PRACTICE INFORMATION We are delighted to serve you and your family by providing quality and compassionate healthcare to both the insured and uninsured in Williamson and surrounding counties - with a focus on Primary and Integrated Behavioral Healthcare for the entire family! 2 Mission & Vision Mission statement: Mercy Community Healthcare exists to reflect the love and compassion of Jesus Christ by providing excellent healthcare for all and support to their families. Vision statement: Mercy Community Healthcare will transform the physical, mental and spiritual health of individuals through the healing power of Jesus Christ to the glory of God for generations to come. Fundraising At Mercy, no one is turned away for their inability to pay. Because of this, we must raise nearly $1 million a year to help cover the costs to care for those that are uninsured. The money we raise is donated by individuals, churches, corporations, and foundations. We also host three fundraising events throughout the year to raise money and awareness: Mercy Golf Classic, Franklin Classic and the American Girl Fashion Show. 3 Appointments & Same-Day Care To make an appointment, please call our office during normal business hours at (615) When calling to make an appointment for your child, please choose the Pediatric option. When calling to make an appointment for yourself, choose the Adult option. You will be directed to a patient service s representative who will assist you in making an appointment. For your convenience, walk-in appointments are available for Pediatric acute care medical needs. Please be on time for your scheduled appointments. As a courtesy, we allow a 10 minute grace period for appointments. If you are later than 10 minutes you may be asked to reschedule or have to wait for the next available appointment that day. Appointment Reminders You will be contacted via automated phone call approximately 2-3 days prior to your appointment. You will also be notified via text message and/or if you provide us with your cellular phone number and/or address. Cancellation Policy Mercy Community Healthcare requires that you provide us with 24 hours notice when canceling your appointment. Multiple cancellations without proper notice, and/or failure to show up for three appointments, may result in termination of services. As a non-profit, Mercy must raise nearly $1.2 million a year to provide healthcare to our patients. Please assist us in this effort by keeping your scheduled appointment. If you need to cancel or reschedule your appointment, doing so in a timely manner allows us to offer that appointment to another patient. No Shows cost us money. Medical Record Information Please call our office at (615) and ask for the Medical Records clerk with your medical record related questions or requests. Requests for medical records may take between 7-10 business days.

4 Financial Policy All copays and visit self-pay fees are due at check-in. Additional fees for labs, procedures; tests, etc. are collected upon checkout the day of service. The patient/legal guardian who is present for the visit is responsible for all fees on the day of service. Mercy understands that sometimes financial circumstances make it difficult to pay for your visit at the time of service. If you are experiencing a hardship, you may contact our Billing Department at (615) who will work with you to set up alternative payment arrangements. Self-Pay Program Mercy Community Healthcare exists to provide healthcare services to everyone. We provide a Self-Pay Program for those without insurance. Our sliding fee discount schedule is determined by the 2014 Federal Poverty Guidelines. By Federal Law, qualification is based on two factors: household size and income. You will be asked to complete an application and provide proof of income for all adult members in your household. The completion of those documents will determine if you are eligible for the program, the category your household size and income place you in, and the fee(s) required at time of service. Admittance into the Self-Pay Program is valid for a period of one year. Your fees could change if your income or household size increase or decrease during that time. Please inform us if this occurs. Mercy trusts that the information you provide is true and accurate. We reserve the right to disqualify you from the Self-Pay Program if you have misrepresented information. Billing and Insurance Information Please call our office at (615) and ask for the Billing Department with your billing and insurance related questions. Mercy accepts all types of insurance including; commercial, high-deductible plans, TennCare and Medicare. We are not responsible for knowing what your medical and behavioral health insurance benefits are and recommend you call your insurance company to find out if Mercy is in your insurance s network of providers.

