to engage in self-care. Determine the influence of basic conditioning factors that influence self-care demand and self-care agency. Identify deficits in self-care. Design and initiate a plan of care to address self-care deficits. Evaluate the success of the nursing plan and identify needed continuation or modifications to the plan. Orem defines the nursing process as a conscious, deliberate and systematic method of selecting and performing activities that: Identify who will benefit from nursing care. Identify the types and amount of nursing that will best assist the patient in achieving therapeutic self/dependent care now and in the future. Initiate and sustain the planned nursing care. Judge whether or not nursing care has achieved its intended goal. Upon completion of the education program, the entry-level Associate Degree in Nursing graduate shall demonstrate skills and knowledge within each of the following categories: The entry-level graduate shall recognize self-care as learned behavior derived from interaction and communication. The entry-level graduate shall possess a broad psychosocial base of knowledge and be able to implement this knowledge within the scope of caring for self and to assist the individual with the development of a personal plan for self-care. The plan will include teachinglearning strategies designed to promote the patient s ability to respond to and adapt to selfcare changes, environmental changes and psychosocial changes. The entry-level graduate will be able to develop the strategies required to intervene in those cases when the individual no longer has the capability to respond independently to the demands of self-care. In the application of Nursing Agency, the nurse will through critical thinking, apply the methods of clinical decision-making to assess the patient and to identify the major self-care deficits using data obtained from multiple sources. In addition, the nurse will analyze the sociologic and psychological influences, identify the presenting physical and social needs, and define the environment within which the patient lives. Using these data, the nurse will through collaboration with health team members and the patient, develop the plan of intervention. 7 P a g e

8 Clinical Objectives The clinical objectives are based on the core competencies and performance expectations, which are differentiated into four levels. Level 1: Fundamentals Level 2: Medical-Surgical I and Pharmacology Level 3: Medical-Surgical II, Obstetrical Nursing, Mental Health and Pediatrics Level 4: Medical-Surgical III, Community Based, and Management The Core Competencies with Associated Outcomes: 1. Professional Behaviors: The nurse demonstrates professional behavior by adhering to standards of practice, demonstrating accountability for his/her own actions and behaviors and practicing nursing within legal, ethical, and regulatory frameworks. Level 1: Define and discuss standards of nursing care, nursing practice and professional behavior. Level 2: Demonstrates standards of nursing care, nursing practice and professional behavior. Level 3: Differentiate nursing practice and professional behaviors. Level 4: Develop autonomy and accountability for nursing actions based on standards of nursing care, nursing practice and professional behavior. 2. Communication: The nurse recognizes communication as an important therapeutic tool and as an interactive process through which there is an exchange of information that may occur verbally, in writing or through information technology with the patient, members of the health care team, support persons and community context. Level 1: Define and discuss beginning communication skills using a variety of techniques. Level 2: Demonstrates competence in various communication techniques including, written documentation with individual patients, families and groups. Level 3: Differentiate appropriate communication techniques, including written documentation with individual patients, families and groups. Level 4: Develop and implement a variety of communication techniques, including written documentation with individual patients and families within the community context. 3. Assessment: A primary function of the nurse is to conduct an accurate and informative overview of the patient. As the first step in the clinical judgment process, assessment is the orderly collection of information from multiple sources about the physical, spiritual, developmental, emotional, psychosocial, cultural and functional status of the patient to establish a foundation for the provision of nursing care. Level 1: Develop beginning skill in basic physical, developmental, emotional, psychosocial, cultural and functional assessment. 8 P a g e

