The home is located in the seaside resort of Blackpool overlooking the south promenade. All bedrooms have en suite facilities. A hairdressing salon and therapy room is also in place for the use of people staying at the home. Off street parking is available for visitors. 4 Summary of this inspection Why we carried out this inspection This was a routine inspection to check that essential standards of quality and safety referred to on the front page were being met. We sometimes describe this as a scheduled inspection. This was an unannounced inspection. How we carried out this inspection We looked at the personal care or treatment records of people who use the service, carried out a visit on 30 July 2014, observed how people were being cared for and checked how people were cared for at each stage of their treatment and care. We talked with people who use the service, talked with carers and / or family members, talked with staff and were accompanied by a pharmacist. We reviewed information sent to us by commissioners of services, reviewed information sent to us by local groups of people in the community or voluntary sector, talked with commissioners of services and talked with local groups of people in the community or voluntary sector. We used information from local Healthwatch to inform our inspection. What people told us and what we found The inspection team was made up of two Inspectors and a Care Quality Commission (CQC) Pharmacist. During the inspection the team gathered evidence to help answer our five key questions; Is the service caring? Is the service responsive? Is the service safe? Is the service effective? Is the service well led? Below is a summary of what we found. The summary is based on our observations during the inspection, speaking with people using the service, the staff supporting them and from looking at records. We also spoke with Blackpool council's contracts monitoring team and Healthwatch Blackpool who are an independent consumer champion for health and social care. If you want to see the evidence supporting our summary please read our full report. Is the service safe? People told us they felt safe and their rights and dignity was respected. They told us they were receiving safe and appropriate care which was meeting their needs. Safeguarding procedures were in place and staff understood how to safeguard people they supported. The home had policies and procedures in relation to the Mental Capacity Act and Deprivation of Liberty Safeguards. Relevant staff understood when an application should be made and in how to submit one. We saw best interest meetings had been held when necessary and these were clearly recorded. The records confirmed safeguarding procedures had been followed and the people had received appropriate support and representation during the meetings. 5 The service was safe, clean and hygienic. Equipment had been maintained and serviced regularly ensuring people were not put at unnecessary risk. People living at the home told us they were happy and well looked after. We looked at the recruitment of new staff. This showed that some required recruitment checks had not been undertaken. We found gaps in employment history were not being explored at interview. This meant the home did not have a full employment history of the people employed. People did not always receive their medicines at the times they needed them and in a safe way. Medicines were not always administered and recorded properly. We have asked the provider to tell us what they are going to do to meet the requirements of the law in relation to recruiting new staff and administration of medicines. Is the service effective? People's health and care needs had been assessed with them, and they were involved in writing their plans of care. Specialist dietary needs had been identified where required. Care plans had risk assessments completed to identify the potential risk of accidents and harm. Staff members we spoke with confirmed guidance was provided to ensure they provided safe and appropriate care. We found care plans were flexible, regularly reviewed for their effectiveness and changed in recognition of the changing needs of the person. People spoken with said their care plans were up to date and reflected their current needs. Is the service caring? People were supported by kind and attentive staff. We saw care workers showed patience and gave encouragement when supporting people. The people we spoke with were happy with the care being provided. One person said, "I love it here. The girls really look after us. They are so pleasant and caring. Nothing is too much trouble for them. The food is wonderful and we have lots of activities to keep us entertained." Two people visiting their relatives told us they were happy with the home and had no concerns about the care provided. One person said, "I cannot believe the difference in my mum since she moved into the home. She is always clean and well presented. How they manage it I don't know. The staff really care. I cannot praise them high enough." Care plans had been maintained recording the care and support people were receiving. Good care practices were observed and people told us they were happy with the support they were receiving. Is the service responsive? Records showed admissions to the home were well planned. Information about people's care and dietary needs had been recorded. We also saw potential risks to people's health and welfare had been identified. Guidance had been provided for staff to ensure they provided safe and appropriate care. We found people completed a range of activities. On the day of our inspection we saw people playing dominoes and bingo in the afternoon. People we spoke with said they enjoyed the activities organised by the home. We found people had access to healthcare professionals. People received regular health checks with their General Practitioner and the outcome of these visits were recorded on their care records. Is the service well-led? The service had quality assurance systems in place. Records showed that identified problems and opportunities to change things for the better were addressed promptly. As a result the quality of the service was continuously improving. Staff had a good understanding of their roles and responsibilities. 