5 Emergencies Mercy Community Healthcare providers are dedicated to serving our patients 24 hours a day. If you are unsure whether an illness or injury sustained by you or your child requires emergent attention, please call our office at (615) During office hours, you will be referred to a triage nurse. After hours, you will be referred to our answering service, where a nurse and/or provider on-call will speak with you. If you feel that you or your child are experiencing a medical emergency that requires immediate attention, please dial 911 or proceed to the nearest emergency room. Prescription Refills In order to provide quality care, medication refills are best addressed at the time of your visit with your provider. This allows you to update the provider and plan any necessary change in your medication or to schedule any testing that may need to be done. If you are unable to be seen by the provider before you are out of medication, a once a year bridge script may be provided to last until your next scheduled appointment. A bridge script will only be provided once within a year. It is important that you keep your appointments and plan follow up appointments BEFORE you will be out of medication. Your provider will only replace one lost or stolen prescription/medication during your time as a patient at Mercy Community Healthcare. After this has been done, no further prescriptions/medications will be replaced as a result of being lost or stolen. If you no-show or miss your appointment without notifying the clinic and canceling, you will not be given prescriptions to cover your medications until your next appointment. Pharmacy Discount Program As a Federally Qualified Health Center, Mercy has access to many prescription medications at greatly reduced prices. Working in conjunction with many Walgreens stores throughout the area, Mercy s uninsured and Medicare patients can now obtain needed prescription medications at substantially reduced prices*. For Mercy s commercially insured patients, our plan is accepted by Walgreens, too. Your plan works just like always and because you are a patient of Mercy, you are creating needed revenue for Mercy when you use the designated Walgreens Pharmacies or its mail order program. This new pharmacy program is a further extension of Mercy Community Healthcare s desire to provide total healthcare delivery for our neighbors throughout Middle Tennessee. *excludes TennCare (Medicaid) and CoverKids patients

6 Behavioral Health Appointment Types & Allotted Times Mercy Community Healthcare s Behavioral Health services are by appointment only. The length of the appointment varies based on the type of services provided. Behavioral Health Consultation Intake Sessions are minutes in length. Clients under the age of 16 must be present and accompanied by their parents. Behavioral Health Follow up Appointments Sessions are 30 minutes in length. These appointments are for the client and or/ parents (dependent upon the individual treatment plan.) Clients under the age of 16 must be checked in by a parent or guardian for their follow up. Individual Counseling Intakes Sessions are minutes in length. The first appointment (the intake session) is for the parents alone unless the client is over the age of 16, then the client may be present for the appointment. Individual Counseling Follow up Appointments These sessions are for the client only, unless the counselor has previously communicated a change of plans to the parents. Clients under the age of 16 must be checked in by a parent or guardian for their follow up. Psychiatry Intake Sessions are minutes Clients under the age of 18 must be present and accompanied by their parents to all intake sessions. Psychiatry Follow up Sessions are minutes. Clients under the age of 18 must be present and accompanied by their parents to all follow up sessions (unless previously discussed with the provider)

7 Patient Rights Mercy accepts all persons seeking service regardless of race, national origin, sexual orientation, gender, religion, handicap or disability (assuming capacity is available.). As a patient of Mercy Community Healthcare: You have the right to receive considerate, respectful and quality care. We promise to treat you with dignity and regard for your medical, psychological, cultural, and spiritual beliefs. You have the right to a safe, clean and comfortable facility as determined by OSHA and OES (Office of Emergency Services) and implemented by an Emergency Planning Committee. You have the right to an environment free from sexual, physical and emotional abuse. You have the right to a smoke and nicotine free facility, both inside and outside the clinic. You have the right to be given the name of your provider, their professional capabilities, licensure, education, training, specialization, and limitations. You also have the right to the names and functions of all other health care persons involved in your care. You have the right to expect your medical information be treated with the utmost privacy and confidentiality, except as otherwise provided by the law or third-party arrangements (insurance). You have the right to access your medical information upon signing an authorization and in most cases receive a summary of your behavioral health file or request its transfer to another provider or mental health agency. You have the right to your diagnosis, treatment, and prognosis. You have the right to appropriate continuity of care and specialist referral services. You have the right to a foreign language or sign language interpreter in order to properly understand and participate in your care. You have the right to request and receive a written explanation of your fees, payment, insurance, and number of visits upon signing an authorization. Mercy Community Healthcare strives to keep and respect your rights as our patient. We provide the opportunity for all patients to express their concerns for quality of care through a grievance process. If you feel your concern or complaint has not been addressed in a sufficient manner, you may contact our Chief Operations Officer in person or by calling our office at: (615)