9 Level 2: Demonstrate nursing skill in basic physical, developmental, emotional, psychosocial, cultural and functional assessment. Level 3: Differentiate Orem s dependent care concept vs. self care deficit/ self-care agency concepts in physical, developmental, emotional, psychosocial, cultural and functional assessment of individuals, groups and families. Level 4: Demonstrate autonomy in completing a physical, developmental, emotional, psychosocial, cultural and functional assessment of individuals, groups and families within the community context. 4. Clinical Decision-Making: The application of clinical judgment skills results in finding solutions, individualizing care, and assuring the delivery of accurate, safe care that moves the patient toward positive outcomes. Level 1: Demonstrate application of clinical judgment skills and provide basic nursing care based on national patient safety goals for long-term care. Level 2: Demonstrate application of clinical judgment skills and provide basic nursing care based on national patient safety goals for hospital care. Level 3: Differentiate elements of evidenced-based nursing knowledge, theory and national patient safety goals as the basis for making practical decisions. Level 4: Demonstrate autonomy in making nursing practice decisions for positive patient outcomes. 5. Caring Interventions: According to Orem, the inability of a person to provide for self-or dependent-care results in self-care or dependent-care deficits. These deficits justify the need for the helping services of nursing. The nursing system is comprised of nursing behaviors and actions based on knowledge and understanding of the natural sciences, behavioral sciences, nursing theory, nursing research, and nursing experiences that assist patients in meeting their needs. Nurses intervene to help patients meet needs or self-care deficits they are unable to meet themselves. Level 1: Define and discuss Orem s self-care deficits requiring the use of the nursing process in the provision of care to individuals, families, and groups. Level 2: Demonstrate provision of care according to Orem s theory of self-care deficit and apply critical clinical judgment for the provision of care to individuals, families, and groups. Level 3: Differentiate Orem s self-care deficits versus dependent care for the provision of care to individuals, families, and groups. Level 4: Demonstrate autonomy in applying Orem s self-care deficit theory of nursing for the provision of care to individuals, families, groups within the community context. 6. Teaching and Learning: An ongoing expectation of the nurse, both within the health care setting and in the community is the provision of health education to promote and facilitate informed decision-making, achieve positive outcomes, and support self-care activities of the patient. Level 1: Define and discuss teaching-learning principles in planning health teaching for individuals in a structured setting. 9 P a g e

10 Level 2: Demonstrate teaching-learning strategies to meet specific learning needs for health teaching for individuals, families, and groups. Level 3: Differentiate appropriate teaching-learning strategies in providing health teaching for individuals, families, and groups. Level 4: Develop and implement a variety of teaching-learning strategies in providing health teaching for individuals and families within the community context. 7. Collaboration: As health care delivery has become increasingly complex, collaboration among health care professionals has become an important factor for the well-being and safety of the patient. Collaboration is the shared planning, decision-making, problem-solving, goal setting and assumption of responsibility among those who work together to achieve patient-based outcomes. Level 1: Define and discuss the nurse s role as a member of the health care team in providing basic nursing care for individuals and families. Level 2: Demonstrate use of collaborative skills as a member of the health care team in providing basic nursing care for individuals and families. Level 3: Differentiate between the roles of the nurse and other members of the health care team. Level 4: Demonstrate autonomy in nursing leadership and management leading to collaboration as a member of the health care team for the provision of care for individuals and groups within the community context. 8. Managing Care: Through the application of skills, knowledge and clinical experience, the nurse achieves the ability to manage care for the patient. The management of care is accomplished through planning, organizing, directing and evaluating patient care activities for the purpose of assisting the patient in moving toward established outcomes. Level 1: Define and discuss beginning skill in using the nursing process for the provision of basic nursing care to individuals. Level 2: Demonstrate the nursing process when providing care for individuals and families in acute care settings. Level 3: Differentiate the elements of the nursing process applicable to Obstetrical Nursing, acute care and mental health settings. Level 4: Demonstrate autonomy in applying the nursing process in managing care for patients across the health-illness continuum. The clinical and laboratory experiences shall: Meet the established course objectives or outcomes. Be closely aligned to the course content in the acute care/extended care setting and the community-based setting. At the beginning of the term, the focus of the clinical assignment will be limited, but will expand to incorporate more content as the term progresses. Provide a student the opportunity to practice cognitive, psychomotor, and affective skills in the performance of a variety of basic nursing functions with individuals along the life span. 10 P a g e

