2 Page 1 of 7 SECTION I PURPOSE AND GENERAL PROVISIONS 1.0 PURPOSE OF REQUEST FOR PROPOSAL (RFP) California State University, Chico ( CSU, Chico or University ) is soliciting proposals from certified/licensed geotechnical consultants to provide geotechnical consulting services in support of the Siskiyou II Science Replacement Building Project (Project #SSKU04). 1.1 PROJECT DESCRIPTION The California State University, Chico, Siskiyou II Science Replacement Building project will develop and construct a new structure that will provide four to five floors of academic space. This project, of up to 135,000 GSF, will include laboratories, support space, offices, classrooms and a vivarium. The building will be located on the site of the existing Siskiyou, just north of Meriam Library, south of Big Chico Creek and on the east side of Warner Street. (Refer to Exhibit 1, Project Site Map). 1.2 PROJECT TEAM Sound technical skills and project approach will be provided by strong comprehensive management for each service request. Professionals selected for the project are to have the skills and experience necessary to successfully complete a project. The Service Provider fully commits the time needed by his/her professional staff to perform the work of this project at the highest standard of quality and in a timely manner. The primary contact individual named for this contract shall not be substituted without the prior consent of the University. 1.3 CONTRACT TERM The contract term shall commence upon full execution of the contract and shall terminate upon the acceptance of completion of the Siskiyou II Science Replacement Building Project by the University. 1.4 PREVAILING WAGES Should any work under this contract be performed by workers in the classifications covered by prevailing wage laws, they shall be paid not less than the prevailing wage for the craft or work they perform as determined by the Division of Labor Standards Enforcement (DLSE). The prevailing wage obligation applies to working partners, sole proprietors or owner/operators as well as employees of the service provider and its subcontractors. 1.5 UNSATISFACTORY OR UNACCEPTABLE BEHAVIOR The University s Project Manager will investigate and evaluate any case of unacceptable behavior or unsatisfactory performance by the personnel performing work under any resultant contract. Unacceptable behavior or unsatisfactory performance by contract personnel will be cause for their removal from the project at the request of University s Project Manager. The decision of the University s Project Manager will be final in these matters. SECTION II SCOPE OF SERVICES 2.0 SCOPE OF SERVICES Service Provider shall provide all labor and materials necessary to perform professional geotechnical consulting services under a Service Agreement on an as-needed basis specifically for the California State University, Chico Siskiyou II Science Replacement Building Project. Service Provider shall provide the requested services in accordance with this Request for Proposal. Service Provider understands and acknowledges that this will be a non-exclusive agreement between the Service Provider and California State University, Chico and that CSU, Chico retains the right to procure geotechnical consulting services from others as may be needed by the University during the term of an agreement.

3 Page 2 of 7 Service Provider shall provide a standard geotechnical and geologic seismic investigation and report for the purpose of providing the project architect and structural engineer adequate information to design the subject project to meet all current codes and physical conditions. This report should be considered only a Geotechnical Report or Soils Investigation Report. This report will not include complete geologic hazard studies. The DSA requirement to include a Geologic Hazards Report ( Geohazard Report ) is intended for K-12 schools, community colleges and state essential services buildings, but does NOT apply to CSU non-essential campus buildings. This project falls under a nonessential building and therefore does not require the Geologic Hazard Report. Reasonable and normal interface with the project architect will be required as part of this service. Services in this agreement shall include, but not be limited to, the following work at the project site. 2.1 Service Provider will recommend the approximate number of borings required for the scope of the project, currently estimated to be a total of five (5) to eight (8) borings. The Service Provider will work with campus personnel to identify and coordinate boring locations. Service Provider is responsible for setting up a safe work environment, including temporary fencing as-needed to fence off work areas for safety. Service provider is also responsible for removing spoils generated from the borings. Actual drilling work for borings may need to take place during non-standard work hours due to campus pedestrian traffic. 2.2 To evaluate subsurface soil conditions, Service Provider shall conduct the following laboratory tests on site soil samples acquired from boring operations: A. Visual classifications as per unified soils classification system. Coordinate visual review with University s Representatives. B. Moisture content of undisturbed soils at various depths. C. Densities of undisturbed samples at various depths. Densities to be performed on samples collected in the field and tested in the lab. D. Plasticity and sieve analyses of representative samples. E. Unconfined or triaxial compression tests of suitable samples. 