8 Patient Responsibilities In order for us to provide you with the highest quality of care, you are responsible for: Providing accurate and complete information about your past and present illnesses and hospitalizations. Providing accurate and complete information about your medication history, reactions and concerns. Providing information about your medical condition(s) as they occur. Asking questions if you do not understand the directions or instructions given to you by any health care worker involved in your care. Completing all necessary paperwork as required completing registration, ensuring accurate billing for services and allowing for proper treatment and quality care. Update contact information (telephone number and address) as necessary by informing a patient service s representative by calling our office at (615) or when checking in for an appointment. Presenting a photo id and insurance card for all appointments. Compliance with our smoke and nicotine-free policy. Not bringing the following items onto Mercy s premises: alcohol, tobacco/nicotine products, weapons or any illegal substances. Providing us with all custody and legal documentation, including but not limited to: Guardianship, Power of Attorney, Orders of Protection, Advanced Directives, etc. Keeping your scheduled appointments. If you should need to cancel your appointment, please provide our office with 24 hours notice. Failure to show up for 3 appointments without proper notice may result in dismissal from services. Being on time for your scheduled appointments. As a courtesy, we allow a 10 minute grace period for appointments. If you are later than 10 minutes you may be asked to reschedule or have to wait for the next available appointment that day. Fulfilling your financial obligations by paying all fees at the time of service. All copays and self-pay fees are due at check-in. Additional fees for labs, procedures and tests are not included in this fee and will be expected at check out. Charges for all services are due on the day of service, and will be collected at check-in. Providing us with the necessary documents in order to qualify for our Self-Pay Program. These documents are required in order to determine eligibility and your sliding fee scale category.

She has dedicated her career to caring for all children. She feels that every child deserves outstanding medical care and is committed to building caring relationships with her patients and families to achieve that goal. Now back at Mercy, Dr. Brunner often shares stories of God s presence and provision while in Peru. Invaluable to him is the support of his wife, Laura, and the love of his children, Alexandra, Sofia, Lydia, Joshua, and Daniel. He is most thankful for the Lord Jesus Christ, his Savior. Dr. Brunner s hope and prayer is to glorify God in all that he does and an integral part of this purpose is reflecting Christ s love through his service to the patients and staff at Mercy.

Although she initially didn t know what type of medicine she would practice, God has led her stepby-step into Pediatrics, then to Tennessee, and now to Mercy. It is a tremendous blessing to know that she is doing the work that God has called her to do and to know that she has the support of her friends and family. Caleb Hancock, Family Nurse Practitioner BA Psychology: University of Tennessee, Knoxville BSN: University of Tennessee, Knoxville MSN: University of Tennessee, Knoxville Board Certified, Family Nurse Practitioner Caleb was raised in Franklin, TN before going to college in Knoxville ten years ago. Since then, Caleb has been blessed to marry his high-school sweetheart, Katie, and they now have two wonderful children. The Hancocks are excited to move back to this area to serve in a young church plant, as well as to join the staff at Mercy. Caleb believes that health is not just the absence of disease, but the active pursuit of well-being: physically, intellectually and spiritually. Caleb believes in the mission and purpose of Mercy and is blessed to be able to extend the grace of God that he received in Christ to his patients as he cares for their health.