6 a good quality service at all times. You can see our judgements on the front page of this report. What we have told the provider to do We have asked the provider to send us a report by 29 August 2014, setting out the action they will take to meet the standards. We will check to make sure that this action is taken. Where providers are not meeting essential standards, we have a range of enforcement powers we can use to protect the health, safety and welfare of people who use this service (and others, where appropriate). 7 Our judgements for each standard inspected Care and welfare of people who use services Met this standard People should get safe and appropriate care that meets their needs and supports their rights Our judgement The provider was meeting this standard. People using the service experienced safe, appropriate care and support to meet their identified needs. Reasons for our judgement During our inspection we looked at a sample of care plan records. We found these described the assessed needs and support people required. The records were informative and enabled us to identify how staff supported people with their daily routines and personal care needs. Care plans were flexible, regularly reviewed for their effectiveness and changed in recognition of the changing needs of the person. The home had policies and procedures in relation to the Mental Capacity Act and Deprivation of Liberty Safeguards. Relevant staff understood when an application should be made and in how to submit one. We saw best interest meetings had been held when necessary and these were clearly recorded. This confirmed good communication protocols were in place for people to receive continuity with their care. During our inspection we observed staff interactions with people in their care. This helped us assess and understand whether people using the service were receiving care that was meeting their individual needs. We saw staff being responsive and attentive. This confirmed people who required support with their personal care were being treated with respect and dignity. We observed the routines within the home were being arranged around people's individual and collective needs. We saw they were provided with the choice of spending time on their own or in the communal areas. The home had a relaxed atmosphere. We observed staff were kind and caring when providing support. 8 meeting their needs. One person said, "I love it here. The girls really look after us. They are so pleasant and caring. Nothing is too much trouble for them. The food is wonderful and we have lots of activities to keep us entertained." Two people visiting their relatives told us they were very happy with the home and had no concerns about the care provided. One person said, "I cannot believe the difference in my mum since she moved into the home. She is always clean and well presented. How they manage it I don't know. The staff really care. I cannot praise them high enough." All of the people we spoke with said they were fully satisfied with their care and with the choices and quality of the food provided. Care plans had been maintained recording the care and support people were receiving. Good care practices were observed and people told us they were happy with the support they were receiving. We saw staff assisting people with mobility problems. We observed they were patient and took care to ensure people were assisted safely. The staff members we spoke with confirmed they had received mandatory moving and handling training. We found people completed a range of activities. On the day of our inspection we saw people playing dominoes and bingo in the afternoon. People we spoke with said they enjoyed the activities organised by the home. During our inspection we contacted the Blackpool contracts monitoring team. They told us they had no concerns about the service. We also contacted Healthwatch Blackpool prior to our inspection. Healthwatch Blackpool is an independent consumer champion for health and social care. They did not provide us with any information about the home. Inspection Report Feng Shui House Care Home August

9 Cleanliness and infection control Met this standard People should be cared for in a clean environment and protected from the risk of infection Our judgement The provider was meeting this standard. People were protected from the risk of infection because appropriate guidance had been followed. Reasons for our judgement When we arrived at the home we saw it was clean, tidy and well-maintained. A tour of the building confirmed bathroom and toilet area's were clean and hygienic. Hand washing dispensers were readily available in all rooms and clean towels were provided daily. Hand hygiene and infection control facilities were available throughout the home. We saw staff had been provided with protective clothing and gloves. This should ensure they were protected from potential infection when delivering personal care and undertaking cleaning duties. Staff were observed undertaking their duties in an unhurried and professional manner. Cleaning schedules were in place and these were being audited by the manager. People we spoke with said they were satisfied with the hygiene standards provided by the home. One person said, "The home is spotless and always clean and smells fresh." The staff we spoke with had a good understanding of infection control procedures. They had all read the homes infection control policy and signed confirming they understood the procedures to be followed. When we undertook this inspection a number of staff had had attended infection training provided at the home by Blackpool council. Inspection Report Feng Shui House Care Home August