2.3 Service Provider shall analyze the results of the aforementioned borings and tests, and formulate recommendations into a report suitable for providing design criteria for foundation support and earthwork construction of the subject project. The Service Provider will prepare a draft report for the University s review and comment. After revising if necessary, provide four (4) copies of this final report and also provide one electronic copy of the final report in PDF format. The Service Provider shall address in the report the following: A. Description of subsurface conditions encountered in the boring and tests. B. Recommendations for site preparation and grading operations. C. Appropriate foundation systems for proposed structure. D. Allowable bearing pressure for spread and continuous foundations. E. Anticipated total and differential settlements for foundation system used. F. Allowable passive pressures and coefficient of friction for existing soils. G. Allowable equivalent fluid pressure (active) for retaining structures. H. Special recommendations for expansive soils (if needed). I. Allowable vertical and lateral capacity of piles (if needed). J. Settlement of pile foundation. K. Appropriate 2016 CBC seismic parameters L. Sub-grade reaction (for paving work) 2.4 The Service Provider shall provide periodic consultation with the University s representatives during the design phase of the project. The Service Provider is also required to review the 95% construction documents relating to foundation and earthwork design to ensure conformance with recommendations provided in the above report. A letter stating design conformance is the final deliverable for this contract and will be part of the geotechnical investigation work. The service provider is also required to review the foundations shop drawings and any revisions which may occur to the foundation design (which service provider previously reviewed for conformance). 2.5 Geotechnical Consulting Services required during construction are not a part of this Request for Proposal. The University reserves the option to add these services under separate agreement to the selected Service Provider as a result of this Request for Proposal or to bid the services separately at a later date.

4 Page 3 of 7 SECTION III BIDDING REQUIREMENTS & PROPOSAL SUBMITTAL INSTRUCTIONS 3.0 QUESTIONS REGARDING THE RFP All questions or requests for interpretations or clarifications must be submitted in writing to: California State University, Chico Procurement & Contract Services ATTN: Michael McNairn, Contracts Coordinator 400 West First Street Kendall Room 206 Chico, CA mjmcnairn@csuchico.edu 3.1 GENERAL BIDDING REQUIREMENTS AND INFORMATION Errors And Omissions If a respondent discovers any ambiguity, conflict, discrepancy, omission, or other error in this RFP or any of its exhibits, he/she shall immediately notify the individual listed in paragraph 3.0 above of such error in writing and request clarification or modification of the document. Modifications will be made by addenda. Such clarifications shall be given by written notice to all parties who have been furnished an RFP for bidding purposes. If a respondent fails to notify the individual listed in paragraph 3.0, prior to the date fixed for submission of questions, of an error in the RFP known to him/her, or an error that reasonably should have been known to him/her, he/she shall respond at his/her own risk; and if awarded the contract, shall not be entitled to additional compensation or time by reason of the error or its later correction Addenda The University may modify the RFP, any of the submittal dates, or any of its attachments, prior to the date fixed for submission of proposals by issuance of an addendum to all parties receiving the RFP for bidding purposes. Addenda will be numbered consecutively Alternative Proposals Only one proposal is to be submitted by each respondent. Multiple proposals will result in rejection of all proposals submitted by the respondent Rejection Of Proposals The University may reject any or all proposals and may waive any immaterial deviations in a proposal. The University s waiver of any immaterial deviation shall in no way modify the RFP documents or excuse the respondent from full compliance with the RFP specifications if he/she is awarded the contract. Proposals referring to terms and conditions other than the University s terms and conditions may be rejected as being non-responsive. Oral communications of CSU officers and employees concerning the RFP shall not be binding on the CSU and shall in no way excuse the respondent of his/her obligations as set forth in this RFP. The University may make such investigations as deemed necessary to determine the ability of the respondent to perform the work, and the respondent shall furnish to the University all such information and data requested by the University for this purpose. The University reserves the right to reject any proposal if evidence submitted by, or investigation of, such respondent fails to satisfy the University that the respondent is properly qualified to carry out the obligations of the contract and to complete the work specified Errors In The Proposal If errors are found in a proposal, the University may reject the proposal RFP Cancellation This solicitation does not obligate the University to enter into an agreement. The University retains the right to withdraw this RFP at any time should the project be canceled or it is deemed in the best interest of the University. No obligation either expressed or implied, exists on the part of the University to make an award or to pay any cost incurred in the preparation or submission of a proposal.