13 Medical Providers Dr. Chris Belleau, Adult Primary Care Physician BA: University of Illinois MD: University of Illinois Residency: University of Tennessee, Memphis Family Medicine Diplomate, American Board of Family Medicine Board Certified, Family Medicine Member, American Academy of Family Practice, Tennessee Academy of Family Practice Dr. Belleau is originally from Illinois and has lived in Tennessee since She is blessed with a beautiful family; Joe, her husband, two children and a lab named Beau. Dr. Belleau loves being with her family and especially treasures the time they get together to swim and play on the lake, hike, or ride bikes. The Belleaus are members of Brentwood United Methodist Church. Dr. Belleau is very proud to be a part of Mercy and its mission by caring for the adults in this wonderful community we live in. Dr. DeAnna Brown, Pediatrician BA: University of Tennessee, Knoxville MD: East Tennessee State University Quillen College of Medicine Residency: Vanderbilt Children s Hospital - Pediatrics Fellow, American Academy of Pediatrics Board Certified, Pediatrics Clinical Instructor, Vanderbilt University Medical Center Hospital Privileges: Vanderbilt Children s Hospital Williamson Medical Center DeAnna Brown, MD, is a board-certified pediatrician and a member of the American Academy of Pediatrics. She received her medical degree from East Tennessee State University s Quillen College of Medicine in Johnson City, Tennessee, and completed her Pediatric residency at Vanderbilt Children s Hospital in Nashville, Tennessee. Dr. Brown sees patients of all ages ranging from birth through adolescence. Her clinical interests are very broad and include routine well child and preventative care, breastfeeding education and support, managing chronic medical conditions such as asthma and ADHD, along with promoting nutrition and weight management. Being a Pediatrician allows Dr. Brown the opportunity to apply her skill set to the benefit of future generations, and she enjoys caring for children in collaboration with their parents. Dr. Brown is happily accepting new patients and welcomes the opportunity to meet and care for your child as he or she grows. Originally from rural southwestern Virginia, Dr. Brown is now proud to call Middle Tennessee her home where she lives with her husband and two young sons. In her spare time, she enjoys spending time with her family, following UT football, reading, snowboarding, and traveling.

From the day she moved to Nashville over 10 years ago, she had her eye on Mercy. She is excited to share the love of Jesus while helping others with their medical concerns. She loves to spend time with her family and is married with a sweet little girl. Jennifer likes to catch up with friends, play sports, exercise, bake and read. She is thrilled to have the opportunity to serve the Lord in this capacity! Dr. Melissa Hixson, Adult Primary Care Physician BS: Vanderbilt University MD: University of Tennessee College of Medicine, Memphis Residency: Vanderbilt University Medical Center - Internal Medicine Board Certified, Internal Medicine Member, American College of Physicians Dr. Hixson is thrilled to be a working with such a wonderful team at Mercy! She is a wife and mother to 3 young children and loves spending time with her family and friends at home and in the outdoors. Her interests include hiking, jogging, cooking, and reading. Her husband, Reid, is a Pediatrician and together they have two wonderful children. At their church, Otter Creek, Dr. Collins sings on the praise team. She loves reading, running, hiking, being in theatrical productions, and spending time with her family. She is honored to work in a Christian healthcare environment where the primary mission is to care for children of all socioeconomic backgrounds and where there is true concern for all aspects of the lives of the patients, the families, and the staff. Dr. Ruth Long, Pediatrician BS: Auburn University MD: Auburn University Residency: Vanderbilt University - Pediatrics Member of American Academy of Pediatrics, TN Medical Association, Nashville Academy of Medicine & Cumberland Pediatric Foundation Board Certified, Pediatric Medicine Hospital Privileges: Vanderbilt Children s Hospital Williamson Medical Center Dr. Long is originally from Georgia, but has lived in Nashville since She s happily married to her husband, Will Long, and is the mother of three children: Barry, Hallie, and Cynthia. Her greatest spiritual struggle and spiritual growth has come through having a son with Down Syndrome and developmental challenges. As she says, He is a great blessing, as well as a great challenge.