11 Practice of psychomotor skills will begin with fundamentals; include cognitive and affective skills through case studies and role-play. These criteria are incorporated in each course varying in emphasis with the content and complexity of each course. Be provided concurrently with the related theory instruction. 11 P a g e

12 Program Outcomes 1. Demonstrate autonomy in completing physical, psychosocial, and spiritual assessments of patients. 2. Plan and Implement an individualized plan of care using evidence base practice and technology for patients across the life span. 3. Assess and integrate the educational needs of patients in the promotion, maintenance, and restoration of health. 4. Practice within the realm of the professional nurse as defined by law and guided by ethics. 5. 13 SOCIAL MEDIA POLICY Effective: 12/1/13 POLICY 1.9 Social Media venues will not be used as a means of communicating any patient or display of patient information regarding clinical experiences for Terra State Community College. PROCEDURE A student shall maintain the confidentiality of patient information. The student shall communicate patient information with other members of the health care team for health care purposes only, shall access patient information only for purposes of patient care or for otherwise fulfilling the student's assigned clinical responsibilities, and shall not disseminate patient information for purposes other than patient care or for otherwise fulfilling the student's assigned clinical responsibilities through social media, texting, ing or any other form of communication. To the maximum extent feasible, identifiable patient health care information shall not be disclosed by a student unless the patient has consented to the disclosure of identifiable patient health care information. A student shall report individually identifiable patient information without written consent in limited circumstances only and in accordance with an authorized law, rule, or other recognized legal authority. A student shall not use social media, texting, ing, or other forms of communication with, or about a patient, for non-health care purposes or for purposes other than fulfilling the student's assigned clinical responsibilities. 13 P a g e

14 NURSING PRECEPTOR POLICY AND GUIDELINES Effective: 8/17/2013 POLICY 1.10 The purpose of the policy is to ensure compliance with Ohio Board of Nursing rules regarding agency nurses serving as preceptors, to ensure communication between the student, agency and the nursing program. TSCC shall use various clinical agencies that may provide services for Community Based Nursing, Pediatric and Nursing Management and Leadership and to provide alternative clinical assignments. Preceptor and student contracts will be used in Nursing Management and Leadership as this course requires a one to one relationship only between student and preceptors. Other courses use multiple preceptors throughout the learning experience and a long term professional relationship is not established. Terra State Community College will: Obtain a written affiliation agreement. PROCEDURE Communicate with the site to secure the preceptor. Review the Ohio Board of Nursing website for licensure of each preceptor each semester. Provide the preceptor qualification form to the preceptor. Provide the preceptor with the educational plan, via the student. Aggregate the student observation of the preceptor data and provide feedback. Aggregate the preceptor observation of the student data to use for program analysis and feedback. Conduct site visits as deemed necessary: includes but is not limited to site identification, problem resolution, communication, and meetings. Reserve an agenda item on advisory committees to discuss preceptors. Students will: Complete the required number of preceptor hours. Contact the assigned agency and preceptor to arrange for scheduling the experience. Will schedule a meeting with the preceptor to discuss the Preceptor Qualifications Form (6.30). 14 P a g e