5 Page 4 of Award Of Contract The University reserves the right to reject any and all proposals and to award one or more contracts. Award, if any, will be to the respondent or respondents whose proposal best complies with all of the requirements of the RFP, and deemed to be in the best interest of the University. Evaluation methodology and basis for award are described in Section IV, Evaluation Criteria/Award of Contract Contract Documents Contract documents will consist of the Service Agreement. In the event of a conflict between documents, the following order of precedence shall apply: 1. Service Agreement, including the CSU Agreement General Provisions 2. CSU, Chico Request for Proposal (RFP) 3. Respondent s Proposal Contract Execution The selected Service Provider(s) and the University shall commit to negotiation and execution of an agreement, in substantial accordance with the terms and conditions herein. Should the parties be unable to reach final agreement within a reasonable amount of time, the University reserves the right to terminate negotiations and proceed with secondary and tertiary finalists Proposals Property Of University Proposals become the property of the University and information contained therein shall become public documents subject to disclosure laws Materials Marked Proprietary Or Confidential Proposers should note that marking proposal documents as Confidential or Proprietary will not exclude the document from being released as part of a public record after notice of intent to award. Proposers believing their materials to be proprietary or confidential must provide a listing, by proposal page number, of all information identified as confidential and a justification supporting the need for such information to be kept confidential. The University will make the determination whether such materials can be released as a public record or not. Please be advised that a blanket confidentiality or proprietary legend identifying the entire proposal as confidential, or proposals that have had every page identified as confidential, will not be accepted and may cause the proposal to be rejected Disposition Of Proposals All materials, ideas, and formats submitted in response to this RFP will become the property of the University on receipt and may be returned only at the University's option and at the Proposer s expense. One copy shall be retained for official files Non-Endorsement If a proposal is accepted, the Service Provider shall not issue any news or other statements pertaining to the award or servicing of the agreement which state or imply University endorsement of Service Provider's services. 3.2 FEE/RATE SCHEDULE INVOICING The respondent is to provide a fee/rate schedule detailing hourly rates for providing professional geotechnical consulting services as specified herein. All fees/rates are to be inclusive of overhead costs, deliveries to laboratories, insurance, taxes, and other associated costs. Rates shall remain firm throughout the term of the Agreement. All invoices will be paid in arrears within thirty (30) days of receipt of proper invoicing and approval by the University s Project Manager. In the event that consulting services are performed after the initial investigation and report, the Service Provider will invoice the University monthly for the work hours expended in the preceding month. All invoices shall clearly identify tasks and deliverables and shall be fully itemized to match and reflect the rates provided in the Service Provider s fee/rate schedule. 3.3 REIMBURSABLE EXPENSES No reimbursable expense shall be reimbursed without prior approval from the University s project manager. Compensation for reimbursable expenses will be for actual expenses, without markup, accompanied by supporting receipts. It is expected that all items will be included in the fees/rates proposed and that additional reimbursement will be minimal, if any. Reimbursable expenses shall be actual expenditures made by the Service Provider working on the project and must be directly related to the project work. Reimbursement shall be made upon receipt of appropriate billing, supporting receipts, and approval of the project manager.

6 Page 5 of TRAVEL EXPENSES Travel Expenses: No travel expense or per diem will be reimbursed without prior approval from the University s project manager. The California State University Internal Procedures/Regulations Governing Reimbursement for Travel Expenses and Allowances can be found here: Current rates are listed below but are subject to change: Airfare commercial airfare coach or economy class Lodging not to exceed $275/night excluding taxes Meals & Incidental Expenses actual meal expenses (no alcohol) up to $55/day +$7/day incidentals Itemized receipts are required Mileage $0.535 per mile NOTE: if there is any known or anticipated reimbursable travel or other planned reimbursable expenses not listed in the Service Provider s fee/rate schedule, please identify and include these in your proposal. 3.5 SMALL BUSINESS, MICRO BUSINESS AND DISABLED VETERAN BUSINESS ENTERPRISE (DVBE) PARTICIPATION The University supports statewide goals for participation of small, micro, and disabled veteran business enterprises in state contracts. As such, respondents to this RFP are to include documentation if they are a small, micro, or DVBE vendor/contractor and data on any small, micro or DVBE suppliers or subcontractors that will be used to satisfy the requirements of the contract. Information on State of California Small Business, Micro Business and Disabled Veteran Business regulations is located at GENERAL PROPOSAL SUBMITTAL INSTRUCTIONS To be considered responsive to this RFP, respondents must submit proposals in the format identified in this section. All requirements and questions in the RFP must be addressed and all requested data must be supplied. The University reserves the right to request additional information, which, in the University s opinion, is necessary to assure that the respondent can adequately perform the work of the contract. It is presumed the quality assurance standards employed in the preparation and delivery of the proposal is reflective of the respondent s overall quality assurance standards to be used in the performance of the contract. Emphasis should be on completeness and clarity of content. 3.7 SUBMISSION OF PROPOSALS Proposals must be submitted in electronic form. Submit a full and complete proposal in electronic PDF format, including the signed cover letter, in one of the following methods: 1. Submit the electronic copy via to the address listed in Section 3.0 with the subject line reading, RFP MM OR 2. The electronic copy can be submitted on a CD or flash drive and delivered to the address listed in Section 3.0. (Materials will not be returned.) Proposals must be received no later than the date and time specified on the RFP cover sheet. Proposals must be submitted sufficiently in advance of the specified date to ensure delivery. California State University, Chico assumes no responsibility for delay in delivery of the proposal either by the United States Post Office, by University Mailroom, electronic or telecommunication delay, or by any other means.