16 Medical Providers Sam Paris, Pediatric Nurse Practitioner BS: Middle Tennessee State University MSN: Vanderbilt University School of Nursing Member, National Association of Pediatric Nurse Practitioners Board Certified, Pediatric Nurse Practitioner Sam Paris, MSN, CPNP-PC is a board-certified Pediatric Nurse Practitioner who is looking forward to establishing and growing relationships with the patients and families of Mercy. He is focused on providing compassionate and intentional health care to children of all ages regardless of background or barriers. He has interests in rural and community health care as well as preventive and primary care. Sam grew up in Middle Tennessee and is looking forward to utilizing his training and expertise to positively impact the lives of children in the area he has always called home. Sam loves every moment of time he gets to spend with his wife, Sarah, and his son, Keane. He enjoys reading, running, sports, and music. He is also a member of Fellowship Bible Church of Rutherford County where he actively lives in community with his fellow believers in Christ.

17 Mental & Behavioral Health Providers Dr. Anne Fottrell, Clinical Director of Psychiatry BS: Middle Tennessee State University Science MD: University of Tennessee College of Medicine Health Internship in Internal Medicine Baptist Hospital Memphis Adult and Child Psychiatry Vanderbilt University Board-Certified Adult Psychiatry Board-Eligible Child Psychiatry Held teaching positions at Vanderbilt University for Residents and Nurse Practitioners Worked at a private practice in Nashville since 1993 Held various consulting positions currently Medical Director of Scarab Behavioral Dr. Fottrell came to Mercy originally, because she had always participated in public mental health in some capacity, and it was closer to home. What began as a matter of convenience has turned into a mission and calling. As she says, There is no better staff anywhere and I am very privileged to work beside such wonderful people. She had felt God s call on her life over the years, but was fairly certain she was not called to parrish ministry. Through working at Mercy, she has come to realize that her path is to practice psychiatry, especially with those who may not be able to access care from the private sector. She feels very blessed to have found like-minded people with whom to serve. Emily Hoffberg, LPC-MHSP, Clinical Therapist BA: Psychology, Union University MS: Counseling, Lipscomb University Emily is originally from Northwest Illinois, which is where her love for counseling began. In high school, her church presented her with the opportunity to lead groups for middle school girls. She began mentoring the girls individually as well, and before long she knew that it was something she wanted to continue doing at a professional level. In 2005, Emily moved to Tennessee to study psychology at Union University in Jackson. During that time she completed internships working with adolescents at Mercy Ministries of America and Juvenile Court Services. From there, Emily moved to Nashville to pursue a Master s degree in counseling at Lipscomb University. As a part of that program she was introduced to Mercy and became a counseling intern. Upon graduating, Emily was asked to continue counseling as part of the Mercy Mental & Behavioral Health team. On a personal note, Emily is a music lover and has wanted to live in the Nashville area for as long as she can remember. But, as much as she loves living in Nashville, she enjoys visiting family back in Illinois, especially since the additions of her three-year-old niece, Callie, and one-year-old nephew, Bennett. Her hobbies include hiking with friends, reading good books, and eating homemade guacamole on a weekly basis.

18 Mental & Behavioral Health Providers Gretchen Dorman, LCSW, Behavioral Health Consultant BS: University of Tennessee-Knoxville, Psychology MS: Erikson Institute, Child Development MS: Loyola University-Chicago, Social Work Gretchen grew up in Wisconsin and went back and forth between the Midwest and Tennessee for her schooling. She ultimately settled in Franklin, Tennessee and is thrilled to be a part of the Mercy family. Gretchen has years of experience working with children and families in various settings, including residential treatment, home visitation programs, and intensive in-home services. Gretchen loves spending time with her family and enjoys cooking and experimenting with new recipes in her free time. After completing her bachelor s degree in psychology, she went on to pursue her master s degree in professional counseling. During graduate school, she had the opportunity to be an intern with Mercy, providing counseling at New Hope Academy. Upon graduation, Hillary joined the case management department at Mercy as the Intensive Social Services Case Manager. In this position, she participated in symptom identification management, crisis planning and support, accessing community resources across life domains, and education about physical health, life skills, and social skills. Now, Hillary is excited to be one of Mercy s school counselors at New Hope Academy. With a passion for working with children and families, Hillary is thrilled to be a part of Mercy Community Healthcare and its mission! She is licensed as a Marriage and Family Therapist by the State of Tennessee and a clinical member of the American Association of Marriage and Family Therapists, the Tennessee Association of Marriage and Family Therapists, and the Nashville Area Association of Christian Counselors. Josie and her husband, Craig, moved to Spring Hill in 2006 and are so thankful to have family living close by. Josie s hobbies include photography, enjoying her porch swing, watching fireflies, talking to dear friends and family, exercising, and loving her Beagle puppies, Summer and Sophie.