15 Provide course objectives and the course educational plan (calendar) to the preceptor. Provide a student observation form to the preceptor. Community Based Nursing Form 6.18; Nursing Management and Leadership Form 6.19 and Pediatric Form Record the time spent with the preceptor on the Time Log (Form 6.26). Complete a Preceptor Observation Form (Pediatric Form 6.36, CBN Form 6.17 and Nursing Management and Leadership Form 6.20). Preceptors will: Agree to serve as a preceptor at their employing agency. Limit the number of students to be precepted to two. Complete a student observation form (Pediatric Form 6.35, CBN Form 6.18 and Nursing Leadership and Management Form 6.19) and return it to the college. Provide the required hours as defined in the course syllabus. Complete the Preceptor Qualifications Form (Form 6.30) The experience portion of this form must include both years and months to ensure qualification is met. Implement the educational plan Preceptors may delegate patient activities to another RN to observe and monitor the students. Preceptors will be oriented through the Preceptor Manual that will be given to them electronically as a new preceptor and will be updated as the manual is revised and updated. Preceptors can be mentored through correspondence, advisory board meetings and preceptor recognition meetings. Visits may be made to preceptor sites as deemed appropriate by nursing faculty. Preceptors will be monitored through the use of (forms) student feedback, time logs and preceptor observations of the students. Preceptor licenses will be verified the Ohio Board of Nursing website each academic year used and renewal year. 16 NURSING FEES AND EXPENSES POLICY 2.1 The student enrolled in the Associate Degree Nursing Program shall be responsible for the following fees and expenses. These fees and expenses are subject to change with each academic year. 17 ASSOCIATE DEGREE NURSING TRANSFER POLICY POLICY Applicants for transfer must apply for General College Admission. 2. Applicants who are enrolled in another college, but have not taken nursing courses must meet the requirements cited in the Nursing Program Admission Policy. 3. Applicants enrolled in nursing courses at another college must show evidence of good academic standing (Grade of C or better) at the current or previous Nursing Program in order to be eligible for transfer status. Nursing courses from another program will not be accepted by Terra State. 4. The application for transfer will be reviewed by the Nursing advisor. Any student who have failed out of another program must meet with the Director and nursing faculty for an interview for possible admission. 5. All transfer students must complete a minimum of 51% of the nursing courses at Terra Community College and comply with the college graduation requirements in order to be eligible for a Degree in Nursing from Terra State Community College. PROCEDURES 1. Transfer students who wish to enroll in the Nursing Program must meet with the Nursing Program Director/Designee to review transcripts and to determine the extent to which nursing courses taken at a previous college are equivalent to the Terra Community College program. 2. Once the admission requirements are met the application for the Nursing Program is submitted to the Nursing Program Director/Designee. 3. Selection for admission to the Nursing Program by way of transfer is based on the following: a. The number of college credit hours completed toward the nursing degree. b. The grade point average of 2.8 or higher based on a 4.0 grading scale of the courses that apply to the degree. 17 P a g e

18 c. Date and time the application was submitted to the Nursing Program Director/Designee. 4. The applicant will receive written notification from the college regarding the admission status. 18 P a g e

19 NURSING PROGRAM READMISSION POLICY POLICY 2.5 A. Students who withdraw from the Nursing Program for personal or academic reasons and are eligible to return, and may apply for readmission to the program one time only. B. Students who seek readmission into the Nursing Program, after having earned a failing grade in a nursing course, must repeat that course and earn a grade of C or better before being permitted to progress to other nursing courses. C. Students who withdraw from the Nursing Program for two or more years must apply for admission as a new student. At that time, the catalog and criteria currently in effect will supersede the prior catalog. D. Readmission to the Nursing Program regardless of the reason for withdrawing will be on a space available basis. PROCEDURE 1. Students seeking reinstatement must meet with the Nursing Director/Designee and Faculty to review courses and to present a plan of how they will be successful in future courses. 2. Students who have been away from the program for two years must meet with the Nursing Director/Designee to review the student s current standing and obtain an application for readmission. 3. The applicant will receive notification in writing regarding his/her reinstatement or readmission status. 4. Because the availability of a seat may hinder the return of a student, the following process will be used: a. Students who completed the course in its entirety will return first. The order if more than one student is involved with be the final numerical grade of the course; b. Students who withdrew from the course will be next, and again, the order if more than one student is involved will be based on the numerical grade at the time of withdraw. 5. The Nursing Faculty does reserve the right to readmit a student at any time based on the individual student circumstance. 19 P a g e