7 Page 6 of PROPOSAL FORMAT AND SUBMITTAL REQUIREMENTS Proposals are to be submitted as follows: Cover Letter The proposal is to be submitted with a cover letter that includes a brief statement of intent to provide the services of the RFP. The letter is to be signed by an authorized officer of the firm who has legal authority in such transactions. An unsigned cover letter/proposal shall be considered non-responsive and shall be rejected. If applicable, the letter should include the firm s State of California small business certification number and/or copy of the Department of General Services certification letter Statement of Qualifications The purpose of the statement is to demonstrate the firm s ability and experience in providing the services. A. General Firm Information/Firm Experience The respondent is to describe the firm s background and experience in a level of detail adequate to demonstrate the firm s qualifications in the field of geotechnical services including: 1. Brief history of the firm, years of experience, 2. Range of services offered, size of the company, 3. Location where management and coordination of services will be performed, 4. General project experience and work performed, 5. References: list of clients (including name, address, contact name and telephone number, and description of services) for which work was performed over the past three (3) years. B. Personnel Experience/Qualifications/Organizational Chart The respondent is to provide: 1. Names and backgrounds of the firm principals (background is to include education, experience, certifications, accreditations, and licenses held by each individual), 2. The name of the primary contact professional who would be assigned to California State University, Chico; this individual would coordinate and execute the work of the contract, 3. An organizational chart showing, at a minimum, the key personnel to be assigned to the University and their reporting relationship within the organization, 4. Names of subcontractors typically used by the firm and their roles/involvement in projects. C. Operational Plan/Approach/Quality The respondent is to provide a detailed conceptual approach of the methodology to be used to perform the work described in Section II, Scope of Services. The response to this section should demonstrate the respondent s ability and commitment to render high quality geotechnical consulting within a compressed time table, provide testing, prepare construction documents, establish appropriate cost and time estimates, and delivery quality assurance and construction administration services required by the University. It should describe the firm s approach/monitoring of work Fee/Rate Schedule The Respondent s proposal must include: A. A cost proposal for performing the geotechnical investigation report as identified in this RFP for the Siskiyou II Science Replacement Building Project. This includes an itemized detail of each task to be performed with estimated number of hours and cost to perform each task. B. An hourly fee/rate schedule for all personnel and other costs associated with the work of this RFP.

8 Page 7 of 7 SECTION IV EVALUATION CRITERIA/AWARD OF CONTRACT 4.1 EVALUATION TEAM An evaluation team comprised of California State University, Chico employees, will evaluate proposals. All proposals will be reviewed, evaluated and scored in accordance with the point schedule for each evaluation criteria noted below: Available Points A. Personnel Experience/Qualifications/Organizational Chart Overall experience and qualifications of firm principals and key personnel Organizational chart/staffing plan C. Operational Plan/Approach/Quality Ability to respond quickly and overall approach to the geotechnical consulting process Approach/monitoring of work D. Fees/Rate Schedule Overall reasonableness of proposed fees Total Possible Points: 30 points 20 points 20 points 30 points 100 points 4.2 ADDITIONAL INFORMATION/PROPOSER PRESENTATION If a proposal response(s) is unclear, the University may, but is not required to, request additional information or clarification of the respondent. At the discretion of the University, a respondent may be asked to make a presentation at the campus regarding their proposal. 4.3 AWARD OF CONTRACT Award of a contract will be made to the respondent receiving the highest number of points, whose proposal best complies with all of the requirements of the RFP, and deemed to be in the best interest of the University. 4.4 CONTRACT ISSUANCE After proposal evaluation, a Service Agreement will be issued to the selected firm. Please view a sample standard Service Agreement, and accompanying General Provisions at The selected Service Provider shall commit to execution of the Agreement, in substantial accordance with the terms and conditions of the RFP and the sample Service Agreement and will also be required to provide or comply with the following: A. Insurance documentation requirements as specified in Rider B, Paragraph 19 of the Agreement General Provisions, and Rider B1 of the Supplementary General Provisions (shown in the sample Service Agreement).
