3 EPTEK South Ostrobothnia Health Technology Development Center SERVICES: Project coordination: Several health technology projects, especially related to homecare support and assisted living technologies Previous projects: Nordic Telemedicine Venter, Connected for Health, PrimCareIT, ICT for Health, TUPA, Hyvinvointialan Living Lab, Kulkurinvalssi, Assisted living technologies: Support citizens in finding the right technologies for individual needs, in order to enable independent living as long as possible Consultations and education/training services Nordic Telemedicine center a showroom / demonstration environment for the technologies
4 Social & health care reform in Finland Preparation for the national social & health care reform in Finland is ongoing, current plan is to establish: The reform will bring secondary health care, primary health care & social care together Regional governance reform is being done at the same time Patients will have bigger freedom to choose their health care service provider Regions will be funded by state in the beginning The reform is planned to take place from onward 18 autonomous regions (=provinces) South Ostrobothnia is one of these 18 regions, These regions are responsible for arranging social & health care services Services can be provided by regional public companies, private sector or associations 5 university hospitals, 7 central hospitals with wide emergency services (incl. Seinäjoki) + 6 central hospitals with limited services

10 APOTTI Apotti is an extensive change project of the social services and healthcare field. The central objective of this project is to build a regionally cohesive social services and healthcare system that enables developing the operations and increasing their quality. All information pertaining to the client or patient is up-to-date and available for treatment and service situations. This enables the renewal of operational methods. With the help of Apotti, the services can be better targeted to those needing them the most. The parties participating in Apotti are Helsinki, Vantaa, Kirkkonummi, Kauniainen and the Hospital District of Helsinki and Uusimaa (HUS). The other HUS-district municipalities can also participate in the project via the procurement company KL-Kuntahankinnat Oy.
11 UNA UNA is national social- and healthcare project where build new information system in social- and healthcare area Together with Kanta services UNA is part of national ekosystems UNA projects and few other big development projects cooperate social and healthcare reform project. Every hospital districts and few big towns are involve in UNA - project
12 Connected for health Pilot activity: Distance consultation in wound treatment Test distance consultation and electronic home record Pilot participants: 7 home care nurses, 5 home care clients + 2 specialised nurses from the Alavus primary health care center FTTH was used at 4/5 client cases & in the primary health center; Mobile (4G) was used in 1/5 client case (didn t work -> client was dropped out of the pilot) In the pilot activity, video communication technology was used for distance consultation during the wound treatment situation Distance consultation was carried out between the primary health centre (specialised nurse) & the client s home (where home care nurse & client were present) Solution was operated by the health & home care professionals Distance consultations were carried out on a weekly basis / depending on need
13 Connected for health Pilot activity: Distance care for diabetes patients During the pilot, regular diabetes nurse controls were replaced by virtual appointments Appointments were carried out via Vidyo video communication solution Link to the virtual meeting room was sent to patient together with the appointment time information Participation with own device e.g. from home or office Patients were encouraged to carry out self-monitoring Blood pressure meters, blood glucose meters, reading programmes were provided Electronic template for recording the data was created Electronic health care service portal (Hyvis) was used for communication between health care personnel & patient Data secure information delivery Self-monitored data sent to nurse prior to the appointment
14 TUPA: Process Support and wellbeing to living at home 1. Home care worker notices challenges related to living at home 2. She reports them to a KAT group member 1 time/ month 5. Chosen solution supports customer s life at home. Customer s case is followed and if necessary, discussed again later on. 4. KAT group attempts to find a good solution to the situation. They find out customer s own opinion, organise the funding and delegate tasks to correct persons 3. KAT group member informs the challenge to the multiprofessional KAT group

22 Nordic Telemedicine Center The overall aim of the project is to establish a sustainable cross border Nordic telemedicine competence center between the regions of Västerbotten, Sweden and (South) Ostrobothnia, Finland The competence centre will consist of two physical nodes (in Umeå, Sweden & in Seinäjoki, Finland) and a joint virtual node consisting of a database etc. The project aims at enhancing the ehealth awareness & knowhow in the programme region through: Sharing best practices & experiences Learning from one another Enhancing the crossborder co-operation Developing joint solutions
23 Nordic Telemedicine Center The telemedicine center s purpose is to promote the use of telemedicine technologies in the Baltic Sea region The center will support the development, implementation and evaluation of innovative telemedicine technologies and work models The centre will provide companies with an opportunity to reach relevant target audiences in the health sector more efficiently, with an aim of increasing the cross-sector collaboration The center will be a learning environment for the telemedicine & ehealth education The centre will have active cooperation with researchers for studying and evaluation of the various telemedicine service models