20 NURSING PROGRAM SECURITY CHECK POLICY 2.6 A. At least two-fingerprint checks will be required during the course of enrollment in the Nursing Program. B. The fingerprint check must be completed and the report submitted to the Program Director/Designee prior to the nursing interview and admission into the nursing program. PROCEDURES 1. The State Bureau of Criminal Identification and Investigation (BCI) and Federal Bureau of Investigation (FBI) is required prior to the nursing interview and admission to the nursing program. Students should consult with the Security at the College for available dates and times for fingerprinting. 2. At the end of the course of study, State Bureau of Criminal Identification and Investigation (BCII) and FBI fingerprinting are required prior to taking the NCLEX. Additional security checks may be required by the Health Care Agency to which the student is assigned for clinical experience. 20 P a g e

21 BASIC LIFE SUPPORT CERTIFICATION POLICY 2.7 A. Each student must maintain a current American Heart Association Healthcare Provider card during the time of enrollment in the Terra State Community College Nursing Program. B. The school does not recognize a grace period after the card expires. C. No student will be able to attend clinical rotations if his/her Healthcare Provider card lapses. D. If a student misses a clinical rotation due to lapse of certification, the lost time will be considered unexcused clinical absence. E. If a student chooses to schedule re-certification during clinical hours, it will be considered an unexcused absence. PROCEDURES 1. Prior to the beginning of nursing classes, each student must show proof of current certification as an American Heart Association Healthcare Provider. 2. The student is responsible for presenting evidence of re-certification to the Program Director/ Designee. 3. The Program Director/Designee will maintain records on each student regarding current American Health Care Provider certification. 21 P a g e

22 HEALTH REQUIREMENTS POLICY 2.8 A health examination is required prior to the nursing orientation for admittance into the program. Title IX pregnancy requirements will be observed. PROCEDURE A. The following immunizations and evaluations must be completed prior to the nursing orientation and the written documentation submitted to the Nursing Director/Designee. 1. Varicella (chicken pox) screening report if no documentation of vaccine. 2. Two-step Mantoux results with one-step repeat every year if done within one year. 3. Completed immunizations: Diphtheria and Tetanus within the past ten years MMR screening report if no documentation of vaccine. Hepatitis B Vaccine (series of three). Yearly Flu vaccine administered in fall. B. The nursing student must have documented evidence (if requested) of the following abilities: 1. The student must have the physical capability to: Provide nursing care for 6 to 8 hours Perform one person CPR Lift and move patients and objects weighing 50 pounds or more Demonstrate fine motor manual dexterity skills Work at varying heights and levels 2. The student must have the sensory ability to: Hear and acknowledge verbal instructions, recognize changes in equipment sounds, perform auscultation with or without assistive devices See and read medication labels, patient records, and equipment instructions Speak clearly to provide information, to explain procedures, to conduct interviews and to ensure patient understanding 22 P a g e

The student must have the cognitive capability to: Read with comprehension at the college level Apply formulas and critical thinking to solve mathematical problems and data analysis Interpret graphs, numerical tables and charts Write concisely Use appropriate grammar and vocabulary Correctly spell common words and medical terminology 4. The student must have the emotional capability to: Function quickly and accurately under stressful conditions Adapt to a changing environment Acknowledge and accept cultural differences Provide nursing care to patients regardless of age, gender, race, national origin, religious beliefs, physical condition and/or disease process. PROCEDURES 1. All documentation is to be submitted to the Nursing Director/Designee prior to the nursing interview. 2. Students are required to obtain their own physical examination and immunizations. *Statement regarding pregnancy: The student is required to bring, in writing, any restrictions given by her healthcare provider. Restrictions will be honored as long as course objectives can be met. A personal plan will be developed as needed for each student who is pregnant to meet the Title IX requirements. If a student must withdraw from any courses while pregnant, a clinical and classroom seat will be guaranteed. *Statement regarding hospitalization: Any student missing clinical because of hospitalization must present written permission from his/her physician in order to return to clinical. 23 P a g e

24 LIABILITY AND HEALTH INSURANCE POLICY 2.9 A. Nursing student must carry professional liability insurance; students are covered under the College s policy. B. Nursing students will be required to have health insurance. PROCEDURES 1. Nursing students of the College will be covered for liability through the College s policy. 2. Nursing students of the College must show proof of personal health insurance. For information on student discounts, see the Admissions Office. 24 P a g e

