1 Guidance regarding completion of the build over or near to a public sewer application form 1 Applicant details Please provide details of the applicant, i.e. the person we will be corresponding with regarding the application. 2 Site owners details Please provide details of the site owner. If the applicant is the site owner this section will not need to be completed. 3 Site details Please provide details of the site itself, i.e. the location where the proposed works will take place. 4 Development details i/ Please confirm whether the proposed works involves a new building, an extension/alteration of an existing building or both. If the proposed works entails an extension please indicate the location of the extension in relation to the existing building and the number of storeys. i Please confirm whether the proposal includes the demolition of existing buildings on the site and if so please indicate these on the site plan. The provision of this information is important as may affect the suitability of your proposals. Please stipulate the Local Authority area, i.e. the name of the District/Borough Council area in which the site indicated in Section 3 is located. v) Please confirm whether you have applied for Building Regulations approval for your proposed works and if so please add the Building Control Body reference number. If you have applied to a Building Control Body that is not the District/Borough Council, please add the name of the body. vi) Please state the date of the Building Regulations application. 5 Other details i) Please state the diameter of the sewer(s) (in mm), that will be affected by your proposed works. Please state the depth of any manholes (in metres) that are located upstream or downstream of the sewer(s) that will be affected. The depth of the manhole is the vertical measurement from the channel within the manhole to the manhole cover. Application form guidance notes 11 May 2018

2 i If the proposals involve the extension/alteration of an existing property or there is an existing property on the site, please advise, if you are aware when the property was originally constructed. This information is important as it may help us to establish the status of the affected sewer. Please indicate whether your proposals will require a connection to the public sewer. If the answer is yes you will need to make a separate application to us, information regarding which is available on our website: 6 Supplementary information i) Site location preferred scale, 1:2500, (coloured in GREEN). Site plan, preferred scale 1:100, indicating the position of the public sewer (coloured in RED) in relation to the proposed building/extension. i Elevations of the proposed buildings. Sectional drawing indicating the proposed foundations in relation to the public sewer. 7 Consultation If you completed our Build over or near to a public sewer consultation form and paid the 50 consultation fee, please provide us with your reference number which will start with BC ; the date you made the payment, the payment reference number we gave you once payment was processed and the cheque number (if applicable). 8 Payment and declaration The technical vetting and administration fee is currently set at If a Build over or near consultation was made prior to this application. Please deduct the consultation fee from the technical vetting and administration fee and pay Cheques should be made payable to Anglian Water Services Ltd or you can pay by credit/debit card by calling our Income Team on , option 4, option 1. Please note that you are still required to complete and submit this form even if you have already submitted a Building Regulations Application. Please ensure that you fully complete sections 1 to 8, as failure to do so will delay the processing of your application. Please note that there is a Technical Vetting and Administration Fee for the technical assessment and processing of your application. This fee is applied per application and payment must be made by cheque made payable to 'Anglian Water Services Ltd' with your application or by credit or debit card before you submit your application by calling us on , option 4, option 1. Please note we do not issue proforma invoices however we will issue a VAT receipt once we have processed your payment. Failure to provide any of the information listed above could result in your application being delayed or returned. Please note that works within the vicinity of the sewer must not commence until approval has been granted and any works carried out prior to approval by Anglian Water may need to be removed at your expense.