21 Mental & Behavioral Health Providers Leiah Drew, Master s Level Clinician BS: Psychology, University of Tennessee MS: Counseling, Trevecca Nazarene University From the age of 8, Leiah knew that God was preparing her to counsel children. All four years of high school she was a babysitter for a child with autism and through the relationship with this family and child she felt her heart shift towards working with children who have special needs. After graduating from the University of Tennessee, Leiah served as the Assistant Director of autism services at The King s Daughters School Center for Autism, a residential and educational program for individuals with autism. There, Leiah had one-on-one experience with non-verbal students who used a variety of ways to communicate. While there, Leiah was more than a teacher or counselor for these children; she was also the Easter Bunny, Santa Claus, the Tooth Fairy, as well as mentor and nurturer. She led and supervised a team of Special Education teachers, and paraprofessionals helping to improve the quality of life for each of her students. She also gained extensive experience in developing behavior plans and individualized educational plans (IEPs), which included transition plans, daily living/ vocational preparation, Picture Exchange Communication (PECS), BoardMaker software, social stories, de-escalation, programmatic skills, augmentative communication, TEACCH, and behavior modification. Through this position, God revealed to Leiah her true vision and purpose for life working with children who have come from hard places. Returning to school for her Master s degree, Leiah enrolled in Trevecca University s counseling program. During her internship she worked at Mercy as a school-based counselor with New Hope Academy and as a mental health counselor with Williamson County Schools Tier 3 program. Leiah currently serves as the Clinical School Therapist at the Tier 3 program in Williamson County Schools. Leiah has a heart for nature; if the weather is delightful you are likely to find her perched somewhere outside, likely next to a lake or stream. She also has a passion for cooking, reading, and making something out of nothing. Most of all, she loves spending time with her family, especially her twin nephews and niece. She has also worked for several years at Smoky Mountain Children s Home as a counselor/social worker and has three years of experience working as a behavioral specialist for children on the autism spectrum. While earning her Master of Arts Degree from Liberty University in Professional Counseling, she was welcomed by the Mental Health staff of Mercy as a counseling intern and upon graduation, she was brought on full time as the first Behavioral Health Consultant (BHC) for Mercy. Stacy has a love for people, the study of the soul (psychology) and the study of the way people go about finding solutions to their problems. She sees people from a bio/psycho/social/spiritual perspective and knows that each person is unique in the way they see the world, learn, and ultimately, heal. Her call to counsel allows her to work with people using Brief Solution Focused Therapy, Cognitive Behavioral Therapy, and Dialectical Behavior Therapy with the goal of helping people improve their daily functioning. All the while, she understands people from a psychodynamic perspective never forgetting that we are fearfully and wonderfully made (Psalm 139) in the image of the Almighty (Genesis 1:27). When Stacy is not at work, she loves to spend time with her husband and daughters doing crafts, playing board games, riding bikes, hiking, watching movies, having dance parties, and making music together in their home studio. On any given weekend, you will find the Phillips family hanging around Thompsons Station Church where they are blessed to be a part of an awesome group of believers who help sharpen them to become all God has made them to be.