25 GRADING POLICY POLICY 3.1 The grading scale for all nursing courses (NUR) will be as follows: A = 100% - 94% B = 93% - 87% C = 86% - 78% D= 77% -70 F = 69% -0% PROCEDURE A. To successfully complete any course the student must: Achieve an overall C average on all tests and quizzes and written assignments within the nursing course. Achieve satisfactory competency evaluation of all required nursing skills in the campus laboratory if included in the course. Achieve a satisfactory competency evaluation of all required nursing skills in the clinical setting if included in the course. B. A grade of less than C for any nursing course (NUR) counts as a failing grade. C. A student who earns a grade of less than a C in a nursing course must repeat the course and earn a grade of C or better before proceeding to the next nursing course(s). D. A student, who earns a failing letter grade in any two required nursing courses throughout the program, will be academically dismissed from the program, and/or any student failing the same course twice will be academically dismissed. PROCEDURES 1. There will be no rounding of grades (e.g., is a grade of D ). 2. Grading methods will include evaluation of the understanding of theories and concepts through written and oral tests. 3. When a math test is administered in a nursing course, the test will focus on the types of math problems and calculations that reflect the complexity of the nursing course. 4. A grade of 85% must be achieved in order to be eligible to proceed to the next clinical nursing course. 5. Any math tests may be retaken once. If 85% is achieved, the student may progress to the next clinical course. 25 P a g e

26 6. Evaluation of campus laboratory performance and clinical laboratory performance will be graded as satisfactory or unsatisfactory based on performance criteria. An unsatisfactory grade for the clinical or campus laboratory performance will result in a failing grade for the course. 7. Non-exam points will only account for 4% of the course grade. 8. Professional points will account for 1% of the course grade. 9. Exam points count for 95% of the course grade 26 P a g e

27 PERIODIC EVALUATION OF STUDENT PROGRESS POLICY 3.2 A. All students enrolled in a nursing course will be evaluated in writing by their clinical instructor and/or preceptor at midterm or half of the total clinical session, and at the end of each full semester course. A final evaluation will be given at the end of an eight-week course. B. At the end of each term, the Nursing Director/Designee will review the course grades of each student. Students who have received a grade of F in any course will be placed on probation. C. In all cases, students must meet prerequisites for each course. If a student has not passed a nursing course, the student may have to wait one year to repeat that course before progressing to the next course. D. Once a student has received two failing grades (D or F) he/she will be dismissed from the program. PROCEDURES 1. Additional conferences to discuss clinical performance may be requested at any time, either by the student or by the clinical instructor. 2. The student will be notified in writing, by the Nursing Director/Designee, within two weeks of the end of the term that he/she has been placed on academic probation. 3. A student will be notified within two weeks of the end of the term that he/she has been dismissed from the program. Once a generic student has been dismissed from the program, he/she may not return. 27 P a g e

28 NURSING PROGRAM PROGRESSION POLICY 3.3 In accordance with OBN rule: , Student progression will be analyzed each academic year to identify pattern or trends leading to attrition, determine a course of action and review current practices as needed. PROCEDURES A. Once enrolled in the Associate Degree Nursing Program, the student must achieve a grade of C or better in each nursing course (NUR) listed in the program curriculum. All nursing courses required for the program must be completed within four years from the date of enrollment in the first nursing course. B. Students may enroll in any one nursing course two times throughout the entire nursing program. If not successful on the second enrollment the student will be permanently dismissed. C. When a math test is administered in a nursing course, the test will focus on calculations used in the nursing profession. A grade of 85% must be achieved; the test may be retaken, one time only, after the student undergoes math remediation. If unsuccessful on the second attempt, the student will fail the course. D. Students, who fail one nursing course, must repeat the course and will be considered on academic probation within the Nursing Program. The student will be notified by the Nursing Director/Designee. If the student fails on the second attempt, the student will be dismissed from the program permanently. E. If a student fails a course, it may be possible that the student will need to wait one year to repeat the failed course before taking any other nursing courses. F. If a student fails two nursing courses, the student will be dismissed from the program permanently. G. If an LPN student fails NUR 1130, Nursing Transition twice, the student may apply to the program as a beginning (generic) student by completing an application available from the Nursing Director/Designee. H. The student will be notified by the Nursing Director/Designee that he/she has been dismissed from the program. Once a generic student has been dismissed from the program, he/she may not return. I. This policy may be appealed, in a written letter, to the Program Dean/Designee. The letter must include the following: The nursing course(s) in which the failing grade(s) were received. 28 P a g e