10 Management of medicines Action needed People should be given the medicines they need when they need them, and in a safe way Our judgement The provider was not meeting this standard. People were not protected against the risks associated with medicines because the provider did not have appropriate arrangements in place to safely manage them. We have judged that this has a moderate impact on people who use the service, and have told the provider to take action. Please see the 'Action' section within this report. Reasons for our judgement As part of this inspection we looked at medicines records, supplies and care records relating to the use of medicines for nine people. We talked with two care workers and one person who used the service about how medicines were handled. Overall we found people were at risk because medicines were not always handled safely. Medicines were not safely administered. We saw some medicines for two people had been signed as given on the morning of our inspection but stock checks showed they had not been. Our checks showed three other medicines had been administered to people over the previous week but a record had not been made. We found people did not always get their medicines at the right time. For example, one person was given two medicines together but one was supposed to be given before food and the other afterwards. This meant there was a risk to people's health and wellbeing. Appropriate arrangements were not in place in relation to the recording of medicines. Medicines stocks were not always properly recorded when supplies were received midmedicines cycle. This meant these medicines could not be fully accounted for. Handwritten records were not accurately completed and were not witnessed by two care workers to ensure accuracy, as recommended by NICE (National Institute for Health and Care Excellence). One person was prescribed insulin that was being administered by district nurses but this was not accurately listed on the current records. A suitable trolley, cupboard and a fridge were used for storing medicines to help ensure medicines were safely kept. However we found insulin was not stored according to the manufacturer's requirements so there was a risk it would not be safe for use. We also found unlabelled sachets of a medicine in the medicines trolley so it was not clear who they were prescribed for. In the controlled drug cupboard we saw an unlabelled plastic pot containing tablets that were still being used, some of which were dirty and unfit for use. We looked at medicines care plans and information kept with the medicines records. We saw effective systems for managing people that refused their medicines. 11 meetings were held when necessary and these were clearly recorded. This meant people's rights were safely protected. However we saw no information about how 'when required' medicines should be administered. The manager told us there had previously been plans with the medicines records but these were no longer in place. This meant there was a risk care workers might not safely administer these medicines. 12 Requirements relating to workers Action needed People should be cared for by staff who are properly qualified and able to do their job Our judgement The provider was not meeting this standard. People were not supported by staff who had been properly recruited. We have judged that this has a minor impact on people who use the service, and have told the provider to take action. Please see the 'Action' section within this report. Reasons for our judgement We looked at the recruitment procedures the service had in place. We found relevant checks had been made before new staff members commenced their employment. These included Disclosure and Barring Service checks (DBS), (formerly CRB checks) and references. These checks were required to ensure new staff were suitable and safe for the role of protecting the people in their care. Members of staff we spoke with said they had found their recruitment very thorough and professional. We found application forms had been signed and dated by the applicant. We also saw the date the person had commenced their employment had been recorded. This enabled us to follow an audit trail of the person's recruitment from the date of application to the date they commenced their employment. However we found gaps in the employment history of two people recently appointed had not been explored at interview. One person had only declared their employment in 2013 and a second person had only declared their employment from Providers are required to operate effective recruitment procedures in order to ensure staff they employ are safe and suitable to work with people in their care. 13 Supporting workers Met this standard Staff should be properly trained and supervised, and have the chance to develop and improve their skills Our judgement The provider was meeting this standard. People were supported by a trained and competent staff team. Reasons for our judgement We looked at staff training records and spoke with the staff members on duty. We did this to ensure the registered person had suitable arrangements in place for staff to receive appropriate training, professional development, supervision and appraisal. The home had a training matrix in place confirming all staff had access to a structured training and development programme. This included mandatory training covering health and safety, manual handling techniques, food hygiene, infection control, safeguarding, personal care and medication administration. In addition some staff had achieved national care qualifications. This confirmed people in their care were being supported by a well trained and competent staff team. The staff members we spoke with told us they were supported by the provider. They told us they were happy with the support structure in place which included performance appraisals with as well as team meetings. Inspection Report Feng Shui House Care Home August