If a student fails two nursing courses, the student will be dismissed from the program permanently. 4. If an LPN student fails NUR 1130, Nursing Transition, the student may repeat the course once. 5. If an LPN student fails NUR 1130, Nursing Transition twice, the student may apply to the program as a beginning (generic) student by completing an application available from the Nursing Dean/Designee. 31 P a g e

32 WITHDRAWING FROM A NURSING COURSE POLICY 3.6 Students enrolled in a nursing course who have earned a passing or failing grade and withdraws from a nursing course before completing the tenth week of the course will receive a grade of Withdraw. Students withdrawing from an eight week course must withdraw by the end of the fifth week. Students are only permitted to withdraw from one course throughout the entire nursing program. PROCEDURES A. Students who withdraw from a nursing course must meet with the Nursing Director/Designee to request permission for re-enrollment. a. Students withdrawing from a course before completing the tenth week of the term must do so through their Banner Self Service portal after meeting with the Director of Nursing/Designee b. Students withdrawing from a course after completing the tenth week (or the fifth week of an eight week course) of the term must do so through their Banner Self Service portal after meeting with the Director of Nursing/Designee B. Failure to officially withdraw from a course will result in an automatic F and the student will be placed on academic probation. 32 P a g e

33 COMPLETION OF THE NURSING PROGRAM POLICY POLICY 3.7 To be eligible to graduate with an Associate of Applied Science Degree in Nursing the student must: 1. Complete all credit hours or equivalent work in the degree program. 2. Earn a minimum of 2.0 cumulative grade point average. 3. Complete at least one-third of all courses required for an Associate Degree at Terra State Community College. PROCEDURES 1. Students nearing the completion of an Associate Degree must apply for graduation. 2. Students initiate the graduation process by filling out the application for degree form. This information must be submitted to the College cashier one academic term prior to the projected graduation date. 3. If a student files the application for degree but will not complete the course requirements to be eligible to graduate by the next commencement, the student will be notified in writing and will be encouraged to withdraw his/her application. 4. Letters of Completion to the Ohio Board of Nursing will be issued based on the recommendation of the Assessment Technology Institute (ATI) RN Predictor results as follows: a. Any student who obtains below a 90% probability will be required to take the Virtual ATI (VATI) review and obtain a green light prior to release of his/her completion letter. 33 P a g e

34 CELL PHONE AND TEXTING POLICY POLICY 3.8 A. Cell phones are an important part of communication. However, it is important that they should not be a distraction in the classroom. B. In the classroom, the cell phone volume should be set to vibrate or turned off while in class. C. Cell phone conversations are not allowed in the classroom. Also, there will be NO texting in the classroom. 1. If you are expecting a phone call in an emergency, please talk to your instructor and then leave the room. 2. The administrative assistant can be called at ext and she will contact the student. *Campus Lab: All rules applied to the classroom will be the same in the lab. * Clinical Experience: Cell phones will not be allowed in the clinical practice area. 34 P a g e

35 CLASSROOM ATTENDANCE POLICY POLICY 4.1 Regular attendance and punctuality at all classes, campus laboratories and clinical experiences is considered essential to academic success at Terra State Community College. Classroom: Regular attendance at all lectures is expected for success in the course. Tardy: If tardy (arriving after faculty has started class), student may be required to wait until break to enter the classroom. Absence: If absent on the day of exam, student will receive 5% deduction and must take the exam within 24 hours. Classroom Illness: If a student becomes ill in the classroom, security is to be contacted at 2253, if it is a medical emergency, contact security and/or The procedures for a Medical Emergency are outlined in the Emergency Procedures Quick Reference Guide that is located in each classroom on the campus. 35 P a g e