14 Assessing and monitoring the quality of service provision Met this standard The service should have quality checking systems to manage risks and assure the health, welfare and safety of people who receive care Our judgement The provider was meeting this standard. Systems were in place for monitoring the quality of the service people receive. Reasons for our judgement The provider had procedures in place to monitor the quality of the service being provided. Audits were being completed by the provider. These included monitoring the environment, infection control, reviewing care plan records, medication procedures and staff training. Service contracts were in place confirming the building was maintained and a safe place for people to stay. We found the provider had sought the views of people being supported about their service by a variety of methods. These included meetings to discuss the service being provided. We saw documented evidence that these had been held and the people being supported had attended. Relative surveys were also in place. These had been summarised by the provider and action considered where shortfalls in the service had been identified. People we spoke with during our inspection confirmed they were happy with the service being provided. 17 About CQC inspections We are the regulator of health and social care in England. All providers of regulated health and social care services have a legal responsibility to make sure they are meeting essential standards of quality and safety. These are the standards everyone should be able to expect when they receive care. The essential standards are described in the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010 and the Care Quality Commission (Registration) Regulations We regulate against these standards, which we sometimes describe as "government standards". We carry out unannounced inspections of all care homes, acute hospitals and domiciliary care services in England at least once a year to judge whether or not the essential standards are being met. We carry out inspections of other services less often. All of our inspections are unannounced unless there is a good reason to let the provider know we are coming. There are 16 essential standards that relate most directly to the quality and safety of care and these are grouped into five key areas. When we inspect we could check all or part of any of the 16 standards at any time depending on the individual circumstances of the service. Because of this we often check different standards at different times. When we inspect, we always visit and we do things like observe how people are cared for, and we talk to people who use the service, to their carers and to staff. We also review information we have gathered about the provider, check the service's records and check whether the right systems and processes are in place. We focus on whether or not the provider is meeting the standards and we are guided by whether people are experiencing the outcomes they should be able to expect when the standards are being met. By outcomes we mean the impact care has on the health, safety and welfare of people who use the service, and the experience they have whilst receiving it. Our inspectors judge if any action is required by the provider of the service to improve the standard of care being provided. Where providers are non-compliant with the regulations, we take enforcement action against them. If we require a service to take action, or if we take enforcement action, we re-inspect it before its next routine inspection was due. This could mean we re-inspect a service several times in one year. We also might decide to reinspect a service if new concerns emerge about it before the next routine inspection. In between inspections we continually monitor information we have about providers. The information comes from the public, the provider, other organisations, and from care workers. 18 How we define our judgements The following pages show our findings and regulatory judgement for each essential standard or part of the standard that we inspected. Our judgements are based on the ongoing review and analysis of the information gathered by CQC about this provider and the evidence collected during this inspection. We reach one of the following judgements for each essential standard inspected. Met this standard This means that the standard was being met in that the provider was compliant with the regulation. If we find that standards were met, we take no regulatory action but we may make comments that may be useful to the provider and to the public about minor improvements that could be made. Action needed This means that the standard was not being met in that the provider was non-compliant with the regulation. We may have set a compliance action requiring the provider to produce a report setting out how and by when changes will be made to make sure they comply with the standard. We monitor the implementation of action plans in these reports and, if necessary, take further action. We may have identified a breach of a regulation which is more serious, and we will make sure action is taken. We will report on this when it is complete. Enforcement action taken If the breach of the regulation was more serious, or there have been several or continual breaches, we have a range of actions we take using the criminal and/or civil procedures in the Health and Social Care Act 2008 and relevant regulations. These enforcement powers include issuing a warning notice; restricting or suspending the services a provider can offer, or the number of people it can care for; issuing fines and formal cautions; in extreme cases, cancelling a provider or managers registration or prosecuting a manager or provider. 19 How we define our judgements (continued) Where we find non-compliance with a regulation (or part of a regulation), we state which part of the regulation has been breached. Only where there is non compliance with one or more of Regulations 9-24 of the Regulated Activity Regulations, will our report include a judgement about the level of impact on people who use the service (and others, if appropriate to the regulation). This could be a minor, moderate or major impact. Minor impact - people who use the service experienced poor care that had an impact on their health, safety or welfare or there was a risk of this happening. The impact was not significant and the matter could be managed or resolved quickly. Moderate impact - people who use the service experienced poor care that had a significant effect on their health, safety or welfare or there was a risk of this happening. The matter may need to be resolved quickly. Major impact - people who use the service experienced poor care that had a serious current or long term impact on their health, safety and welfare, or there was a risk of this happening. The matter needs to be resolved quickly We decide the most appropriate action to take to ensure that the necessary changes are made.