36 Unprofessional Behavior CLASSROOM CODE OF CONDUCT POLICY 4.2 PROCEDURE Students are expected to behave in a professional manner at all times. This includes arriving on time, following the dress code, and conducting oneself in a positive manner with peers, instructors, visitors, and agency representatives. Unprofessional behaviors include insubordination (noncompliance or disrespectful behavior) to an instructor; not following dress code; unauthorized use of cell phone; or engaging in impolite or disrespectful behavior to instructors or other students. It shall also include any inappropriate actions or decisions that have the potential to cause harm 1. Insubordination is defined as: Verbal or non-verbal refusal to comply with a directive given by the instructor, nurse, unit manager, or nursing school Dean/Designee. Students who question a directive should use the appropriate chain of command beginning with their instructor. Refusal to change disrespectful behavior when redirected by the instructor. 2. Disrespectful attitudes and behaviors can include: Swearing or obscene language toward an instructor or other students. Behaviors that undermines the rightful authority of instructors. Name-calling, gossiping, spreading rumors, or any attempts to embarrass, ridicule, or degrade instructors or others Argumentative, grumbling, harassing, hitting or slamming things, or displays of tantrums and anger outbursts Refusing to listen or interrupting others before they are finished. This may include controlling the conversation by sighing or groaning, rolling eyeballs, closing one s eyes, putting one s head on the table, or walking out on conversations or presentations. Intimidating actions or words toward peers or instructors. 3. Students will begin each semester with 10 professional points. This will constitute one (1) % of the grade. If a student is unprofessional, points will be deducted using the Classroom Professional Tool. Students will be notified by the instructor that they have had points deducted. 4. One copy of the completed Classroom/Laboratory Professionalism Tool will be available to the student in the Nursing Department office for pickup from 8 a.m.- 4:30 p.m., and one copy will be kept in the student s academic file. 5. Any student who has reached 6 or greater points is mandated to complete a Performance Improvement Plan and have it approved by faculty within one (1) calendar week. 36 P a g e

37 6. A student who accumulates 9 or greater points will receive a due process hearing from the Dean of Students and the college conduct procedures will be followed. CONDUCT RULES AND REGULATIONS (From the Terra State Community College Code of Conduct, College Catalog.) Any student found to have committed the following misconduct is subject to the disciplinary sanctions outlined in Article F: Acts of dishonesty, including but not limited to the following: Cheating, plagiarism, or other forms of academic dishonesty, furnishing false information to any College official, faculty member or office. Forgery, alteration, or misuse of any College document, record, or instrument of identification. Helping or attempting to help another student commit an act of dishonesty Any form of academically unethical behavior involving misuse of College computers Tampering with the election of any College recognized student organization. Disruption or obstruction of teaching, research, administration, disciplinary proceedings, other College activities, including its public-service functions on or off campus, or other authorized non- College activities, when the act occurs on College premises. Physical abuse, verbal abuse, threats, intimidation, harassment, coercion and/or other conduct, which threatens or endangers the health or safety of any person. Attempted or actual theft of and/or damage property of the College or property of a member of the College community or other personal or public property. Hazing, defined as an act, which endangers the mental or physical health or safety of a student, for the purpose of initiation, admission into, affiliation with, or as a condition for continued membership in a group or organization. Failure to comply with directions of College officials or law enforcement officers acting in performance of their duties and/or failure to identify oneself to these persons when requested to do so. Unauthorized possession, duplication or use of keys to any College premises or unauthorized entry to or use of College premises. Violation of published College policy, rule or regulation published in hard copy, posted on campus or available electronically on the College s website. Violation of federal, state, or local law on College premise or at College-sponsored or supervised activities.
